FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

I am reporting a potential safety defect involving unintended automatic emergency braking/forward collision warning/brake-control behavior, reverse automatic braking, parking brake behavior, and driver-assistance/sensor calibration. Incident XXX ~11:15 AM, [XXX] driving 2024 Chevrolet Blazer EV ~20–25 mph on/near [XXX]. Vehicle displayed red collision alert and abruptly braked hard although the lane was clear; only a car was parked in a driveway on the right. Sudden braking created serious risk of rear-end collision/loss of control. Vehicle used for family transportation; toddler (~30.8 lb) had been in vehicle ~10 minutes before and family includes blind/visually impaired passenger. History of unwanted reverse emergency braking ~every 45 days: Jan 2026 near Crust Pizza Woodforest (backing up) and March 2026 at H-E-B 7988 FM 1488 Magnolia (backing out), jolting child in rear-facing car seat. Prior service at Keating Chevrolet Nov 12, 2025 (including recall/service for Unintended Parking Brake Engagement N252503010 and missing chassis harness plugs update) did not resolve unwanted braking. After June 10, AEB/front pedestrian braking/intersection collision braking were disabled as temporary safety mitigation. Vehicle removed from daily family use, available for inspection, and I request NHTSA review. Any inspection should preserve relevant braking/parking/AEB/forward collision/sensor data before any reset/update/repair. This is now the second report from this vehicle with similar issues. This incident happened when in reverse with nothing behind me, the vehicle slammed on its brakes. No audible warning in the vehicle and nothing showing on the dashboard. The vehicle just applied emergency braking. This was a witnessed event.

While driving in a plaza parking lot at approximately 18mph the AEB engaged for no reason. There were not any objects near our car to activate the system. The AEB activated so quickly and abruptly that my spouse and I needed to seek medical attention due to whiplash.

The contact stated that while driving at an undisclosed speed, the Forward Collision Avoidance system activated while there were no objects or persons in front of the vehicle. The onstar module failed. Also controlling the pedestrian px Proximity sensor and crosstraffic sensor. When driving slowly waiting a parking lot the vehicle locked up on the brakes with the warning message pedestrian alert. The area was clear. No pedestrians were in the vicinity This would be the 3rd incident 1st was cross traffic already braking while entering a light only at a speed of 15 mph. 2nd was the cross traffic and 3rd was the pedestrian alert.. We discovered the onstar module contrs all of these functions. Contacting the dealer theu stated the module need to be replaced but there is not an ETA on the part arrival as this is a common issue with all vehicles that contain. This module. We are scared it will happen if a driver pulls in front when in traffic or put us in a precarious even deadly situation

While driving, most often at dawn/dusk and commonly with cruise control engaged, the vehicle will falsely detect a vehicle ahead when none is present. It then triggers sudden Automatic Emergency Braking along with a crash/forward-collision indicator, causing an abrupt speed drop of 15–20 mph. This is a major safety concern for my family and me while commuting in our family vehicle. If a vehicle behind us follows too closely and does not brake as rapidly as this faulty feature activates, it could cause a severe rear-end collision, or the driver may swerve into the other lane and collide with other vehicles to avoid hitting my vehicle, which has suddenly decreased speed. Other safety concerns related to this faulty braking feature include passenger whiplash and strains when the vehicle suddenly decelerates, instability on wet/snowy/icy roads, and the loss of an important safety feature. This vehicle safety feature cannot be used due to faulty braking.

The contact stated that after the vehicle was repaired under NHTSA Campaign Number: 25V433000 (PARKING BRAKE), while driving approximately 45 MPH, the Forward Collision Avoidance system erroneously activated, causing the vehicle to abruptly stop in the middle of the road. During the failure, the message "Front Brake Applied" was displayed. The contact stated that approximately 10-minutes prior to the failure, the message that the Forward Collision Avoidance sensors were disabled. The cause of the failure was not yet determined. At a low speed, between 10 and 15 mph, the forward collision system automatically triggered and brought the car to an immediate stop. While we were lucky that no car was following behind us, the passenger and I experienced significant whiplash and lasting back pain as a result of this. The vehicle has not been inspected by anyone, and not been reproduced. The forward collision system flashed (red warning light, alert).
