THE STEERING INPUT SHAFT YOKE PINCH BOLTS ON THESE VEHICLES HAVE BEEN FOUND TO BE DEFECTIVE AND IN SOME CASES, MISSING. IF THE PINCH BOLT IS DEFECTIVE OR MISSING, LOSS OF STEERING CONTROL COULD RESULT, LEADING TO A POSSIBLE CRASH WITHOUT PRIOR WARNING. CORRECTION: VEHICLES WILL BE INSPECTED AND CORRECT PINCH BOLTS WILL BE INSTALLED.
THE STEERING GEAR PITMAN ARM PINCH BOLTS IDENTIFIED BY A "KS" ON THE BOLT HEAD, HAVE BEEN FOUND TO BE DEFECTIVE. LOSS OF THIS PINCH BOLT CAN RESULT IN LOSS OF STEERING AND A CRASH WITHOUT PRIOR WARNING. CORRECTION: PINCH BOLTS WILL BE REPLACED ON AFFECTED VEHICLES.
CERTAIN ROCKWELL FL-941 FRONT STEERING AXLES ON THESE VEHICLES COULD EXPERIENCE FATIGUE CRACKING OF THE AXLE BEAM WHICH IN TURN COULD RESULT IN COMPLETE RUPTURE OF THE FORGED CENTRE SECTION OF THE BEAM. A BEAM FAILURE COULD RESULT IN LOSS OF VEHICLE CONTROL AND A CRASH WITHOUT PRIOR WARNING. CORRECTION: VEHICLES WILL BE INSPECTED AND ALL SUSPECT AXLES WILL BE REPLACED.
THE STEERING ARM USED ON THE EATON EFA-12FA FRONT AXLE MAY FAIL WHEN USED IN CONJUNCTION WITH ROSS HFB-70 POWER STEERING GEARS IF TORQUE LIMITING FEATURES ARE NOT MAINTAINED. A FAILED STEERING ARM WOULD CAUSE LOSS OF VEHICLE CONTROL AND COULD RESULT IN A CRASH WITHOUT PRIOR WARNING. CORRECTION: INSTALL A NONADJUSTABLE 1500 PSI POWER STEERING PUMP PRESSURE RELIEF VALVE TO LIMIT STEERING ARM FORCES. REPLACE STEERING ARM WITH A DESIGN CAPABLE OF WITHSTANDING HIGHER FORCES. ADJUST MAXIMUM POPPET VALVE SETTINGS TO 500 PSI AND ADJUST AXLE STOPS.

NOTE: MODEL FLC-112 CONVENTIONAL TRUCKS. VEHICLES WERE BUILT WITH INCORRECT SIZE AIR BRAKE ACTUATOR CHAMBERS. THIS COULD RESULT IN INCREASED STOPPING DISTANCES AND A POSSIBLE VEHICLE CRASH. CORRECTION: AIR BRAKE CHAMBERS WILL BE REPLACED WITH CHAMBERS OF THE CORRECT SIZE.
AN ELECTRICAL SHORT CIRCUIT CAN DEVELOP BETWEEN A POSITIVE BATTERY POST AND THE BATTERY RETAINER FRAME. THE POSITIVE BATTERY CABLE MAY ALSO CHAFE ON THE BATTERY RETAINER FRAME AND RESULT IN A SHORT CIRCUIT AND POSSIBLE FIRE. CORRECTION: "PICTURE FRAME" BATTERY RETAINER WILL BE REPLACED WITH "I-FRAME" TYPE BATTERY RETAINER AND CONVOLUTED TUBING WILL BE INSTALLED OVER POSITIVE BATTERY CABLE.
THE HANDHOLD MOUNTING SYSTEM CONSISTS OF A TUBULAR HANDLE HELD IN THREE PLACES BY CAST ALUMINUM BRACKETS WHICH ARE, IN TURN, ATTACHED TO THE ENGINE EXHAUST MUFFLER HEAT SHIELD. VIBRATION FROM THE EXHAUST SYSTEM CAUSES THE HANDHOLD PARTS TO WEAR THIN AND COULD CAUSE THE HANDHOLD TO GIVE WAY UNDER THE WEIGHT OF A DRIVER. THIS COULD RESULT IN PERSONAL INJURY. CORRECTION: REPLACE THE HANDHOLD WITH A MODIFIED DESIGN WHICH UTILIZES A RUBBER ISOLATOR. ADD A PLASTIC SLEEVE TO THE CENTER BRACKET FOR ISOLATION.

THE TUBULAR GRABHANDLE FOR CAB INGRESS/EGRESS IS HELD IN PLACE BY THREE BRACKETS AT TOP, MIDDLE AND BOTTOM OF THE TUBE. THE CENTRE BRACKET CONTAINS A RIVET WHICH SECURES THE TUBE IN THE BRACKET. THE DEFECT OCCURS IF THE RIVET IS NOT IN THE PROPER ALIGNMENT, WHICH RESULTS IN SMALL GAP BETWEEN THE RIVET HEAD AND THE BRACKET. THIS COULD ALLOW A RING FINGER TO BE CAUGHT IN THE GAP AND RESULT IN INJURY TO A PERSON GETTING OUT OF THE CAB.
THE ROCKWELL "FF" SERIES FRONT AXLE BEAM ASSEMBLIES MAY CONTAIN CRACKS AT THE AXLE BEAM FORGED ENDS WHERE THE STEERING KNUCKLES ARE ATTACHED BY THE KING PINS. THESE CRACKS MAY LEAD TO A FAILURE OF THE AXLE BEAM END UNDER NORMAL OPERATING CONDITIONS AND COULD CAUSE A COMPLETE SEPARATION OF THE WHEELS, TIRES, BRAKE ASSEMBLIES AND STEERING KNUCKLES FROM THE VEHICLE, LEADING TO A LOSS OF VEHICLE CONTROL AND POSSIBLE CRASH.
THE HOLE IN THE THROTTLE LEVER AND THE STOP PIN MAY BE MISMATCHED WHICH COULD ALLOW THE PIN TO LOOSEN AND FALL OUT. SHOULD THIS HAPPEN, THE TENSION SPRING ATTACHED TO THE THROTTLE LEVER ASSEMBLY WOULD SEPARATE FROM THE LEVER CAUSING THE LEVER TO MOVE TO FULL THROTTLE POSITION, IDLE POSITION OR REMAIN IN THE POSITION THE LEVER WAS IN AT THE MOMENT OF FAILURE. THIS COULD CAUSE THE VEHICLE TO CRASH WITHOUT PRIOR WARNING.
THE SPRING BRAKE AIR CHAMBERS ARE POSITIONED TOO CLOSELY TO A BOLT ON THE SUSPENSION. IF CONTACT BETWEEN THE CHAMBER AND THE BOLT OCCURRED WITH SUFFICIENT FORCE, DAMAGE TO THE PARKING BRAKE AIR CHAMBER COULD RESULT. THIS WOULD CAUSE THE SPRING BRAKES TO AUTOMATICALLY APPLY. THESE VEHICLES DO NOT COMPLY WITH CMVSS 206 - DOOR LATCHES, HINGES AND LOCKS.
THE BOLT ATTACHJING THE PITMAN ARM TO THE STEERING GEAR SHAFT MAY HAVE BEEN INSUFFICIENTLY TORQUED. IN ADDITION, THE RETAINING SET SCREWS MAY NOT HAVE BEEN INSTALLED OR WERE NOT FULLY SEATED. THESE CONDITIONS WOULD CAUSE THE PITMAN ARM TO SEPARATE FROM THE SHAFT AND RESULT IN LOSS OF STEERING CONTROL.
