Certain vehicles may have a bead damage to the tires which occurred during tire mounting. The damage to the tire reinforcing structure in the bead "toe" area would likely manifest itself as a bulge in the sidewall after the tire is inflated. This damage could result in rapid air loss, which could cause injuries to anyone if they were handling the tire when this occurred. Correction: Tires will be demounted and inspected for bead damage. If bead damage is present, the tire will be replaced.

On certain G3 vehicles, the lower control arms could develop a crack in the flange end of the control arm near the ball joint pad. With continued use, the crack could propagate to the ball joint mounting hole resulting in the separation of the ball joint from the control arm. This could result in loss of steering control and a crash without prior warning. Correction: A reinforcement brace will be installed on each of the front lower control arms.

Suspension

Certain G2 vehicles may have been built with an incorrect spare wheel which was specified for use with a G1 model van. The use of the incorrect spare wheel at a regular wheel position could, under certain loading conditions, eventually cause wheel cracking and possible wheel separation. This could result in loss of vehicle control and a crash without prior warning. Correction: Vehicles will be inspected and correct wheel installed if necessary.

Certain vehicles were built with a brake pedal push rod retainer missing, mispositioned or not properly seated. This condition could cause the stop lamp switch and the brake booster push rod to disconnect from the brake pedal, resulting in loss of brake operation and a possible vehicle crash. Correction: Vehicles will be inspected to ensure the brake pedal retainer is secure and fully seated in the groove and can be rotated a full 360 degrees in both directions.

Fuel System

Certain vehicles may have a lower fuel fill pipe which will leak during certain situations (fuel fill, fuel slosh during maneuvers, etc.). The leaking occurs through small holes created by faulty projection welds that attach the mounting bracket to the pipe. In the presence of an ignition source, leaking fuel could ignite and result in personal injury. Correction: The lower fuel fill pipe will be replaced on affected vehicles.

On certain vehicles, the push-pull headlight switch may have been manufactured with electrical contacts which allow an intermittent headlight circuit. An intermittent circuit could cause the headlights to suddenly go out.
