Issue: On certain trucks, the fasteners that attach the steer arm to the steering knuckle may fail. If this happens, the steer arm could separate from the steering knuckle and cause a loss of steering control. Note: This recall only affects trucks equipped with a Dana D-series steer axle. Safety Risk: A loss of steering control could increase the risk of a crash. Corrective Actions: PACCAR will notify owners by mail. The corrective actions for this recall are under development.
Issue: This recall is for certain used trucks that Trojan Welding Inc. converted to dump trucks in 2019 and 2020. The company applied the National Safety Mark and a new compliance label that indicated the trucks were built in 2020. This is not allowed under the Motor Vehicle Safety Act. Safety Risk: Contrary to Trojan's labelling, the modified trucks do not meet all requirements under the Motor Vehicle Safety Act. Corrective Actions: Trojan Welding will notify the owner(s) in writing and arrange for the removal of the incorrect compliance label and National Safety Mark.
Issue: On certain trucks, the welds that attach the trailer hitch may crack due to improper welding. As a result, the hitch could detach from the truck while towing a trailer. Note: This recall only affects certain trucks with trailer hitches installed by Commercial Truck Equipment. Safety Risk: A trailer that detaches from a truck could create a risk of a crash. Corrective Actions: Commercial Truck Equipment will notify owners by mail and instruct you to take your truck to a dealer to inspect the trailer hitch and repair or replace it, as necessary. You should stop using the trailer hitch until the inspection and repair have been completed.

On certain vehicles equipped with an Eaton automated manual transmission and electronic clutch actuator (ECA), the clutch may have been manufactured improperly and could fail. If this occurs, the broken clutch components could prevent the transmission from disengaging or shifting to neutral. This could result in unintended vehicle movement, even if the parking brake is applied, increasing the risk of a crash causing injury and/or damage to property. Correction: Dealers will recalibrate the Transmission Control Module (TCM).
On certain trucks built with a Cummins X15 or ISX15 engine equipped with nylon fuel lines and a banjo bolt on the fuel pump, a blockage in the fuel cooling system may cause a fuel line to burst. This could result in a significant fuel leak onto the roadway, which would create a road hazard for other motorists. Additionally, this condition could also cause an engine stall without warning, without the ability to restart the engine. Both of these conditions may increase the risk of a crash causing injury and/or damage to property. Correction: Cummins-authorized dealers will replace the affected nylon line with a steel line.

On certain fire trucks, the retaining ring(s) in the relief valve assembly may not have been made using high grade stainless steel, causing it to deteriorate over time due to frequent exposure to water. If the retaining ring fails, the relief valve may break down and prevent the fire pump from building adequate pressure, which would increase the risk of injury during firefighting operations. Correction: Dealers or certified technicians will inspect and replace the retaining ring(s) as necessary.
On certain vehicles, the castellated nut and cotter pin on the steer axle may not have been tightened to specification. This could cause the tie rod to become loose in the steer axle, which would result in audible noise and/or looseness in steering. If the vehicle were continued to be operated in this condition, the tie rod could disconnect from the knuckle, resulting in a complete loss of control of a vehicle wheel, which would increase the risk of a crash causing injury and/or damage to property. Correction: Dealers will inspect the torque of the castellated nuts on the tie rod, and replace the tie rod stud and knuckle as necessary.
On certain vehicles equipped with Cummins ISX 15 engines and certain fuel pumps, the drive gear on the fuel pump may spin loose on the drive shaft. This could result in a loss of pumping function, which could result in an engine stall without warning and not being able to be restarted, which could increase the risk of a crash causing injury and/or damage to property. Correction: Cummins-authorized dealers will replace the fuel pump.

This could result in an unexpected engine stall without the ability to restart the engine, which could increase the risk of a crash. Correction: Dealers will replacing the existing, affected ECM with a new ECM.
Certain vehicles fail to comply with the requirements of Canada Motor Vehicle Safety Standard 121 - Air Brake Systems. The two service brake connections of the SR-7 spring brake valve assembly that provide the valve with feedback from the primary and secondary service brake applications may have been reversed during installation. Although the SR-7 valve would still function properly if there were normal reservoir pressure, in the event of a failure in the primary or secondary circuit, the redundant circuit would not provide adequate service brake functionality, and the vehicle would fail to meet the emergency brake system performance standard. This could result in reduced braking performance which could result in a vehicle crash. Correction: Dealers will inspect, remove and reconnect the air lines to the correct port of the SR-7 valve.

On certain trucks equipped with Bendix ATR-6 Antilock Traction Relay valves, in extremely cold conditions (at or below -18 degrees Celsius), internal ATR valve leakage can occur. This could cause unintended brake application, which could overheat the affected brakes and cause a fire. Also, in certain low traction conditions, unexpected brake application can cause a loss of vehicle control and a crash. These issues could result in property damage and/or personal injury. Correction: Dealers will update the cover assembly on the ATR valves.

Should a steer axle slack adjuster fail, it could also cause the vehicle to pull to one side during braking. Either problem could result in a crash causing property damage and/or personal injury. Correction: Dealers will inspect and, if necessary, replace the slack adjusters.

If undetected, the condition may result in universal joint failure, allowing the inter-axle driveline to detach from the axle, which may cause the vehicle to become disabled in the roadway. Lost propulsion, in conjunction with traffic, road conditions, and the driver's reactions, could increase the risk of a crash. In addition a driveline component separating from the vehicle could strike another vehicle, stationary object, or bystander, causing property damage and/or personal injury. Correction: Dealers will inspect and, if necessary, replace the interaxle driveline universal joint.

Engine oil leaking onto hot exhaust components, such as when the engine is under significant load, may create the possibility of a fire which can result in personal injury or property damage. Correction: Dealers will replace the EGR valve.
On certain vehicles, one of the harness spade terminal connections to the chassis load center may become completely unseated. An unseated connection may result in a power loss to a portion of the vehicle's exterior lighting, potentially inhibiting the visibility of the operator and/or detection of the vehicle in certain conditions, which could increase the risk of a crash. Correction: Dealers will inspect the harness and, if necessary, re-seat the female connectors on the male spade terminals.
On certain trucks, the weld around the steering shaft tube, which joins the splined tube to the non-splined intermediate tube, may be mislocated. An improperly located weld bead may allow the splined tube and non-splined tube to rotate relative one another during a low speed, high torque turn, resulting in a possible separation of the tubes. This could cause a loss of steering control which could result in a crash causing property damage and/or personal injury.
