2025 Mazda CX-5 Sacramento, CA 1 injured

At about 6:50am July 7th 2026, I was getting in the exit lane of interstate 50 to exit. I merge into the lane and being applying my breaks since I was going about 55-60mph. However, my vehicle did not slow down. I began to press harder, it still did not slow down, so I pressed as hard as I could because I was rapidly approaching stopped vehicles. The vehicle began to make a grinding sound, it started shaking, and then a "break" light appeared on the instrument panel. After this it came to a sudden and violent stop. I have taken it to the dealership for similar issues many times since I got the vehicle in October 2025. Each time they state that they cannot replicate the issue and the vehicle operates as it should. It is under warranty so I have not taken anywhere other than the Mazda dealership.

I have a serious safety issue with my 2025 Mazda CX-5, purchased new in January 2026 from Island Mazda on [XXX]. A few days before the critical alert, I noticed the car stopping on its own before pressing the brake. While driving on the highway, I received a “faulty sensor” alert that went away. On July 10, 2026, I received a Critical Vehicle Malfunction Alert in the MyMazda app. The app gave no specific details. I called Mazda Customer Experience. They confirmed via email the system labels it as a Parking Brake Malfunction and instructed me not to drive the vehicle. They advised it could stop at any time or not stop. I also experienced pulling and unprompted braking while driving. On XXX, I called Island Mazda service multiple times (calls went to a call center). No callbacks. I went in person. General Manager [XXX] was unavailable; Sales Manager [XXX] advised towing to [XXX]. The car was towed XXX at 1:29 PM. No confirmation call. I called again at ~2 PM. Technician [XXX] called first. Service Manager [XXX] later called and was rude/dismissive. He yelled when I mentioned lemon law saying he can no longer speak to me when i mention that, downplayed the issue, refused to discuss documentation, and suggested I test drive the unsafe vehicle with them to figure out whats wrong. He spoke over me and said it would take a long time to fix because they are busy. He said he would forward my case to someone else and i agreed he is not capable of handling this appropriately. This is a brand new vehicle under warranty. I have a [XXX] son with congenital adrenal hyperplasia (rare condition requiring stress-dosing for injury). I do not feel safe driving it since the alert never even appeared in my actual car just the mazda app on my phone i do not trust the dealership after this treatment to fix it. 2024 Mazda CX-5 Davenport, FL

On 06/18/2026, the vehicle displayed multiple safety warnings, including ABS malfunction, dynamic stability control/traction control malfunction, Smart Brake Support and Forward Obstruction Warning disabled, possible forward sensing camera malfunction, and airbag/seat belt pre-tensioner malfunction. These warnings involve braking, stability control, collision avoidance, airbag, and seat belt safety systems. This was not the first occurrence. On 04/24/2025, the vehicle displayed similar warnings, including critical brake system/Electronic Brake Force Distribution malfunction, ABS malfunction, Smart Brake Support and Forward Obstruction Warning disabled, possible forward sensing camera malfunction, and keyless entry malfunction. The vehicle was towed at that time. The vehicle was inspected by an authorized Mazda dealer in June 2026. The dealer replaced the battery under warranty, but I was not provided diagnostic trouble codes, a root-cause explanation, or written confirmation that the braking, ABS, airbag/seat belt, stability control, Smart Brake Support, Forward Obstruction Warning, and forward sensing/camera systems were fully diagnosed and verified safe. The dealer inspection report also listed the mileage as “No Mileage.” The vehicle is available for inspection upon request. I do not feel safe driving it because the repeated warnings involve braking, collision avoidance, stability control, airbags, and seat belt systems. I am concerned that an electrical, braking, or safety-control defect may disable safety systems while driving.

2025 Mazda CX-5 Issaquah, WA

On May 21st , 2026, while waiting in line to make a right turn (positioned 3rd in queue), the vehicles ahead moved forward. As I transitioned from the brake to the gas pedal to follow, the vehicle experienced an instantaneous engine stall and total electrical failure (NOT an i-Stop event). Pressing the gas caused the vehicle to slowly roll backward. When I immediately tried to reapply the brakes, the brake pedal became extremely stiff and nearly impossible to press down, failing to hold the vehicle effectively. The driver behind me began honking aggressively due to the imminent collision risk. I attempted to lower the windows to signal the other driver, but all electronic controls were completely dead. I had to quickly shift the gear into Park (P) and hit the start button to successfully restart the engine, which temporarily restored functions.

The vehicle's Automatic Emergency Braking (AEB) / Smart Brake Support system intermittently triggers "false positive" emergency braking events during critical driving maneuvers, specifically while accelerating to merge into highway traffic. During these events, the system violently applies the brakes with no external hazard present. Crucially, the vehicle fails to acknowledge driver override input. Depressing the accelerator pedal fully does not disengage the automatic braking, stripping the driver of vehicle control and exposing the vehicle to a severe risk of being rear-ended at high speed. The contact owns a 2025 . The contact was in a parking lot of a shopping center, put the car in gear and the steering wheel became inoperable , vehicle would not stop while depressing the brakes. Four additional vehicles were damaged from the impact with one vehicle deemed a total loss. The right front end was impacted including the hood and the right wheel. No additional property damage. The vehicle came to a stop in the parking lot from impacting the vehicles. Both front driver and passenger side air bags deployed. The vehicle was towed to a tow yard. The vehicle was deemed as a total loss by the insurance company. The contact sustained injuries left broken clavicle, left arm, left and right knees received lacerations and lacerations and 3 broken ribs on the left side and hematoma and received medical treatment at the emergency room intensive care. A woman in the back seat of one of the parked vehicles sustained minor injuries and received medical treatment. The dealer nor manufacturer was notified of the failure. The failure mileage was 2,837

2018 Mazda CX-5 SAN Jose, CA Crash

Unintended acceleration incident. I entered the parking lot of a business where the Salvation Army is located to drop off a donation. The parking spaces were on the left side. As I pulled into a parking space and was in the process of parking, the vehicle suddenly lurched/accelerated forward unexpectedly and struck the concrete wall of the building. The airbags did not deploy despite the frontal impact. No one was seriously injured, but my husband, daughter, and I experienced soreness the following day from the impact. Thankfully, I had chosen a parking space in front of a concrete wall rather than the glass storefront windows, which could have resulted in far more serious injuries or damage. I contacted my insurance company, and the vehicle has since been repaired.

2021 Mazda CX5 AWD- Non Turbo Engine. Sunny Day, Dry. My Car was clean. No Precipitation. Some shadows of the interstate light poles across the road 200-300 feet in front of me, but they were thin shadows. Sun Was to my Left. I was driving South on May 01 2026 about 10am on the Interstate going Highway Speeds, with adaptive cruise control "on", in the 2nd lane of a 4 lane interstate. Thankfully no other traffic ahead or behind me. Car slammed on the brakes suddenly, pretty hard. I counteracted and added more gas pedal to keep in going. I turned off the Cruise Control. After a while I re-engaged the Cruise Control and had no issues. This could have been a huge accident in busier traffic. Will get the car checked out at Mazda Dealership Service this week.

My wife's vehicle has a "Infotainment screen" (dashboard touch screen). There is a class action suit (CSP13d dealer letter) for issues regarding the screen performing tasks (Ghost screening touch) on its own. In lieu of a Mazda vehicle mass recall, they agreed to "FIX" the issue with a software update and replace the CMU Module of the vehicle to rectify this issue. I am here to report / advise you that MANY Mazda owners are NOT happy and have taken it upon themselves to disconnect these screens and do other "work around self repairs (removing/ disabling the unit) in order to fix the issues that occur. I have directly contacted Mazda in regards to my issues and they stat they can ONLY do what was advised on the court ordered settlement and NOTHING more to fix the issue. I want to advise you first hand, their FIX for this issue does NOT fix the problems. In fact, it is making it much worse! NOW my wife's vehicle is operating this screen by itself, WHILE driving, creating a MAJOR distraction. This "Ghost Touching" is actually going into the braking system settings and trying to change/ alter them AS she is driving! A MAJOR SAFETY ISSUE. She has had to, on several occasions, pull over /off the road, shut down the vehicle and try to "reset" what has occurred and check her system for ANY altered setting..if possible. As a retired Police Officer, I can tell you first had that if we ever encountered such thing/ safety issues reported by drivers, we would NOT allow them to travel any further with the vehicle and have it safely towed to a dealer or mechanic for repair. For their safety as well as the rest of the publics safety!! Mazda has refused to fully fix this issue and you can even see on numerous social media posts of MANY others reporting similar issues, disabling their devices on their own and doing other work arounds to mute or disable their devices totally. I Implore you to do a FULL RECALL of ALL affected vehicles to fix the problem ASAP!

While driving at various speeds, including highway speeds, the vehicle intermittently fails to slow down when the brake pedal is applied. On multiple occasions, the brake pedal was pressed but the vehicle did not respond appropriately and continued moving forward. On December 1, 2025, while traveling at approximately 65 mph on the interstate in rainy conditions, the vehicle began making a grinding sound and started sliding instead of slowing down. The vehicle briefly lost the ability to decelerate, creating a near-collision with a semi-truck. On December 22, 2025, the issue occurred twice in one day. While attempting to brake at a stop sign and in traffic, the vehicle did not slow down, nearly causing collisions with vehicles in front and adjacent lanes. In the instance where approaching the stop sign where a vehicle was stopped, the dashboard flashed “break” while I was breaking as hard as I could. Warning lights and sensor-related alerts have appeared intermittently prior to and during some incidents. The issue has continued intermittently, including additional incidents where the brakes were either unresponsive or overly sensitive. They do not respond in a consistent or predictable manner, making safe driving nearly impossible. This problem places myself and others at significant risk of collision due to loss of braking control. The vehicle has been taken to multiple dealerships for inspection. The problem has not been successfully replicated, and no repairs have been performed to resolve the issue. The vehicle exhibits unintended acceleration behavior when the brake pedal is applied. Instead of slowing down, the engine revs and the vehicle does not decelerate as expected. On March 16, 2026, while merging onto a highway and applying the brakes, the engine suddenly revved to approximately 3,500 RPMs while the vehicle failed to slow down. There have been previous issues with the vehicle not slowing when breaks are being applied, nut this is the first instance when it was noticed that the RPMs increased while breaking. This issue has also been reported to the dealership, with service documentation noting that when applying the brakes, the engine revs and the vehicle does not slow or accelerate properly. This creates a dangerous condition where braking input does not result in expected deceleration and may instead cause acceleration-related behavior. The issue has not been resolved. This issue significantly increases the risk of collision and loss of vehicle control.

While driving on the highway in traffic, I was applying the brakes (slowly and smoothly) and moving with the flow of traffic when the SBS system activated the brakes harshly and slammed me to a stop without reason. In addition to throwing me forward against my seatbelt, it also resulted in the car behind me rear-ending me. I did not hit the car ahead of me, since I was far enough behind them BEFORE the smart brake support engaged, but the feature caused an accident that could have been avoided.

Incident Description: On multiple occasions in January and February 2026, while operating a 2024 Mazda CX-5 in reverse on low-friction surfaces (snow/ice) with a slight incline, the vehicle suffered a total systemic failure. System Failures: While reversing at low speeds, the engine spontaneously shut off without warning. This resulted in an immediate loss of power braking and power steering. Simultaneously, the electronic gear selector (shifter) became non-functional, preventing the vehicle from being placed into 'Park' to stop the uncontrolled rearward slide. Safety Risk: The vehicle became a "runaway" object on several occasions. In one instance, the vehicle narrowly avoided crushing a pedestrian against another parked car, stopping only when the tires reached a different surface texture. In a separate incident, the vehicle slid uncontrolled down a residential hill, striking snowbanks. There is a grave concern regarding whether the Supplemental Restraint System (airbags) remains functional during these engine-stall events, as the dealer could not confirm occupant protection status during a total power loss. Inspection & Reproduction: The vehicle was presented to an authorized Mazda dealer on March 3, 2026. The service department attempted to replicate the issue by reversing over soapy water on a gravel surface. However, due to the high friction of the gravel, they were unable to induce the specific tire-slip/low-traction conditions required to trigger the malfunction. No error codes were found during an informal scan, and no warning lamps or messages appeared prior to or during the failures. Component for Inspection: The vehicle is available for inspection. I believe the failure is rooted in the traction control or engine management software, which appears to improperly shut down the powertrain when it detects specific wheel-spin patterns in reverse, subsequently disabling critical safety systems including brakes and transmission locks.
