BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING

The R1T has four cameras to assist the driver and provide warnings for lane changes, etc. The cameras are routinely failing while I am driving the vehicle. This started after an OTA software update. A software reset will turn the cameras back on, though they have repeatedly failed. Rivian attempted to correct the issue by replacing the front camera, which they determined was faulty. The cameras have continued to fail since then.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

I was driving late night to a campsite in a very rural part of the Big Island of [XXX] , about 45 minutes from where I live. I was by myself except for my dog, in the passenger seat. I was on a country road with a 30 mph speed limit, going about 25 mph. I engaged the cruise control and it immediately went up to 40-45 mph, far faster than the limit and far faster than was safe given the wet conditions and windy road. As I tried to slow the car down, using the steering while cruise control adjustment buttons, I steered through a curve and rather than turn with the road, as I had intended, the truck slid off the road to the left, effectively hydroplaning, into two trees, at about 40 mph. The airbags deployed as the truck came to a complete halt against the two trees. The car automatically called 911 as I sat there trying to assess what had just happened, whether I was hurt, and whether my dog was hurt. I told the 911 operator that the truck had called them automatically adn that I was ok and I would call in the morning after assessing what had happened. I am waiting for the police report and also waiting for the incident report from Rivian, which has an on-board log of events. I don't know why the cruise control engaged at such a high speed, rather than at the posted speed limit, but this appears to be a known flaw with Rivian's cruise control that I was not aware of. The cruise control and software are available for inspection by NHTSA. There is also an active recall on this vehicle that I was aware of but had been unable to schedule for service yet b/c it required shipping the truck to [XXX] on a barge. I had only had the truck on island for a little more than a month. From previous operations I had no reason to believe that the cruise control was faulty. It is possible that the recall issue may have been a contributing factor to the crash but I don't know b/c the vehicle has not been inspected yet by Rivian. FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

Initially, my instrument screen on my vehicle went black and stopped working. I had to drive roughly 10 miles home without the use of my speedometer. After replacing the AXM module, the cruise control and lane-keeping functions intermittently stop working. The vehicle was at the service center on and off for 57 days before it was returned to me without a resolution. I’m told this issue is affecting 30 vehicles in the Rivian fleet; however, they don't yet have a permanent fix. I brought up my safety concerns and was assured that this issue will not affect the automatic emergency braking and other safety features, which I hope is true.

This occurred intermittently from approximately 1,200 miles onward and continued for over a year despite multiple repair attempts by the manufacturer’s service centers. The malfunction directly affected safe operation, as the system could disengage without warning at highway speeds, forcing sudden driver intervention. The issue was reproduced and documented by the manufacturer’s technicians during at least one service visit, which resulted in replacement of the vehicle’s driver-assistance computer. The failure continued even after that replacement. The vehicle has been inspected and serviced numerous times by the manufacturer, with diagnostics performed on the cooling system, cameras, sensors, and ADAS module. Each repair attempt either failed to duplicate or failed to permanently correct the defect. The safety risk is loss of Adaptive Cruise Control and Highway Assist functions while driving, unexpected disengagement of automated steering/braking, and lack of reliable warning timing to the driver. Warning messages appeared frequently throughout 2024–2025, beginning within the first few months of ownership. The defect remains unresolved and appears systemic rather than isolated to this vehicle.

The entertainment and gauge cluster displays begin to slow down and stutter, eventually leading to a full reboot. This causes the ADAS view, as well as the blind spot cameras, and reverse camera to display a still image, even while the vehicle is moving, or display no image at all. Also, if this happens while Highway Drive Assist is active, the state of the system is not displayed and an audible chime is played.

Rivian alerted me that there was a “service action” related to the hub, and while waiting for that appointment, the hub came loose while driving and began grinding against the brakes and went into full system shutdown. The truck needed to be towed and the hub and brakes needed to be replaced. The truck was undrivae while driving at highway speeds and could have crashed. When questioned, the service center reiterated that this was “not a recall” but simply a head up. The repairs were covered under warranty.

I ended up turning off LKA, but that should not be the solution.

2023 Rivian R1T at 7,000 miles Bakersfield, CA

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL:SOFTWARE

The contact owns a 2023 Rivian R1T. The contact stated while driving at approximately 70 MPH with the cruise control activated while overtaking another vehicle, the vehicle independently activated the brake. The contact had not turned off the cruise control and there were no other vehicles nearby. The contact stated no warning light was illuminated. The contact stated that when the other vehicle had passed the contact’s vehicle, the brakes disengaged, and the cruise control resumed and accelerated the vehicle back to the pre-set cruising speed. The contact stated that the failure was intermittent but had become a recurring failure. The contact had taken the vehicle to a service center; however, the failure could not be duplicated. The vehicle was not repaired. Additionally, the contact stated that when the vehicle fully charged the battery, the indicator showed that the vehicle was at 100 percent charge with a range of 369 miles. The contact was only able to drive a range of 260 miles on a full charged battery before needing to recharge the battery. The manufacturer was informed of the failure. The failure mileage was approximately 7,000.
