While driving at about 40mph on a city street I complete lost steering, brakes and vehicle control. I suddenly started severely swerving/ spinning. I suddenly had no control over the vehicle. The vehicle threw a bunch of codes at once including: anti-lock brakes malfunction, pre-collision system malfunction, secondary collision brake system malfunction, braking power low, parking brake malfunction. I tried to turn off and restart the car and it did not reset any codes. I have brand new tires on the vehicle and just had an oil change and check up two weeks ago. There was no reason for this to happen other than a complete and sudden safety failure of apparently multiple systems. Vehicle is currently being inspected by local Toyota dealer.

For over a year now, I have been experiencing a random issue in my van with the brake system. It feels like a slippage when I hit the brakes to slow down and it will randomly feels like something slips and just give gives out and gets loose and like you are free falling until the brakes grab and actually stop. I have had it in the dealership twice to fix this issue. Last year they reprogrammed the brake actuator assembly control module, and it worked for a while, but then once again after several months, it started again and the second time I took it in they drove it around to try to get it to mimic the problem, but was not able to so Couldn’t do anything about it and just said to watch it and see if it became more frequent. They also said that once you have the actuator reset the computer shows updated and can’t redo it.The problem with this issue is that it is random and sometimes it’s less and sometimes more and I can’t give specific dates because of the randomness of the problem. I have never not had it to stop, but I am including my two invoices from Toyota of Richardson where I’ve had it seen as well as the research that I have looked up in different forums where other people are describing the very exact same problem with their vehicles. [XXX] . It is scary because you don’t know if it’s going to stop or not but so far it has. [XXX] . There is an issue, but don’t know how to permanently solve it or know for sure what it is. The forum does explain some different things because of the vehicle being hybrid, but who knows whether that’s it. Please look into this and advise because I do not wanna wait until this is a problem and I have a wreck because I am not able to have my brakes work Correctly. P.s. Brakes release briefly when slowing to a stop. While slowing to a stop with steady pressure on the brake pedal, the braking action will momentarily give out, resulting in the vehicle lurching forward beyond the anticipated stopping/slowing distance before the braking action re-engages. This has resulted in near collisions with cars, bikes, and pedestrians as the vehicle suddenly stops slowing without warning and unexpectedly increases stopping distance. The problem is intermittent and difficult to reproduce, but has occurred repeatedly over at least 6 months of ownership. No warning lamps, error messages, or other related symptoms from the vehicle either before or after occurrences of the problem. Has been taken to the dealership for an update to the vehicle but it didn’t fix it. There are multiple reports online and multiple lawyers talking about this.

I am writing to formally raise a serious and unresolved safety issue with my 2022 Toyota Sienna. The braking system on the vehicle behaves abnormally: when the brake pedal is pressed, the van does not stop immediately. In fact, it momentarily accelerates before decelerating, an extremely dangerous malfunction that has nearly led to multiple accidents. I have brought this issue to your attention several times: • In 2022, I contacted O'Regan's Toyota, the Halifax dealership, which has been unable to diagnose or repair the issue to date after several visits. • In 2023, I contacted Toyota Canada directly. I was referred to the dealership in Halifax. • The Halifax dealership has been unable to diagnose or repair the issue to date. • Despite the unresolved nature of this problem, the dealership has recently asked to charge me diagnostic fees for the same issue, one that remains unsolved and is clearly a manufacturer-level safety defect. This is not a minor inconvenience; it is a fundamental failure of the braking system and presents a critical safety hazard. The fact that this issue persists without a resolution, and that I am being asked to pay diagnostic fees for a problem that Toyota has not addressed, is unacceptable.

When decelerating, particularly on hills or into corners, there is an intermittent surge that occurs after the brake pedal has been depressed. It feels as though there is a release of the braking action, this is unsettling as if you are starting to approach a vehicle or pedestrian, this sudden "delay" of braking could result in contact with other objects. We have changed our driving behavior as a result of this. We reported this behavior at 5000 miles and there was not a solution until last year. We were not notified of this, and our vehicle is now out of warranty. Toyota has released as of last year a TSB related to programming and we are going to pay out of pocket $300 this week to get that TSB applied. This doesn't seem fair to consumers. When braking, the vehicle "releases" and lurches forward at a faster rate, but then catches again and continues to brake as it was before. It does not flash or light up on the dash with any issues when this happens. We have tried to find how to specifically recreate this issue to show to the dealership, but it just happens randomly anywhere from 2-12 times a day. This is especially terrifying coming down steep hills or taking quick offramp with stopped traffic in front of you. It began about a year and a half ago and used to take place once every few weeks, it is now happening daily, with increased "release" strength and time. It is still only about 1-2 seconds of released brake pressure, but again is terrifying when behind the wheel and expecting your vehicle to continue to brake as normal while foot is on the pedal. The brake pedal does not depress further or move from its position when braking. The car is acting and performing just as it should be 'normally', except that the brakes give way for 1-2 seconds.

Unexpected sudden acceleration (when braking) as I was slowing down from exiting a highway, just as I was approaching full stop, it felt like the regenerative braking stopped grabbing momentarily and I violently lurched forward for what seemed like a feet or two. I thought it was a fluke back then and thought nothing of it. It just happened again today as I approached a stop sign - again as I neared complete stop with brake depressed it felt like the brake stopped working for a second and the car again lurched forward violently as if I had lost control for what seemed like a foot. On May 18, 2025, 4:00pm, my wife was applying brakes to stop for a stop sign at a T-intersection. Suddenly, the car lurched forward and accelerated uncontrollably. Despite her attempts to stop, her vehicle crossed four traffic lanes, vaulted over curb, nosedived into turf, and then crashed through brick wall into a fire station. My wife was unable to brake and stop the forward progression of the vehicle. My wife steered to avoid hitting trees but could not avoid the brick of the fire station wall. The airbags did NOT deploy. My wife sustained back injuries and bruising of the front torso. Her right foot is bruised from pressuring the brake pedal. The vehicle sustained heavy damage as did the fire station.

The brakes temporarily let up and the vehicle lurches forward before the brakes re-engage, resulting in an unpredictable, delayed stop. It is unpredictable and can result in the driver's stop being dangerous. This usually takes place at stop lights, stop signs, and parking spots, where the vehicle is under 20mph. The car sometimes does not brake as it should, meaning that the nose of the vehicle or the majority of the vehicle will extend past a stop sign or stop light, or contact in a parking lot could occur. The dealer was unable to reproduce the problem. The dealer inspected the vehicle twice. They turned the rotors (unrelated to the issue) the first time I took it in. The second time I took it in, I made them apply the T-SB 0047-24 software update. The T-SB 0047-24 update says that some drivers might be "sensitive to the momentary transition from regenerative braking to hydraulic braking under certain driving conditions." The update to the software was designed to "improve the feeling" for the condition. The brakes are not slowing the vehicle predictably. There are no warning lights or errors given by the vehicle.

During braking the vehicle will lose braking force and “lurch” forward before regaining braking power. It usually happens at about 15-20mph decelerating. There are no warning lights associated with this. The dealer recommended we brake harder to prevent the issue. It still happens and the extra braking force leads to unnecessary wear on braking and suspension components. This caused us to have a near-mis accident while braking in traffic. It also causes quite a distraction during deceleration because it’s unpredictable and uncomfortable when it happens.

When coming to a stop at various speeds the car will sometimes surge during the braking process. It feels like either a brief acceleration or a release of the brakes. It then continues to brake as normal. It is an intermittent issue and can occur when coming to a slow stop or at faster stops. It started in November of 2023, and started happening only once a week or so. As time went on, it became more frequent until it was happening several times a day. By the summer of 2024, the Toyota dealership was finally able to replicate it, and they fixed it with a “computer update” as best I understood. The problem was fixed for a while but began again a few months later. The frequency at this time , early 2025, is every 1-2 days. I have returned multiple times to Toyota, and they have no fix since they have not been able to replicate it this time. I have pointed out to them that they did replicate it when they had previously fixed it. This issue presents a danger when the driver is coming to a stop. The first time it happened my reaction was to move my foot to the accelerator and I jumped a small curb. Although I am prepared now for this to happen, it is possible my instinctive reaction could happen again, resulting in a collision or injury. There are multiple forums and I see multiple complaints on this website, but I see no investigation cases. Please help me understand why this safety issue has not been investigated.

On a regular basis when applying the brakes the brake pressure will release causing the vehicle to lunge forward or momentarily stop decelerating. This release of the brake pressure is only for a second but during that time the van will stop or slow decelerating and if you are slow when this happens you will feel the van lunge forward. It has even happened at a stop with the brake applied and the van would move slightly forward as the pressure released and then reapplied. It almost feels like a computer signal is being reset when this happens causing the momentary release of pressure.

This needs to be addressed by Toyota or someone is going to end up killed or seriously injured in an accident from this malfunction. The brakes in my 2022 Sienna Hybrid XLE randomly slip when stopping. It happens rarely, but is completely unexpected when it does and I can't reproduce it on demand. It feels like I've hit a small patch of black ice or released my foot from the brake for a split second (when I didn't!). My husband rarely drives the van, and it happened to him yesterday. Last week it 'released' for the longest it's ever done, maybe a full second or more. It is summer here and I was driving on normal pavement, so there is no reason that I shouldn't have been in full control of the stop! Thankfully there wasn't anything in front of me, but if it were a different situation it could have been VERY BAD.
