The contact owned a 2023 Chrysler Pacifica Hybrid. The contact stated that while driving at 35 MPH, the vehicle accelerated without the application of the accelerator pedal. The accelerator pedal depressed against the floorboard. The brake pedal was applied, but it failed to respond, and the vehicle continued to increase in speed. There were no warning lights illuminated. The contact crashed into a concrete barrier and crashed into a Ford, and another unknown van. The driver sustained an unknown injury and was taken to the hospital by ambulance to receive medical attention at the hospital. There was no reported fire. The air bags were deployed. A police report was filed. The vehicle was towed to a tow lot. The contact called the local dealer, but the vehicle was not diagnosed or repaired. The black box data showed the vehicle was traveling at 100 MPH. The passenger's side door had detached, and the windshield was shattered. The front and rear of the vehicle were shoved inward. This is the second time we received an alert that our 2023 Pacifica Hybrid minivan while driving sends an alert message to STOP SAFELY VEHICLE WILL SHUT OFF SOON. A green Turtle and Red Hazard Triangle image accompanied the message. First time it occurred was Friday 3/7/2025 and again after servicing on 3/14/2025. Both times the vehicle was traveling at a lawful speed of 65 to 70. The first time it lost power and in order to get to side of road safely the gas pedal had to be pushed to floor to get a reaction and any power. The second time the power seemed to stay until we got to side of road. After turning vehicle off and then on- them message was off but the check engine light remained steadily on. Then upon restarting a second time after driving to dealer it turned off and stayed off. Dealer stated they could not get vehicle to demonstrate any issues and found a stored code that showed only P06DE and No Active Faults. Car appeared to drive fine until the second incident on 3/14 when it occurred again. We have video of codes

Vehicle had been operating less than 5 minutes. When I was on an on-ramp for a left entrance to a highway, several warning lights illuminated in addition to messaging on my dash. The car entered "turtle mode" which extremely limited the speed of the vehicle. I immediately pressed the button for my hazard lights to come on, but they did not operate correctly. They flashed so rarely that they did not warn vehicles behind me that I was having an incident. During this incident, the vehicle speedometer was inoperable, displaying "-- --" instead of a speed. I was lucky to be able to pull to the shoulder, but with the narrow shoulder and my child in the car I felt safer limping off the highway using primarily the shoulder. I was terrified and had some cars nearly hit me/honk as I was going approximately 10 miles per hour on a 55mph roadway. I have seen similar reports on online forums for chrysler pacifica minivans, so I am not the only one to encounter the issue. The vehicle has also at times refused to go into gear while in Park, which is a related problem but I don't deem it a safety issue. The problem is intermittent and the service center is able to see 'codes' when they examine the vehicle but as the issue is intermittent they have returned the car to me twice with no repairs performed since it didn't occur while they had it. The warning lights and turtle mode occurred simultaneously or within a few seconds. I have a video during the incident. I can see the fuel gauge needle rising and falling wildly, turtle mode, a picture of a wrench, check engine, parking brake, brake, collision avoidance, ABS, and others. Each time the vehicle has gone in for service they have seen and cleared codes but they did not come back immediately, so no repairs were attempted. It is in the shop again currently and I have escalated to the manufacturer to hopefully get greater assistance.

We were driving on the freeway, 65 MPH, 0% battery life. Warning came on the dashboard stating something like “ Pull over safely, engine will stop”. Engine had already stopped and we luckily avoided impending doom and pulled off to the side of the highway. Turned car off, waited a couple min, and started the car again. Car turned on with check engine light. Able to slowly get to the next exit. Next morning, took to dealer. After two days, they said it needed a NEW transmission?! Still waiting after 2 weeks. 2023 Chrysler Pacifica. 17,000 miles.

Hello. Writing to you about a serious safety concern on my 2023 Chrysler Pacifica Hybrid. Overnight, I had it parked in my driveway, and this morning, I found it had rolled down the driveway. In addition, because it was plugged in charging overnight, the movement of my van rolling, caused it to pull on the cord and ultimately snapping my charging port. Fortunately,no one was walking at that time as you could understand the concern this would be if this had been the case and my van would’ve harmed someone. I checked my cameras, and it shows the van beginning to slowly roll on its own around 9:40pm until it reaches the end of my driveway. I believe the only reason it did not go any further is because our sidewalk is a bit raised where it meets the end of the driveway, which stopped it from sliding even further than it did. I would like to see if this is a known issue and if not make it aware as this is a serious concern to the public. Also what steps can I take to Chrysler to fix this issue, I don’t feel comfortable now with the car sitting idle in the driveway with kids playing. Also corrected steps to repair the damage to my car. I contacted the dealer and have an appointment scheduled for Friday 12/27/2024. The backover prevention camera failed to display the image from the rear of the vehicle due to there being an unexplained fracture. No warning lights illuminated. The vehicle was taken to the dealer who was unable to diagnose the cause of the failure. The vehicle had not been repaired for the brakes and gear shifter. The back-over prevention camera had been replaced but the failure persisted. The vehicle was returned to the dealer however had not been diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was 30,600.
