Complete loss of motive power due to engine failure

On December 13, 2023, the Office of Defects Investigation (ODI) opened PE23-023 to investigate allegations of engine failures leading to loss of motive power in model year (MY) 2021-2023 Nissan Rogue; MY 2019-2021 Nissan Altima; and MY 2019-2021 Infiniti QX50 vehicles. At that time, ODI had received 6 Vehicle Owner Questionnaires (VOQ) and multiple early warning reports, with consumers reporting engine knocking noises accompanied by a loss of engine power while driving. The investigation identified bearing failures in certain vehicles equipped with the subject 3-cylinder 1.5L or 4-cylinder 2.0...

Issue: On certain vehicles, the engine may not have been manufactured properly. As a result, the engine could make abnormal noises and a warning light may turn on. If you continue to drive the vehicle with these symptoms, the engine could fail. Note: This recall only affects certain vehicles equipped with 1.5 L or 2.0 L variable compression engines. Safety Risk: An engine failure could cause a sudden loss of power to the wheels and/or increase the risk of a vehicle fire. Both issues could increase the risk of a crash or injury. Corrective actions: The company will notify owners by mail and advise you to take your vehicle to a dealership to inspect and, if necessary, replace the engine. On certain vehicles, the dealer will also update the engine control module.

Issue: On certain vehicles, the rear view camera system does not return to the default setting when the transmission is shifted to reverse. This could cause a reduction in rear visibility if a driver had previously adjusted the camera display. Canadian regulations require that the rear view image returns to its original setting each time the camera is turned on. Safety Risk: Reduced rear visibility could increase the risk of a crash. Corrective Actions: The company will notify owners by mail and instruct them to take their vehicle to a dealer to update the software for the rear view camera.

The contact owns a 2019 Infiniti QX50. While the contact's son was driving at an undisclosed speed, the engine seized with several unknown warning lights illuminated. The contact stated there was smoke and fire coming from the engine compartment. In addition, the smell of burning oil and smoke was inside the vehicle. The vehicle was towed to an independent mechanic, where it was diagnosed with engine failure. The contact was informed that the independent mechanic was not an engine expert and recommended that the vehicle be towed to an engine expert. The vehicle was towed to an alternate independent mechanic, where the vehicle was diagnosed with total engine failure. The contact was informed that the engine needed to be replaced. The vehicle was repaired. The engine was replaced with a used engine. The contact later received notification of NHTSA Campaign Number: 25V437000 (Engine and Engine Cooling); however, the part to do the recall repair was not available. The manufacturer was made aware of the failure, and a case was filed. The contact submitted a claim for reimbursement for the repair. The contact was required to submit several documents, including a printout of the repair. The contact was informed that the report needed to include the engine failure code and engine report for the request for reimbursement. The independent mechanic was contacted by the manufacturer and the manufacturer received verbal confirmation of the repair; however, the manufacturer insisted on printed documentation of the repair. The failure mileage was approximately 98,500.

My 2019 Infiniti QX50 has the KR20DDET 2.0L VC-Turbo engine involved in NHTSA Recall 25V437/Infiniti Campaign R25B2 for potential engine-bearing failure, but my VIN does not currently show an open recall. The vehicle is at Infiniti Milwaukee, [XXX], [XXX]. During inspection, bronze-colored metallic glitter/debris was found in the engine oil. This may indicate internal bearing wear and appears consistent with the metal debris that Recall 25V437 requires dealers to check for in the lower oil pan. The dealership is refusing to acknowledge or report the bronze-colored debris as possible bearing material. It reported only “engine sludge” to Infiniti and is treating the engine repair as customer-pay. Sludge and bronze-colored metallic particles are visibly different conditions. The vehicle and engine are available for inspection at the dealership. I requested preservation of the oil, filter, oil-pan contents, photographs, video and technician findings. Bearing failure could cause engine damage or loss of power while driving, increasing crash risk. Warning lights before inspection: UNKNOWN. The exact composition of the debris is UNKNOWN pending testing. You see metal debris in oil pan of the video of an engine that failed citing sledge while clearing seeing metal debris in the photo provided by the dealership. I request that NHTSA investigate whether this vehicle was incorrectly excluded from the recall and whether recall-related metallic debris is being misclassified as sludge. While attempting to open the rear passenger’s side door to remove the child from the vehicle, the contact discovered that all four doors were locked. The Police and Fire Department was called to the scene. The window was intentionally fractured to unlock the door, and the child was removed from the vehicle. The vehicle experienced hesitation, delayed acceleration, and multiple warning lights while driving. The loss of power was severe enough that the driver became afraid to make left turns across traffic because the vehicle would not accelerate quickly or consistently when needed. This created a significant safety risk, as the driver could not reliably judge whether the vehicle would clear an intersection before oncoming traffic. A diagnostic trouble code related to the turbocharger control system was retrieved during the first visit to the dealership. The vehicle was brought in for a recall inspection involving potential engine bearing wear and oil pressure issues, but the recall inspection was not performed on the first visit and was rescheduled. On the way to the recall appointment, additional warning lights illuminated and the vehicle again lost power. The dealership kept the vehicle and stated that further diagnostics were needed. Over several days, the dealership reported that diagnostics were being performed, but no diagnostic results, measurements, technician notes, or recall determination were provided. The manufacturer later confirmed that they had not received any diagnostic information from the dealership and could not proceed with the recall case. The dealership verbally stated that the turbocharger issue was “not related to the recall,” but no documentation was provided to support this. Instead, the dealership sent a cost‑sharing repair proposal without identifying the failed component, the cause of the failure, or whether the failure was related to the recall. Warning lamps and reduced performance first appeared on the initial date of symptoms and continued through the recall appointment. The recall process remains incomplete because the dealership has not provided the required diagnostic documentation.

I am the owner of a 2019 Infiniti QX50, VIN [XXX] , and I am filing this complaint due to repeated, severe mechanical failures that have rendered my vehicle unreliable and unsafe. Since purchasing the vehicle, I have experienced catastrophic failures requiring two complete engine replacements, two turbocharger replacements, and one transmission replacement. My vehicle has spent over one year cumulatively at repair shops, leaving me without transportation and in constant fear for my safety. I am unable to drive this vehicle on the highway or for long distances due to the very real risk of sudden engine stall or failure at speed. I am aware that Nissan has issued Recall 25V-437 acknowledging manufacturing defects in the VC-Turbo engine bearings on the 2019–2022 Infiniti QX50, and I believe my vehicle's repeated failures are directly related to this defect. Despite multiple repair attempts by authorized Infiniti dealerships, the underlying issues have not been resolved. I am requesting that NHTSA investigate this vehicle and pressure Nissan/Infiniti to provide an appropriate remedy, including a full vehicle repurchase or buyback. Good morning, I am writing to formally escalate a serious and unresolved safety concern involving my vehicle and to follow up on my existing case (Case #XXX). On April 22, 2026, while driving on the highway at approximately 55 mph, my vehicle suddenly performed a hard stop as if it detected an imminent collision. At the time, there were no visible obstructions or vehicles directly in front of me that would have warranted automatic emergency braking. This is a repeat issue. A nearly identical incident occurred on August 8, 2025, which was previously reported. Despite that report, the problem remains unresolved and continues to present a significant safety risk. These unexpected braking events create a hazardous driving condition with a clear risk of rear-end collision, particularly at highway speeds. Based on the pattern and circumstances, this strongly suggests a false activation of the automatic emergency braking system or a related safety system malfunction. The vehicle continues to exhibit the same dangerous symptoms, including shaking, engine knocking, and loss of power, and remains diagnosed by another repair facility as having engine bearing failure. This is a serious safety concern because the vehicle can lose power unexpectedly while driving at highway speeds.
