The contact owns a 2016 Mazda Mazda3.The contact stated that while driving at various speeds, the contact stated that the vehicle's steering wheel would fail to operate as needed. The contact stated that an abnormal, clunking noise would emit from the steering wheel while turning at various speeds. The contact also stated that the steering wheel felt loose while driving at high speeds, fearing that he would lose control of the vehicle if he did not firmly grip the steering wheel. The contact had taken the vehicle to a dealer where they acknowledged that the heard the noise coming from the vehicle; however, they could not duplicate or diagnose the failure. The manufacturer was notified of the failure but offered no assistance. The vehicle was not repaired. the failure mileage was approximately 117,000.

On 14th, 2026, while traveling at approximately 60 mph on the northbound 55 Freeway near the Katella exit, my vehicle suddenly began to shake violently without any prior warning. In response, I attempted to slow down by applying the brakes; however, the speedometer rapidly increased even as I maintained firm pressure on the brake pedal. Immediately afterward, the vehicle pulled sharply to the left, causing a complete loss of steering control and resulting in a collision with the center median barrier. Despite the force of the impact, the airbags did not deploy, raising additional concerns about potential safety system failures. At no point before the incident were there warning lights, alerts, or indications of mechanical or electronic issues. The vehicle had also undergone its regularly scheduled maintenance at the Tustin Mazda dealership on January 2nd, 2026, just prior to the accident. Given the sudden shaking, unintended acceleration, loss of braking effectiveness, steering failure, and non‑deployment of airbags, I believe this incident may involve one or more manufacturer defects affecting critical safety systems. I respectfully request that NHTSA investigate this matter. I also believe the vehicle’s Event Data Recorder (EDR) may contain information relevant to identifying the root cause of these failures.

Intermittently spanning several months, there is a sudden loss of power steering as the vehicle is in motion. Warning lights light up on the vehicle as power steering and power brakes are disengaged. Even though I provided a video of power steering loss, where a slight turn of the steering wheel engages this failure, where the dash board lights flash as power steering fails, the dealership will not fix anything they can’t duplicate in their shop. I guess they don’t believe their lying eyes, and don’t accept the video as proof of anything. It is doing exactly what it was recalled for back in September 15, 2010. Recall #10V374000. Mazda Corporate has been notified. Case # 00342901. There are known fixes for this problem. Mazda has done these repairs on hundreds of thousands of times. Dealership told me they will do nothing unless they can duplicate the problem. In reality it might take a month or more for this to show up at some unpredictable time and place. I have a potentially dangerous scenario awaiting me. Driving this vehicle in it’s present condition is recipe for a crash. This was a problem that Mazda addressed and fixed in the past. Being that it was done hundreds of thousands of times, I’m fairly certain that none of those fixes were predicated by mechanics having to see those issues in their shops before they did the repairs.

The power steering on my 2008 Mazda 3 went out. I took it to a mechanic shop who diagnosed the power steering pump is creating pressure that is getting to the rack and pinion and the fluid is just bouncing off of the rack and pinion and the computer system does not know how to function, so the power steering is not working.

During the incident, the vehicle exhibited an uncontrollable malfunction—the steering wheel suddenly jerked sharply to the right while accelerating violently, leaving me unable to steer or brake effectively, the car was looking forward. The car ultimately came to a stop upon colliding with a truck parked diagonally in front of me. After impact, I observed that the front tires remained stuck in a turned-right position, with the steering wheel also locked in that position. This was not an isolated event. Multiple reports from Mazda owners describe similar experiences with their vehicles unexpectedly veering to the right and accelerating without driver input. Given these recurring reports and my own experience, I strongly suspect that a manufacturing defect—whether mechanical, electronic, or software-related—may have contributed to this accident.

Lane departure warning activated and attempted to correct during a gentle curve. The location had shifted lanes due to construction, so clear lines were visible but not the actual lane to follow which is a frequent occurrence in New York City. The system came close to steering me into the adjacent lane occupied by another vehicle forcing me to fight the correction. There was a lane departure warning on the dash and I believe the blind spot indicator was on (i.e. the adjacent vehicle was detected in some capacity). I considered this especially dangerous as in this zone also had very tight lanes so my physical correction could have also caused an accident. There was no malfunction of the physical components. No dealer or service center has reproduced the issue however I have discussed with other drivers of the same line (2020-23 Mazdas) who have had similar experiences leading to a decision to disable these "safety" features. Mazda provides a reasonable description of scenarios where lane departure and assistance will disable itself due to poor conditions or low confidence. I would recommend at a minimum requiring the system to detect and potentially differentiate lane widths, as city driving sometimes has extremely tight lanes (for example, the inner roadway of the Williamsburg Bridge where two adjacent lanes are perhaps 8-9ft with less than 2ft on either side to the wall). In these scenarios, any steering correction at all is more dangerous than if it were off.

When driving and trying to negotiate a turn, or sometimes when going straight, the power assist steering simply fails (stops assisting the steering). I have to "muscle" the car off the roadway and either restart the engine or stop the car (off the road) straighten the steering wheel, restart the car, and the power assist is restored. Power steering failure causing rust on car components. When turning my steering wheel to the left or right there is a noise the car makes and it jumps gear shifts making it very unsafe to drive this car. Also there is an electrical power issue with the car. I just bought a new battery for the car but there still seems to be a shortage And I have to power jump the car every time I tune the car off and have to start the the car up. When I’m driving the car, electricity of the car goes off, including headlights. The steering wheel gets locked. It doesn’t do that regularly. Sometimes, it takes weeks to do that, but sometimes it happens once in every five minutes. The red airbag light on the odometer turns on or off randomly. But, the worse case is that the airbags are not working while my steering wheel gets locked at the same time. They all start happening around the same time. I took my car to the Mazda Service and another mechanics. Neither of them found the issue. When I leave the car with them, the car doesn’t show these malfunctions. It literally randomly occurs. Therefore, they told me that I need to bring the car when it start happening in regular basis to understand the exact cause for the issue. I don’t know when it becomes a regular issue. 4-5 days ago, when I was turning right on a small street, the wheel locked again. I was about to crash. It’s a deadly malfunction, so I don’t want to wait until then.

I can be turning left from a full stop and a few times almost went up onto the median seperating traffic. The car doesn't keep straight in it's own lane unless i put a lot of pressure with both hands on steering wheel and that doesn't always keep it straight, it veers left and right going down the road. Tire pressure light keeps coming on. when backing up hear like a grinding sound in passenger front tire. I bought this used at a Ford car dealership in June 2021. thats all i can think of right now. This has happened more than once no just one time.

On Multiple occasions the Power Steering goes out and warning light on dash comes on. Very hard to steer without the PS on. This has happened on highways as well as in town in traffic. Sometimes it comes back in a few seconds on its own. Other times the car must be turned off and restarted, if you can get to a safe place. No codes appear when checked.
