On June 7, 2026, I was driving westbound on I-290 in the leftmost lane in a 2024 Tesla Model Y. I recall FSD (Supervised) was engaged and making a lane change into the left lane. Instead of settling in the lane, the vehicle continued left and struck the divider/median. I braked. The vehicle then bounced back and moved right toward a large truck and collided. I did not receive a takeover warning before the event. The front bumper came off and the vehicle became inoperable. A fire truck traveling behind me blocked traffic after the crash. Tesla event data records a Crash Algorithm Wake-Up at about 9:49 a.m. Central and includes speed, steering, braking, acceleration, ABS, lane-assist, and cruise-control signals. I reported the safety concern to Tesla and asked for review of the FSD/software issue and data, but I have not received written findings. I do not have an expert determination that FSD caused the crash. I am asking NHTSA to review the incident and data for a possible driver-assistance, steering, or lane-control safety defect.

On August 25, 2025, I was in a crowded Trader Joe's parking lot using the vehicle's FSD parking feature. The vehicle was moving backward into a parking position. A stationary vehicle was in the area. Instead of detecting the other vehicle and stopping, the Tesla kept backing up and made contact with the stationary vehicle. The other vehicle was not moving, and there was enough room for the Tesla to park without contact. I do not remember the exact positioning or direction because of the time that has passed. I did not receive a warning that prevented the contact. I reported to Tesla that the vehicle appeared not to detect the stationary vehicle, but I did not receive an inspection result or written technical finding. On XXX, at approximately 12:30 PM, I was driving eastbound on XXX through [XXX]. The weather was mildly rainy, and I was traveling at approximately 65–70 mph. Without any input on my part, the vehicle suddenly steered sharply to the left, causing it to strike the center divider. The vehicle then crossed multiple lanes of traffic, slid toward the shoulder, and impacted barriers a second time. The collision resulted in the vehicle being declared a total loss, and all airbags deployed. Two days later, after reviewing the vehicle recordings, I observed that immediately after water splashed up from the vehicle in front of me, the Tesla appeared to make a sudden steering maneuver to the left as if attempting to avoid an object or hazard. Based on my review of the footage, I did not observe an actual obstacle in the roadway that would have required such an evasive action. As a result, I am concerned that the vehicle may have incorrectly detected a hazard and initiated an unintended steering maneuver. While I cannot determine the exact cause, the behavior appears consistent with a false or erroneous perception by the vehicle's automated driving or driver-assistance system. Whenever the self driving feature is disengaged, a pop up menu is forced into the user asking for a reason for the disengagement. The user is forced to submit a response for the pop up menu to go away. This menu shifts sections of the screen around and can cause distractions due to the user looking for specific information that is typically there but is not due to the pop up menu.

On XXX, at approximately 5:00 PM, my daughter was operating our 2024 Tesla Model Y on [XXX] near the [XXX] exit in [XXX]. The vehicle suddenly became uncontrollable. According to the driver, the steering wheel appeared to move on its own, the vehicle accelerated unexpectedly, braking was ineffective, and the vehicle zig-zagged across multiple lanes of traffic from the far left lane to the far right lane. The vehicle reportedly performed approximately half of a 360-degree spin before control was regained. Several motorists reportedly pulled over after observing the vehicle’s erratic behavior. Tesla Service later inspected the vehicle and advised that no fault was found and that the vehicle was operating within factory specifications. However, no explanation was provided for the loss-of-control event. Due to the seriousness of the incident and the risk of severe injury or death, I am requesting that NHTSA investigate this matter as a potential vehicle safety defect. I am reporting an issue with Supervised FSD regarding school zones in Ohio. In my area, active school zones are indicated by flashing lights. The FSD system successfully recognizes these lights and reduces the vehicle's speed to the legal limit of 20 mph. However, if there is a traffic light within the school zone, the vehicle loses track of the school zone speed limit after stopping or cycling through the intersection (red/green transitions). Once the traffic light turns green, the car quickly accelerates back to the normal speed limit (e.g., 35 mph) while still inside the active school zone. I have posted this issue to @Tesla and @Tesla_AI on X, but have not received a response.

After updating to Tesla FSD 14.3, whenever I take over from FSD, the vehicle displays a mandatory “intervention reason” prompt on the center screen. The prompt does not disappear unless the driver selects a reason. This occurs immediately after a disengagement/takeover, when the driver may still need full attention on the road. INCIDENT: On April 17, 2026 at ~8:30 AM, my 2024 Tesla Model Y was operating on FSD (Supervised) approaching Wunderlich County Park parking lot entrance, Woodside, CA. Clear daylight, 11-12 MPH, turning into the parking lot. WHAT HAPPENED: A stationary metal W-beam guardrail (~2-3m long, ~1m tall) was directly in the forward path, clearly visible. FSD failed to detect the obstacle, then silently disengaged 2 seconds before impact. TIMELINE FROM IN-VEHICLE REPLAY: - 08:30:23: Vehicle fully aligned with guardrail, direct collision course - 08:30:24: FSD actively engaged (Self-Driving indicator displayed), 12 MPH, guardrail clearly visible - 08:30:25: FSD disengaged - 08:30:27: Impact TWO FAILURES: 1) DETECTION: For ~2 sec (08:30:23-08:30:25), FSD in active control with clearly visible stationary standardized guardrail directly in path. System failed to detect, decelerate, or alter course. 2) SILENT DISENGAGEMENT: FSD disengaged at 08:30:25 with no audible alert, no haptic feedback, no visual alert in driver's forward field of view, no "Take Over" prompt. Only indication was center display change, outside driver's forward line of sight required by Tesla's Owner's Manual to keep eyes on the road. DRIVER STATE: I was monitoring road per Owner's Manual. I received no warning of FSD handoff. I had no awareness FSD disengaged. I took no manual action (no brake, no steer) because I was not notified. I discovered disengagement only upon reviewing in-vehicle dashcam replay the following day. The 2-sec interval was insufficient reaction time even for an informed driver (below AAA/NHTSA 1.5-sec baseline). Research on automated handoff indicates 3-7 sec needed even when notified. Date & Time: December 25, approximately 6:00 PM Location: Northbound I-880 between the Mowry Avenue and Thornton Avenue exits, Fremont, CA Weather & Road Conditions: Heavy rain at the time of the incident. Road surface was wet with reduced visibility. Vehicle & System State: Tesla Model Y. Driver assistance / self-driving functionality was active at the time of the incident (traffic-aware cruise control / Autopilot functionality). Description of the Incident: While driving northbound on I-880 in heavy rain, the vehicle had been operating normally. Without warning, the vehicle appeared to incorrectly detect that it was drifting out of its lane and attempted to correct this perceived drift. Instead of a minor lane correction, the vehicle abruptly applied heavy braking and initiated a sudden and aggressive left steering maneuver. The car crossed approximately four lanes of traffic in the same direction of travel and continued rotating, ultimately performing an unintended U-turn while remaining on the northbound side of the highway. As a result, the vehicle came to a stop oriented against the direction of travel, facing oncoming northbound traffic. Immediately afterward, the vehicle unexpectedly began reversing at high speed without driver input, creating an additional imminent risk of collision with the outer highway wall (noise barrier separating the highway from adjacent residential areas). During this entire sequence of events, I was unable to regain control of the vehicle despite attempting to intervene. There were no vehicles immediately surrounding us at that moment. Had there been nearby traffic, this incident would likely have resulted in a severe or fatal multi-vehicle collision. Occupants: My wife and two young children were in the vehicle. The children were asleep in the back seat, secured in car seats. Outcome: No physical collision occurred. However, the incident caused extreme fear and distress, and we remain shaken by the event.

The heating system in my 2024 Tesla Model Y failed after only one winter season. Under normal HVAC operation, the vehicle blows cold air and does not provide cabin heat. Heat only intermittently works when defrost mode is selected. This failure affects the vehicle’s ability to safely operate in cold weather, including proper windshield defogging and defrosting. A loss of heating presents a safety risk to occupants and reduces visibility. Tesla diagnosed the issue and provided an estimated repair cost of approximately $3,235.00. The vehicle is just outside the warranty period, and this appears to be a premature failure of the heating or heat pump system, not normal wear and tear. Vehicle appears to be giving off toxic fumes causing medical symptoms like headaches for multiple drivers over last 2 months since vehicle was purchased used, fumes have a sweet smell but source of fumes was unable to be identified by Tesla service center. Fumes appears to be coming from front vents of vehicle. Suspecting there is a coolant leak but source of leak is unknown. Tesla had indicated that coolant reservoir was slightly below expected levels. Tesla replaced cabin air filters but that did not resolve the issue, cabin air filters had already been replaced prior to tesla replacing the filters. Tesla service centers indicated they do not have the equipment needed to test what chemicals are in the fumes and have no idea what they could be.

After Tesla Berkeley performed an alignment on my 2024 Model Y on July 22, 2025, the car suffered a complete systems failure while driving on the freeway. Within minutes of leaving the service center, multiple safety systems shut down at once — steering assist, emergency braking, lane keeping, stability control, etc... all turned off, and the car went into manual mode. The vehicle became extremely difficult to control at highway speeds, forcing me to steer and brake manually without assist. Because the Service Center was closed that evening, I couldn’t contact anyone, so I drove back first thing the next morning (July 23) to report the issue. I was initially told the next available appointment was two weeks later, but I had to beg to be seen the same day because the car was unsafe to drive. Tesla redid the alignment and told me the car was fine. After reviewing the alignment reports, both the July 22 and July 23 data are mechanically impossible. The reports show rear camber changes from −1.8° / −2.3° to −1.5° / −1.6°, then to −2.0° / −1.8°, even though rear camber on a stock Model Y is not adjustable. This means the alignment equipment or calibration was inaccurate, and therefore the toe settings were also unreliable. The car was almost certainly released with incorrect rear toe, which explains the immediate handling instability and safety system shutdowns. A later independent alignment on October 13, 2025 (America’s Tire, Roseville) confirmed the correct geometry (−1.9° / −2.0°) and proved Tesla’s readings were wrong. Since the faulty alignments, my car developed severe inner-edge tire wear, and I had to replace all four tires prematurely. This incident involved a total system and steering-assist failure immediately after service, making it a clear safety hazard. I was driving in McKinney, TX on [XXX] taking a friend to his destination. FSD (full self driving) was turned on and enabled for this drive. Normally the car is extremely good at noticing its surroundings, including school zones amidst other things. It is getting better all the time with software updates. However, I turned right into a school zone where the car did not notice that there were other cars going to pick up children. The school zone sign looked like it was abnormally above the ground and so there is a possibility FSD didn’t read it. Unfortunately a police officer noticed me and by the time I did the car was going 16 miles over in a 20 mph school zone. No warnings or anything was shown on the screen to indicate that I had come into a school zone, the speed limit was showing at 40 mph and not 20. Car did not indicate to me that I was not paying attention to the road or doing anything dangerous; it was just going with what it perceived to be flow of traffic. Sun glare is a well-documented driving hazard, yet Tesla’s Y model’s visors are too short to offer sufficient coverage. The vehicle comes with a software feature that automatically shifts the car into park "to prevent rollaway". It does it at random times when we are driving and has multiple times almost caused an accident because my vehicle will suddently stop when I am in the middle of a road or pulling out onto a busy road. The most recent incident was on March 11 2025, I was pulling out from a parking lot, taking a right onto a very busy road. The software unexpectedly shifted the vehicle to park, stopping it completely while I was halfway in the road, with other cars approaching me at 45-50mph. In a panic it was difficult to shift back into drive quickly, and nearly caused an accident that could have been fatal. I called the manufacturer and was uanble to get through to an agent, so I sent an email and have not yet received a response.
