Recurring front passenger safety restraint system fault causing incorrect occupant detection for airbag deployment. Failed and was ‘repaired’ 3 times since 2024, including 2 consecutive failed repairs of the same part within the last 3 months. Tesla service advisor also mentioned informally this is a common issue. Tesla is now refusing further no-cost repair despite both recent repairs remaining under Tesla’s own parts warranty.

We were trying to drive home and the middle seat belt, where my youngest child was sitting, would not latch. it would 'click' in, but if you pulled on it, it would pop out. This was terrifying because we had to drive home on a 2-lane highway with oncoming traffic. I checked online and found that many others have reported this issue because the seat belt latch sits rigidly upright in a divot in the seat, which acts as a funnel to direct any crumbs or dropped items to go into the seat belt latch, which then apparently causes the issue. This is a death trap design for children, because of course children drop things and make messes all the time, and applies to all three of the seat belt latches in the back. What is even worse about this issue is that as a parent I listen for the Click to know they have buckled, and I also check the seat belt indicator on the dash screen. Even when this model Y seatbelt latch isn't going to hold the seatbelt in place, it makes the Click sound and also shows as latched on the dash screen, giving parents a false sense of security. It looks like no children have been injured or killed yet because of this design flaw, but please do not wait for that to happen. Please make them fix this issue immediately before tragedy strikes a family. 2023 Tesla Model Y Georgetown, TX 1 injured

I lease a 2023 Tesla Model Y. The rear passenger side seat belt/buckle assembly had previously experienced issues involving unexpected disengagement/unlatching. The vehicle was previously brought to Tesla for service regarding the restraint system/rear passenger buckle area and Tesla represented that the issue had been addressed. On April 4, 2026, during an emergency braking event to avoid another vehicle that abruptly cut in front of me and slammed on its brakes, I was forced to brake hard and maneuver to avoid a collision. During that braking event, the rear passenger side buckle released/unlatched while my child was properly secured in a child safety seat in that seating position. As a result, the entire child safety seat violently pitched forward and my child’s face struck the seat in front of her. This incident occurred after Tesla had already been given notice of restraint system concerns involving this seating position and after prior service attempts. The issue raises serious concerns regarding rear passenger restraint integrity and child occupant safety during emergency braking situations. Tesla was formally notified again regarding this incident on May 12, 2026. As of the date of this complaint, approximately one week has passed and Tesla has not responded or attempted contact regarding the reported child occupant restraint failure. The vehicle has not yet been reinspected by Tesla following this incident and remains available for inspection. I am requesting preservation of all vehicle telemetry, restraint system data, diagnostic information, and service records related to this vehicle and prior restraint system complaints/repairs.

Driver’s seatbelt: This issue occurred twice. The first time, the car was parked; when I attempted to pull the seatbelt, it would not extend and instead locked in place. The second time occurred after I summoned the car. Again, I was unable to pull the seatbelt out. Because the vehicle was stopped in the middle of the street, I had to drive without wearing a seatbelt, which felt very unsafe. Middle rear seatbelt: This occurred once. I was initially able to fasten the seatbelt, but when I tried to pull the strap down to loosen it because it was too tight, it retracted and locked. I was unable to extend it again, leaving the belt tight against my neck. This also felt very unsafe. I brought the car to Tesla twice, but they were unable to identify the issue. I showed them a video demonstrating the seatbelt remaining locked while I attempted to pull it, as well as footage showing that I was driving without a seatbelt. No warning lights appeared during any of these incidents.

I am reporting multiple safety-related failures with my Tesla Model Y, purchased December 18, 2025. Within four months of ownership, the vehicle has experienced escalating issues: On April 19, 2026, while driving approximately 70 mph on a freeway in heavy rain, the vehicle suddenly lost power and decelerated to approximately 20 mph. Multiple safety systems became unavailable simultaneously, including automatic emergency braking, traction control, stability control, and vehicle hold. I was able to pull to the side of the road, but the vehicle would not restart and required towing. This created a dangerous situation and significantly compromised my ability to safely control the vehicle. On April 9, 2026, a rear seatbelt unlatched under braking while in use. This is a serious safety concern, as the passenger was not properly restrained and had to brace themselves. On February 10, 2026, the vehicle experienced repeated software failures causing lockouts. In one incident, I was locked out while my [XXX] child was inside the vehicle. On January 2, 2026, the vehicle required service for a cracked window shortly after delivery. The vehicle has been taken in for service multiple times, and although repairs were performed, new and increasingly serious failures have continued to occur. I am concerned about the safety of operating this vehicle, particularly due to the loss of power at highway speed and failure of safety systems. I am reporting a safety issue with my 2026 Tesla Model Y. The rear seatbelt system has experienced three separate mechanical failures, each requiring service and replacement. The vehicle was purchased new from Tesla, Inc.. Due to these repeated failures, I have had to adjust seating arrangements for my two children in booster seats, including temporarily relocating one child to the middle seat when a side seatbelt became inoperable. The issue has since recurred on the opposite side. This pattern represents a significant and ongoing safety concern.

Vehicle: 2023 Tesla Model Y Long Range Complaint: I am reporting a serious safety issue involving a vehicle sold by Tim Dahle Nissan Murray in Murray, Utah. Approximately three weeks ago, I purchased a 2023 Tesla Model Y Long Range from this dealership. After purchase, I discovered that the rear seat belts were not functioning. I have evidence indicating that the dealership was aware of this defect prior to the sale. This is a critical safety issue, as seat belts are required safety equipment under federal motor vehicle safety standards. The vehicle should not have been sold in this condition. Additionally, the vehicle had an issue with the right rear wheel/tire losing air. When I returned to the dealership, they failed to properly inspect or repair the issue. Instead, they reduced the air pressure in the other tires to match the leaking tire, rather than addressing the defect. After leaving the dealership, the tire again lost air. Upon inspection by another service provider, the wheel rim was found to be bent. When the tire was inflated to proper pressure, it began leaking air immediately. This created an unsafe driving condition, as the vehicle could not maintain proper tire pressure. I was forced to have the vehicle repaired by Tesla at my own expense ($1,390). The combination of non-functioning seat belts and an unsafe wheel condition placed my family, including my newborn child, at significant risk. I am requesting that this matter be investigated as the dealership knowingly sold a vehicle with defective safety equipment and failed to properly address additional safety-related issues. Right rear passenger seat belt malfunctioned after owning the vehicle for one month. When I called the service department to discuss the issue, they said the part would take 5 days to come in. They denied me a loaner car to drive while I waiting for the part. The malfunction was confirmed by the dealership the following week and required replacement. I still cannot believe that a company who values safety, expected me to continue to drive my children in a car that had a malfunctioning seatbelt, until the part came in.

Rear seat belt latch won't latch. This becomes a problem because dirt and debris fall into the seat belt latch since the seat belts latches are lower than the seat cushions. Child eats something with crumbs and the crumbs fall into the seat belt latch. Over time these build up to the point the seat belt won't click in.

Vehicle had "front passenger safety restraint system fault" multiple times intermittently. Serviced by Tesla service center in Dublin, CA who confirmed the problem. Documentation on the invoice stated that there was an open service bulletin related to this issue and was related to "the occupancy sensory in the seat was not functioning as expected." Upon inspection by the technician they found "an open connection in the seat harness and seat cover that can cause intermittent alerts." Additional repair notes indicate that this was to "address a service bulletin related to the front passenger seat's occupant classification system. We identified the need for an update to ensure proper functionality. To resolve this, we retrofitted the system in the front passenger seat as per the bulletin requirements." The fact that there is an open service bulletin for this issue indicates that it is not isolated to just this vehicle. An issue with the occupant sensors of a front vehicle seat directly impacts whether the air bags will deploy in a crash, therefore this is a serious safety risk.

Safety complaint: intermittent uncommanded rear-seat folding while driving; rear (3rd-row) seat belt anchorage integrity. Issue 1 — 2nd-row right seat: Since the purchase, the right-most rear passenger seat in the 2nd row intermittently activates its seat-folding motor while the vehicle is moving. In each event, a passenger was seated in that seat with the seatbelt fastened. The motor engages (audible whirring), and the seat attempts to fold fully forward; the passenger’s body prevents a complete fold, so the seat partially collapses while occupied. This has never occurred when the seat is unoccupied. The most recent documented event times provided to the manufacturer for electronic log review (CST): • Fri Jan 23: 7:40 AM; 7:41 AM • Mon Feb 2: 7:26 AM; 7:29 AM; 7:53 AM; 7:54 AM The manufacturer requested these exact times to check vehicle logs, but reported they could not find evidence in the logs and could not reproduce the issue during service/test drive. Issue 2 — 3rd-row seat belts: During a service visit on Feb 3, 2026, the manufacturer found that the 3rd-row seat belt anchors were not secured to the vehicle (documented as a safety concern) and performed a repair/adjustment to secure the left 3rd-row seat belt, then validated its operation afterward. The manufacturer had also previously documented (March 17, 2025) that the 3rd-row seat belts were not properly mounted and were unsafe/operational for passengers, but took no action in March 2025. No crash, fire, or known injuries to date. Request to NHTSA: Please log and review this as a potential safety defect involving (1) uncommanded activation of a rear seat-folding mechanism resulting in partial seat collapse while the seat is occupied and the vehicle is moving, and (2) rear (3rd-row) seat belt anchorage/installation integrity, based on recurring real-world events and manufacturer service documentation.

A few weeks ago my seatbelt censor started acting up, and then two weeks later that my heater quit. They blamed Chewed wiring because of soy based coatings, and set its not in the warranty and accused rodent damage since the rear seat was eaten. I got the call during diagnosis demanding i bring the loaner car because they wanted $1,349.07 which i did not have because a 9/11 first responder and don't have other insurance accept mandated state minimums, they told me bring the loaner back, on the way home FSD worked till i got towards my putnam county rural route 22 when power steering FSD and power breaks died. I skidded at 25 mph managed to get to park and plug it and leaving it this. So the tech put the plugs together and made the situation worse by plugging chewed harness together. No heat, transport, no car, no door dash money for me, no medical appointment and cant risk losing all steering. When TESLA works, it works very and great car. had they refused to use soy coatings and stayed with rubber coatings this issue would not be here. how Many FSD crashes are result of rodent damage?

In early Nov 2025 my Tesla Model Y began intermittently showing warnings about the forward collision warning system and multiple sensors being offline. Tesla service replaced a faulty connector on Nov 3. On Nov 15, 2025, about two weeks later, the car had a sudden drivetrain failure while I was parking on a steep hill. I heard a loud bang from the front of the car and the vehicle immediately lost drive power and had to be towed. Tesla diagnosed a failed front motor inverter that blew the high-voltage pyrofuse and damaged the 12V battery; they replaced those parts. The vehicle is available for inspection and the repaired components and records are with Tesla. After that repair the car needed another visit for alignment and has continued to show worrying behavior: “VCFRONT offline/unable to contact VCFRONT” in the service menu, repeated “autopilot computer overheating” messages, intermittent loss or reduction of regenerative braking (braking feel changes unpredictably), a rear right seatbelt malfunction warning (later inspected and cleared by Tesla), and what appears to be significantly increased battery drain compared to before the inverter failure. This has now resulted in four service visits within about a month, all at Tesla service centers (no police, insurance, or independent shops involved). Tesla has confirmed the inverter/pyrofuse/12V failure and says the overheating was due to an air pocket in the cooling system, but the VCFRONT messages, inconsistent regen braking, and increased drain have not been fully resolved or clearly explained. I am concerned that a sudden loss of power, loss of regen, or failure of VCFRONT/autopilot systems could recur at speed or in traffic, creating a serious safety risk. Please note that I'm only able to view partially-complete paperwork from the latest service appointment in the Tesla app. I have requested this documentation from Tesla and will be able to provide all the service reports and communication as a follow-up.
