Something needs to be done because Toyota is stalling on sending out a critical safety component that has been recalled in thousands of vehicles. It is the HV floor wire connector, when the corrosion is beyond repair, the parts have been back ordered for months. Toyota and dealerships are not approving loaner vehicles. This is a serious safety issue because: If the wire corrodes or shorts out, the vehicle can experience unexpected power loss, stalling, or complete hybrid system failure. this could happen in the middle of driving in fast, traffic, leading to serious injury or loss of life. I have been waiting since February for this part and it is still on backorder. Toyota is not taking this seriously because it’s a $10,000 part and they don’t want to pay for the thousands of people who are affected by this. My life is more important than that, as is the life of my child. I exhort you to put pressure on Toyota. Thank you.

In regards to recal 25TA08/25TB08, Toyota has yet to release a fix for the very critical digital instrument cluster going blank forever. I got a notice in the mail that they had a software update for me but when I was able to schedule an appointment with the dealer, the dealer advised that within 10 days of Toyota releasing the “fix” they rescinded/repealed the “fix”. So by the time I got the letter giving me hope that my car would live, I realistically had 2 days to get it in to get updated with a fix that probably wouldn’t have worked. With such a critical part/software issue, I feel Toyota is massively dragging their feet and I am driving a ticking time bomb and possibly having this car become unsafe to drive and potentially useless all together.

The car was under warranty and experienced a transaxle failure while on a highway between Yuma, AZ and Tucson, AZ. The dealer who inspected and repaired the car determined the cause to be a flawed cooler replacement by a body shop in which transmission fluid was not properly replaced subsequent to a left front fender bender accident, which voided the warranty. (Fluids were dry on inspection; no leaks/issues identified at our residence or during "Toyota Care" X2 following the body shop repair.) The car worked flawlessly for one year after the body shop repair until the transaxle failed. Issue: There were no warning messages to indicate a problem with the fluid level until immediately before the catastrophic failure; we were able to get the car to the side of the road right before the wheels locked but had we not, a serious crash could have occurred. Toyota tells us there is no alert for fluids since they have designed the transaxle as a "closed system". However, conditions exist that could compromise this closed system so consumers (and service dealerships) should have a way to proactively identify a fluid level problem so it can be remedied to avoid potential safety hazards related to an abrupt failure.

The instrument cluster (combination meter) failed completely and without warning, going entirely blank. This eliminated all speedometer, warning light, and indicator functionality. The failure began occurring intermittently before becoming persistent. No warning lamps or messages preceded the failure — the cluster simply went dark. The loss of speedometer functionality made the vehicle unsafe to operate, as the driver had no means of monitoring vehicle speed. The loss of all warning indicators eliminated visibility into any safety-critical vehicle alerts that may have been active during the failure periods. The problem was confirmed by the dealership and is the subject of Safety Recall 25TA08 (Combination Meter May Go Blank). The vehicle was brought to a Toyota dealership where the recall was documented and the repair was eventually completed. The replacement part required VIN-specific production and programming by Toyota, resulting in an extended out-of-service period of nearly two months at the dealership, and approximately five months from the first occurrence of the failure. During the out-of-service period, the vehicle could not pass state inspection and was legally inoperable. The component has been replaced and is no longer available for inspection.

The 12.3 inch display of the instrument panel, where the speedometer and malfunction indicator lights are shown, goes blank when I turn ON the engine. It remained blank even when I drive. It is NOT safe to drive as I will not know which part is malfunctioning. The dealer has confirmed that it is a known issue. There were NO warning lamps, messages or other symptoms of the problem prior to the failure I took the car to the Toyota Dealer in Fremont, CA in October 2025. Till today ( March 29th 2026 ) the issue has NOT been repaired yet. The dealer has mentioned that they have NOT received any Fix for this issue from the Toyota Manufacturer.

The dealer has tried to order a new dash three times and each time Toyota has canceled it because it is a recall issue however this has been going on 39 days so far.

There is an active recall on this car with no available remedy. Upon starting the car, the instrument panel/instrument cluster screen remains completely black and doesn't turn on. This means there is no way to see: (a) speedometer, (b) odometer, (c) tire pressure, (d) available fuel range, (e) available EV range, (f) check engine lights or other emergency alerts, (g) lane assist, (h) drive menu settings, (i) temperature outside, (j) what gear the car is in, (k) whether the car is in EV mode or HV mode, (l) average fuel economy, (m) whether the parking break is engaged, (n) whether all seatbelts are fastened, (o) whether any doors or trunk is open, (p) battery status, and (q) oil status. To be clear, Toyota is well aware of this issue and has sent out the recall notice. Upon taking my car to the dealership, they confirmed that this is the active recall issue and there is no available remedy. If the instrument panel has not yet gone blank, there is a software update that can be given; however, if the instrument panel HAS ALREADY gone blank, there is ABSOLUTELY NOTHING THAT CAN BE DONE TO REMEDY THIS. The dealership would not release my car to me as it is not drivable. The recall has been active since SEPTEMBER 11, 2025 and there is no remedy available FIVE months later.

On Friday Jan 30 around 3pm, this Rav 4 entered the short drive way in front of the garage. The car stopped, the automatic garage door was engaged. Suddenly, the car advanced, passing through the opening garage door and into and through the back wall of the garage. This is a sustaining way for the second storey. The front of the car displaced the counters inside, demolished the kitchen, almost toppling the fridge. Given the damage to the garage done and the damage inflicted, my interpretation is that an unintended acceleration event occurred. The airbags did not deploy. My wife, [XXX] , was driving at the time and was alone in the car. The air bags did not deploy Significant damage was done to the kitchen. And because the wall was a sustaining wall, there was serious damage to the support beams throughout the house. Estimate is $118,770 plus $30,000 for the repair work in the attic. My premium plug-in hybrid SUV (RAV4 Prime XSE) has been continuously grounded at my local authorized Toyota Service Center since December 29, 2025, due to Safety Recall 25TA08 (NHTSA Campaign 25V-595). The 12.3-inch digital instrument panel in my vehicle had failed completely, removing access to the speedometer, system warnings, tire pressure alerts, and other critical information, which poses a severe risk of crash or injury. As of today February 5th, 2026, my vehicle has been out of service for over 37 consecutive days with no available remedy or estimated repair date. This constitutes an unreasonable delay in repair and a failure by the manufacturer to remedy a safety defect in a timely manner as required by the National Traffic and Motor Vehicle Safety Act (49 U.S.C. § 30120), to my best understanding. This grounding has created a severe family hardship primarily because this is our only vehicle. We are a family of three, including a [XXX] toddler with daily transportation needs. Toyota’s attempts to mitigate our hardship have been substandard: (1) Initially, I was provided an economy SUV (Corolla Cross) which I was forced to return on January 16th because the dealership allowed the temporary registration to expire; (2) I was then downgraded to an economy sedan (Corolla) which was unfit for local winter conditions (snow and ice) and failed to meet the safety and transportation needs of my toddler; and (3) Only on February 5th was I offered a gas-powered RAV4. Despite owning a premium plug-in hybrid, I have now been forced to pay unfairly for gasoline for 37+ days due to Toyota's inaction. Because my vehicle is unsafe to drive and the manufacturer cannot provide a timely fix, I am formally requesting a prompt manufacturer action in the form of a prompt repair or a fair repurchase (buyback) to resolve this ongoing safety and financial burden. My 2023 Toyota RAV4 Prime is currently subject to an active safety recall. I brought the vehicle to my Toyota dealership on 12/20/25 to have the recall addressed, but I was informed that Toyota does not yet have an available fix or remedy. As of January 28, 2026, there still is not fix or remedy. As a result, my vehicle has been sitting at the dealership for an extended period of time with no estimated completion date. Toyota has provided a rental vehicle, which I appreciate, however there is still no clear timeline for when my own vehicle will be repaired and returned. I am filing this complaint because I was told there should be a fix/remedy for this recall and the recall remedy delay has effectively taken my vehicle out of service indefinitely. Owners should not be left without their vehicles for unknown periods while manufacturers work on recall solutions. I respectfully request that NHTSA review this situation, as the lack of an available remedy and open-ended repair timeline is creating hardship and uncertainty for affected owners.

The dashboard screen (which shows all data, including speed, gas, charge, etc) has been completely blank/black regularly and will stay that way for days. There is no way to know how fast we are going, how much charge the car has, if there are engine issues, etc. This has been happening for a few months now. The car has not yet been to the dealer to be seen but will be soon. I found many reports of this same thing in the same models and years on reddit and other places on the internet.

The mechanized power liftgate— the rear door, the primary way to access the back of vehicle—fails to open & close as intended. A continuous beeping noise sounds due to the liftgate’s failure to fully open and close. We reported this issue to the dealer multiple times during 2025. On July 24th, I experienced sudden unexpected acceleration with my 2021 Toyota Rav4 Prime XSE (41,600 miles) while driving in the electric mode. I was lining myself up to reverse into my garage and as I paused to switch gears to reverse, the car suddenly accelerated full-speed towards my neighbor's house. I slammed the brakes but that had little effect. The car slammed into the house, through the garage door and my car was only stopped when hitting an SUV parked in the garage, pushing it forward about 2 feet. No air bags were deployed, the collision detection system on the vehicle did not seem to work and tire marks from my braking were visible from the middle of the road. I have found a number of very similar incidents with Toyota cars described online. Cars were often turning into parking spaces at slow speeds and ended up accelerating into lamp posts or buildings. If there hadn't been a car parked in the garage, I can't imagine what might have happened as the car was probably going 35 mph+ upon impact. There were no warning lights and the collision shop informed me that there were no diagnostic codes when they downloaded the computer.

Moonroof spontaneously exploded while driving on the highway. The moonroof was closed, no one was trying to open it. All windows were closed, no one was trying to open them. My husband was driving and differently we heard a loud explosion which sounded like a shotgun going off and thought someone was shooting at us. I looked behind the car and could see glass flying everywhere. We safely pulled over and the entire moon roof had randomly and spontaneous exploded. No collision. Nothing fell on us etc. A quick Google search with show you that this is not uncommon for my car's make and model. Something needs to be done! I was stopped at a red light at an intersection, when a car pulled up next to me, and the driver rolled down the window to tell me my vehicle was on fire. I got out of my vehicle while the driver of the other car called 911. Brookline Police & Fire came, firefighters extinguished the fire, and one of the emergency services responders had a wrecker come & take the car away. The car was towed to Watertown, then collected by Safety Insurance, which got a repair estimate of $95,000. The car was deemed a total loss and is in the possession of the insurance company. When the vehicle caught on fire, it had 3,627.4 miles on it. I did not see any warning lamps, messages or other symptoms of the problem prior to the fire. My safety and the safety of others was at risk due to the sudden and unexplained fire.

2021 Toyota Rav4 Prime, 69K miles ***** On [XXX] while driving at highway speeds, many warning messages began to scroll across the display. The gas pedal immediately became unresponsive. The power steering stopped working. The car was unable to be stopped with the brake pedal, and it had to be allowed to coast stop. ***** The car was then towed to Fred Anderson Toyota in Asheville, NC. The service advisor refused to scan the codes and said that all I needed to do was change the battery. I purchased a battery from Fred Anderson and changed it myself. The car turned on without any warning lights. ***** I started driving the car home. After approximately 45 minutes of driving, while at highway speed, the same dangerous situation occurred: Many warning messages began to scroll across the display. The gas pedal immediately became unresponsive. The power steering stopped working. I was unable to stop the car with the brake pedal, and it had to be allowed to coast stop. ***** Twice that same day, this vehicle lost all power and turned off power steering and braking without any prior warning. [XXX] that nobody was injured during either occurrence that day.
