Driver Assist & Collision Avoidance complaints by model year
The rear-view camera intermittently fails to engage when the vehicle is shifted into reverse. When the camera does display, the image sometimes shows horizontal glitch lines/artifacts. This affects backing visibility and parking safety and creates a risk of collision with pedestrians, objects, vehicles, or property. The concern was reported to Audi service along with other electrical/driver-assistance symptoms, suggesting a possible camera, wiring, module, signal-processing, or broader electrical fault. No crash, fire, injury, or fatality occurred. This complaint is being submitted because the rear camera/reverse display is a backing-safety system and the intermittent failure is unpredictable. The vehicle/component is available for inspection, and photo/video evidence and service communications are available.
NHTSA complaint #11747799 — an unverified report by a vehicle owner.
I am reporting recurring safety-related defects on my 2018 Audi A4 involving Automatic Emergency Braking, Forward Collision Warning, Audi pre sense/front-collision functionality, and related electrical/driver-assistance reliability. The AEB/forward-collision system has shown two unsafe intermittent behaviors. At times it has failed to activate as expected when crash-mitigation assistance appeared reasonably available. At other times it has unexpectedly warned, jolted, or applied braking when there was no apparent vehicle, object, pedestrian, or hazard close to the car. Unexpected braking or false intervention can startle the driver, create rear-end collision risk, and reduce confidence in the system. Failure to activate can also increase forward-collision risk. The system was not intentionally disabled by me, and I am not aware of any driver-caused obstruction, modification, or normal explanation. The condition is intermittent and may not appear during a brief service visit or basic fault-code scan. This concern was reported through Audi/dealer service channels, including a documented repair request identifying the AEB system as malfunctioning and failing to activate as designed. A later dealer communication stated that there were no stored AEB faults and later clarified that no work was done on the AEB system, no inspection or verification of AEB operation was performed, and confirmation of operation would require an additional estimate. I am reporting unresolved safety status because intermittent active-safety failures or false interventions may not store permanent DTCs, and lack of stored faults is not complete safety verification. I have not received final written safety closure confirming inspection, calibration review, road-test verification, factory scan/freeze-frame review, module communication review, and technical verification. No crash, fire, injury, or fatality is being reported. The vehicle is available for inspection if requested.

NHTSA complaint #11741957 — an unverified report by a vehicle owner.
This problem is extremely sporadic, and often remedies itself after car shut down, and sometimes require that the car sit for a period of time. I estimate the following issue happens ~1 time a month, or estimated 5% of the time. Weather (temperature specifically) does not appear to impact occurance. When reversing, at times the back up camera does not function properly. During these times, the parking sensors appear to work, but it's unclear if the emergency reverse automatic braking system is working. The backup camera issues present themselves in several different ways and are described below. 1) Scrolling view (think of an old time movie, where the image "scrolling" through the screen 2) Black, but not completely black, includes what appears to be interference (sometimes white pixels flash randomly throughout the screen) 3) Off color (almost like a color inversion), darks appear light, light colors appear dark, similar to how things when developing film in a dark room 4) Similar to #1 but instead it freezes, halfway through a scroll. Often times the picture is split, with the bottom on the top, and the top on the bottom, and a clear black line in the middle Opening and closing the trunk, tapping the camera, turning the parking sensor (and camera) off and on do not solve the issue when it happens. Neither does changing from R to P on the transmission lever. Issue always requires a restart, rarely does an immediate restart fix the issue. The tracking lines are also distorted when this happens. When not malfunctioning, camera system works perfectly. Recently, I came across NHTSA ID: 25V900000 which almost exactly describes my issues but my car is not included. I did recently ask the Dealer to look into it during my 40,000 mile service. They stated that no codes were in the system and they could further troubleshoot but the intermittent nature would likely result in no findings unless the issue is present on site.
NHTSA complaint #11709539 — an unverified report by a vehicle owner.
My screen turns on and off consistently not allowing me to use my backup camera. Causing a small collision while backing up when the camera turned off unexpectedly. Additionally when it’s off it will auto break when I’m not close enough to hitting and it’s loud and yanked me. This has been an issue for some time and I’m seeing a significant amount of my models have this issue and it’s not safe.
NHTSA complaint #11702346 — an unverified report by a vehicle owner.
While driving southbound on [XXX] near [XXX] in Mississippi, the advanced driver-assistance systems (lane assist and radar) did not operate as designed. At highway speed, the vehicle gave no warnings, no steering correction, and no radar-based intervention prior to a severe crash. This failure of the safety systems placed my life at extreme risk. The vehicle had been properly serviced at an authorized dealer, and there were no warning lights or fault messages prior to the incident. After the crash, nearly all airbags deployed, but the crash data recorder shows abnormal timing information. The Bosch Event Data Recorder (EDR) captured a force of 292 G’s, confirming the severity of the impact. Dash camera footage also documents that the systems failed to intervene before the collision. My primary complaint is not about the airbags but about the malfunction of the lane assist and radar systems and the manufacturer’s refusal to provide the requested reports that would verify the failure. When I requested the radar and lane assist reports from the manufacturer after their inspection, they admitted they would not provide them. Instead, they stated there was no defect. This is unacceptable because the data clearly exists, and withholding it prevents owners from knowing whether these safety systems can be trusted. The fact that these systems failed in a critical moment and the manufacturer is unwilling to disclose the information raises a serious safety concern not just for me, but for all owners of this model. I can provide supporting materials upon request, including dash camera video, the full Bosch crash data report, and correspondence with the manufacturer. My concern is both the malfunction itself and the lack of transparency. Without access to the radar and lane assist data, owners cannot verify that the advanced safety systems they rely on are functioning properly. This failure and refusal to provide proof of system performance must be addressed to protect other dri INFORMATION REDACTED PURSUANT TO THE FREEDOM O

The instrument cluster has failed per the recall 90VC. Audi issued the recall for every model made in 2021 with the exception of the A3 and A4 and I believe this was a mistake as my instrument cluster has the same failure.

I have a 2021 audi a4 that is also being affected by the instrument cluster failure/recall. My model , a4 was not included but I am experiencing the same failure and this was reported to the dealership. An online search will show just how many people have issues with the instrument cluster. The instrument cluster screen just went out while driving. I have to monitor my functions via the app on my phone. The dealer wants to charge me full price when every other model in 2021 has been recalled for the same issue. The A4 should be added to the instrument cluster recall for 2021 audi.

The MMI system, which is the display in the car, keeps rebooting every few minutes. It is extremely distracting while driving. Also during parking or back up the parking assistance and backup camera would suddenly disappear because the system reboots. The rear cross traffic warning stops working during reboot as well.

MMI (infotainment), and the dashboard screen will turn off randomly and reboot every 2 minutes. This is incredibly dangerous as it turns all sound off, and driving visuals like directions, and freezes or reboots, not where you last had it but from a different check point. It has happened while I have been backing up and backup camera has rebooted during backup. Parking Collision has also malfunctioned multiple times - but primary issue is the MMI rebooting even while driving on the freeway.

MMI Constant Rebooting - Follow up My 2020 Audi A4 Quattro's MMI system constantly reboots (~60 seconds), causing the screen to go black/blank. Component Failed: MIB3 infotainment software. This persistent issue leads to reboots and loss of digital display. System available for inspection. Safety Risk: Significant, recurring hazard. Critical safety features are inoperative for ~1 min during reboots, including: Rearview camera (backing/parking safety) Navigation Communication No manual bypass exists; driver loses vital visual information Problem Confirmation: Consistently reproducible. Dealer proposed costly hardware replacement with no guarantee, contradicting manufacturer guidelines for known software issues. Symptoms: continuous black screen, Audi splash, then re-initialization. Problem worsened over weeks. Manufacturer's Knowledge: Extensive owner research (AudiWorld, Reddit) confirms this as a widespread, documented MIB3 software defect for 2020 Audi A4/A5 models. Audi's own Technical Service Bulletins (TSBs) address this: TSB 2069112 (MIB3 improvements) TSB 2067021 (MIB3 malfunctions) These TSBs outline a specific, sequential software update process (e.g., older versions need TSB 2067021 first for TSB 2069112). Other owners successfully resolved rebooting with these updates. This malfunction is analogous to other Audi MIB3 software instabilities, as evidenced by NHTSA Recall Number 22V742 (Audi Campaign Code 91DZ). This recall for 2021-2022 Audi models addresses similar digital display/infotainment failures (e.g., rearview camera loss) via a no-cost software update. This demonstrates Audi's awareness of systemic MIB3 software issues impairing safety. I urge NHTSA to investigate this widespread 2020 Audi A4 issue and compel Audi to issue a recall for a no-cost software update (per TSBs) to all affected owners. More detailed documentation, including dealer records and owner videos, is available upon request as there is limited space to detail here.

I have started experiencing issues with the MMI (infotainment system) system rebooting every few minutes. It was very sporadic at first, but now it is at the point where I frequently don't have time to get into the system before it starts the reboot cycle again. When this happens, the screen will go blank for several seconds. Then a message will show for several seconds saying that the system is loading. The system will then try to guide me through several prompts to try to log back into the car's MMI system. Even if I choose "Guest User" I am shown multiple prompts before I get into the system and can use the functions located in the MMI. This can and does happen while driving, while parked, and while backing up. This reboot sequence makes all of the safety features, including collision avoidance and the backup camera inaccessible during this time. Sometimes, the system reboots before I have time to even back out of my driveway. Frequently, I don't have time to sign in or work through the prompts to get to the working system before it starts rebooting again. This issue started in February, 2025, but it was infrequent then. Over the past month, the issue has been persistent with the system rebooting every 5 minutes or less. This is very distracting while trying to drive and takes the driver's attention away from driving. As stated before, it also makes the safety features of the vehicle inaccessible - such as collision avoidance and backup camera.

Vehicles MMI constantly cycling through reboot - inordinate amount of previous complaints about this exact issue - Dealer advised a min $1040 ($370 to diagnose + $670 new MMI) to diagnose and change MMI console with NO guarantee it will work and that's because: the dealer has not updated MIB2 (aka the MMI) using the instructions on a different bulletin, titled "TSB 2067021 - 91 MIB3: Garage opener/SXM/Online Radio or other malfunctions B129B11, B129C22, B123B13, B123C13, U044700 or B200000" (latest version is from July 27, 2024, they've had 6 revisions), before applying the most recent update. So it looks like, at a minimum, I'm two updates behind (encompassing of course, a ton of intermediate versions). So right there, the dealerships have been failing by not even taking the time to read Audi's own TSBs. The dealership said: they have seen this before and they have seen many leave without resolve May I please be placed on the list of VINs that when all complaints are proven to be true and a recall is issued, I can partake in the MMI update? It truly is dangerous as the entire console shuts down, meaning cameras and system are not available for viewing protection (the minimum safety precautions expected with cameras, etc) Audi needs to recognize this is a widespread issue. It appears that they are picky with who they provide updates to and who they ignore as some owners received an update and while their MMI doesn't function 100%, it doesn't cycle through reboot every 60 seconds.
