Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. (Dorman) is recalling certain Accelerator Pedal Assemblies part numbers 699-114 and 825-5029-1, sold as replacement parts for 2003-2006 Acura MDX, 2004-2008 Acura TL and TSX, 2003-2007 Honda Accord, 2005-2006 Honda CR-V, 2007-2011 Honda Element, 2005-2008 Honda Pilot and 2006-2014 Honda Ridgeline vehicles. The rotating portion of the accelerator pedal assembly may bind. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 124, "Accelerator Control Systems."

Risk: If the accelerator pedal binds, the engine may not quickly return to idle after the pedal is no longer pressed, increasing the risk of a crash.

Structure

Issue: On certain vehicles, the rear frame could rust. In extreme cases, this could result in a rear trailing arm separating from the vehicle. This recall applies to vehicles originally sold or currently registered in areas of heavy road salt usage (Ontario, Quebec, New Brunswick, Nova Scotia, Prince Edward Island, and Newfoundland & Labrador). Safety Risk: A rear trailing arm that separates could cause a loss of control and increase the risk of a crash. Corrective Actions: Honda will notify owners by mail. Owners will be instructed to take their vehicle to a dealer for an inspection of the rear trailing arm front mount bolts. If the vehicle passes the inspection, the dealer will apply corrosion protection and install support braces. Honda will offer to repurchase vehicles that do not pass the inspection. In the event the repurchase is declined by an owner, a secondary inspection and body shop repair method may be possible.

Issue: On certain vehicles, there could be a problem with the driver-front airbag inflator. This could cause the airbag to deploy with more force than normal. If the airbag inflator ruptures, pieces could be propelled toward the occupants. This could also cause damage to the airbag assembly and prevent it from inflating properly. Note: This recall only affects certain vehicles that had a Takata airbag inflator installed during a previous airbag recall or collision repair. Safety Risk: In a crash where the airbag deploys, there could be an increased risk of injury. Corrective Action: The company will notify owners by mail. Owners will be instructed to take their vehicle to a dealer to have the driver-front airbag inflator replaced.

On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the passenger frontal airbag inflator. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. These issues could increase the risk of injury. Correction: Dealers will replace the passenger frontal airbag inflator. Note: Canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected.

On certain vehicles, the driver frontal airbag inflator could produce excessive internal pressure during airbag deployment. Increased pressure may cause the inflator to rupture, which could allow fragments to be propelled toward vehicle occupants, increasing the risk of injury. This could also damage the airbag module, which could prevent proper deployment. Failure of the airbag to fully deploy during a crash (where deployment is warranted) could increase the risk of personal injury to the seat occupant. Powertrain

On certain vehicles, the secondary shaft bearing in the automatic transmission may fail. This could cause abnormal noise, the malfunction indicator lamp to turn ON, and allow contact between the transmission idle gear and an electronic sensor housing within the transmission, resulting in a short circuit that causes the engine to stall. Engine stalling would result in lost vehicle propulsion which, in conjunction with traffic and road conditions, and the driver's reactions, could increase the risk of a crash. Additionally, broken pieces may become lodged in the parking pawl resulting in the vehicle rolling after the driver has placed the gear selector in the PARK position. These issues could result in property damage and/or personal injury. Correction: Dealers will update the automatic transmission control module software.

On certain vehicles, during the self-check process that occurs shortly after each time the engine is started, the self-diagnostic software for the Vehicle Stability Assist (VSA) brake modulator causes a condition that can allow air to enter the modulator pump. Over time, and after starting the engine numerous times, it is possible for enough air to enter the system to change the brake pedal feel and affect braking performance. Reduced braking performance could lead to a crash causing property damage, personal injury or death. Correction: Dealers will remove existing air in the brake system then apply sealant and caps to the VSA modulator to prevent air from entering the brake system.

I was driving on a 65 mph highway when another vehicle forced me off the road causing my vehicle to spin out and hit a guardrail pretty much head on at about 60-65mph. Should my airbags have gone off? My safety could have been at risk because of how my vehicle went from highway speed into a guardrail, my car bounced off of the guardrail and spun so I was facing traffic coming towards me. I have sent a message to Honda asking about the airbags but I have not received a response, no the vehicle has not been inspected by anyone. I had no airbag sensors light up on my dash. I have a sensor for the passenger airbag as well which said it was off because no one was in the passenger seat. My car is totaled and the frame is bent on the driver side. When my vehicle hit the guardrail my head kept banging on the door, headrest and the beam where the two doors join. I have pretty bad whiplash and have been having some nerve issues on my left side, I am not sure if that constitutes as an injury. I have dash camera footage of me hitting the guardrail as well, my vehicle lost power once I hit it though. With the dash camera footage I do have to get permission from my attorney to release it if you need just because we are pursing the other driver for damages and injuries. I still have the vehicle on my property if it does need to be inspected. My main concern is making sure other drivers will be safe.

I was driving on a 65 mph highway when another vehicle forced me off the road causing my vehicle to spin out and hit a guardrail pretty much head on at about 60-65mph. Should my airbags have gone off? My safety could have been at risk because of how my vehicle went from highway speed into a guardrail, my car bounced off of the guardrail and spun so I was facing traffic coming towards me. I have sent a message to Honda asking about the airbags but I have not received a response, no the vehicle has not been inspected by anyone. I had no airbag sensors light up on my dash. I have a sensor for the passenger airbag as well which said it was off because no one was in the passenger seat. My car is totaled and the frame is bent on the driver side. When my vehicle hit the guardrail my head kept banging on the door, headrest and the beam where the two doors join. I have pretty bad whiplash and have been having some nerve issues on my left side, I am not sure if that constitutes as an injury. I have dash camera footage of me hitting the guardrail as well, my vehicle lost power once I hit it though. With the dash camera footage I do have to get permission from my attorney to release it if you need just because we are pursing the other driver for damages and injuries. I still have the vehicle on my property if it does need to be inspected. My main concern is making sure other drivers will be safe.

I took my very well maintained Honda Element into the shop for routine maintenance and a pre-spring/summer driving check. They indicated that the driver's side subframe where it meets the rear trailing arm has a dangerous hole and that I should not drive this car any more. We live in Michigan where snow/salt are factors. This specific area has been known to fail in Elements and CR-V's and there was a recall on this specific issue for Honda Elements in Canada and CR-V owners in the US and Canada: "The 2007-2011 Honda CR-V is being recalled because the rear trailing arm can fall off due to excessive corrosion, the NHTSA disclosed Thursday. In some cases, the rear frame may be so corroded that Honda will offer to buy the vehicle back from owners to get the safety hazards off the road." Why have the US Elements not been recalled when this is a known safety hazard and the exact same issue as the CR-V's? I am in numerous online forums of Element owners and have seen photographs where this caused life-threatening issues while people were driving. Thankfully, we caught it before that happened to us, but now my loved daily driver is out of commission for a known safety hazard. No one in our family was harmed, but this could have been life threatening if it had not been discovered. The area has not been inspected by police or insurance. Only by our mechanic who specializes in servicing Hondas, Toyotas and Subarus. There were no warning lamps or other warning signs. The contact owns a 2008 Honda Element. The contact stated while driving 55 MPH, the contact lost control of the vehicle. The contact was able to regain control of the vehicle and pulled over to the shoulder of the roadway. Upon inspection, the contact discovered that the rear driver’s side tire was leaning outward from the vehicle. The contact called a friend with a trailer and had the vehicle towed to a friend's residence. Upon further inspection, it was discovered that the front mounting bracket on the rear trailer arm had become severely corroded which led to the failure. The manufacturer was notified of the failure and was informed that there were no recalls on the vehicle, but offered no further assistance. The vehicle was not repaired.
