On February 9, 2026, our 2023 Mazda CX‑9 experienced a sudden and complete system malfunction while being driven. Multiple system failures began approximately six minutes before the crash. During this time, the driver reported: • Total loss of steering • Complete brake failure • No response to acceleration or driver input • No ability to slow, stop, or maneuver the vehicle • No meaningful warnings that allowed corrective action The vehicle became completely uncontrollable. At the crash site, there were no skid marks, no braking evidence, and no signs of evasive action, consistent with a full loss of vehicle control. The vehicle only stopped after entering approximately two feet of water and striking a tree. The vehicle was declared a total loss. This incident represents a severe safety defect involving critical electronic and mechanical systems. A modern vehicle should not lose all steering, braking, and control functions under any circumstances. We are requesting a full investigation into the cause of this failure and whether similar incidents have occurred in other Mazda vehicles. We can provide photos, crash documentation, diagnostic information, and driver statements upon request.

I was pulling into a parking spot in Brooksville, Florida. It was the last parking spot on the block. As I pulled in, I went to hit my brake so that I could back up and the car did not stop. My foot was pressed all the way down to the floor and it kept going, and I hit the car parked in front of me. At that point, a warning came up on my odometer stating “Brake Malfunction “. . Thank goodness no one was hurt. I called the police. When the officer arrived, I explained what happened. He did see the odometer warning. He advised me to take a picture of it which I did. My car was towed to Crash Champions in Wesley Chapel. I explained to them what happened. They repaired my car and told me that They couldn’t find anything wrong with the brakes and everything was calibrated properly. I then asked Mazda to please check the brake system as I was not comfortable driving in this car anymore. I had just gotten off the Suncoast Parkway and thank goodness it didn’t happen on the parkway or I and my sister may have been killed. Mazda checked out the car and told me the same thing “ everything calibrates properly”. They were unable to help me exchange cars without a large sum of money so I had to take it back. I will not drive anywhere except locally and keep a good distance between me and the person in front of me. Again, last week, I was driving on a residential street in Lutz FL there was no cars in front of me and no cars behind me and my odometer lit up “BRAKE” which is what happens when I am too close to the car in front of me. But there was no car in front of me. I called Mazda to speak to the service manager and explained to the receptionist what had happened and he has not responded. It’s been almost a week. I am submitting this recall request because my vehicle has experienced repeated incidents of a serious safety issue. While driving—most often on the highway—a warning appears that says “Smart System Brake Malfunction.” Immediately after, the car begins braking on its own without driver input. This has happened multiple times and, based on online reports, it appears to be affecting many Mazda vehicles across different models and years. Each time, I have been forced to pull over, turn off the car, and restart it to temporarily resolve the issue. This is extremely dangerous, especially at highway speeds, as it creates a high risk of rear-end collisions and sudden loss of vehicle control. When reported to Mazda, the concern was dismissed as a “sensor issue,” but the repeated nature and widespread reports suggest a potential systemic defect that requires urgent investigation and recall.

My infotainment screen goes black 10-12 times a year, and therefore I lose access to my backup cameras. I have to turn the car off, and then back on, in order for the screen to function again. Vehicle randomly displays "smart brake system malfunction," "forward smart city brake malfunction" and " front radar system malfunction." This has been going on for at least a year. I will be driving on a highway, trying to accelerate and these messages will pop up. I'm not able to accelerate when these messages pop up and I have to turn on my hazard lights and pull over to the side of the road. The only thing that clears these warnings are for me to completely shut down my car and restart it. Luckily I have been able to get over to the side of the road on the highway without an accident, but these warning lights pop up when it's sunny out and there is no reason for the car to act the way it does. I took my car to the dealer the first time it happened and paid several hundred dollars for them to tell me that they don't know why it happened. There are several forums posted online with people having the same issues. Online posts say to make sure the sensors are clean and that the malfunction can activate if it's too hot, rainy, snowing or foggy? In my experience, it has happened only when it has been sunny. When the system decides to malfunction, the car will not accelerate, and it could make it easier for the car to be hit by another car. It's like the car just shuts down.

Smart City Break Malfunction warning light appears while driving. Simultaneously the tire pressure warning appears. Also, at the same time, the temperature reads -20 to -40 degrees. Yes, vehicle is available for inspection. Fearful that car could randomly break while driving putting myself and family in danger since there is a malfunction. Mazda service center says the problem can’t be reproduced so there’s nothing they can do. The problem seems to appear when the outside temperature is 60 degrees Fahrenheit or higher.

1. The navigation has a frosted crackle appearance framing the screen. It is difficult to read the navigation when needed. On [XXX], [XXX], [XXX] I was driving on an interstate highway with a 65 mph speed limit with no warning and received the following codes which slowed the vehicle to the limp mode. 1) Smart Brake Support and Forward Obstruction Warning 2) Activsense system malfunction 3) Front Radar Sensor Malfunction 4) Engine Malfunction 5) Dynamic Stability Control Malfunction I reported all three incidents to the dealer and no codes were left and the dealership had no solution for the problem. They suggested dirt, sunlight, or blowing debris caused it, but that was not the case. Luckily the first two incidents happened with little or no traffic around but the third was during rush hour and almost caused a major accident trying to get off the highway. I shut the car off, restarted and the problem resolved no lights left on or warnings appearing all three times. Contacted Mazda several times and all representatives at the dealership and California Headquarters claim no knowledge of this happening. I was backing out of a parking spot while watching the backup camera when I felt a bump as if I hit something but the camera was showing I have enough space to the cars behind me. It turns out the backup camera image was frozen on the display and I had hit a car parked behind my car. Please investigate and make the manufacturer solve problems with the hardware or the software. Make sure the display shows a warning when the camera fails and the vehicle drives backwards. There may be children behind the car. Shifting into park and then into reverse again caused the camera to work correctly.

I was in a parking lot and came to a stop and placed the car in reverse to back into a parking space. The back-up camera was on and I began to back into the parking space when the car suddenly lurched/jerked forward and accelerated on its own. The car jumped the parking lot median strip and I hit a tree head on. I had my seatbelt on and the airbag deployed. The car then reversed back off the median into the parking lot. I turned off the car. Insurance company will have it inspected. The vehicle is a total loss. I was injured with a large lower abdomen hematoma and airbag burns on arms, wrists, and chest as well as various bruising. I was fortunate to have not hit pedestrians who were walking in the lot or other vehicles that were parked in the lot. There were no warning lights, messages, sounds. The brake assist did not engage. It was frightening.

Dealer said these faults due to bad battery--replaced battery per Service Manager [XXX] at Lakeland Mazda. 9/10/2023--Incident #2 with same faults as above with vehicle stopping suddenly at risk of creating a rear end collision. Lakeland Service Manager [XXX] now said that the faults are due to a faulty ERG valve. Lakeland Mazda and [XXX] are absolutely clueless as to the root cause of this problem and are releasing the vehicle back to the customer with the potential of causing a significant collision that can result in serious injury and/or death.
