The front driver-side lower ball joint suffered a catastrophic structural failure and sheared completely apart while moving. This vehicle was previously serviced under Safety Recall Campaign 15V451000 / Mazda Recall 8515G for this exact defect. The recall remedy part failed a second time, collapsing the front wheel assembly, ripping the CV axle completely out of the transmission casing, causing a hazardous transmission fluid spill, and leaving the vehicle stranded and immobilized in the middle of a live roadway in the dark

Front suspension broke while my husband was driving back from a fire as he is a volunteer firefighter. He states car did not have any lights on dash to report there was a problem. The next thing he knew he heard a loud squealing noise and lost control of the vehicle taking him off road in a small ditch. As the axle broke it went upward and hit both belts and shredded them. He was not able to move or drive the car at all. I am the owner of this vehicle had I been driving I would have totaled and crashed,also I do have A [XXX] son I drive to school most days. Thank goodness my husband is a heck of a driver. I have contacted the Mazda manufacturer and they are stating that the vehicle was already fixed that they will not do a safety remedy. As I found out there was a 8515G recall for the front suspension interior corroding. I just purchased this vehicle on 2/24/2026 it broke on 3/19/2026. The vehicle states to have been fixed in 2017. I have the inspection papers from the previous dealer from whom I bought it from in 2021 and the one from the dealer that I purchased it and that inspection was in 2025. There has been no inspections after I purchased or after the catastrophic failure happened I am in process of this and my insurance is the next I'm contacting. There was no police report as there was only damage done to the vehicle,luckily no one else was involved and my husband was ok. I am reporting a serious safety issue involving a defective suspension part (lower control arm) manufactured by Dorman and purchased from an online auto parts retailer. On October 7, 2024, I ordered a Dorman lower control arm for my vehicle and had it professionally installed on November 7, 2024 at Johnston and Magwood Wheels & Tires. On November 8, 2024, at approximately 5:40 p.m., while I was driving and made a hard brake, I suddenly lost steering control and my front left wheel collapsed. Fortunately, I was the only one in the car and managed to avoid an accident. My vehicle had to be towed to a nearby shop (Kal Tire) and was later sent back to the original shop for further inspection. On November 12, technicians confirmed that the newly installed lower control arm had failed, which caused serious damage to the suspension. I paid $1,774.10 in repairs in order to make the car safe to drive again. This is extremely dangerous — if I had been driving on a highway or with passengers, it could have resulted in a major accident or injuries. When I contacted the retailer about the warranty, they only offered to refund the part itself and refused to take responsibility for the damages caused by this defective product. I believe this part should be investigated for safety defects. When I searched online, I noticed Dorman has had similar recalls on control arms, which makes me even more concerned. I am reporting this incident to NHTSA in hopes that you will look into this part to protect public safety.

Failed water pump, blown head gasket, major coolant leak, and warped head causing the vehicle to overheat and die when coming to a stop. Vehicle died when stopped in construction on a highway and we were nearly rear-ended. Vehicle never showed check engine light and temperature gauge never showed that vehicle was overheating but significant damage was caused to the engine from “excessive temperatures.” Failure of warning signals allowed us to continue driving the car with a significant engine issue. Now the entire engine needs replaced at only 83,000 miles. Car was diagnosed by licensed independent repair shop. Additionally, front struts went out at approximately 40,000 miles making the car wobble at higher speeds. Was inspected and replaced by dealership.

While I have not been involved in an accident yet, I have discovered an issue that is believed to be the same issue that other models are covered under a recall, yet for some reason my VIN is not included. The bulleting reference number is 02-001/20 that covers MAZDA 2016-2020 CX-9 models. I have a 2018 CX-9 touring that is exhibiting the exact action of the recall which is "creaking noise from front lower arm bushing/clunking noise from front shock absorber coil spring when driving over bumps". It is only getting worse as time goes on. I am confused as to why some vehicles would be included in this recall yet my year model is experiencing the same issues but is not covered under the recall. I don't know if people are not reporting it, which is why I am filling out this report today.

I started smelling coolant after exiting the vehicle from a drive. Coolant level was very low. Adding coolant did not last long. I could not see where the leak was. The dealership evaluated and determined I had a cracked cylinder head and the car is leaking coolant near the rear of the engine. This could cause the engine to seize should coolant get into the engine. The dealer (New Rochelle Mazda) explains the cylinder head needs replacing. The dealer also observed bad rear sway bar links. One was detached. The car is serviced at the same dealership every 5k miles. The car has approximately 85k miles. The cylinder head has been repaired by the dealer under the extended warranty. Safety risk was moderate as the car could have stalled on the highway. The component was inspected by the warranty company and dealer only. The only warning lamps that showed up once were both brake indicator lights came on and the climate control would not warm the car.

The dealer referred the contact to the manufacturer for assistance. An independent mechanic replaced the passenger's side lower control arm; however, the contact stated that after the repair was performed, the engine rattled and the vehicle stalled. The vehicle was then taken back to the independent mechanic, who determined that the driver's side lower control arm spacers were missing and that the front and rear axles, brakes, and rear shocks needed to be replaced. Additionally, the cage bolts needed to be retightened. The vehicle was not repaired. The manufacturer was made aware of the failure, and a case was opened. The failure mileage was approximately 110,000.

In 2017 had my front control arms replaced after the driver side CV arm came off of the axle due to a recall. 5 years later (about 30k miles) the same exact situation happened now with the passenger side making a left turn. Thankfully both times I didn’t cause an accident but came close due to the wheel coming off of the axle. Brought to dealership, they had to get approval from corporate for the repair. Got a call 6 days later saying corporate denied the goodwill repair due to being “normal wear and tear” and they told me to call corporate. Just called corporate and they say that the dealership makes that decision. I shouldn’t have to pay for something that was recalled, repaired, and happened again 100% in the same fashion. Normal wear and tear on a CV arm is not 20-30k miles.

I purchased this vehicle with 19K miles on July 31, 2020. I noticed on April 12, 2022, that there was a knocking noise coming from the left front suspension after leaving a Mazda dealership for an oil change. I called Mazda service and was told they only did an oil change. I have put 4 brand new tires and have replaced the front/back brakes, rotors and still hear noise going over uneven pavement. I brought it to my own mechanic twice and he hears the noise but informed me that he cannot order any suspension parts at all for Mazda CX9. He said Mazda are forcing Mazda customers to go to a Mazda dealership. He called 5 different places to purchase strut. suspension and was told they had none. He also said Mazda has a special wrench to work with having to do with suspension and he cannot get one. My mechanic had my vehicle for 3 days and said it's definitely a suspension issue. My Mazda only has 35K miles. In 2 years, I have only driven 16K (8K) per year. There is no way a suspension should go after only 35K miles. I am not off roading or abusing the vehicle. Can't NHSTA assist with this issue? This is very serious. I am afraid to drive this Mazda now.

The front end especially on the drivers side front suspension. The noise seems to be coming from the control arms and ball joints area. I have reported this to the dealership many times since purchasing the vehicle new. They continue to say that there is no issue but the noise is not going away. I believe it is associated with this TSB but no recall has been made. I am very concerned as the steering feels looser than usual and may cause an accident. TSB 10170655 - DESCRIPTION Some vehicles may exhibit one or more of the following noises from the front suspension: 1. A creaking noise from under the front of the vehicle (front lower arm bushing) when driving over bumps at approximate 12 - 25 mph (20 - 40 Km/h). The noise occurs when the road surface is wet or after the vehicle has been washed. The noise becomes more noticeable as the bumps get higher. The creaking noise is caused by the front lower arm bushing (A) making a stick-slip noise during wet road conditions. 2. A clunking noise from the front coil spring when driving over bumps. The clunking noise is due to stick- slip between the front strut bearing (B) and the coil spring (C). 3. A popping noise from the bump stopper (D) when driving through gutters.
