The vehicle's Connectivity Master Unit (CMU) / infotainment system has experienced a complete, permanent internal electronic failure, logged by Diagnostic Trouble Code DTC U3000:49 (Control Module Internal Electronic Failure). The main display screen is completely dark at all times and never powers on or responds. The unit is available for inspection upon request. This total hardware failure creates critical safety hazards: 1. Non-Compliance with FMVSS No. 111 (Rear Visibility): Because the rearview camera relies entirely on the main display screen, the backup camera is 100% inoperative. When shifting into Reverse, the screen remains black, offering zero rearward visibility and creating an immediate backover risk to pedestrians, children, and property. 2. Complete Loss of Driver Assistance & Emergency Systems: The permanently dark display eliminates access to active safety guidance, rear cross-traffic visual alerts, parking distance indicators, turn-by-turn navigation, and hands-free emergency communication interfaces. 3. Ongoing Driver Distraction: Operating the vehicle without essential safety/navigation displays requires additional driver focus, causing persistent cognitive distraction. The contact stated that while driving at various speeds, or while attempting to reverse, the rear camera flashed on and off for approximately five minutes and then started working as intended. No warning light was illuminated. The vehicle was repaired under the NHTSA Campaign Number: 23V487000 (BACK OVER PREVENTION) by the owner of the vehicle; however, the fix failed to repair the vehicle. The contact stated that the failure had occurred after the recall repair was performed. The manufacturer was not notified of the failure. The failure mileage was 180,000.

BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING

The rear backup sensors no longer work and did not give any indication they stopped working. They do not detect cars driving up from behind when driving or switching lanes or objects or people behind the car anymore when you reverse. This happened without any notice or warning indications.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

left-most lane of a three-lane highway, I pressed the accelerator to maintain the flow of traffic. The vehicle suddenly began revving between 4,000–5,000 RPM without increasing speed. When I attempted to accelerate again, the RPM remained around 3,000, and I immediately received multiple warning messages on the dashboard, including: “Skyactiv Partially Disabled” warning Engine Malfunction warning ATF (Automatic Transmission Fluid) warning light Simultaneously, the vehicle rapidly decelerated from 65 MPH to approximately 55 MPH, creating an extremely hazardous situation and nearly resulting in a collision. I had to carefully maneuver across lanes to reach the shoulder. After shutting the vehicle off and restarting it, the warning lights temporarily cleared; however, the vehicle appeared to be operating in limp mode. I had the car towed home via AAA. After this near-fatal incident, I scanned the vehicle using an OBD2 reader and retrieved diagnostic trouble codes P0780 and P0761, indicating Shift Malfunction and Shift Solenoid 3 Performance issues. To address the issue proactively, I had my mechanic perform two full transmission fluid and filter exchanges on Monday, February 16, 2026, and again on February 18, 2026. Both services included a pan drop, replacement of the transmission filter, use of Mazda OEM transmission fluid (0000A7115QT and FZ01-21-500), and a complete transmission relearn procedure after each service. Despite these efforts, the same codes (P0761 and P0780) reappeared when scanned with a professional-grade diagnostic tool. The battery has been tested and confirmed fully charged. Additionally, my mechanic identified oil leaking from the timing chain cover.

The contact owns a 2015 Mazda Mazda3. The contact received notification of NHTSA Campaign Number: 23V487000 (BACK OVER PREVENTION). However, the dealer refused to complete the recall repairs due to the vehicle having a Salvage Title. The contact stated that the rearview camera displayed a black screen while the vehicle was in reverse(R). The contact stated that the failure obstructed the driver's visibility, which prevented the driver from safely operating the vehicle. FORWARD COLLISION AVOIDANCE: WARNINGS

To whom it may concern, I was on a back road late in the evening not far from home [XXX] and a small deer ran into the road. I needed to brake to avoid hitting the deer. After breaking, a Brake System Malfunction warning message and i-ACTIVESENSE System Malfunction warning illuminated on my dashboard. I brought the car to a Mazda dealership the next morning. The technician determined that the brake fluid level sensor became stuck, requiring replacement of the brake fluid reservoir and the brake fluid level sensor. What failed: The repairs include replacement of the Brake Fluid Reservoir and the Brake Fluid Level Sensor. This is the link to the dealer report [XXX] . No one other than the dealer has inspected these parts to my knowledge. There was no accident of any kind. I personally felt unsafe after these parts failing and reached out to Mazda Customer Care to request investigation of the issue. The case manager just called me back today to say they could not help me. Mazda has denied warranty coverage, claiming the parts are not covered. However, I really do feel this is a critical safety system component that failed after normal emergency use with no collision or impact. Why I feel this is a safety concern: - The brake warning system became unreliable after a single emergency stop. - My car only has 42k miles and is in excellent condition and is fully dealer serviced. - A driver could lose the ability to receive proper brake system warnings if this failure occurs again. - A critical brake sensor should not fail under normal driving conditions, and its failure could lead to delayed driver response in an actual brake fluid loss scenario. It makes me worry for the future. For your consideration: I believe this represents a safety-related defect that should be investigated, as it affects the reliability of the brake system warning indicators in the 2019 Mazda 3. Car gives me emergency brake warnings about imminent collision maybe once or twice a week with nothing in front of me what so ever and once even straight up just brought me to a complete halt in the middle of the road with absolutely nothing in the road. The dealership basically told me it was a known issue and that “some models are more sensitive than others” and that they will be doing nothing about it. Different models of Mazda 3s from the same year are currently being investigated for the same recalls. This definitely needs to be addressed before I get into an accident.

2024 Mazda MAZDA3 Chico, CA Crash 1 injured

LANE DEPARTURE: BLIND SPOT DETECTION

On July 1, 2025, I was involved in a severe rear-end collision while driving my 2024 Mazda3 manual hatchback. The impact was forceful enough to shatter the rear windshield, leave a deep dent in the tailgate, and bend the trailer hitch mount under the vehicle by several inches. Despite this, none of the airbags deployed, and there were no airbag warning lights before or after the crash. After the incident, I checked my vehicle’s VIN on NHTSA’s recall lookup tool and discovered that Recall 7525E had been issued on June 6, 2025, for a critical airbag sensing system defect (SAS module), which can disable airbag deployment after battery depletion or improper power conditions. According to Mazda and NHTSA documents, this defect can completely disable airbag function, increasing the risk of injury in a crash. As of July 11, 2025, I have not received any recall notice from Mazda by mail, email, phone, or dealer contact—despite being the registered owner. Mazda’s documentation shows that: Dealers were notified as early as June 3, 2025 Owner letters are not scheduled until July 28, 2025 This means that I was driving a defective vehicle without knowledge of a serious safety issue at the time of the crash. I believe that: Mazda failed in its legal obligation to notify me in a timely and adequate manner. The non-deployment of airbags in my crash is consistent with the failure described in Recall 7525E. Other affected Mazda owners may remain unaware and at significant risk. I respectfully request that NHTSA: Investigate whether Recall 7525E was the cause of airbag failure in this incident. Determine whether Mazda’s recall notification practices met regulatory requirements. Ensure immediate outreach to affected owners to prevent further injury or risk. 2017 Mazda MAZDA3 Green Lake, WI Crash 1 injured

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

I made a right turn onto a gravel driveway that has a slight incline. My speed was less than 10 mph. At the top of the incline I attempted to make a U turn so I could park the car. Midway through the U turn the car's acceleration increased rapidly to high speed without my stepping on the accelerator. I pressed the brake 3 times. The brake pedal went to the floor without seemingly any effect. In seconds the car traveled across the front lawn, picked up concrete from and old cistern top and smashed into the front porch tearing it off it's foundation. Portions of the porch and tree limb fell shortly after the impact. The front tire was spinning at a rapid rate when I exited the vehicle. Extraction of the vehicle was not accomplished for 2 weeks while experts determined how best to accomplish the extraction. The car insurance company and 2 towing businesses declared the vehicle a total loss. The vehicle is with the towing company per car insurance company request. I don't know if it has been inspected. I did not see any warning lights. I was treated at the hospital for a slight concussion and significant head and facial bruising and lacerations as well as a wrist burn likely caused by air bag deployment.

Hello frustrated people! Second time back to the dealership...backup camera they say got wet from moisture, (after 2 drained batteries in less than 2 years), my cost. I think that this is a very dangerous vehicle that I am driving now!!!! Besides the fact, at my age, being stuck in a parking lot in 20 degree weather for 3 hours waiting for roadside assistance. The camera goes haywire...I cannot connect to the phone or navigation system. I can't leave my house for fear that my car is not going to start! Now I have to pay for a backup camera??? WE think that is the problem, they told me! So far the quote is $600.00...... A DANGER to drive! Can you imagine , pulling off the side of the road, when your navigation goes out! And now warmer weather, it doesnt do that. So how are they going to know now????

I have had to keep this feature turned off to prevent it wrecking the car, because I could be on a straight road, and it will detect a curve that isn’t there and begin turning the wheel, which could result in a fatal accident if not corrected and disabled by the driver. It is available for inspection upon request. As the dealer has not driven the vehicle, they have not recreated the error, however if anyone were to drive the vehicle, they would be able to observe the malfunction. The dealer has been made aware on many occasions, though they never documented it and only suggested that I turn the feature off, which automatically turns itself on upon ignition. There are no warnings that there is a malfunction; you only know there is an error once an alert pops up thinking you’re missing a curve and attempts to turn your steering wheel though there is no curve. This issue has persisted over the life of the vehicle.

The active braking support feature of this vehicle falsely warns me of an imminent collision. It happens pretty often, the system is obviously not quite smart enough. Last week, while accelerating away from a stop light in moderately heavy traffic, the active braking system APPLIED THE BRAKES in a panic manner, causing the vehicle to almost come to a complete stop, and I missed being rear-ended by the car behind me by inches. There appears to be no method to permanently deactivate this "safety" feature. I can turn it off through a series of menus in the infotainment system, but each time I start the car the system turns itself back on again.
