Owners have filed 625 complaints
about the 2004 Mazda RX-8 with NHTSA between 2003 and 2026.
The largest single group is the transmission & drivetrain.
Across this model's run it ranks 1 of 5 worst.
The letter says how bad. This says what kind — whether this year drew
an unusual number of complaints, unusually dangerous
ones, or failures that arrived unusually early.
The dashed ring is 50 — the national norm. Further out is worse.
Complaint volume — 100
Against the median year of the Mazda RX-8. 50 means typical for this
model; 100 means roughly double.
Severity — 48
9.3% of these complaints
involved a crash, fire, injury or death, against a national rate of
9.7%.
Early failure — 34
20% of failures with a mileage on record happened under
36,000 miles, against a national rate of
29%.
NHTSA files every complaint against a named sub-assembly, not just a
system. This is the difference between knowing it was "the engine" and
knowing which part of it — the list to hand a mechanic.

When owners complained
A spike years after the car went on sale usually means a part wearing
out on schedule across the whole fleet — the kind of pattern no single

NHTSA investigations into the 2004 Mazda RX-8
An investigation is the step before a recall: the agency has seen enough
complaints to look into a pattern itself. An open one means it thinks
something may be wrong and nothing has been fixed yet.
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. Recalls on the 2004 Mazda RX-8
A complaint is one owner's allegation. A recall is the point at which
the manufacturer or a regulator agreed the defect is real and dangerous
enough to fix for free — however old the car is.

AIR BAGS:FRONTAL:PASSENGER SIDE:INFLATOR MODULE
Mazda North American Operations (Mazda) is recalling certain 2003-2008 Mazda6, 2004-2005 MPV, 2004 RX-8, and 2006-2007 Mazdaspeed6 vehicles. These vehicles had their passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.
Risk: An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.

FUEL SYSTEM, GASOLINE:DELIVERY:FUEL PUMP
Mazda North America Operations (Mazda) is recalling certain model year 2004-2008 RX-8 vehicles. Heat from the engine and the exhaust may cause the main fuel pump pipe to crack.
Risk: If the fuel pump pipe cracks, fuel may leak, increasing the risk of a fire.
17V355000
SUSPENSION:FRONT:CONTROL ARM:LOWER ARM
Mazda North American Operations (Mazda) is recalling certain 2004 Mazda RX-8 vehicles. The ball joint socket on the front suspension lower control arm may crack and the ball joint may separate from the socket.
Risk: If the ball joint separates, steering control may be lost, increasing the risk of a crash.
16V793000
FUEL SYSTEM, GASOLINE:DELIVERY:FUEL PUMP
Mazda North America Operations (Mazda) is recalling certain model year 2004-2008 RX-8 vehicles manufactured April 10, 2003, to February 18, 2008. The affected vehicles have fuel pump sealing rings that may deteriorate due to exposure to heat from the engine or exhaust pipe.
Risk: The deteriorated seals may leak fuel, which, in the presence of an ignition source, can increase the risk of a fire.
05V325000
SUSPENSION:FRONT:CONTROL ARM:LOWER BALL JOINT
ON CERTAIN PASSENGER VEHICLES, DUE TO IMPROPER FORGING OF THE LOWER CONTROL ARM, CRACKS MAY OCCUR IN THE BALL JOINT SOCKET.
Risk: IN SEVERE DRIVING CONDITIONS, THE BALL MAY SEPARATE FROM THE BALL JOINT SOCKET AND A LOSS OF STEERING MAY OCCUR, WHICH COULD RESULT IN A CRASH.
05V317000
ENGINE AND ENGINE COOLING
IF CERTAIN PASSENGER VEHICLES ARE PARKED AND THE ENGINE IS OPERATED AT HIGH RPM'S FOR AN EXCESSIVE LENGTH OF TIME, SOME OF THE PARTS AROUND THE EXHAUST SYSTEM CAN MELT AND PRODUCE A VARIETY OF MALFUNCTIONS. PROBLEMS CAUSED BY THE EXCESSIVE HEAT BUILD-UP CAN RANGE FROM INOPERATIVE OXYGEN SENSOR, NEUTRAL SWITCH AND BACK UP LIGHTS, PROBLEMS WITH THE PARKING BRAKES, MALFUNCTIONS OF THE GAS GAUGE AND/OR POSSIBLE FUEL LEAKS RESULTING FROM HEAT DAMAGE TO THE FUEL TANK.
Risk: FUEL LEAKAGE, IN THE PRESENCE OF AN IGNITION SOURCE, COULD RESULT IN A FIRE.

Airbag
Issue: On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, may eventually degrade the propellant contained in the passenger-front airbag. This could cause the airbag to deploy with more force than normal. If the airbag inflator ruptures, fragments could be propelled toward vehicle occupants or cause damage to the airbag assembly, preventing its proper function. Safety Risk: If the vehicle is involved in a crash requiring an airbag deployment, the airbag inflator could rupture and create a risk of injury. Corrective Action: Owners will be notified by mail and instructed to take their vehicle to a dealer to have the passenger-front airbag inflator or airbag module replaced. Note: The Canadian climate results in the propellant degrading slowly. This recall is being conducted as a precaution to address future risk. It is expected that all airbag inflators will be replaced before their function would be affected. Additional note: This recall supersedes recalls 2014-245 (Mazda Recall 7714F), 2018-038 (Mazda Recall 2018A) and 2018-584 (Mazda Recall 2018A). Vehicles repaired under the previous recalls do not require repair under this recall.

On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the passenger frontal airbag. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. These issues could increase the risk of injury. Correction: Dealers will replace the passenger frontal airbag inflator. Note: Canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected. Additional Note: This recall supersedes recalls 2016-259 (Mazda Recall 9416E) and 2017-011 (Mazda Recall 0617A) for RX-8 models only. RX-8 model vehicles completed under those two recalls do not need to be completed under recall 2018-038 (Mazda Recall 2018A).

Fuel System
On certain vehicles, the fuel pump discharge pipe may crack and cause a fuel leak. A fuel leak in the presence of an ignition source could result in a fire, increasing the risk of injury and/or damage to property. Correction: Dealers will replace the fuel filter body with an improved part.
Recall 20172883,004 units in Canada
Suspension
On certain vehicles, the ball joint socket area on the front lower control arm may become cracked due to an inappropriate manufacturing process of the ball joint. If a large load is applied to the suspension while driving, the ball joint may suddenly separate from the socket, which could result in a loss of steering control, increasing the risk of a crash causing injury and/or damage to property. Correction: Dealers will replace the lower control arms with modified parts.
Recall 20165686,788 units in Canada
Fuel System
On certain vehicles, the fuel pump sealing rings may deteriorate from being exposed to heat from the engine and exhaust pipe. Over time, this could cause the fuel pump rings to crack, which could result in a fuel leak. A fuel leak in the presence of an ignition source could result in a fire causing injury and/or property damage. Correction: Dealers will replace the fuel pump rings with new parts and add additional insulation to the fuel tank.

On certain vehicles, the driver frontal airbag inflator could produce excessive internal pressure during airbag deployment. Increased pressure may cause the inflator to rupture, which could allow fragments to be propelled toward vehicle occupants, increasing the risk of injury. This could also damage the airbag module, which could prevent proper deployment. Failure of the airbag to fully deploy during a crash (where deployment is warranted) could increase the risk of personal injury to the seat occupant. Correction: Dealers will replace the airbag module. Note: This recall is superseded by recall 2018-036. THE CLUTCH WAS REPLACED BY MAZDA 30,000KMS AGO AND AT A MAZDA DEALERSHIP USING OEM CLUTCH KIT. THE FORCE OF THE GEARBOX EXPLOSION WAS SO GREAT THAT THE CLUTCH PEDAL IMPACTED MY LEFT FOOT AND SHATTERED THE BONES OF THE LEFT FOOT IN PIECES (YES I WAS WEARING THICK SOLED SNEAKERS) AND CAUSED SEVERE TRAUMA TO MY FOOT IN GENERAL. AT THE HOSPITAL, THE SURGEONS WORDS WERE " IT LOOKS REALLY BAD". I WAS OPERATED ON AND TITANIUM PLATES AND 4 SCREWS WERE INSERTED TO TRY TO HOLD MY SHATTERED BONES IN PLACE. 8 MONTHS AND A LOT OF HOSPITAL VISITS LATER, I AM STILL RECOVERING FROM THE DAY. I HAD FURTHER SURGERY FEW WEEKS AGO TO PERFORM A BONE GRAFT BECAUSE ONE BREAK WASN'T HEALING. I HAVE APPROACHED MAZDA REGIONAL HEAD OFFICE. INITIALLY MAZDA CONTACTED ME VIA PHONE ASKING FOR DETAILS OF THE CRASH. I SENT THEM PHOTOS VIA EMAIL. I HAVE NOT HEARD FROM THEM SINCE. I THEN ALSO CONTACTED THE DEALERSHIP. THEY PROMISED TO RING BACK AND THEY TOO, HAVE STOPPED. IT'S BEEN DIFFICULT TO FIND A LAWYER WHO CAN SEE THE OBVIOUS PROBLEM, THAT A VEHICLE SHOULD NOT BLOW UP, OR CATCH FIRE ETC NO MATTER WHAT AGE IT IS. HELP!

No worries — here it is trimmed to under 2,000 characters: I am reporting a recurring safety defect on my 2004 Mazda RX-8 (VIN: [XXX] a repair under NHTSA Recall 1017E (Fuel Pump Ring / Discharge Pipe). Recall 1017E was completed on my vehicle on September 19, 2017. The same fuel leak has since recurred, with fuel vapor now entering the passenger cabin. Only a few thousand miles have been driven since the recall repair. The vehicle is currently at Della Mazda, Queensbury, NY at approximately 72,000 miles. On May 8, 2026, I contacted Mazda Customer Experience Center. Mazda confirmed the dealer submitted a pre-authorization request for recall coverage, which Mazda corporate denied, stating the recall was previously completed and further repairs are my responsibility. I have a chat transcript documenting this. A recall remedy that fails to correct the covered defect should not be considered complete. I am requesting NHTSA review whether Recall 1017E has produced recurring failures, direct Mazda to honor the recall remedy, and advise on available consumer reimbursement. ENGINE
Engine naturally wears out especially after 200k miles due to friction in the natural design of the Rotary engine, resulting in losing its overall function such as turning off in the middle of a drive due to loss of compression from worn Apex Seals of the Rotary. The rotary engine requires a replacement of the Apex Seals to continue functioning normally to avoid any unpredictable accidents due to engine failure. An event occurred where my RX-8 went into limp mode in the middle of the freeway as i was commuting due a misfire from poor compression, this is more then likely a common event nowadays for RX-8 owners starting at the S1 2004 models with newer models soon to follow the same incident as me. This is completely unsafe especially traveling at 65 MPH

UNKNOWN OR OTHER
PASSENGER DASH COVER HAS LOST INTEGRITY/CRACKED ALONG THE LINES WHERE THE AIRBAG DEPLOYS AND AM CONCERNED THAT THE FORCE OF POTENTIAL AIRBAG DEPLOYMENT COULD BE AFFECTED AND NOT WORK AS DESIGNED OR DEPLOY WITH TOO MUCH FORCE TO CAUSE INJURY OR DEATH TO THE OCCUPANT IF THE AIRBAG COVER SHOULD FLY OFF UPON DEPLOYMENT. THIS IS AN ISSUE WITH THE DASH OF MANY MAZDA VEHICLES AND SHOULD AT A MINIMUM BE REVIEWED. THE CAR HAS NOT BEEN IN ANY ACCIDENTS AND THE AIRBAGS RECALL HAS RECENTLY BEEN PERFORMED BY A MAZDA DEALER.

STEERING
WHILE DRIVING MY 2004, MAZDA RX8 THE ELECTRIC POWER STEERING MODULE FAILED, AND JERKED THE CAR AGGRESSIVELY TO THE LEFT INTO THE ONCOMING LANE OF TRAFFIC. THE FORCE WAS SO STRONG THAT IT CAUSED MOMENTARY LOSS OF CONTROL. FORTUNATELY, THERE WASN'T ANY ONCOMING TRAFFIC AT THAT MOMENT ON THAT PARTICULAR STRETCH OF ROAD. WHEN REVIEWING THE MAZDA FORUM THERE WERE SEVERAL OTHER OWNERS THAT EXPERIENCED THE SAME LOSS OF CONTROL OR FAILURE. ADDITIONALLY, MAZDA HAD POWER STEERING ASSIST RECALLS FOR THE SAME PROBLEMS, REF. MAZDA3/MAZDA5 POWER STEERING RECALL NOTIFICATION 6010H.

STEERING
MY POWER STEERING INTERMITTENTLY GOES OUT FROM TIME TO TIME. I HAVE TAKEN IT TO THE SHOP 4 TIMES. THERE IS SOMETHING SERIOUSLY WRONG WITH THIS DESIGN AND THE VEHICLE IS VERY DANGEROUS.
