NHTSA investigations into the 2007 Mazda RX-8

An investigation is the step before a recall: the agency has seen enough complaints to look into a pattern itself. An open one means it thinks something may be wrong and nothing has been fixed yet.

Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. Recalls on the 2007 Mazda RX-8

A complaint is one owner's allegation. A recall is the point at which the manufacturer or a regulator agreed the defect is real and dangerous enough to fix for free — however old the car is.

Canada — Transport Canada

Canada runs its own recall programme, and its campaigns do not always mirror the American ones.

Recall 2019496 4,460 units in Canada

Airbag

Issue: On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, may eventually degrade the propellant contained in the passenger-front airbag. This could cause the airbag to deploy with more force than normal. If the airbag inflator ruptures, fragments could be propelled toward vehicle occupants or cause damage to the airbag assembly, preventing its proper function. Safety Risk: If the vehicle is involved in a crash requiring an airbag deployment, the airbag inflator could rupture and create a risk of injury. Corrective Action: Owners will be notified by mail and instructed to take their vehicle to a dealer to have the passenger-front airbag inflator or airbag module replaced. Note: The Canadian climate results in the propellant degrading slowly. This recall is being conducted as a precaution to address future risk. It is expected that all airbag inflators will be replaced before their function would be affected. Additional note: This recall supersedes recalls 2014-245 (Mazda Recall 7714F), 2018-038 (Mazda Recall 2018A) and 2018-584 (Mazda Recall 2018A). Vehicles repaired under the previous recalls do not require repair under this recall.

Recall 2018038 5,251 units in Canada

Airbag

Note: This recall is superseded by recall 2019496. Please see <a href="http://wwwapps.tc.gc.ca/Saf-Sec-Sur/7/VRDB-BDRV/search-recherche/detail.aspx?lang=eng&&rn=2019496">recall 2019496</a> for more information. On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the passenger frontal airbag. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. These issues could increase the risk of injury. Correction: Dealers will replace the passenger frontal airbag inflator. Note: Canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected. Additional Note: This recall supersedes recalls 2016-259 (Mazda Recall 9416E) and 2017-011 (Mazda Recall 0617A) for RX-8 models only. RX-8 model vehicles completed under those two recalls do not need to be completed under recall 2018-038 (Mazda Recall 2018A).

Recall 2018036 38,909 units in Canada

Airbag

On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the driver frontal airbag. Correction: Dealers will replace the driver frontal airbag inflator. Note: Canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected. Additional Note: This recall supersedes recall 2015-247 (Mazda Recall 8215F). Vehicles completed under 2015-247 do not need to be completed under recall 2018-036.

Recall 2017289 6,788 units in Canada

Fuel System

On certain vehicles, the fuel pump discharge pipe may crack and cause a fuel leak. A fuel leak in the presence of an ignition source could result in a fire, increasing the risk of injury and/or damage to property. Correction: Dealers will replace the fuel filter body with an improved part.

Recall 2017011 8,853 units in Canada

Airbag

Note: The recall for the 2005-2008 RX-8 models has been superseded by recall <a href="http://wwwapps.tc.gc.ca/Saf-Sec-Sur/7/VRDB-BDRV/search-recherche/detail.aspx?lang=eng&&rn=2018038">2018038 (Mazda Recall 2018A)</a>, and the recall for the 2005-2006 MPV models has been superseded by recall <a href="http://wwwapps.tc.gc.ca/Saf-Sec-Sur/7/VRDB-BDRV/search-recherche/detail.aspx?lang=eng&&rn=2018578">2018578 (Mazda Recall 2618F)</a>. On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the passenger frontal airbag inflator. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected.

Recall 2016568 6,788 units in Canada

Fuel System

On certain vehicles, the fuel pump sealing rings may deteriorate from being exposed to heat from the engine and exhaust pipe. Over time, this could cause the fuel pump rings to crack, which could result in a fuel leak. A fuel leak in the presence of an ignition source could result in a fire causing injury and/or property damage. Correction: Dealers will replace the fuel pump rings with new parts and add additional insulation to the fuel tank.

Recall 2015247 57,861 units in Canada

Airbag

On certain vehicles, the driver frontal airbag inflator could produce excessive internal pressure during airbag deployment. Increased pressure may cause the inflator to rupture, which could allow fragments to be propelled toward vehicle occupants, increasing the risk of injury. This could also damage the airbag module, which could prevent proper deployment. Failure of the airbag to fully deploy during a crash (where deployment is warranted) could increase the risk of personal injury to the seat occupant. Correction: Dealers will replace the airbag module. Note: This recall is superseded by recall 2018-036. Please see recall 2018-036 for more information. <a href="http://wwwapps.tc.gc.ca/Saf-Sec-Sur/7/VRDB-BDRV/search-recherche/detail.aspx?lang=eng&&rn=2018036">Click here for more information</a>.

Mazda Canada is conducting a voluntary Safety Improvement Campaign concerning the driver's airbag inflator on certain vehicles equipped with Takata airbags. Mazda will replace the driver's inflator on affected vehicles. This action is not being conducted under the requirements of the Motor Vehicle Safety Act. Note: This Safety Improvement Campaign is superseded by recall 2015-247 (Mazda Recall 8215F). STEERING

WE HAVE OWNED THIS 2007 MAZDA RX-8 FOR 3 YEARS. WE BOUGHT IT FOR OUR SON TO DRIVE, AND HE DRIVES IT DAILY TO WORK AND TO SCHOOL. THE CLUTCH PEDAL BRACKET HAS BROKEN 3 TIMES, AND EACH TIME CAUSED CONSIDERABLE DIFFICULTY IN SAFELY DRIVING THE CAR HOME, OR TO THE DEALER FOR REPAIR. I CAME VERY CLOSE TO BEING IN AN ACCIDENT DRIVING THE CAR TO THE DEALER FOR REPAIR THE SECOND TIME THE FAILURE OCCURRED. EACH TIME THE BRACKET HAS BROKEN, THE DEALER (HILEY MAZDA, HUNTSVILLE, AL) REPLACED IT FOR NO COST, AS MAZDA EXTENDED THE WARRANTY ON THE PART TO 8 YEARS / 100,000 MILES. BUT, EACH TIME THEY REPLACED THE BRACKET WITH EXACT SAME PART. SO, THE SAME INADEQUATE DESIGN FAILS AGAIN AND AGAIN. I BELIEVE MAZDA SHOULD HAVE REDESIGNED THIS PART AFTER IT FIRST BECAME APPARENT IT WAS PROBLEM, NOT JUST REPLACE IT WITH THE SAME INFERIOR (AND UNSAFE) PART. THIS DEFECT WAS INVESTIGATED BY SAFECAR.GOV IN 2009-2010 (REFERENCE NHTSA ACTION NUMBER PE09045). I BELIEVE THIS IS WHAT PROMPTED MAZDA TO EXTEND THE WARRANTY. THE CLUTCH PEDAL BRACKET BROKE, AND WAS REPLACED THE FIRST TIME ON 10/15/13, WITH 78,586 MILES ON THE CAR. THE SECOND FAILURE AND REPLACEMENT OCCURRED ON 4/23/14 WITH 83,115 MILES (ONLY 6 MONTHS LATER !). TODAY (7/13/15), THE BRACKET BROKE AGAIN WITH 93,560 MILES ON THE CAR. IT IS CURRENTLY AT THE HILEY MAZDA, HUNTSVILLE, AL DEALERSHIP FOR REPAIR. MY WIFE AND I ARE WRITING TO ENLIST THE HELP OF THE NHTSA TO RESOLVE THIS ISSUE. POWER TRAIN

I WAS DRIVING ON I4, AND WENT TO SHIFT FROM 4TH GEAR TO 5TH GEAR; I HEARD A SNAP AND THE CLUTCH PEDAL WENT TO THE FLOOR. I THEN HAD ABOUT 2 INCHES OF TRAVEL IN MY CLUTCH PEDAL. THE CLUTCH PEDAL BREAKING IS A KNOWN ISSUE WITH MAZDA RX8S. NO, I DO NOT HAVE A HEAVY DUTY CLUTCH.

AIR BAGS

THE PASSENGER SIDE DASHBOARD AREA IS ALL CRACKED UP AD SPLIT OPEN. THERE IS NO REASON FOR IT TO BE CRACKED OR SPLIT. THE CRACKING FOLLOWING THE SHAPE OF THE AIRBAG THAT IS UNDERNEATH THE DASHBOARD. IT HAS VERY JAGGED EDGES AND EXPOSES THE AIRBAG TO EXCESS LIGHT AND MOISTURE BY BEING SPLIT OPEN IN SO MANY PLACES. THE DASHBOARD STRUCTURE IS NOW WEAKENED DUE TO ALL OF THE CRACKS. AN EXPLOSION FROM THE AIRLOCK WOULD BE MORE POWERFUL THAN NECESSARY BECAUSE IT NO LONGER NEEDS TO BREAK THROUGH THE PLASTIC. THIS APPEARS TO BE A SAFETY CONCERN. DEALERS ONLY STATE IT AS A COSMETIC CONCERN AND DO NOTHING TO CORRECT IT.
