An incorrect font size is displayed on the instrument panel for the Brake, Park, and Antilock Brake System (ABS) warning lights. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 105, "Hydraulic and Electric Brake Systems" and 135, "Light Vehicle Brake Systems."

Risk: Warning lights with a smaller font size can make critical safety information on the instrument panel difficult to read, increasing the risk of a crash.

Risk: An air bag that is not designed to function with the replacement steering yoke or wheel may deploy incorrectly, increasing the risk of injury during a crash.

Issue: On certain vehicles, a software problem could cause the computer circuit board to short circuit. If this happens, the rearview camera image may not display. Canadian regulations require the rearview image to display when the transmission is shifted into reverse gear. Safety Risk: A rearview camera image that doesn't display could reduce a driver's ability to see behind the vehicle while backing up. This could increase the risk of a crash. Corrective Actions: Tesla has released an over-the-air software update to fix this problem. Tesla will notify owners by email. Tesla will also identify vehicles in which the computer circuit board experienced a short circuit, or damage that could lead to a short circuit, prior to the release of software versions 2024.44.25.3, 2024.45.25.6 or a later release. If your vehicle is identified, you will be advised to contact Tesla Service to replace the car computer.

As a result, you may not be aware that the hood isn't latched properly. If this happens, the hood could open suddenly while driving. Safety Risk: A hood that opens suddenly while driving can block the driver's view and increase the risk of a crash. Corrective Actions: Tesla will notify affected owners by email. Issue: On certain vehicles, the seat belt reminder chime and visual warning may not work when you start the vehicle and your seat belt is not buckled. Canadian regulations require both a seat belt reminder sound and visual telltale when a driver is not wearing their seat belt. Safety Risk: A seat belt warning that doesn't work correctly may not remind you to buckle your seat belt. Not wearing your seat belt can increase the risk of injury in a crash. Corrective Actions: Tesla will notify owners by email and supply an over-the-air software update to activate the seat belt reminder chime and visual warning.

Issue: A previous over-the-air (OTA) software update could cause the vehicle not to display the correct visual warning indicators when there are problems with the brake system or when the parking brake is applied. As a result, the BRAKE, PARK, and antilock brake system (ABS) visual indicators would display as text instead of symbols. Safety Risk: While there is a low risk to safety, Canadian regulations require visual warnings to use common symbols, instead of words, to avoid driver confusion. Corrective Actions: Tesla will notify affected owners by email. Tesla has released an over-the-air software update to fix this problem. i was in full self driving mode. the car wanted to turn left but i thought it was going in the wrong direction so i tried to continue straight by turning the wheel to the right forcefully but the car would not let me take over and i crashed into a guard rail. when i tried to brake it was not able to stop soon enough, i have a dash cam footage. i have taken over the this way numerous times and it usually does not require much force to do so, this time it simply would not allow me to control the wheel. no one was hurt other that a guard rail. the car was totaled by insurance company. the police assisted me but i never received a police report. no pictures to share, just a dashcam video but i dont know if that would be helpful since there isn't a way to upload it that i can see. VEHICLE SPEED CONTROL

On July 29, 2024, when my husband backed the vehicle out of the garage slowly, the vehicle suddenly accelerated and it hit the stone column causing a big damage to the car. I believe the pedal speed control and emergency brake have problems. When I reported to Tesla, they said the vehicle was not at fault. They said the issue had been resolved remotely by the engineer and cancelled my service appointment. When I asked for the engineer report, Tesla rep told me to download the data report. After I downloaded the data report, it shows no data 2 hours before and after the time of the incident. When I tried to retrieve the video on the replay function in the vehicle, the video was not there anymore. Fortunately, my husband recorded the video on his phone and I have a copy of the video. I feel very disappointed and frustrated with how Tesla handled the issue. If they are not at fault, how come the data report and video are not available? I filed a claim under my auto insurance policy and got the vehicle fixed. This sudden unintended acceleration happened to me back in June when the vehicle accelerated by itself to 41mph. My daughter, who was sitting next to me in the front passenger seat, saw my foot was in the air but the vehicle kept moving at 41mph. We were so shocked and I had to use both feet to step on the brake to stop the vehicle. Tesla eventually replaced the steering column control in the beginning of July. Then, on July 29, the sudden acceleration happened again to my husband. This safety issue needs to be addressed by Tesla more seriously. I should at least get a written explanation of what happened and not just a phone call telling me the vehicle was not at fault. Since the damage to vehicle is fixed now, we would drive the vehicle more often. I don't want this sudden unintended acceleration to happen again, hitting something or someone on the road!

On XXX, at approximately 3:30 PM while using FSD (Supervised) on software 2026.21.6, my Tesla Model X came to a sudden complete stop in the middle of a busy four-lane intersection. I had to intervene. The FSD feedback prompt appeared; I selected Critical. Approximately one mile later, while programming a destination, FSD pulled over unsafely on a busy multi-lane road with barely enough room for 50 mph traffic. I intervened again and selected Critical on the prompt. Neither event appears in the Dashcam Viewer (only Sentry parking clips visible). These continue a pattern. Previously, while driving from [XXX] to [XXX], the car jumped across freeway lanes unsafely. I reported it via the Tesla app and to Cathedral City Service. Technicians lacked diagnostic equipment, said they would escalate to the FSD team for research, and promised to realign the front cameras on a return visit. Months later: no follow-up and cameras still cosmetically misaligned. Camera Calibration menu only shows Clear option (I did not clear it). I have submitted a Service request and emailed Tesla requesting internal logs/telemetry and full escalation. Filing this for an independent record. On 7/29/2026, 2:45 PM PST, I tapped the in-car myQ interface to send an "Open" command. Due to network latency/ connection drops in the Tesla infotainment system, there was a significant delay before the open command executed. After it finally opened, I proceeded to back out in Reverse. However, due to buffering/retry logic during the network reconnect, the system unexpectedly transmitted a follow-up "Close" command without any driver input. Because this automated ghost command executed while the vehicle was actively backing out, the overhead garage door unexpectedly began closing, striking the car and shattering the left Falcon Wing door glass. Safety Concern: The infotainment software suffers from a critical retry/buffering flaw where latent network API payloads execute out of sequence or trigger unintended secondary commands after initial connection drops. Transmitting an unprompted "Close" signal to an overhead garage door while a vehicle is actively moving in Reverse creates a severe collision hazard and safety risk for occupants and bystanders.

On XXX at approximately 465 miles, multiple critical safety systems simultaneously failed on my 2024 Tesla Model X (VIN: [XXX]). Dashboard warnings indicated that ABS, stability control, traction control, steering assist, and automatic emergency braking were all simultaneously disabled. FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

On XXX at approximately 465 miles, multiple critical safety systems simultaneously failed on my 2024 Tesla Model X (VIN: [XXX]). Dashboard warnings indicated that ABS, stability control, traction control, steering assist, and automatic emergency braking were all simultaneously disabled. The vehicle was taken to Tesla Service immediately. Tesla diagnosed the issue as a faulty wheel speed sensor and performed a replacement (RO# XXX). This simultaneous disabling of five critical safety systems on a vehicle with only 465 miles is a serious safety defect. Additionally, on XXX at approximately 21,180 miles, a passenger airbag warning light illuminated requiring service. This was resolved and addressed by slight moisture on son's shorts triggering the alert but unsure why this would cause such sensitivity. The vehicle has required 12 service visits since delivery in November 2024 for various recurring safety and mechanical issues. The door operated normally for the preceding 12 months. In my experience, this prompt does not automatically dismiss and requires manual interaction with the touchscreen to clear. This design creates a safety concern because: - It introduces a mandatory driver interaction immediately after a disengagement event. - It diverts driver attention away from the road during a potentially critical moment. - It may encourage rushed or inattentive responses just to clear the interface. It was a windy day I was driving to work I was only driving max 14 mph and there were leaves flying due to the wind and without any warning the phantom break came on and my car just randomly stopped. Tesla remotely blocks a car powertrain without warning if a service invoice wasn’t paid before leaving a service center. This poses a safety risk because the car might unexpectedly become non-driving, such as after returning from a hike with young kids in a remote area, without water, food, or medications. To reproduce this issue, follow these steps: 1. Leave a Tesla service before a service or part is charged to the card on file (this can happen for various reasons). 2. Park the car in an underground parking garage or a remote area with no cellular connectivity. 3. Attempt to return home. The result is that the car will be blocked by an unknown PIN, even if the invoice was automatically charged to the card on file while driving. Without Internet connectivity, it’s impossible to work around the PIN. The car has be towed to an area with cellular connectivity to resolve the issue.

On each occasion, acceleration was either severely delayed or completely unresponsive, and the vehicle rolled backward when attempting to move forward on an incline. The following warnings appeared on the dashboard: “Stability Control Disabled,” ESC (Electronic Stability Control) warning light, and Air Suspension System warning. On both occasions, Tesla stated that the issue could not be reproduced and returned the vehicle without performing any repairs. i was in full self driving mode. the car wanted to turn left but i thought it was going in the wrong direction so i tried to continue straight by turning the wheel to the right forcefully but the car would not let me take over and i crashed into a guard rail. when i tried to brake it was not able to stop soon enough, i have a dash cam footage. i have taken over the this way numerous times and it usually does not require much force to do so, this time it simply would not allow me to control the wheel. no one was hurt other that a guard rail. the car was totaled by insurance company. the police assisted me but i never received a police report. no pictures to share, just a dashcam video but i dont know if that would be helpful since there isn't a way to upload it that i can see. Purchased this vehicle new from Tesla on March 29, 2024. As I got home I hear a pop sound come from underneath the car and the abs warning light came on. The vehicle lost some of the functions like regenerative breaking and cruise control. The suspension was also inoperable. I brought the car in on the first availible service date to get the repair done which took a few days. After repair was completed I went back to pick the car up only to have the same warnings appear on the odometer and lost of functionality again 20 minutes into driving home. I contacted Tesla and they gave me another appointment at a different service center. I then brought the car to the other service center and they corrected the ABS and other warning light issue. However, I been experiencing vibration on highways when breaking and regenerative breaking. Since the beginning of ownership. I mentioned it to them but the told me vibrations can be normal with these cars so I endured it ever since until it felt worst after a while. I brought the car in again and they diagnosed it. They recommended changing the rear rotors and brake pads. After picking the car up its still doing the same thing. This costed me $1200 out of pocket. I brought the car in for front end noises and they changed parts under warranty. The noises was gone after the service but a week later I heard the noises again and it worsened. Now they are refusing further repairs and recommended that I take the car to a auto body repair shop.
