The Office of Defects Investigation (ODI) received a Petition dated July 20, 2026. The Petition requests an investigation of road debris / rocks damaging the heat exchangers in model year (MY) 2024-2026 Toyota Grand Highlander vehicles. The Petition contends that the Grand Highlander’s front-end grill has large, open apertures and no protective screen between the grill and the vehicle’s heat exchangers. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Risk: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2024 Toyota Grand Highlander, Grand Highlander Hybrid, Lexus TX350, TX500 Hybrid, and TX550 Hybrid+ vehicles. The driver and passenger side curtain air bags may not unroll as intended and could partially deploy outside of an open window. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 226, "Ejection Mitigation."

Risk: An improper deployment of the side curtain air bag can reduce occupant protection during a crash, increasing the risk of injury.

Issue: Under certain conditions, a software problem could cause the rearview camera image to freeze or not to display. If this happens, the rearview image may not display when the transmission is shifted into reverse. Canadian regulations require the rearview image to display when the vehicle is reversing. Note: This recall only affects certain vehicles equipped with a panoramic view monitor system. Safety Risk: A rearview camera image that doesn't display correctly could reduce a driver's ability to see behind the vehicle while backing up. This could increase the risk of a crash. Corrective Actions: The company will notify owners in writing and advise you to take your vehicle to a dealership to update the parking assist ECU software.

Issue: On certain vehicles, a software problem could cause the combination meter in the instrument panel not to display when the vehicle is started. As a result, you may not be able to see certain information about the vehicle, including the speedometer, fuel gauge, warning lights and malfunction tell-tales. Note: This recall only affects vehicles equipped with a 12.3-inch display. Safety Risk: Warnings, tell-tales and gauges that don't display could increase the risk of a crash. Corrective Actions: The company will notify owners by mail and advise you to take your vehicle to a dealership to update the combination meter software. Alternatively, certain eligible vehicles will be sent a wireless over-the-air software update, which you can install yourself. For certain vehicles, the dealer will also inspect and, if necessary, replace the combination meter.

On February 25, 2025, I took my car into Freeman Toyota dealership in Santa Rosa for a recall on the airbags (24TA09 SAFETY (NONCOMPLIANCE) RECALL 24TA09(Remedy Notice) - Certain 2024 Model Year GrandHighlander and Grand Highlander HV - Curtain ShieldAirbags May Not Deploy As Intended). In the labor notes, they claimed to have performed the recall - 1. PERFORMED THE 24TA09 AND REPLACED BOTH LEFT AND RIGHT HAND CSA ASSEMBLIES. OP CODE: 24TA09R1.3.1 HRS. However, on June 24, 2025, the Toyota was involved in a collision with a deer. The autobody shop confirmed that the airbag sensors went off, but the airbags did not deploy. A claim was filed with my insurance company, and the airbags have been replaced by the autobody shop. The vehicle did not get back to the dealership.

The contact owns a 2024 Toyota Grand Highlander. The contact stated that after driving into the garage and while pressing the brake pedal, the vehicle suddenly experienced unintended acceleration, lurched forward, and crashed into the husband's workbench. During the incident, the brake pedal was forcefully pressed, but the vehicle did not immediately stop. No injuries were reported. The husband's workbench was destroyed. The vehicle was later driven to the local dealer, who was unable to determine the cause of the failure. The manufacturer was notified of the incident, and an investigator was sent to examine the vehicle. The investigator reported that no trouble was found with the vehicle, and the crash was due to driver error. The contact insisted that no driver error had occurred, and the incident occurred while the brake pedal was being forcefully pressed. The failure mileage was 14,000.

Road debris from normal driving on paved roads (perhaps related to a trip from Flagstaff to Phoenix on [XXX] in April 2026- in Phoenix the AC took a very long time to cool down the car), road debris punctured the ac condenser. The AC stopped working entirely in July of 2026, so it was taken to Findlay Toyota Flagstaff for review. They noted all refrigerant had leaked from the system, and that road debris had punctured the condenser. The warranty claim was denied, and I paid about $1450 out of pocket for a new condenser and system recharge. I have the estimate from the dealer along with the receipt for the actual repair; however, the file upload functionality of this app is not working. The contact believed that the odor was caused by a fuel leak. The vehicle was taken to a dealer, where it was diagnosed with an evaporator filter failure, and it was determined that a fuel leak was not the cause of the odor. I have a 2024 Toyota Grand Highlander (FWD Gas) with about 37,000 miles and had 2 wheel bearings fail - front right and rear right. I noticed a constant humming noise starting around 20-25 mph that increased with vehicle speed. Thinking it was tire noise, I replaced all 4 tires, and the sound was still present. The dealership first replaced the right front bearing, and when they drove it after the repair, they noticed the sound decreased but did not eliminate the noise, and they further diagnosed a failed rear wheel bearing.

POWER TRAIN

My vehicle has a recurrent issue with failing front axle assemblies. The vehicle experienced rotational clicking and had both from axle assemblies replaced around 20,000 miles. Around 40,000 miles the clicking returned and toyota has refused to replace the failing axles again. I currently am at 63,000 miles and I won an arbitration case regarding the axles. The arbitration decision ordered toyota to replace the axles and determined that the parts were defective and covered under warranty. Toyota breached the arbitration and refused to replace the defective axles as ordered in a legally binding arbitration. Rotational clicking is more apparent at low ambient temperatures. A metallic grinding is more apparent at high ambient temperatures. I have had several drive train skips, a sensation of drive train parts slipping and slamming together. Toyota knowingly put defective parts back on my car during the 20,000 mile repair. Toyota release a technical service bulletin T-SB-0082-24 in sep of 2024. It stated that they redesigned the axle to fix the problem. The tsb said to put the original part number on instead of the updated, fixed part number. I received the old, broken part number when my repair was done in 2025. There is a new bulletin T-SB-0028-26 release in april 2026 for the same issue and it states that the previous bulletin is obsolete. The new bulletin states that the correct part number is the new part number not the old one. Additionally, the tsb indicates toyota changed to the new part number with a vin cutoff sometime in 2024. The fixed part number was changed at the production line level in 2024 yet I received the old, known defective part. These are drive axles and a failure can lead to loss of control and a serious accident. A full investigation and recall of this part should be conducted.

While conducting independent research, the contact identified Technical Service Bulletin: T-SB-0028-25, which was associated with the vehicle shuddering while at a complete stop, and believed that the failure was associated with the technical service bulletin. The contact stated that while driving at an undisclosed speed and turning the steering wheel to the left or the right, occasionally there was a clicking sound coming from the vehicle. The dealer was contacted and diagnosed the failure as an axle failure. The vehicle was repaired, but the failure reoccurred approximately two to three months later. The vehicle was repaired five times due to failures. There was no sound coming from the vehicle after the last repair. The manufacturer was not made aware of the failure. I own a 2024 Toyota Grand Highlander Platinum Hybrid MAX with approximately 34,000 miles. The vehicle has experienced repeated delayed or absent acceleration from a complete stop, creating a serious risk when entering traffic. Before the most recent repair, I reported intermittent hesitation to the Toyota dealer. On Sunday, July 12, 2026, shortly after startup, the vehicle hesitated twice within the first half mile. At the second stop sign, I attempted a left turn into traffic and applied approximately 30% accelerator pedal. The engine was running, but the vehicle only crept forward at idle-level power for approximately three seconds. There was no noticeable increase in engine RPM and no warning lights. Normal power then returned. On July 15, 2026, the Toyota dealer performed TSB T-SB-0017-24 and updated the engine-control software for hesitation during acceleration. After that repair, the same loss-of-acceleration condition happened again while my wife was leaving a parking lot. She was stopped at a stop sign, making a right turn into traffic on a slight uphill grade, when the vehicle failed to provide normal acceleration. This was substantially similar to the earlier incident and occurred after the software update intended to correct the problem. The repeated failure to respond to accelerator input from a stop creates a collision risk because the vehicle can begin entering traffic without providing the expected power. I am requesting that NHTSA investigate whether this is a broader safety defect affecting the Grand Highlander Hybrid MAX and whether the existing software repair is effective.

I've had consistent issues with the vehicle accelerating and shifting. These include sluggish acceleration, loss of power and sputtering when we've come to a complete stop and starting to move again. We've almost been hit by other vehicles at least over 20 times because the vehicle has failed to accelerate when pulling into a roadway. I am fearful of getting t-boned when my car hesitates while turning at an intersection, going through an intersection or pulling out from a parking lot. I have been stuck in several intersections when traffic was coming toward me, but thankfully those cars had patience and avoided us. I've also found that this issue always happens when the hybrid battery shows that it only has a charge of 3 bars. It seems that the engine, transmission and hybrid battery don't know how to communicate when the battery charge gets that low. This has been happening since Dec 2023 when I bought the vehicle and the dealer has never been able to duplicate or find any error code, they are refusing to fix a known issue when many others are experiencing the same issue. There are Toyota Technical Service Bulletins (TSBs) out there regarding this problem but my dealer told me they won't do anything as it could label my vehicle a lemon. This is creating an unnecessary safety risk for me and my family.

The passenger-side second-row bench seat (60 percent section of the 60/40 split) has a defective seat track/latch assembly that fails in both directions: it seizes completely when it should slide, and it failed to stay latched while the vehicle was in motion. The seat saw almost no use for the first 34 months of ownership (third row kept folded). Within two weeks of its first regular use (XXX, approx. 45,700 miles), it seized completely and could not be moved in either direction. An authorized Toyota dealership in Placerville, CA disassembled the seat and documented in writing that the seat track was twisted and rubbing against the seat frame. The dealer straightened the track rather than replacing it and stated on the invoice that if the seat stopped moving again, both seat tracks should be replaced and the seat frame itself may need replacement or adjustment. After that repair, in approximately mid-June 2026, the bench slid FORWARD on its own as my wife braked for a stoplight, the track failed to remain latched with the vehicle in motion. Two rear-facing infant car seats are installed on this bench, and our toddlers reach the third row through this seat. On XXX (approx. 46,416 miles, 33 days and about 700 miles after the repair), the seat seized again. The vehicle is currently at a second authorized Toyota dealership in [XXX] for diagnosis and is available for inspection upon request. Safety risk: a second-row seat that does not remain latched cannot properly restrain occupants or anchored child restraints during hard braking or a crash, and a seized seat blocks third-row access and egress for children, including in an emergency. There were no warning lamps or prior symptoms before either failure. Note: NHTSA recall 26V128 covers 2021-2024 Highlander second-row seats that fail to lock; this Grand Highlander is not included in that campaign but exhibits a similar failure to remain latched. While driving my 2024 Toyota Grand Highlander Hybrid AWD at highway speed with only 16,755 miles on the vehicle, multiple warning messages suddenly appeared, including warnings related to the engine, hybrid system, AWD system, and other vehicle functions. The vehicle instructed me to pull over immediately and significantly reduced power output, making it difficult to maintain highway speed and safely maneuver through traffic. I was able to reach the shoulder and had the vehicle towed to a Toyota dealership. The dealer diagnosed a punctured radiator caused by a rock or similar road debris that allegedly passed through the front grille and struck the radiator. The dealer confirmed that coolant loss from the puncture triggered the vehicle's protective reduced-power mode. This failure created a significant safety risk. A sudden loss of power while traveling at highway speed increases the risk of rear-end collisions and other accidents, particularly in heavy traffic where drivers may not expect a vehicle to rapidly decelerate. There were no warning lights, messages, leaks, drivability concerns, or other symptoms before the incident. All warnings appeared suddenly at the time of failure. Toyota has declined warranty coverage, classifying the damage as a road hazard. However, I believe this may indicate a design defect. I have identified numerous reports from other GH owners describing nearly identical failures involving rocks penetrating the radiator through the front grille, often at relatively low mileage. These reports suggest the radiator may be inadequately protected from ordinary roadway debris. My concern is not simply radiator damage, but that a small piece of road debris encountered during normal driving can disable a critical component and cause the vehicle to enter a severe reduced-power mode at highway speed. I request that NHTSA investigate whether the front grille and radiator protection design of the Grand Highlander creates an unreasonable safety risk
