While driving on a highway at highway speed of approximately 62 miles per hour, a rock from the highway went through the front grille's hole, and punctured the area of the radiator, resulted in a large loss of coolant, and the vehicle's speed suddenly lost highway speed and lost propulsion on the highway, while driving at a highway speed. Thankfully noone was driving behind me. However, If someone was behind me, they would have hit me from the rear due to my loss of propulsion.

The contact believed that the odor was caused by a fuel leak. The vehicle was taken to a dealer, where it was diagnosed with an evaporator filter failure, and it was determined that a fuel leak was not the cause of the odor. The dealer stated that the vehicle did not require a repair and could still be driven with the odor inside the vehicle. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 45,000.

Vehicle intermittently fails to accelerate when accelerator pedal is depressed after stopping. Vehicle may move extremely slowly for several seconds and then suddenly regain power. The component that was available for inspection was our gas tank. It provided a profuse smell of fumes coming out of the tank and/or exhaust. You could also see vapors coming out of the gas tank when driving. The fumes alone are extremely toxic and extreme levels. The safety of me, my two young children and husband is all at risk. The smell of the gas and the view of the vapors from the gas tank puts the vehicle at an extreme fire risk. The smell of the fumes out it at a toxic inhalant risk - especially having two young children. This has caused great fear within me and the safety of my children is at extreme risk. No. I have taken the vehicle to the dealer 2-3 times, advised each time there is no issue with the vehicle. However, the smell of the gasoline has happened 6-8 times since purchasing the vehicle in May 2025. The vehicle has only been inspected by the dealer. Unsure who else to bring it to. The first time it was towed because the smell of the gas was in the cabin of the vehicle and I did not feel safe having my two small children in it with the extreme gas smell. No, no warning lamps. No indicators. The first time it happened I was alone with my two small children and the only indicator was the vapors coming out of the tank that were visible, and the extreme smell of the gasoline when I would roll my window down.

After a complete stop at a stop sign there is hesitation/stalling when trying to accelerate. Sometimes the hesitation is several seconds and then the engine kicks in and lurches forward. Couple times this has been a safety issue as it happens when I’m trying to pull out on a busy road. The delay/hesitation has caused me to almost get hit a few times. There are no lights or warning messages when this happens. It happens a multiple times a week especially during hot weather when AC is on full (LO temp setting and maximum fan) and is usually during the first few minutes of operation usually at the same stop signs on my route to pickup my kids after the car has been sitting unused overnight. I also notice that the hybrid battery indicator is usually always at 3 bars. Dealership hasn’t been able to reproduce as it's a very specific scenario (not driving/in garage overnight and short drive). Instead they went and applied T-SB-0040-25 but this hesitation is still happening after the software update. This needs to be handled like a safety issue/recall worthy with a complete fix! Not just some drivability issue that only gets applied if we ask about specific T-SBs.

I've had consistent issues with the vehicle accelerating and shifting. These include sluggish acceleration, loss of power and sputtering when we've come to a complete stop and starting to move again. We've almost been hit by other vehicles at least over 20 times because the vehicle has failed to accelerate when pulling into a roadway. The car hesitates during acceleration regardless of how much I push the gas. Once it starts to go, it feels as though the car can't shift from 1st to 2nd gear, The RPMs get extremely high while the car is going 25-40 mph and remain there for a few seconds up to ~20 seconds. I am fearful of getting t-boned when my car hesitates while turning at an intersection, going through an intersection or pulling out from a parking lot. I have been stuck in several intersections when traffic was coming toward me, but thankfully those cars had patience and avoided us. I've also found that this issue always happens when the hybrid battery shows that it only has a charge of 3 bars. It seems that the engine, transmission and hybrid battery don't know how to communicate when the battery charge gets that low. This has been happening since Dec 2023 when I bought the vehicle and the dealer has never been able to duplicate or find any error code, they are refusing to fix a known issue when many others are experiencing the same issue. There are Toyota Technical Service Bulletins (TSBs) out there regarding this problem but my dealer told me they won't do anything as it could label my vehicle a lemon. This is creating an unnecessary safety risk for me and my family.

Radiator issue summer of 2025, when in back to back traffic at low speeds my vehicle overheated at a stand still took vehicle in to dealership with no real answers. The following summer 2026 same scenario but the engine was smoking. Took into dealership and they stated the connection for the radiator fan was disconnected which meant it was not colling the engine. A month later the AC is blowing hot air when AC is set to the coolest setting, scheduled another appt with dealership likely another radiator issue. Fuel gauge inaccuracy. Manual states 17 gallon tank capacity but my car states empty and can only fill the tank with 14 to 15 gallons. Dealership had no help and stated it does that to protect the hybrid engine. Transmission shifting issues, in the summer hotter temperatures I notice at times the transmission has difficulty shifting in lower gears and really revs up the RPMs to shift sometimes. At times the the transmission shift is jerky or harsh when going speeds form 20- 40 mph.

On a recent road trip I experience loss of acceleration twice. I was at a stop sign making a left hand turn. I step on the accelerator it start forward but then stop suddenly leaving me in the 1st lane of on coming traffic. I smashed on the accelerator several time before the vehicle finally responded. I took the vehicle to the dealer on 6/30 told the dealer what had happen along with my google findings acceleration hesitation and sudden lurching in the Toyota Grand Highlander Hybrid MAX Technical Service Bulletins (TSBs), including T-SB-0017-24. The dealer could not duplicate or find any error code, they are refusing to fix a known issue. They are creating an unnecessary safety risk for me and my family.

While driving my 2024 Toyota Grand Highlander Hybrid AWD at highway speed with only 16,755 miles on the vehicle, multiple warning messages suddenly appeared, including warnings related to the engine, hybrid system, AWD system, and other vehicle functions. The vehicle instructed me to pull over immediately and significantly reduced power output, making it difficult to maintain highway speed and safely maneuver through traffic. I was able to reach the shoulder and had the vehicle towed to a Toyota dealership. The dealer diagnosed a punctured radiator caused by a rock or similar road debris that allegedly passed through the front grille and struck the radiator. The dealer confirmed that coolant loss from the puncture triggered the vehicle's protective reduced-power mode. This failure created a significant safety risk. A sudden loss of power while traveling at highway speed increases the risk of rear-end collisions and other accidents, particularly in heavy traffic where drivers may not expect a vehicle to rapidly decelerate. There were no warning lights, messages, leaks, drivability concerns, or other symptoms before the incident. All warnings appeared suddenly at the time of failure. Toyota has declined warranty coverage, classifying the damage as a road hazard. However, I believe this may indicate a design defect. I have identified numerous reports from other GH owners describing nearly identical failures involving rocks penetrating the radiator through the front grille, often at relatively low mileage. These reports suggest the radiator may be inadequately protected from ordinary roadway debris. My concern is not simply radiator damage, but that a small piece of road debris encountered during normal driving can disable a critical component and cause the vehicle to enter a severe reduced-power mode at highway speed. I request that NHTSA investigate whether the front grille and radiator protection design of the Grand Highlander creates an unreasonable safety risk

Failed System: Fuel Injector Failure (?) at high speeds, under load (uphill), causing an immediate and life threatening loss of power. While traveling on the highway, going up mountainous areas, the vehicle loses power and shakes violently. During multiple repairs/visits, the dealerships state that it is the failure of fuel injectors. They are replaced, and it happens again with the same explanation. Safety: Three times my family has lost power at 70mph while traveling uphill surrounded by many cars. The last event ended in a a scary and close interaction with an 18 wheel truck that was unable to stop as we lost power. Dealer Confirmation: The dealer has confirmed, on two separate occasions, that the fuel injectors needed replaced. The vehicle has 3,000 miles on it. At 2,000 miles, damage was found to the fuel injectors. They were replaced and the car was placed back in my possession. At 3,000 miles, the incident occurred for the 3rd time and damage was again found with the fuel injectors. Inspection: Dealerships as well as Toyota's field technician have inspected/repaired the vehicle 2 times and is currently at a dealership for the 3rd time. Warning messages: After the incident occurs, multiple warning lights appear (check engine and hybrid system malfunction).

On multiple occasions (four) my 2025 Toyota Grand Highlander Hybrid Max has failed to accelerate when I push the gas pedal. Once in traffic, which was extremely scary and a huge hazard. I have contacted the dealership about this issue and have not heard back from them. It has been almost 2 weeks. No warming lamps turned on and this issue lasted about 15 seconds each time. I press the gas, the car does not respond other than an audible RPM raising sound.

I have reported the problem every time I take the car to the dealer for a service. The problem has been happening since I purchased the car in November 2024 and occurs randomly--sometimes every couple of months, sometimes days in a row. The dealer can't reproduce it, but they were able to pull a code from the transmission and found it had an acceleration error. They updated the software from a TSB, but that didn't help.

I am submitting this complaint regarding a catalytic converter failure on my vehicle and the manufacturer’s refusal to honor the warranty. My vehicle is a Toyota that is approximately two years old and currently has about 31,000 miles. The catalytic converter has failed well before what would normally be expected for this component. When I brought the vehicle to the dealership for repair under warranty, Toyota declined to cover the repair. They stated the failure was due to “contaminated fuel,” but they did not provide any evidence or test results showing that contaminated fuel was present in the vehicle. I have not been provided with documentation, diagnostic data, or fuel analysis supporting this claim. The denial appears to be based on assumption rather than proof. Because catalytic converters are emissions components that are typically expected to last much longer and are often covered by extended emissions warranties, I am concerned this may represent a broader issue affecting consumers. Vehicle details and service documentation can be provided upon request. The entire life of the car from initial purchase the fuel gauge is incorrect. The car has a 17 gallon gas tank and the fuel gauge is calibrated to around 13 gallons. The car thinks it is out of gas around 13 gallons when there is actually 4 more gallons left.

The car only accepts 11-13 gallons of gas when refueling an empty tank despite the advertised 17.2 gallon capacity. I believe this is a fuel gauge or DTE system problem. I’ve seen many many similiar complaints on the forums. Fuel gauge issue could leave drivers stranded on the road.

Description of Issue: We have experienced multiple episodes of violent shaking in our 2024 Toyota Grand Highlander while stopped at red lights or in intersections. Both my wife and I have had this happen independently, around two to three times each. Most incidents occurred while our young children were in the vehicle. The most recent incident occurred today, when my wife experienced it twice in a row while stopped at an intersection. Our children were not in the vehicle during today’s event. Component or system involved: UNKNOWN at this time. We suspect a major mechanical or drivetrain-related failure. The entire vehicle shakes violently and feels unstable. We are willing to make the vehicle available for inspection upon request. Safety risk: The severity of the shaking is extreme and it feels as though the car may explode or fall apart. We have had to put the car in park, shut it off immediately, and in some cases, exit the vehicle in the middle of intersections or while stopped at a red light. This places everyone in the vehicle at serious risk, especially our children, and could result in a crash or secondary accident due to the abrupt stop and unexpected behavior in active traffic. Has the problem been reproduced or confirmed by a dealer or service center? The issue has not yet been reproduced by the dealership. We brought the vehicle to Toyota of Escondido after a previous incident. They inspected it but stated they could not find anything wrong. Has the vehicle or component been inspected by others? The vehicle was only inspected by the dealership (Toyota of Escondido). It has not been reviewed by any other parties such as law enforcement or insurance representatives. Warning lamps, messages, or other symptoms prior to the issue: There were no warning lights, error messages, or abnormal behavior prior to the onset of the violent shaking. The issue occurred suddenly each time, without warning.

Ever since I got my car, sometimes it smells like gas on the right side of the vehicle. By driver side and right passenger side in the back. The vehicle was in PARK and the IGN was in the ON/RUN position and the air conditioning was on. There was no one in the driver's seat but there was an occupant sitting buckled in the front passenger seat. The passenger heard a sound (possibly the ICE turning on) and he felt the vehicle move forward about five inches. He became worried about his safety and pressed the IGN button in order to turn the vehicle off. It could have pushed itself into other cars in the parking lot or pedestrians. A report was made to the local police department on the same day. This occurred in a retail parking lot. Video footage cannot be obtained as a police officer or insurance company must request it. I have not seen a check engine light nor any message prompts and it has not been inspected by a dealership.
