On May 16, 2024, the Office of Defects Investigations (ODI) opened Recall Query (RQ) RQ24010 to investigate the remedy and scope of recalls 23V-213 and 23V-312. The affected vehicles by these recalls included model years (MY) 2021 through 2023 Volkswagen ID.4 vehicles. Volkswagen Group of America, Inc. (Volkswagen) is recalling certain 2021-2023 ID.4, 2022-2023 Audi Q4 e-Tron, and Q4 e-Tron Sportback vehicles. The instrument panel may not properly display the transmission gear lever indicator. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 102, "Transmission Shift Lever Sequence, Starter Interlock, and Transmission Braking Effect."

Risk: Failure to display the gear position may result in a vehicle rollaway if the parking brake is not engaged, increasing the risk of a crash.

Due to a software issue, affected vehicles may experience center displays and instrument panel displays that do not boot, or that sporadically reset. The replacement infotainment central control unit (CCU) installed in the vehicle during a repair contains incorrect software that can cause the rearview camera image to be delayed or deactivated when reversing. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

The affected vehicles may have been manufactured with door handles that allow water to enter the circuit board assembly, which can cause the doors to open unexpectedly when the vehicle is subject to high lateral forces at speeds below 9 mph/15 kmh.

The rearview camera image may be blank or unresponsive when the vehicle is placed in reverse gear or when the park function is manually activated. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

Risk: An inoperative rearview camera display can reduce the driver's rear visibility, increasing the risk of a crash.

Issue: Under certain circumstances, a software problem could cause the ''N'' (Neutral) gear position not to display on the instrument cluster. As a result, you may not be aware when the transmission is in neutral. If this happens, and brakes or parking brake aren't used, the vehicle could roll away. Note: For the Q4 e-tron model, this recall affects both SUV and Sportback models. Safety Risk: A vehicle that rolls away could create the risk of an injury or a crash. Corrective Actions: To reduce the safety risk, the company recommends that you should use the parking brake and confirm the parking brake indicator light is on when you park. The company will notify owners by mail and advise you to take your vehicle to a dealership to update the brake control unit software.

WHEELS

The front wheel assembly detached from the vehicle while I was driving. The vehicle is a Volkswagen ID.4 currently under lease. Prior to the incident, the vehicle had multiple wheel alignment services performed by an authorized Volkswagen dealership on May 24, 2025 and August 11, 2025. After the wheel detached on February 1st, 2026, the vehicle was towed and repaired by an independent repair shop. The vehicle and repaired wheel assembly may still be available for inspection if requested. The wheel detached while the vehicle was traveling on a public street, which caused an immediate loss of control and created a serious risk of collision and injury to myself and other road users. The vehicle could not be driven and required a tow. A police officer assisted at the scene in arranging a tow truck because the vehicle was immobilized.

My car was completely engulfed in flames, nearby cars as well as the parking structure experienced heat damage. My life and others were put in danger because even though my car was electric, it was parked in a resident garage near other cars who are gas operated, which could have caused an even more catastrophic damage if the fire would've spread more. Dallas Fire Department was present, and a fire report was conducted (Incident number #[XXX]). As of today, the vehicle has only been inspected by the Dallas Fire department, as well as a virtual video meeting with my insurance agent from Allstate. Before the incident the car charged normally, was operating normally and there was no indication or warning signs to indicate that there were any issues that would have caused this. This July (2026) I found the power steering to fail on two separate hot days, a similar issue also occurred in July of 2025. Using an OBDII reader it appears that it may have been related to a torque sensor.

The vehicle remained inoperable in this condition for approximately one hour near the intersection at approximately [XXX] , during rush hour traffic. The vehicle recovered without mechanical intervention. This is the third occurrence of this failure mode within approximately 90 days. Safety risk: Loss of propulsion at or near a major intersection during rush hour traffic created a severe collision risk to vehicle occupants and surrounding motorists. The vehicle was unable to move under its own power for approximately one hour in high-density traffic conditions. Dealer/manufacturer confirmation: The vehicle was brought to Deel Volkswagen, Miami, FL on XXX and is currently under active diagnostic evaluation. This is the third reported occurrence of the same failure mode. A related drive error was previously documented at a XXX dealer service visit. Other inspections: UNKNOWN. At 43k miles, 90F outside temps, battery SOC at 20%, charged the car at a fast charger getting 140kw. Daytime Running Lights, DRL, were on. After the charging session was over at 80%, the driver's side DRL went and stayed dim. Dealership replaced the entire headlight under warranty. At 77k miles, 94F outside temps, battery SOC at 20%, using a fast charger and getting 145kw with DRL left on. After the charging session was over at 80%, the passenger's DRL went and stayed dim. I normally turn off the DRL and forgot this one time.

The front wheel assembly detached from the vehicle while I was driving. The vehicle is a Volkswagen ID.4 currently under lease. Prior to the incident, the vehicle had multiple wheel alignment services performed by an authorized Volkswagen dealership on May 24, 2025 and August 11, 2025. After the wheel detached on February 1st, 2026, the vehicle was towed and repaired by an independent repair shop. The vehicle and repaired wheel assembly may still be available for inspection if requested. The wheel detached while the vehicle was traveling on a public street, which caused an immediate loss of control and created a serious risk of collision and injury to myself and other road users. The vehicle could not be driven and required a tow. A police officer assisted at the scene in arranging a tow truck because the vehicle was immobilized.

Subject: Request for Investigation and Recall – Door Handle Failure on Volkswagen Vehicle Dear National Highway Traffic Safety Administration, I am writing to formally request an investigation and potential recall concerning a serious safety issue with my 2022 ID.4 AWD Pro S Volkswagen vehicle. The problem involves the door handles failing to open from either the inside or outside of the vehicle. I can provide video evidence showing that, while the vehicle is fully unlocked, with no child safety mechanisms engaged and my foot on the brake at a complete stop, the doors cannot be opened from either the front or rear seats. This issue occurs repeatedly—approximately four to five times per week—and has become more frequent since the “919” recall was performed on the vehicle. Despite this, the dealership has stated that the issue is a “non-issue,” claiming that other vehicles of the same model exhibit similar behavior. The inability to open a vehicle door in normal operating conditions is a serious safety concern that could endanger occupants in an emergency situation. Volkswagen’s response minimizes a clear risk to consumer safety. If the software controlling door operation cannot be relied upon to perform such a critical function, it should be removed or replaced with a mechanical mechanism that guarantees the ability to open the door at all times.
