The contact stated that after the manufacturer performed an over-the-air (OTA) update to the OnStar system, the radio module and infotainment system became inoperable, rendering the rearview camera and the OnStar system inoperable. The vehicle was taken to the local dealer, who diagnosed that the radio module needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. BACK OVER PREVENTION: WARNINGS
The infotainment center failed completely and does not power on. Because of this failure, I cannot see the front, rear, or side cameras, and there are no audible warning sounds for parking, lane safety, or seat belt alerts. Essentially, none of the safety features connected to the infotainment system are functioning. This failure puts my safety and the safety of others at risk because I cannot see camera views when backing up or parking, and I receive no audio alerts or warnings for seat belts, obstacles, or safety features.

On Sunday September 7th 2025. I was driving 40 miles an hour. And all of a sudden my electronic braking system engaged and locked up. Big jolt and came to stop. Luckily I was not on the highway and also luckily I did not get rear-ended as well.

The contact stated that while his wife was driving at 60 MPH, the brake pedal was depressed but the vehicle continued to drive causing the contact to crash into the vehicle in front. The entire front of the vehicle was damaged. The contact stated that the vehicle was still in motion after the crash, he had to jump out of the vehicle and manually steer the vehicle to the side of the road where it finally came to a stop. All the air bags in the vehicle deployed. The contact mentioned the automatic braking system failed to independently stop the vehicle. No warning lights were illuminated. The contact's wife who was driving sustained air bag and seat belt bruising but no medical attention was required. A police report was filed. The vehicle was towed to the impound lot. The insurance company had not inspected the vehicle as of yet. The manufacturer was not made aware of the failure. The failure mileage was unavailable.

I purchased this vehicle in November 2022, with the understanding that it was not equipped with lane change alert with side blind zone alert and rear cross traffic alert, but the car would be retrofitted as soon as parts were available and that should be in a short time frame. To date I have not heard from GM or Koons of White Marsh on this retrofit, so I started asking questions. I also visited the sales lot at our dealership Koons of White Marsh and found that these safety features are installed on all 2023 Equinox models - so clearly the parts are available. Koons of White Marsh is saying now that they do not know when or if these safety features will be installed. I wrote a letter to GM's CEO and received two calls from the Executive Support team. First call - we would have part by Q1 2023; 2nd call said they do not know when or if we will receive the part. This missing part is putting my safety at risk as well as others driving around me as I do not have full alerts to the traffic around me.

I feel asleep while driving on a residential side street. LANE DEPARTURE: BLIND SPOT DETECTION
Transmission malfunctioned without warning at approx. 4,700 miles. Upon starting the vehicle multiple warnings went off and vehicle could not be shifted into any position; when attempting to put vehicle back in to Park, would not recognize that it was in Park so the vehicle could not be safely shut off and when turning off the ignition a warning alarm would sound and the display would remain on. Vehicle could not be locked due to being stuck “on”. Blind spot warning constantly detects a vehicle even when there is none rendering the feature useless and possibly confusing other drivers behind me into thinking it is a side mirror turn indicator and not BLIS.

LANE DEPARTURE: BLIND SPOT DETECTION
The contact owns a 2022 Chevrolet Equinox. The contact stated that while driving at an undisclosed speed, the vehicle lost motive power. The vehicle was taken to the dealer where it was diagnosed that the throttle body had detached. The dealer reattached the throttle body; however, the failure recurred with the vehicle inadvertently decelerating and then stalling while driving. Additionally, the Lane Change Alert with Blind Side Zone Alert feature malfunctioned. The vehicle was then taken to Rosenthal Chevrolet (6500 Little River Turnpike, Alexandria, VA 22312) where an engineer diagnosed that the vehicle performed as designed and he was unable to duplicate the failure. The vehicle was not repaired. The manufacturer was made aware of the failure and sent an engineer to Rosenthal Chevrolet to diagnose the vehicle. The failure mileage was 4,772.
