Driver Assist & Collision Avoidance complaints by model year
I am reporting a serious and extremely concerning brake failure involving my 2026 Chevrolet Silverado 2500 Diesel. Traffic ahead of me began to slow down. I saw the traffic slowing and immediately began applying the brakes to slow my vehicle. Although I was pressing the brake pedal, the truck was not slowing or stopping as it should have. When I realized the vehicle was not going to stop, I pressed the brake pedal as hard as I possibly could. The brake pedal went completely down to the floor, yet the vehicle still failed to stop, resulting in a collision. I want to be very clear: I saw traffic slowing, I applied the brakes, and when the truck failed to respond properly, I applied maximum pressure to the brake pedal. Despite this, the brake pedal traveled all the way to the floor and the vehicle did not stop. Because of the seriousness of the incident and our concern regarding the braking system, we had the vehicle towed from the scene rather than driving it. This was an extremely dangerous incident involving a 2026 vehicle. A braking system that fails to respond when the driver is actively attempting to stop presents a serious safety concern and could result in severe injury or death. I am requesting that this incident be formally documented and thoroughly investigated as a potential vehicle safety defect. I also request that all relevant vehicle data, diagnostic information, braking-system data, and electronic crash data related to this incident be preserved and reviewed.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Safety Issue. The raised cruise control speed set button on the steering wheel is prone to being touched when rotating the steering wheel while making sharp turns when driving through parking lots at low speed. If the distance cruise was previously set at say 45 mph the last time it was used, and the set button is very briefly hit upward when rotating the wheel through your hands to the left, it can inadvertently cause the truck to start accelerating to that previously-set speed. The cruise button needs to be programmed such that at certain steering angles it will not respond to inputs. This is very dangerous. I ave had this happen to me at least 5 times. Hitting the brakes stopped the acceleration. I reported it to the dealer as a potential engine surge but they could not reproduce it. I just recently realized what was happening. I believe this can easily be addressed by a steering angle lockout of the button. Other issues: Drivetrain vibrates occasionally and shifts very hard (drivetrain literally slams through gear change) when leaving driving from a stop a slow speeds. Dealer cannot replicate. This diesel truck gets very poor mileage possibly from spending too much time in regeneration mode. Truck is super sensitive to fuel. Mileage varies drastically from fill up to fill up depending upon station. I believe the fuel cetane content affects regeneration in unexpected ways. I buy fuel at name brand stations but mileage varies drastically from vendor to vendor. Mileage average goes down to about 11 mpg much of the time due to time in regeneration. Dealer has no comments. I know HD trucks don't have official mileage ratings but this is just crazy. I drive light on the accelerator. Rarely achieved up to 21 mpg but average mileage runs about 11-12. I believe this to be a fault with these trucks as I have read complaints from many others. The infotainment system frequently loses all audio, including turn signal sound, nav. GM has no fix. Possibly software.
NHTSA complaint #11755555 — an unverified report by a vehicle owner.
In May 2026, my vehicle began exhibiting serious and dangerous problems. It would shake violently after starting and occasionally stop on its own while driving. On [XXX], my wife and I got into the vehicle and started driving from our home in West Virginia. While driving uphill at approximately 30 mph, the vehicle suddenly began shaking severely and then came to a complete stop. After a few moments, it restarted itself and accelerated to about 30–40 mph while continuing to shake violently. This happened several times during the trip. Fuel consumption also became extremely abnormal, dropping to approximately 6 miles per gallon. Because Team One Chevrolet in Oakland, Maryland, is the nearest dealer, we drove there for service. After several hours, the dealer returned the vehicle and informed us that they had replaced a sensor. However, after leaving the dealership, the Check Engine light came on again after only three engine starts. I immediately returned to the dealership, where the warning light was cleared. The following day, the Check Engine light returned. When I contacted the dealership, I was told to continue driving the vehicle with the warning light on for one to two weeks. After more than a week, I returned to the dealership, and the light was reset again. A few days later, the same problem reappeared. At that point, I was informed that, according to dealership procedures, replacement parts could not be ordered until the issue had been reported three times. After the third visit, the parts were finally ordered, and I was required to wait several more weeks for another service appointment. I also reported the issue directly to Chevrolet. Despite multiple visits, repeated failures, and a significant amount of time spent attempting repairs, my vehicle remains unrepaired. My factory warranty is approaching expiration, and no one has provided a clear answer regarding what will happen if the upcoming repair attempt fails to resolve the problem. The entire infotainment system has gone black. Nothing shows on the screen. This is a safety issue as it does not show blind spot monitoring, heads up display, lane change warnings, allow hands free calling, gps, and much more. None of the safety features are available with a trailer hookup as well. We have taken it to a certified Chevrolet dealer who has said the truck was the 6th one that they have placed an order for in just over a week. The infotainment system is currently on back order which makes operating the vehicle concerning with its proper safety features.

This vehicle does not recognize that the gear is on park. Due to the issue, it has left me stranded twice. The first time it was at a very remote area up in the mountains. I was stuck for hours due to battery dying. The second time being a very dangerous situation Because it was a very dangerous area late at night.

Following vehicle startup, the entire audio system defaults to a muted state, affecting all sound output channels. This includes, but is not limited to: Critical Safety Alerts: Seat belt warnings, system malfunction indicators, and door ajar chimes. Proximity and Collision Warnings: Parking assist chimes, Rear Cross-Traffic Alert (RCTA) warnings, and Backup Collision Intervention (BCI) alerts. Communication Systems: Navigation prompts and hands-free/Bluetooth call audio. The audio failure persists until manual user intervention or the next vehicle cycle, disabling critical warning redundancy. Safety Implications The system-wide audio failure directly compromises the safety of vehicle occupants and surrounding pedestrians in two primary ways: Risk to Pedestrians: The silencing of proximity and backup collision warnings (RCTA, BCI) removes a vital layer of redundancy, significantly increasing the risk of striking vulnerable road users, such as children, while maneuvering in reverse. Impaired Driver Safety & Compliance: The defect also removes essential alerts like seat belt warnings and renders the mandated hands-free communication system inoperable, forcing potential driver distraction or non-compliance with driving regulations. Conclusion This audio muting defect is a critical safety failure that disables mandated auditory warning systems, severely limiting the driver's ability to operate the vehicle safely and respond to immediate, unseen hazards. Immediate investigation and resolution are necessary.

So the electrical would be when I shut my truck off in park I cannot turn my key completely back to release the key so therefore I’m killing my battery leaving the power on as far as the tires they’re rated for 50 or 55,000 miles I have 37,000 miles and I have no tread left and as far as my drivetrain when accelerating onto a highway pulling my fifth wheel camper that is 16,000 pounds loaded the truck box really bad as far as the crash warning breaking system it does not break

Another driver illegally turned left in front of this vehicle. the vehicles collided on each of their right front corners. The airbags failed to deploy and the shoulder belt did not restrain the driver and keep him striking his head on the steering wheel, and breaking a bone in his hand. this was a serious head-on collision that totaled both vehicles. The truck is currently at the salvage yard and is available for inspection. The driver's safety was put at risk because the two primary safety restraint systems failed. the problem has not been reproduced. Chevrolet is sending someone to download the crash data which will then be provided to their engineering department for review and analysis. No warning lamps, messages or other symptoms of the problem were observed by the owner or driver before the accident.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
While using adaptive cruise control, the system can detect a vehicle and immediately apply the brakes or slow down. In some cases, it may detect a vehicle (the vehicle icon appears) and trigger an immediate red collision light flash, followed by automatic braking. However, during these events, there were no cars ahead of the vehicle. There was no obstruction of sensors or cameras, and the incidents occurred in various weather conditions, including rain, snow, and clear days, both during the day and night. If the road is slick or slippery, this could potentially lead to a loss of control. Additionally, if another vehicle is following closely behind, it could result in a collision.

The contact owns a 2019 Chevrolet Silverado 2500. The contact stated that the main information center changed the information displayed, such as tire pressure. The back-over prevention camera was no longer displayed when the radio was in use. The vehicle was not taken to the dealer. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure. The manufacturer referred the contact to the NHTSA Hotline to report the failure. The failure mileage was 120,000.

Truck has been in and out of the shop since July. For more than 70 days total. I received a printout from a Chevy dealership in Georgia that indicated the main computer system lost communication with other components 11 times. It indicated it lost communication with the brake system control module three times and lost communication with the engine control module twice. Not sure exactly what that means, but it sounds rather serious. I've been trying since July to get GM or the dealership to buyback or replace my vehicle. Neither had ANY interest whatsoever. After I provided the attached photo to GM, they decided to buy my vehicle back. Seems that this issue is bigger than I know and surely needs some oversight from NHTSA so no one gets seriously injured due to GM's carelessness to address issues when they were brought up.

On July 1st, 2023, I purchased a 2024 Chevrolet Silverado 2500 from Orielly Chevrolet in Tucson, Arizona. On July 2nd, 2023, an error message popped up on the dash, “Park Assist Blocked See Owner’s Manual”. It continued to happen on and off til this very day. The dealership "attempted" to fix the issue, but it obviously hasn't been resolved. Due to equipment being faulty, the chances of an accident whilst back up are significantly increased. At one point on this ordeal, the truck was in the shop for 61 days. It had been in my possession for 18 days. I am currently in the process of a buyback for the vehicle. I had to hire a lemon law attorney. A supervisor from GM advised that GM is refusing a buy back for the vehicle. The vehicle also displayed an error, "key not found" while I was driving at 50mph. A bit unsettling. That error showed up twice on the dash. The dealership said they fixed the issue with a software update. Recently, the vehicle has a ticking noise on the left side of the engine, and the engine idles so low, that it feels like it's going to stall. Those issues haven't been addressed at the shop, because I start a new job on Monday(12/4) and had to drive over 2k miles to get here. Thankfully the truck made it. Not a concern I should have with a brand new vehicle.

I took delivery of my 2022 Chevy Silverado in March, 2022. I still does not have the park assist and Emergency Stop when backing up installed, which of course is a safety hazzard.

Failure of the HMI caused a flickering screen during driving operations with warnings and cautions constantly flashing making it very difficult to concentrate on the road conditions and traffic. Parking the vehicle using the back up camera was also affected making it impossible to see approaching objects from the rear.

Front Park Assist (FPA) system did not provide any level of advanced alert until after impact when all conditions were fully met as stated in the user's manual for the Park Assist system. NOTE: Text after bullets are extracts from user's manual while test in () are owner's feedback during incident when FPA failed to alert. •as the vehicle moves at speeds of less than 8 km/h (5 mph) ( the vehicle was going 1-2 MPH) •the sensors on the bumpers may detect objects up to 1.2 m (4 ft) in front (Front Park Assist (FPA) detection was not until pipe contact was made with bumper/sensor) •within a zone 25 cm (10 in) high off the ground and below bumper level. (Pipe was in full zone from 0 to >30" high) •Blocked sensors will not detect objects and can also cause false detections. Keep the sensors clean of mud, dirt, snow, ice, and slush; and clean sensors after a car wash in freezing temperatures. (The sensor was not blocked in anyway. The truck has never been in mud, dirt, snow, ice, or slush.) •When an object is first detected in the rear, one beep will be heard from the rear, or both sides of the Safety Alert Seat will pulse two times. (Initial FPA alert level was never reported) •(There was not an intermediate level description in the user's manual as implemented by GM. Intermediate FPA "yellow" alert was never reported) •or <0.3 m (1 ft) in the vehicle front — a continuous beep will sound (Continuous beep from FPA was reported only after contact was made with the pipe) •The Park Assist system does not detect children, pedestrians, bicyclists, animals, or objects located below the bumper or that are too close or too far from the vehicle. (The pipe as an object was above bumper height and passed from outside the range slowly (1-2mph) until contact was made without any alert from the FPA) This is a single occurrence issue and was reported to GM.
