I was in a collision. Traveling a four lane highway during daylight ours with no in-climate weather. The car in front of a line of approximately 4 cars missed their exit and slammed on his breaks to make a right turn from the driving lane. The line of cars in front of me stopped and/or left the roadway to avoid a collision. I was unable to stop in time and had nowhere to go and I rear-ended the car in front of me, which was fully stopped in the lane. According to my dash camera my speed on Impact was approximately 45 mph. No front end collision warning alarms went off. No automatic breaking occurred. No airbag deployed. None of the vehicle safety and anti collision measures performed as I had expected. $10,000 worth of damage to my car alone.

-The airbag(s) did not deploy. (**Attached police report says it did, but I have pictures showing it didn't, and they are currently in process of correcting the report) I have to pay to get the police report, so since I just obtained it on 3/13, I will wait a few days to get the corrected copy, to ensure it's complete. -The vehicle is either at the towing company or at the insurance company being inspected, but I'm sure it can be available. -I was in a traffic accident with injuries where if the airbags had deployed the injuries could have been prevented. -Unknown if it has been reproduced or confirmed, but I would guess no. -Insurance company was going to pick the vehicle up from the towing company and bring it to their venue to inspect. VEHICLE SPEED CONTROL
While driving 65-70 MPH cruise control will continuously "bog" down and act like it is wanting to cut off the whole vehicle. transmission is starting to act like it is slipping to the point where I am going all the way to the steering wheel before it catches again. Vehicle is under 36,000 miles and should not be having transmission issues. Radio has been replaced once before but is now doing the same thing where it is not recognizing my phone, directions given to it by touch on screen, and will even completely turn off while in the middle of driving. Will freeze and then act like the screen has 'water' damage. back up camera is not calibrated correctly. It states that I have more than enough room to make a turn but when looking into my mirrors, it is off by almost a foot and a half. I was in a car accident due to the back up camera malfunction with the difference in distance and with the screen malfunctioning. I have had the vehicle inspected by the chevy dealership multiple times and they have stated that there is nothing coming up on the code reader. The vehicle is available for inspection. My children ride in this vehicle and after the accident, I do not deem it quite safe with the malfunctioning parts. There has not been any lights coming on for the issues but the issues are there.

The shift assembly is going bad and causing a Shift to Park error that GM is aware of. They have issued a Technical Service Bulletin about this exact part. The error code will cause the battery to run down leaving me stranded. The dealer has inspected the vehichle and ordered the necessary part to replace it. The vehicle has not been inspected by the manufacturer, police, insurance representatives or others but is available to have an inspection done. There were no warning lamps for this other than the Shift to Park error showing on the display when the car has been turned off. These error messages started happening around August 1, 2026.

The contact owns a 2023 Chevrolet Trailblazer. The contact stated that on several occasions while driving at various speeds, the vehicle went into LIMP Mode with the check engine warning light illuminated. The contact stated that on three occasions, the vehicle was stopped and restarted, but the failure persisted. The vehicle was taken to an independent mechanic, where it was determined that the wastegate actuator needed to be replaced, and DTC: P0299 was retrieved. The vehicle was repaired, but the failure persisted. The vehicle was taken to a dealer, where DTC: P0299 was retrieved, and the contact was informed that the turbo or the wastegate actuator needed to be replaced. The vehicle was not repaired. The vehicle was then taken to another independent mechanic, where it was determined that the turbo and waste gate actuator needed to be replaced. The vehicle was not repaired. The contact associated the failure with Manufacturer Recall Number: A222379711. The manufacturer was not notified of the failure. The failure mileage was approximately 33,000.

The contact owns a 2023 Chevrolet Trailblazer. The contact stated that, while driving on several occasions with the auto start-stop feature activated, the vehicle appeared to shut down at a complete stop as the infotainment screen and dashboard lights turned off. The vehicle restarted automatically but shuddered during startup. The dashboard lights and the infotainment screen became operable. The vehicle was not diagnosed or repaired. Additionally, the contact stated that while driving at approximately 30 MPH, the thermostat needle went to Hot, and the Check Engine warning light illuminated. The vehicle was steered to the side of the road and turned off for approximately 15 minutes. Upon restarting the vehicle, the check engine warning light remained illuminated. The vehicle was taken to an independent mechanic, where it was diagnosed that the coolant flow control valve had failed and needed to be replaced. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 88,251.

The contact stated that while parking the vehicle, the computer system failed to recognize that the vehicle had been shifted into the park position, causing a "Shift to Park" warning light to illuminate on the instrument cluster. Although the gear selector was physically in park, the contact was unable to fully shut down the vehicle and had to cycle the ignition on and off multiple times until the system eventually recognized the park position. The vehicle was driven to the dealer where it was diagnosed to be a transmission control assembly failure. The vehicle was repaired. The manufacturer was not notified of the failure. While driving on the highway in light traffic at approximately 70 mph and changing lanes, the side airbags in my vehicle suddenly deployed without warning. There was no collision, no contact with another vehicle, no road debris, and no apparent external cause for deployment. I did not strike anything before, during, or after the deployment. There were no vehicles within several car lengths of my vehicle at the time. Highway Patrol was dispatched to the scene. Officer TJ Carter with Guilford County responded. Upon inspection of the vehicle at the scene, we found no evidence of impact or contact with another vehicle, object, or debris. The vehicle was covered in yellow pollen at the time, and even light contact would have left visible marks or disturbance in the pollen. After examining the vehicle, no signs of contact were identified. The vehicle does have unrelated prior body damage from over a year ago on one side, but that damage predates this incident and was not caused by this event. The responding officer can confirm there was no collision or new impact associated with the airbag deployment. There was no accident associated with this airbag deployment. The airbags deployed while I was traveling at highway speed, which caused a sudden and dangerous driving event and required me to quickly maneuver to the side of the roadway. This created a serious safety risk to me and to surrounding traffic. Prior to this incident, there were no warning lights, safety alerts, or service notifications indicating an issue with the airbag system or vehicle safety systems. However, a notification did appear in the My Chevrolet app at or around the time of the incident. The vehicle has not yet been fully inspected following the incident. A safety claim has been submitted to General Motors. I also have video footage of the Highway Patrol Officer cutting down the airbags on-site but cannot add that below so I have added a screenshot.

The bolts holding the exhaust to the catalytic converter broke while I was driving my car. The car could have flipped, the exhaust could have busted through the vehicle, or I could have caused an accident and died. This comes on the heels of the EXACT SAME THING happening to my wife's Chevy Trailblazer with nearly the same mileage as mine. I have 97K miles and she 96K. This is a KNOWN ISSUE to them and it is NOT THE FIRST time this has occurred. There are thousands of similar instances that have occurred and one can easily find the anecdotes through an online search.

The Onstar module has went out on my car and while researching the issue I found out that it is a very common problem with Chevrolet/GM vehicles years 2021-2025 and although Chevrolet and GM are wwll aware that there's an issue and that the onstar modules are faulty there's been no recall. Mind you the module doesn't just affect onstar features it also is connected to bluetooth microphone and remote start and now none of them work at all.

The contact managed to pull over to the shoulder of the road, where the contact attempted to restart the vehicle; however, the vehicle failed to restart. The vehicle was towed to a local dealer, where it remained for further investigation. In addition, the contact stated that the vehicle was previously taken to the same local dealer on several occasions. The dealer had previously replaced the battery and the turbo boost.
