Issue: On certain trucks equipped with "Fleet Engineer" mud flaps, the bracket for the mud flaps may partially cover the rear reflective tape. Canadian regulations require that reflective tape must be visible on the rear of the truck. Safety Risk: Reflective tape that is covered may make the truck less visible to others and could create an increased risk of a crash. Corrective Actions: Daimler Trucks of North America (DTNA) will notify owners by mail and instruct you to take your truck to an authorized DTNA Service Facility to inspect the reflective tape. It will be moved as necessary to meet the regulations.

Brakes

On certain vehicles with air brakes, after several hard brake applications, the brake light pressure switch may not activate the brake lights with a light application of the brake pedal. If the brake lights do not illuminate, following road users would be unaware of the intent of the driver to slow the vehicle, which could increase the risk of a crash causing injury and/or damage to property. Correction: Daimler Trucks North America authorized service facilities will repair the brake light pressure switch.

Lights And Instruments

On certain vehicles water intrusion could occur, and corrode and damage inside of the Signal-detect and Activation Module (SAM) Chassis. This corrosion could cause electrical shorts or intermittent operation of trailer tail lamps, trailer stop lamps, trailer side marker lamps and trailer lighting. Failure of the stop lamps to illuminate when the brakes are applied may result in following road users being unaware of the driver's intentions, increasing the risk of a crash causing property damage and/or personal injury. Also, under certain conditions, an electrical short in the Chassis SAM could also cause melting in the SAM or other electrical components, which could result in a vehicle fire. Correction: Dealers will inspect the Signal-detect and Activation Module (SAM) Chassis for evidence of corrosion and proper drip loop wire routing, and SAM Chassis damaged by corrosion will be replaced. A drip shield will be mounted inside the cab over the SAM chassis if not previously installed.

On certain trucks, the air tank mounting brackets may crack, which could allow the tank to detach from the vehicle. An air tank separating from the vehicle could strike another vehicle, a stationary object, or a bystander. As well, loss of an air tank would cause a loss of service brakes, which could increase stopping distances, possibly causing a crash. These issues could result in property damage and/or personal injury. Correction: Dealers will replace the air tank mounting brackets.

Fuel System

On certain trucks equipped with a Detroit Diesel DD13, DD15 or DD16 engine, the support system for the high pressure fuel line (between the fuel pump and the fuel delivery rail) may be damaged during a service repair procedure. Incorrect torque applied to the dampener "P" clip fasteners may damage the fasteners, which could allow the clips to loosen. This could result in a cracked fuel line and a diesel fuel leak. Diesel fuel leakage may ignite on a hot engine surface and cause a fire. As well, diesel fuel spilt on the roadway could endanger other road users by making the roadway unexpectedly slippery. These issues could result in property damage and/or personal injury. Correction: Dealers will replace the high pressure fuel line and install a more robust fuel line support system.

On certain vehicles equipped with an MBE4000 (EPA07) engine, the intake air grid heater may short circuit and fail. This could result in a vehicle fire causing property damage and/or personal injury. Correction: Dealers will install an additional external relay to prevent battery connection to the grid heater when the vehicle is not running and when grid heater control signal is absent.

On certain trucks equipped with a rack & pinion steering system, the lower pinion bearing may corrode and fail. As a result, the vehicle may require additional steering input while driving in a straight line. This could also cause a loss of steering power assist. These issues could ultimately result in property damage and/or personal injury. Correction: Dealers will inspect the rack and pinion gear assembly. If it passes the inspection, new bellows will be installed and grease added to the bottom pinion bearing. A steering gear that fails the inspection will be replaced.

On certain vehicles, specific operating conditions (such as tight, successive, highly banked curves in opposite directions), could trigger unintended Electronic Stability Control (ESC) system intervention. As a result, the ESC may unnecessarily apply one of the front brakes in order to correct the perceived oversteer condition. This may cause the vehicle to deviate from the intended path, thereby increasing the risk of a crash causing property damage and/or personal injury. Correction: Dealers will replace the ESC module.

TL* THE CONTACT OWNS A 2010 FREIGHTLINER CL125. THE CONTACT STATED THAT THE DRIVER WAS DRIVING APPROXIMATELY 45 MPH, WHEN THE TRUCK SUDDENLY ROLLED OVER WITHOUT ANY WARNING. THE DRIVER PASSED AWAY DUE TO THE CRASH. THE POLICE WERE NOTIFIED AND A REPORT WAS AVAILABLE. THE TRUCK WAS NOT TAKEN TO HAVE THE FAILURE DIAGNOSED OR REPAIRED. THE CONTACT EXAMINED THE TRUCK AND STATED THAT THE RACK AND PINION WAS FRACTURED. THE CONTACT WAS NOT SURE IF THE FRACTURED RACK AND PINION WAS THE CAUSE OF THE FAILURE. I HAVE BEEN DRIVING A 2010 FREIGHTLINER CASCADIA TRUCK EQUIPPED WITH A SO-CALLED HARD BRAKING DETECTION SYSTEM. I AM ENGINEER MYSELF AND, AFTER A WHOLE YEAR I CAME TO A CLEAR CONCLUSION THAT THE SYSTEM DOESN'T WORK ON ROAD WITH POOR CONDITIONS LIKE BUMPY OR SLIPPERY. THE REAL PROBLEM IS THAT TRUCK DRIVERS TAKE THE BLAME FROM THEIR COMPANY FOR THIS AND IT IS RISKY. TRUCK DRIVING IS BY ITSELF IS VERY STRESSFUL, AND IT BECOME A REAL HAZARD WHEN THEY DON'T WANT TO HIT THE BRAKE BECAUSE OF A HARD BRAKING SYSTEM THAT PUT THEM INTO TROUBLE WITH THEIR BOSS. AS RESULT WE WILL HAVE CRASHES THAT COULD BE AVOIDED. WHAT I HAVE NOTICED IS THAT WHEN YOU TRY TO SLOW DOWN ON BUMPY OR SLIPPERY ROAD THE ABS SYSTEM GETS ACTUATED, THE IT RECORDS A HARD BRAKING EVENT, WHICH IS WRONG. THE SYSTEM NEEDS BETTER JUDGMENT WITH POOR ROAD CONDITIONS. I PERSONALLY THINK THAT IT WILL NEED MORE SENSORS AND A SOFTWARE TO RESOLVE THE ISSUE HERE. IF THE SYSTEM WAS NOT AIMED AT SCARING DRIVERS I THINK IT SHOULD BE CORRECTED BEFORE IT KILLS MANY AMERICANS. I PERSONALLY KNOW SAFE DRIVERS WITH MILLION MILE ACCIDENT FREE WHO REQUESTED TO HAVE THEIR TRUCK SWITCHED FOR THIS PROBLEM AND I AM SURE A SURVEY AMONG DRIVERS WILL CONFIRM MY CONCLUSIONS.
