An investigation is the step before a recall: the agency has seen enough
complaints to look into a pattern itself. An open one means it thinks
something may be wrong and nothing has been fixed yet.
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. I was in a rollover accident on June 26th 2025 in my boyfriends 2007 GMC Sierra 1500 where we flipped 5 and a half times. The wheel separated from the rim. I will upload photos since I am unsure how to explain how the wheel damage is. During the crash the drivers air bags deployed but the passengers did not. When we finally landed we ended up on the roof. My boyfriend tried to unbuckle my seatbelt but it would not release. He ended up cutting my seatbelt and then at that time (approx 3 min after we landed) the passenger airbag deployed striking my boyfriend in the back, neck and head. Which knocked him out for approx 1 hour. My boyfriend had several open lacerations to his back, neck, head and his arms and metal pieces were imbedded in his skin. He also suffered other injuries like a fractured back, and several other medical problems.I got a severe concussion in which it has caused me severe headaches, memory problems and cognitive issues. Both of us have reduced quality of life.

The contact jiggled the key while it was still in the ignition cylinder and the vehicle restarted. The contact stated that the failure was intermittent but was a recurring failure. The contact stated that the failure was also occurring at various speeds. The contact had taken the vehicle to an independent mechanic who diagnosed and determined that the ignition switch was defective and needed to be replaced. The mechanic informed the contact that there was a recall for the failure and referred the contact to a dealer for assistance. The vehicle was not repaired. I was at a stop light and heard a loud noise like a shotgun blast. I felt a concussion hit me in the chest. I thought I had been shot at, then thought I was possibly rear ended but nobody was behind me. I noticed the airbag light was on, the hazards were on and I couldn't shut them off, and the seatbelts were, and still are all locked. But no airbag deployed. There was a smell in the cab that vaguely smelled like a cross between gunpowder and ozone. It's fortunate that I was not at freeway speed.

The contact stated that after the dealer serviced the front passenger’s side air bag inflator under recall, the passenger’s side air bag warning light failed to illuminate with a passenger seated in the passenger’s seat. The vehicle was taken to an independent mechanic and diagnosed with a code reader, which showed an incorrect installation of equipment code. The dealer was made aware of the failure and diagnosed the vehicle with Sensing Diagnostic Module (SDM) sensor failure. The dealer informed the contact that the Sensing Diagnostic Control Module needed to be reprogrammed or replaced. The vehicle was not repaired. The manufacturer was made aware of the failure. THERE HAS BEEN AN AIRBAG RECALL AND EVERY TIME I CALL THE GM DEALERSHIP THEY TELL ME THEY HAVE NO PARTS. ARE THEY HOPING I WILL WRECK OR SELL THE VEHICLE BEFORE THEY ARE REQUIRED TO PAY TO REPLACE THE DEFECTIVE AND LIFE-THREATENING AIRBAG?!!

TL* TAKATA RECALL. THE CONTACT OWNS A 2007 GMC SIERRA 1500. THE CONTACT RECEIVED NOTIFICATION OF NHTSA CAMPAIGN NUMBER: 21V050000 (AIR BAGS) HOWEVER, THE PART TO DO THE RECALL REPAIR WAS NOT YET AVAILABLE. HALEY BUICK GMC MIDLOTHIAN (9831 MIDLOTHIAN TURNPIKE, RICHMOND, VA 23235) WAS CONTACTED AND WAS INFORMED THAT DURING THE REPLACEMENT OF THE AIR BAGS, THERE WAS A HIGH RISK THAT THE DASHBOARD MAY FRACTURE AND THE CONTACT WOULD BE LIABLE FOR THE DAMAGE. THE VEHICLE HAD NOT EXPERIENCED A FAILURE. THE CONTACT WAS CONCERNED THAT THE MANUFACTURER WAS NOT RESPONSIBLE FOR THE DASHBOARD REPLACEMENT IF DAMAGED DURING THE RECALL REPAIR. THE MANUFACTURER WAS NOT MADE AWARE OF THE ISSUE. PARTS DISTRIBUTION DISCONNECT. ## VIN PASSED ## GMC 2007 SIERRA ##

TL* THE CONTACT OWNS A 2007 GMC SIERRA 1500. THE CONTACT STATED WHILE DRIVING 35 MPH, THE VEHICLE STARTED JERKING. THE CONTACT STATED THAT THERE WAS NO WARNING LIGHT ILLUMINATED. THE VEHICLE WAS NOT DIAGNOSED NOR REPAIRED. A DEALER WAS NOT CONTACTED. THE MANUFACTURER HAD NOT BEEN INFORMED OF THE FAILURE. THE FAILURE MILEAGE WAS APPROXIMATELY 120,000.
