NHTSA investigations into the 2022 GMC Sierra 1500

An investigation is the step before a recall: the agency has seen enough complaints to look into a pattern itself. An open one means it thinks something may be wrong and nothing has been fixed yet.

This is in addition to the first report regarding my 2022 GMC Sierra SLT with 10-Speed Transmission. I had previously filed a complaint (NHSTA Tracking# 11728418.) regarding the OnStar and Internet quitting on me. Which was the Serial Gateway Module. I had reported that I was put off many times regarding a loner vehicle so i could get back and forth to my Oncologist for treatment of Head and Neck Cancer. And years of follow-up appt. with specialists Gastrointerologists and Cardiologist for serious issues incurred from 168 intense radiation treatments and Chemo. I had also taken my truck in for all scheduled maintenace intervals specified by the dealer and the manufacturer. "REPAIR ORDER GCCS504610" I had to leave it in the shop for 30 days for a defective Exhaust System, Radio, Charging Port, Remote Key Fob not able to unlock the truck so i could get in. And some other warranty/recall issues regarding my A/C blower and Seat cushion blower motor and Trailer Brake Switch, Fog Lamps running while daytime running lights were on - All Safety issues, My truck was @ Beck & Masten GMC for 30 days during these repairs, recalls and warranty. Will attach INVOICE 123532 to show consistant sheduled maintenance per GMC guidelines. Waited while this service was performed. "REPAIR ORDER / INVOICE 140542" Again my truck was back at Beck & Masten GMC (19 days) for the Serial Gateway Module and Radio not working AGAIN! Again, no OnStar, No Internet, No GPS. And, I reported the oil consumption and Add OIL light coming on; which was dismissed as idling too often (. I had to add 2 qts of oil between scheduled oil changes(The truck had only 13754 miles at this time) The LAST time I had anything to do with Beck & Masten GMC South was when I spent over months waiting on a loaner vehicle because I needed them to please look into still having an oil light coming on. And slipping and front end wobble just before the slipping/clucking and high RPM's that were dangerous- accident!!

ENGINE

Vehicle was traveling at 65 mph on a busy highway when vehicle engine stopped. A message on the dash appeared indicating to driver to shift to neutral and try to restart engine. As vehicle was quickly loosing speed it was decided to coast to the side of road to avoid someone running into vehicle causing multiple cars to crash. After stopping vehicle, an attempt was made to start vehicle. Engine would not turn over. Vehicle was towed to dealership. The following day I was informed that the engine needed replacement.

VEHICLE SPEED CONTROL

Current Unsafe Conditions: Post-Replacement Active Safety Failures. The vehicle presently exhibits two distinct categories of active, ongoing, and dangerous safety failures, both of which were introduced as a direct result of the engine replacement performed by Monument Chevrolet: (a) Severe Drivability Failure. Between approximately 1,500 and 5,000 RPM, the engine repeatedly and without warning lunges forward, drops all power, and lunges again — cycling in this manner between three (3) and six (6) times before stabilizing. This behavior occurs across all gears and driving conditions, which creates a severe and unpredictable hazard particularly when merging into traffic or accelerating onto highways. This failure constitutes an immediate and ongoing risk to the safety of myself and other motorists. (b) Complete ADAS System Failure. The frontal crash response warning light is permanently illuminated on the instrument cluster, rendering the following federally relevant safety systems entirely non-functional: (i) Automatic Emergency Braking / Forward Collision Alert; (ii) Cruise Control; and (iii) Lane Change Assist / Lane Control System. All of these systems were fully operational and properly functioning before the vehicle was submitted for the engine swap. Their failure is an assembly defect introduced by Monument Chevrolet during the course of the repair and constitutes a warranty and safety defect for which General Motors, as the manufacturer and warrantor, bears full responsibility. Medical Hardship and Urgency. This vehicle is my sole personal transportation. It is essential for travel to active medical appointments and upcoming medical procedures. The ongoing failure of General Motors and its authorized service network to restore this vehicle to a safe and fully operable condition has created a serious hardship that directly affects my ability to access needed medical care. Yes, it was driven by a service department supervisor. He did experience the issues.

FORWARD COLLISION AVOIDANCE: WARNINGS

For the past 6 months I’ve had issues with my crash avoidance system, steer assist and lane departure warning. The crash indicator light would illuminate on my dash for no apparent reason. That would disable my crash warning system, steer assist and lane departure warning but, I could still adjust my distance for my brake warning indicator. The other problems that are not safety related is cruise control and automatic high beam lights will not function. I researched the Internet and found this is not an uncommon problem for this year model. The most common fix (if the windshield is not dirty) was remove the F 11 fuse, in the fuse panel beside passenger door for 10 seconds, that would allow the system to reboot. That worked for 3 months with more than 10 occurrences of the light illuminating on the dash. Now the crash indicator light is on continuously and will not reset by removing the fuse. The computer is up-to-date and all the settings in the infotainment system for the crash avoidance system are activated. Check numerous Internet websites, the cameras for this system is on backorder. So there must be a parent problem with the system. I was required to purchase 36 months of OnStar, I did not purchase a renewal agreement after the 36 month and this issue started soon after.

Top Grill/Hood trim piece - it is separate from the main Grill. It detached and flew off at interstate speed. Thank God it missed several cars behind me. The passenger side corner came loose and before I could safely make my way off the interstate the piece ripped all the way across and flew over the top of the truck. May have gone 30 feet in the air prior to landing on the asphalt between the two lanes.

STRUCTURE:BODY

My truck was serviced for the front chrome grill separation recall last year. On Saturday, 8/1/2026 while driving at highway speed the grill deflector separated from the vehicle violently. I was able to pull off of the highway and remove the part as only the passenger corner clip (with screw) was partially retaining the deflector. Upon looking at the "repair" or recall, the corrective action does not appear to have been done correctly per N242490120. The chrome coating was not scuffed let alone removed from the part and an insufficient amount of epoxy was used to attach this deflector. Damage was done to the hood and lower grill during this incident. I have had numerous and considerable issues with the purchasing/servicing dealership where I bought this truck.

POWER TRAIN

While driving my 2022 GMC Sierra 1500 (VIN: [XXX]) in active traffic, the 10-speed automatic transmission suffered a sudden and catastrophic internal failure. Without warning, the vehicle stalled in the middle of traffic and forced itself into a hazardous "limp mode," severely restricting my speed and acceleration. This created an immediate, high-risk safety emergency, leaving my passengers and me vulnerable to a rear-end collision before we could navigate out of the live roadway. The vehicle was towed to an independent facility (Kenny's Transmission, Repair Order #XXX) at 65,579 miles. Diagnostic troubleshooting confirmed a "Number 6 Solenoid Code" and "Internal Transmission Mechanical Failure" (3-4 races away, total loss of reverse gear), requiring a complete replacement of the transmission assembly utilizing an official GM service part at a personal cost of $6,673.10. General Motors has previously acknowledged 10-speed transmission control valve defects and wheel-lockup hazards by issuing safety recalls for diesel variants, but gas-powered models like my 6.2L L87 engine are experiencing identical, highly dangerous sudden losses of motive power without federal recall protection. I am requesting that the NHTSA investigate this systemic failure rate on 2022 gas models before it causes a major collision. POWER TRAIN

My truck is at the dealership for a failed transmission valve body. I was doing some research on this issue and saw recall N242454441. I asked the service writer about this and he said my truck was not covered by the recall. I had all of the same issues as described in the recall. effects can cause harsh shifting, setting a service engine soon message, reduced engine performance or P0747 diagnostic trouble code. I was driving along when the check engine light came on then a message on the dash said reduced power mode. I barely made it to work then called the dealership and told them I was bringing it in if I could make it. When I tried to take it to the dealership the rear wheels seemed to be locked up when in drive but would move in reverse. After coasting forward in neutral then reversing the wheels seemed to unlock in drive but then I didn’t have reverse. I have the same drive train with the 10 speed transmission. I think this recall needs to be expanded to include my truck as well as countless others for this known issue.

ENGINE

I Purchased the vehicle on May 29, 2026 and it stated the 6.2 liter engine recall had been fixed and that I have extended special coverage for the exact issue of the recall until 150008 miles on this vehicle. On July 14, 2026 the vehicle was being driven and stalled out at a traffic light and restarted, yet within a quarter mile it stalled again at the next traffic light and became completely disabled, it would not even allow a shift into nuetral, so I had it towed less than 100 feet into the Walmart on Loudon Road in Concord, NH 03301 parking lot. On July 23, 2026 I was able to get the batter charged and attach a code scanner which showed one code (P0016). Pursuant to Recall N252494001 (which had allegedly been repaired, likely with the bandaid fix of thicker oil) any 6.2 liter GM truck with the loss of propulsion needs to be plugged into a scanner and if code P0016 is present, engine replacement is needed. Specifically noted to the recall is Special coverage N252494003 which provides the coverage to 150,008 miles on this vehicle is directly related to the recall. At the time of loss of propulsion the vehicle had 99,195 miles present on the odometer. The following day July 24, 2026 I had the vehicle towed to Banks Chevrolet GMC in Concord NH, after the vehicle arrived, I spoke with the service department, provided them with the recall paperwork and printout of the special coverage uner the vin as well as a photo of the odometer and my scanner displayining one code P0016. They then informed me it would take them until August 24, 2026 to "diagnose" the vehicle. The recall is clear it just needs a code scan if disabled. I have contacted GM and informed them of the need for a loaner as the vehicle is a vehicle utilized for business. I have since been told they will try to look at it earlier and no loaner will be offered but I "may" be able to be reimbusred $44.00 a day for a rental. There are no rentals for that price that can tow over 9500 pounds. It is unnacceptable.

FUEL/PROPULSION SYSTEM

While driving enroute to a concert in [XXX]., in the passing lane on [XXX] around [XXX] & just before exit 22(mile marker 21) during rush hour on 7-23 at 5.09pm, my truck abruptly lost power and shut off! It would not start, I could not engage "N" from "P" to attempt to push my truck to the far left side of the highway. This was a "VERY DANGEROUS" situation as I could have been rear ended or much worse! I immediately called SOS/ON STAR for assistance. In the meantime, a police officer and state troopers showed up to divert the traffic around my truck. Eventually, a "HERO" vehicle showed up and at the direction of the police, my truck was "DRAGGED" from the passing lane to the emergency lane. As for getting a tow truck, 1 finally showed up after 5+ hours. I found out the you all have a contract with "AAA", which is the worse when it comes to getting any assistance. I was towed back to [XXX] to South Town GMC dealership & arrived at 11.25pm. It was only the next morning that I received an email from OnStar dated 7-23 at 5.07pm, with an "Advance Diagnostics" advising of a critical issue with the engine & transmission. The code was: P129F, which is the fuel pump and/or module. My truck now sits at the dealership with a dead battery, waiting for them to address. I have heard that there are issues and recalls in certain vehicles with this similar issue. I do have "extended warranty" protection thru Ally that I bought. However, the shock is finding out that the only way to get it into "N" is a special dealer tool that goes into the transmission under my truck! Are you SERIOUS??????? To have no ability to put my truck into "N" is not only unacceptable, but extremely dangerous. Especially with what happened to me & my family. At this point, I am at the mercy of my dealership to find the time to address this issue. Until then, I have no transportation & very upset with the stress of all that happened! SERVICE BRAKES

Truck did not stop when applying brakes. Message displayed brake system failure and the breaks were spongy. The truck was towed to the dealer and required the master cylinder, gasket, and reservoir to be replaced with 23,403 miles. This appears to be an ongoing issue with GMC Sierra trucks and needs further investigation as this potential could be catastrophic. Also, as this appears to be an ongoing issue, owners should not be responsible for the cost associated with any repairs and should require an immediate recall.

The upper part of the grill was rattling while I was on a 270 mile road trip. I stopped, inspected it, and found multiple mounting points cracked. I did not feel it was safe to continue driving with it being so loose so I installed four zip ties to continue my trip back home. Safety recall N242490120 had already been completed which means this is the second time the plastic clips on the grill have failed.
