I am reporting a serious safety concern involving my Honda Prologue and believe the repeated failures may indicate an unresolved defect involving the vehicle’s electronic, braking, ABS, propulsion, transmission/shift, or vehicle-control systems. On [XXX], the vehicle first displayed an error/warning. I contacted the Honda dealership in Vancouver, Washington, and was instructed to bring it in for inspection. I was out of town and could not do so immediately. On [XXX], I brought the vehicle to the dealership in the morning. They kept it overnight and returned it to me on [XXX] at approximately 2:00 PM, telling me the problem was a “system glitch” and had been fixed. On [XXX], less than 24 hours later, I was driving home after completing a Lyft ride when the vehicle displayed: “One-Pedal Driving Unavailable – Use Brake for Stopping.” At the same time, the ABS warning and multiple other safety-system warnings illuminated.The vehicle then became immobilized in the middle of an intersection. I was in an unsafe location and had to contact police for assistance. The vehicle remained unable to operate for approximately two hours. After multiple restart attempts, it suddenly became operational again, allowing me to drive it to the Honda dealership.I have photographs and videos documenting the warnings. One photograph shows the “One-Pedal Driving Unavailable – Use Brake for Stopping” warning with the ABS and multiple other warning indicators illuminated. Another shows “Service Transmission Now – Unable to Shift Soon.”This is especially concerning because Honda had already been notified, had possession of the vehicle for approximately a day and a half, and told me it had been repaired. The vehicle then experienced a serious failure the following morning and became disabled in traffic. This is not simply an inconvenience. The contact owns a 2024 Honda Prologue. The contact received notification of NHTSA Campaign Number: 26V306000 (Back Over Prevention). The contact stated that while in reverse, the rear-view camera was glitching, and the red triangle on the rear-view camera screen indicated that there was an object behind the vehicle; however, there was no object behind the vehicle. The vehicle was taken to the dealer for the recall repair however, the part to do the recall repair was not available. The manufacturer was not notified of the failure. FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
I was traveling on Interstate 80 with adaptive cruise control set at 70. The cruise control normally recognizes slower cars ahead and slows or stops as necessary. I saw that cars were stopped ahead so I reduced the cruise control setting to 60 to "help" it slow down. However, we came up fast on the stopped cars and the car did not slow. I had to manually brake to avoid a collision. The car's speed was about 67 when I applied the brakes. The car acted like slowing to the new cruise control setting of 60 was overriding the collision avoidance. I recently had much of the software in the car updated due to a recall and the adaptive cruise control seems to be acting differently now. Weather was clear, around 4pm, and the roads were dry. We were Eastbound around a slight left curve (not sharp) and traffic was stopped ahead in all 5 lanes, so the system should have noticed the obstacles ahead.

WHEN I USED ADAPTIVE CRUISE CONTROL, MY CAR TENDS TO LEAN SLIGHTLY TO THE RIGHT. IT VEERS OVER A BIT AND THEN JERKS TO THE LEFT. ALL IN ALL, I CAN'T USE ADAPTIVE CRUISE CONTROL ON THE FREEWAY WITHOUT PAYING ATTENTION. IT IS GENUINELY DANGEROUS IF VEHICLES ARE DRIVING ALONGSIDE OR BEHIND ME. OTHER THING, WHEN I USED ADAPTIVE CRUISE CONTROL FOR ALMOST ONE HOUR, THEN IT SUDDENLY BECOMES INACTIVE. IF I HAVE BEEN FOLLOW ANOTHER CAR, then what should happen when it suddenly becomes inactive??? I was very lucky at that time because there was no car in front of me. After 5,000 miles, my car began to make a slight noise on the right side when I slowed down in the city. Maybe it happened in the front drive axle assemblies or the front right low arm control.

Airbags deployed on impact, briefly incapacitating me. My foot remained on the accelerator. Standard post-collision safety design calls for automatic power cutoff and brake application after airbag deployment, specifically to prevent continued acceleration when a driver may be incapacitated. This vehicle did neither — it continued accelerating, significantly worsening the collision. I sustained soreness and bruising. Dealer confirmation: I did not take the vehicle to a dealer for the phantom braking. Both front half-shaft CV joints are failing and have been failing for the last SEVERAL months by manifesting a persistent and worsening very loud clicking when loading and unloading the drive-train, driving up hill and executing any type of turn maneuver. I've complained numerous times to the Honda Dealership here in Seattle start 4Q 2025. They informed me that this has been an ongoing "known" problem with this specific model over the entire model 3 year offering from 2024-26, but at that time Honda America had no revised part number to effectively mitigate the problem. They initially replaced the failed CV half-shafts with the same part number, only to have those parts fail again. Honda America subsequently instructed all Honda dealerships maintenance depts to stop replacing these parts until a newly revise part number is released. I was instructed to contact Honda America at their national customer service at 800-999-1009. (My Case # [XXX]). I've spoken with at least 20-30 difference Honda reps over the last 6 months with no commitment to do anything to fix this potentially dangerous problem. When the outer CV joint completely fails, it could potentially turn into a pole-vault stick and literally flip the car at freeway speeds and definitely cause a catastrophic loss of control at freeway speeds or at any velocity/momentum whatsoever. The very disappointing fact is there is NOW a revised/vastly improved Part Number (Honda Part #86284282) that offers customers a fix to this very well known problem. Honda has not officially issued a recall so dealers are not able to perform this fix under warranty. My friend has this same car and her 2024 Honda Prologue is making this same exact clunking noise. Description of Safety Defect: The Automatic Emergency Braking (AEB) / Forward Collision Avoidance system is severely malfunctioning. On multiple occasions, the vehicle has experienced "phantom braking"—abruptly slamming on the brakes for no reason whatsoever. There were no obstacles, pedestrians, or vehicles ahead to trigger the system. This malfunction is happening at high speeds on major highways. Safety Risk and Impact: This issue represents an immediate and terrifying safety hazard. Unexpectedly slowing to a sudden stop at highway speeds completely compromises vehicle control and leaves me at an extreme risk of being rear-ended by high-speed traffic. It is a severe danger to myself, my passengers, and every other driver on the road. While the vehicle is currently subject to a manufacturer recall regarding the backup camera and other software updates, this specific, dangerous braking behavior makes the vehicle incredibly unsafe to operate. BACK OVER PREVENTION
The contact owns a 2024 Honda Prologue. The contact stated while driving at 45 MPH with her wife in the vehicle, the vehicle instrument panel blacked out and displayed blank screen independently than minute and three seconds with instrument panel began to reset it self. A dealer was contacted. The vehicle was taken to the dealer where it was diagnosed that the instrument panel and front camera needed to replaced. The vehicle was repaired as they performed a software update and the front camera was replaced. The contact was relating the failure to NHTSA Campaign Number: 26V112000 (BACK OVER PREVENTION, ELECTRICAL SYSTEM). The manufacturer was notified of the failure and they informed her that they reviewing the case. The approximate failure mileage was 20,000 .

BACK OVER PREVENTION: WARNINGS
I have had two safety issues with this vehicle where there were system failures that did not record DTC codes. And an additional failure that does not record codes and cannot be duplicated. After leasing the vehicle for seventeen months, and during repair for an unrelated non-safety issue, it was discovered that the steering wheel airbag had a loose connection. No warning appeared at any time on the instrument panels. As well the one pedal function would switch into regular drive mode, as in a gas powered vehicle, randomly. After multiple tries and incidents the dealer service department has been unable to duplicate. Then on March 6 of this year I bought in the vehicle yet again for this one pedal failure, and was notified there was an existing recall. This software update was performed. On March 17, just 11 days after the update, while backing into my garage with the garage door going up the windshield wiper was clipped by the door, destroying both the wiper and shattering the back window. I have backed into my garage thousands of times and never had an issue. Why this happened on this date I do not know. What I do know is that the vehicle gave no warnings and did not break for the object. At the time my dogs were clipped into their seat belt harnesses in the center well. By a miracle no glass shattered beyond the back storage area and I was able to safely remove them. I checked after and all of the collision warning systems were listed as active, including automatic braking for rear collisions. I had the dealership run scans twice and all scans showed no DTC codes and all systems active. I can only assume that all prior updates have failed, and there is a substantial defect to the vehicles ability to communicate. I have a copy of the scan performed. The second scan was run on April 9 because the dealership failed to provide a printed copy of the first. I have that second scan. My vehicle fails to communicate or activate essential information, a safety hazard.

The contact stated that while driving on several occasions at various speeds, infotainment screen and instrument panel went black and did not work as intended. The vehicle was turned off for approximately five minutes and then turned on the infotainment screen and instrument panel began working. The vehicle was taken to a dealer where it was diagnosed that a software update was needed. The vehicle was repaired but the failure reoccurred the next day. Sounds like the wheels want to fall off. I got this Prologue with 4 miles on it, started making those noises at less than 3K. It sounds so bad! Honda seems to be waiting until someone is seriously injured to fix this issue.

LANE DEPARTURE: ASSIST
The lane keep assist has never worked. It doesn't not keep you in the lane, it just chimes when you drift out of it. The bigger issue is the axles. I only owned the car for 1 month when the axles started loudly clicking with every turn. Honda is claiming this is just a cosmetic defect and it does not affect safety, but I don't believe that for a second. Clicking axles are NOT normal, and could only indicate signs of deterioration. Not only that, but myself and anyone else with this issue can feel that the drivability is affected. Turns and propulsion are slower as time goes on and the clicking gets louder. Honda knowingly continues to sell these cars for up to $60,000 and no longer offers a remedy for the faulty axles. They now plan to discontinue the vehicle and offer no further support for it, which is completely unnaceptable. This safety defect must be remedied as soon as possible. We should not wait until someone is dead.

FORWARD COLLISION AVOIDANCE: WARNINGS
Honda (really GM) has a problem with the CV joints on the Honda Prologue. It makes a clicking noise on sharp turns at low speeds. I also randomly get forward collision system disabled while driving, this is concerning as it affects the overall safety of driving this car.

Shortly after purchase, I observed damaged/torn underhood insulation and reported the condition to the selling dealership, with concern that the vehicle should be inspected for related issues. Prior to a crash incident, I experienced concerns involving braking-related behavior and advanced driver assistance features, including lane assist and object/collision warning functions. During the crash, while reacting to a roadway hazard, the vehicle swerved and struck a pole, and I did not experience warning or intervention from driver assistance features as expected. Following the incident, a diagnostic pre-scan identified multiple faults involving brake control, radar sensors, image processing, and communication systems. Since the incident, similar concerns have continued, including recurring braking/driver assistance concerns, and the issues remain unresolved. I am submitting this complaint due to concern that braking and advanced driver assistance systems may not have functioned as intended and may present an ongoing safety risk.

BACK OVER PREVENTION
The contact owns a 2024 Honda Prologue. The contact stated that while reversing, the rear-view camera screen was blank. The contact stated that the battery was not fully charged. Additionally, while making a left turn, there was an abnormal clicking sound coming from the front passenger’s side of the vehicle. The vehicle was taken to the dealer several times. The contact was informed that the front axle needed to be replaced. The contact was advised not to fully charge the battery. The vehicle was not repaired. The contact was informed that the Warranty Department had to approve the request for a loaner vehicle, to allow the repairs to be performed. The contact stated that the request was submitted in December 2025, however the vehicle was still not repaired. The contact received notification of NHTSA Campaign Number: 26V112000 (Back Over Prevention, Electrical System). The manufacturer was made aware of the failure. The failure mileage was approximately 20,000.

On April 5, 2026, I was stopped at a busy intersection waiting to turn left. When I proceeded through the light, the car's sensor suddenly went off and the car braked violently. I thought I had hit something in the road. (I was driving in one pedal mode). There was nothing in the road, and I had not collided with anything. The car just decided to brake because it incorrectly sensed something in my way. I was almost rear ended, and it was very frightening. My safety was at risk as well as the person behind me. I have an appointment with the service department at the dealership to address it. This car has been in for service 5 times in the 18 months I have leased it. I have complained to the dealer multiple times, and they have basically ignored me.
