The digital instrument cluster and infotainment/display system repeatedly FAILS WITHOUT WARNING WHILE I AM DRIVING. The screens suddenly glitch, shut down, or go completely black, causing me to lose access to critical vehicle information. The vehicle remains available for NHTSA or manufacturer inspection upon request. This is an ongoing and unresolved safety defect. I did NOT experience the blank-screen condition before Honda Recall 26-056. The recall specifically addressed a condition that could cause the instrument cluster/infotainment display to intermittently go blank while driving. The recall repair was performed April 13–14, 2026. Within approximately one week AFTER that repair, my screens began randomly shutting off while driving. When the failure occurs, the displays can remain blank for approximately 5–20 seconds. During that time, I unexpectedly lose visibility of critical information, including vehicle speed and warning indications. A driver should never suddenly lose access to essential operating and safety information while a vehicle is moving. The unpredictable failure also creates a serious distraction because I must continue driving without knowing when the displays will return. Honda dealers have made MULTIPLE unsuccessful repair attempts. On April 20–22, the dealer performed a factory-data reset. The failure continued. On May 4–7, the dealer contacted Honda Techline, identified an internal defect involving the A11/radio control module, replaced the module, and programmed/configured the system. The defect STILL was not corrected. I reported the continuing screen glitching/blanking on August 6, 2026. There is generally NO WARNING before the screens fail. They can suddenly go completely blank while the vehicle is being driven. Despite a recall repair, subsequent diagnosis, software/programming work, a factory reset, and replacement of an electronic control module, the same dangerous malfunction continues to occur.

I am reporting a serious safety concern involving my Honda Prologue and believe the repeated failures may indicate an unresolved defect involving the vehicle’s electronic, braking, ABS, propulsion, transmission/shift, or vehicle-control systems. On [XXX], the vehicle first displayed an error/warning. I contacted the Honda dealership in Vancouver, Washington, and was instructed to bring it in for inspection. I was out of town and could not do so immediately. On [XXX], I brought the vehicle to the dealership in the morning. They kept it overnight and returned it to me on [XXX] at approximately 2:00 PM, telling me the problem was a “system glitch” and had been fixed. On [XXX], less than 24 hours later, I was driving home after completing a Lyft ride when the vehicle displayed: “One-Pedal Driving Unavailable – Use Brake for Stopping.” At the same time, the ABS warning and multiple other safety-system warnings illuminated.The vehicle then became immobilized in the middle of an intersection. I was in an unsafe location and had to contact police for assistance. The vehicle remained unable to operate for approximately two hours. After multiple restart attempts, it suddenly became operational again, allowing me to drive it to the Honda dealership.I have photographs and videos documenting the warnings. One photograph shows the “One-Pedal Driving Unavailable – Use Brake for Stopping” warning with the ABS and multiple other warning indicators illuminated. Another shows “Service Transmission Now – Unable to Shift Soon.”This is especially concerning because Honda had already been notified, had possession of the vehicle for approximately a day and a half, and told me it had been repaired. The vehicle then experienced a serious failure the following morning and became disabled in traffic. This is not simply an inconvenience. My 2024 Honda Prologue (VIN [XXX]) produces a loud clicking/knocking noise from the front drive axles when turning, most noticeable during low-speed turns and when accelerating out of a turn. The noise began shortly after I acquired the vehicle at low mileage, consistent with the widely reported front CV joint/axle defect affecting 2024-2026 Prologues. This is a safety concern, not merely a noise. The defect involves internal drive-axle joint irregularities that can progressively worsen and impair propulsion, steering response, and drivability, with risk of eventual axle failure and loss of vehicle control while driving. Honda acknowledged this condition in a XXX dealer Tech Line but directed dealers not to repair it if no visible damage is found, leaving the defect unaddressed. Replacement axles reportedly fail again. No effective repair is currently available. I request that NHTSA investigate this drive-axle defect. I went to charge my car Sunday morning, and throughout the day it kept showing on the dashboard that the charge door was open. I stopped several times to check, and the charge door was actually closed, which was really strange. Later, I went into a store for less than 10 minutes. I locked my car before going inside, but when I came back out, all the windows were down. It honestly scared me because I have no idea how that could have happened. I shared my experience with a coworker who has the same car, and he told me the exact same thing happened to him. It’s so strange and concerning. This is the first time it has happened to me, but now I’m nervous about leaving anything inside the car, wondering if the windows could randomly open again.

While getting ready to parallel park the car, the entire screen system (dashboard, center console) went completely blank. This completely eliminates visibility to all safety aids, speed of vehicle, and warning systems. WHEN I USED ADAPTIVE CRUISE CONTROL, MY CAR TENDS TO LEAN SLIGHTLY TO THE RIGHT. IT VEERS OVER A BIT AND THEN JERKS TO THE LEFT. OTHER THING, WHEN I USED ADAPTIVE CRUISE CONTROL FOR ALMOST ONE HOUR, THEN IT SUDDENLY BECOMES INACTIVE. IF I HAVE BEEN FOLLOW ANOTHER CAR, then what should happen when it suddenly becomes inactive??? I was very lucky at that time because there was no car in front of me. After 5,000 miles, my car began to make a slight noise on the right side when I slowed down in the city. Maybe it happened in the front drive axle assemblies or the front right low arm control.

I have had two safety issues with this vehicle where there were system failures that did not record DTC codes. And an additional failure that does not record codes and cannot be duplicated. After leasing the vehicle for seventeen months, and during repair for an unrelated non-safety issue, it was discovered that the steering wheel airbag had a loose connection. No warning appeared at any time on the instrument panels. As well the one pedal function would switch into regular drive mode, as in a gas powered vehicle, randomly. After multiple tries and incidents the dealer service department has been unable to duplicate. Then on March 6 of this year I bought in the vehicle yet again for this one pedal failure, and was notified there was an existing recall. This software update was performed. On March 17, just 11 days after the update, while backing into my garage with the garage door going up the windshield wiper was clipped by the door, destroying both the wiper and shattering the back window. I have backed into my garage thousands of times and never had an issue. Why this happened on this date I do not know. What I do know is that the vehicle gave no warnings and did not break for the object. At the time my dogs were clipped into their seat belt harnesses in the center well. By a miracle no glass shattered beyond the back storage area and I was able to safely remove them. I checked after and all of the collision warning systems were listed as active, including automatic braking for rear collisions. I had the dealership run scans twice and all scans showed no DTC codes and all systems active. I can only assume that all prior updates have failed, and there is a substantial defect to the vehicles ability to communicate. I have a copy of the scan performed. The second scan was run on April 9 because the dealership failed to provide a printed copy of the first. I have that second scan. My vehicle fails to communicate or activate essential information, a safety hazard.

The issue I'm experiencing with my 24 Prologue is the infotainment screen (which controls a lot of the vehicle) sometimes having issues showing the backup camera, or issues with the car not allowing me to utilize ACC (says temporarily unavailable), etc. I feel less safe in the vehicle as a result. The microphone for hands-free calling in the vehicle is not installed properly. It is well known that at the factory where they made these Prologues, they accidentally left the plastic on the microphone, and also did not click it into its place. The microphone is currently floating somewhere in my ceiling, sealed in a plastic wrapper. Whenever I try to use hands-free calling, nobody can ever hear me, so I have to take my hands off the wheel and eyes off the road, pick up my phone, switch it to speaker, and then I will be able to talk to people. This is a major safety concern caused by human error at the factory. It defeats the purpose of hands-free calling, increases the risk of a crash, and forces people to break the hands-free law in most states. Because it is not easy to remove panels and access the microphone floating in the ceiling, this should be recalled.

On Feb 7th, notice a Knocking/Clicking noise coming from right wheel when making slight right turn, but not often. Called my dealership and immediately they said they were aware of issue and said it is common in Prologues. But due to the prologue being a GM make and not Honda, they were told they needed to wait till GM comes up with a solution, as well as parts being backordered. A few weeks later, knocking noise got louder and happening more often. Concerned I asked them to take a look at vehicle. I brought it in on March 4th, tech took it for a drive and yes confirmed issue and told me we just needed to wait or callback in a few months. As of today, April 11th, noise is extremely loud & always happens when making right turns, and is continuous through the whole duration of turn. Now we are slowly hearing it on left turns and when reversing.

To whom it may concern, I am writing to formally express my profound dissatisfaction and escalating concern regarding a persistent mechanical issue with my Honda vehicle, which remains unresolved despite numerous attempts to secure a repair since its purchase in June 2024. Upon acquiring my brand-new vehicle in June 2024, I soon detected a distinctive clicking or thumping sound emanating from the steering mechanism whenever the wheel was turned. I promptly brought the vehicle to Lakeland Regal Honda's service department for diagnosis. A service technician conducted an assessment, confirming the audible anomaly, but I was subsequently informed of a parts unavailability and assured that I would be contacted once the necessary components arrived. Regrettably, no such communication was ever received. The issue persisted unabated throughout 2024, prompting a second visit to Lakeland Regal Honda. During this subsequent appointment, the service department acknowledged the ongoing problem, noting it as a prevalent concern among several customers. I was informed that the component in question, reportedly related to the axle assembly, was currently on backorder and that the dealership was awaiting a factory shipment from Honda. Once again, I was promised a follow-up, a commitment that, unfortunately, was not fulfilled. The situation has now extended to April 8, 2026, nearly two years since the initial report. Upon contacting Lakeland Regal Honda once more, I was advised that the dealership was no longer authorized to procure the necessary parts for this specific issue and that I should direct my inquiries to Honda corporate services. This directive was particularly frustrating, as I had already undertaken this action previously, only to be informed that the part remained on backorder and that I would be contacted. On April 8, 2026, I again contacted Honda corporate. A representative recorded my details and assured me the matter would be escalated to a case management .

The axle on the vehicle clicks when turning. Honda offered to replace the part and I took the vehicle in, only to be told that Honda is not fixing this part because it is a known defect with no fix. This was in December 2025. As of this date, no fix has been found. Additionally, the driver’s side window makes a ton of wind noise when driving over 30mph. Honda failed to find the cause for the issue and could not fix the car after having it for over a week in early 2025.

I need NHTSA assisting in resolving safety recall # 26V112, Instrument Panel and Rearview Camera Displays May Fail/FMVSS 101,111,305. I took my Honda Prologue vehicle to Honda dealer for the recall to be address. The dealer started the update process but failed to complete the update process and is now refusing to continue with the safety recall. The update process is to bring the vehicle operating system to the new and improved version to address failures as reported. The operating system that has the known failures is version Android 12, released in April of 2024. The new and improved operating system is version Android 14, released in May of 2025. This new and improved version were release to address the known failures. My vehicle is still operating on the same outdated operating version (Android 12) knows to have these failures. I have been in contact with Honda corporate trying to address this safety recall to no avail. I need assisting in resolving this safety recall as I do not feel safe driving my vehicle on the road. I am requesting a response regarding this matter.

The onboard navigation and entertainment system, including both the primary dashboard/driver interface and the center console, went fully black for over 1 minute while driving on the highway. When the screen is blank, no input is available from the vehicle as to speeds, behavior of cruise control, warning systems such as lane control, front rear and side awareness, operator awareness of speed, other vehicle warnings. Bringing to dealer today. this is the 2nd time this has occurred. The first time was in December. Vehicle was brought to dealer for software update. Update “bricked” the car, and a new radio module needed to be ordered. This took about 6 weeks. Module installed, software was updated, and this was to have addressed the issue. While records may indicate that the update was not yet performed on the vehicle, it was fully updated and the vehicle (according to the dealer) is fully up to date and the recall for it has been addressed with the software update. However, the update did not address the issue. No other inspections have occurred to date. No other symptoms or warning lights.

When Turning to right and left there is rattling sounds mostly on slow turn. I brought to Honda dealership drive test it with their Tech. They said might be drive shaft or CV joints. But they did not fixed because they did see loose or bad since the car is new parts may not be available. yet they told I can still drive it .
