I am reporting a serious safety concern involving my Honda Prologue and believe the repeated failures may indicate an unresolved defect involving the vehicle’s electronic, braking, ABS, propulsion, transmission/shift, or vehicle-control systems. On [XXX], the vehicle first displayed an error/warning. I contacted the Honda dealership in Vancouver, Washington, and was instructed to bring it in for inspection. I was out of town and could not do so immediately. On [XXX], I brought the vehicle to the dealership in the morning. They kept it overnight and returned it to me on [XXX] at approximately 2:00 PM, telling me the problem was a “system glitch” and had been fixed. On [XXX], less than 24 hours later, I was driving home after completing a Lyft ride when the vehicle displayed: “One-Pedal Driving Unavailable – Use Brake for Stopping.” At the same time, the ABS warning and multiple other safety-system warnings illuminated.The vehicle then became immobilized in the middle of an intersection. I was in an unsafe location and had to contact police for assistance. The vehicle remained unable to operate for approximately two hours. After multiple restart attempts, it suddenly became operational again, allowing me to drive it to the Honda dealership.I have photographs and videos documenting the warnings. One photograph shows the “One-Pedal Driving Unavailable – Use Brake for Stopping” warning with the ABS and multiple other warning indicators illuminated. Another shows “Service Transmission Now – Unable to Shift Soon.”This is especially concerning because Honda had already been notified, had possession of the vehicle for approximately a day and a half, and told me it had been repaired. The vehicle then experienced a serious failure the following morning and became disabled in traffic. This is not simply an inconvenience. Please see details of NHTSA Campaign #25V433000. This is a known issue associated with a platform technology shared by GM and Honda. I purchased the vehicle as CPO on 31 July and almost immediately the parking brake warning light and the 'Service Transmission Now, Unable to Shift Soon' error code appeared on the dash. I returned the car to the dealer on 1 Aug for repairs and was told this is a 'known issue' with Honda Prologues, but there is no recall in effect for Honda. How is this possible? I am concerned with the degree of risk I could have potentially encountered due to a sudden brake failure or being stranded.

While driving my 2024 Honda Prologue Elite at anywhere from 10-30 MPH, the One-Pedal Driving system suddenly and without warning faults out and disabled mid-drive. The regenerative braking abruptly cut out, causing a sudden loss of expected deceleration, creating a 'lurch forward' feeling. A dashboard warning light illuminated instructing me to press the brake pedal immediately. This has happened multiple times, causing a severe safety hazard during normal traffic conditions. The vehicle shares an identical GM Ultium electronic brake modulator architecture with other previously recalled vehicles. The car is unsafe and should be considered for recall repairs. I have read that this is happening to others and Honda really needs to address this issue.

Description of Safety Defect: The Automatic Emergency Braking (AEB) / Forward Collision Avoidance system is severely malfunctioning. On multiple occasions, the vehicle has experienced "phantom braking"—abruptly slamming on the brakes for no reason whatsoever. There were no obstacles, pedestrians, or vehicles ahead to trigger the system. This malfunction is happening at high speeds on major highways. Safety Risk and Impact: This issue represents an immediate and terrifying safety hazard. Unexpectedly slowing to a sudden stop at highway speeds completely compromises vehicle control and leaves me at an extreme risk of being rear-ended by high-speed traffic. It is a severe danger to myself, my passengers, and every other driver on the road. While the vehicle is currently subject to a manufacturer recall regarding the backup camera and other software updates, this specific, dangerous braking behavior makes the vehicle incredibly unsafe to operate. I request that the NHTSA review this issue immediately to determine if a broader safety investigation or expanded recall is required for the 2024 Honda Prologue.

While approaching traffic light at approx 4-5 mph the vehicle slammed to a dead stop with a car length between us and the next vehicle. This was not the auto stop feature and it automatically engaged the parking brake while moving causing whiplash To myself and 2 additional passengers. Causing neck and back injuries. The vehicle was coming to a stop with the assistance of the one pedal drive braking at approximately 5 mph and suddenly while slowing down with more than adequate distance it slammed to a dead stop violently and unexpectedly throwing my passengers and I against our seatbelts causing injuries. Several other occasions the vehicle has had phantom emergency braking with no obstacles, vehicles or pedestrians near it but at slower speeds and did not cause as much trauma. This event was significant and all 3 parties myself included are experiencing pain related to the event.

While driving normally, my 2025 Honda Prologue experienced an immediate, catastrophic mechanical and electronic failure in both the steering and braking systems. The steering wheel completely locked up/failed to respond, and the brakes became completely non-functional simultaneously, causing an absolute loss of control of the vehicle. Due to this sudden system failure, the vehicle veered off course and crashed, resulting in severe structural damage, complete destruction of the front passenger-side suspension, and rendering the vehicle a total loss. American Honda Motor Co. Corporate was immediately notified of this severe safety defect (Case No. 16372717. However, Honda Corporate has explicitly refused to send an engineer or inspector to evaluate the vehicle's electronic or mechanical logs, and they also refused to provide their denial statement in writing. The vehicle remains un-repaired as evidence of this dangerous factory defect.

After charging the vehicle. We reversed out of the parking stall. The vehicle braked really hard with no warning as if we hit something, but there was nothing for us to hit in the parking lot. The braking was so hard my body jerked! This is the second time the emergency braking engaged sudden. The first time a vehicle was near and the warning light did indicate vehicle but not close enough for the braking system to engage. It could have caused the vehicle to hit me from the rear if it was closer.

Shortly after purchase, I observed damaged/torn underhood insulation and reported the condition to the selling dealership, with concern that the vehicle should be inspected for related issues. Prior to a crash incident, I experienced concerns involving braking-related behavior and advanced driver assistance features, including lane assist and object/collision warning functions. During the crash, while reacting to a roadway hazard, the vehicle swerved and struck a pole, and I did not experience warning or intervention from driver assistance features as expected. Following the incident, a diagnostic pre-scan identified multiple faults involving brake control, radar sensors, image processing, and communication systems. Since the incident, similar concerns have continued, including recurring braking/driver assistance concerns, and the issues remain unresolved. I am submitting this complaint due to concern that braking and advanced driver assistance systems may not have functioned as intended and may present an ongoing safety risk.

High voltage system error has flagged and been fixed multiple times starting in early January of 2026. Car randomly warns and breaks in the middle of the road because it sensed something that wasn’t there. Luckily no wrecks have occurred. Both CV joints have a loud clicking noise that the Honda dealerships knows about but has no fix for. I currently have an appointment for the dealership to look at my brake and high voltage system again.

While driving with cruise control engaged, the car had a false positive for forward collision detection and slammed on the brakes at full Highway speed. After about one of second of emergency braking the car released the brakes and resumed cruise control as normal. There were NO other vehicles on the road in front or to the side of us at the time. Due to the unexpected braking, the vehicle following us almost collided into us. The event triggered just before the shadow of the bridge was on road of the underpass. I suspect it was the shadow that might have contributed to this event. However, this is not the first time the vehicle has had false positives in the forward collision detection system. We have experienced many of these, but typically at lower speeds. Generally speaking the “triple beep” and associated Heads Us Display red “brake” symbols briefly flash, then goes away without engaging the brakes automatically, again with these there are no other vehicles present. We have called two different dealers to report this and they tell us there is no associated TSB/service bulletin or recall for our vehicle and no software update for this. We tell them that GM has had several software updates for their Ultium platform with their other vehicles to fix this, but there is still nothing from Honda. There is a SB 25-051 for the 2024 prologues that would seem to address this but it’s not applicable to the 2025 according to them even though the symptoms are the same. This needs to be looked into as BOTH dealers told us that they had several other customers complaining about the same thing.

The primary safety incident occurred while driving on a highway. Multiple warning indicators appeared simultaneously, including battery and maintenance alerts, and the vehicle began to decelerate automatically. When the brake pedal was applied, the brakes were completely unresponsive, creating a life-threatening situation. I was forced to continue driving for approximately one mile before bringing the vehicle to a stop using the parking brake. This incident placed both myself and other road users at significant risk due to the inability to slow or stop the vehicle under normal braking conditions at highway speed. The vehicle was towed to a Honda dealership (Folsom Lake Honda) for inspection and repair. The dealer returned the vehicle stating that the issue had been addressed. However, the same warning messages reappeared, and a second incident involving brake malfunction occurred, confirming that the problem was not resolved. The issue has been inspected by an authorized Honda service center, and the vehicle was diagnosed and repaired once, but the defect recurred after the repair. The vehicle has also been reported to the manufacturer (American Honda Motor Co.) for further action. This is a serious safety concern involving loss of braking capability at highway speeds, and the problem has occurred more than once despite attempted repairs.

On 2/19/2026 I was taking an freeway exit ramp from I 15 south to Carrol Carrol canyon and I stepped on the brake but the car did not slow (my perception was it was speeding up), the vehicle collision warning did not sound and the vehicle did not emergency brake, therefore I hit a stopped vehicle at a stoplight. Vehicle: 2025 Honda Prologue Location: Sacramento, California Date of Incident: [XXX] What failed or malfunctioned The vehicle experienced a sudden loss of directional stability involving the steering system, brake-by-wire system, and electronic stability control (ESC). Steering and braking inputs did not produce proportional vehicle response. What happened While driving under normal conditions, the vehicle began drifting to the left. I attempted to steer back to the right and applied braking; however, the vehicle did not respond as expected. Electronic stability control did not appear to engage or correct the loss of control. The vehicle then deviated from its intended path, overturned, and struck a fence. The vehicle was declared a total loss. No other vehicles were involved, and no injuries occurred. Safety risk This failure created an immediate and serious safety risk. The loss of steering and braking effectiveness could have resulted in severe injury or fatality. The rollover demonstrates a critical safety concern. Warning signs No warning lights, messages, or symptoms were present prior to the failure. The event occurred suddenly without notice. Inspections The vehicle was inspected by police and insurance representatives. The manufacturer has not yet inspected the vehicle. The vehicle remains available for inspection through the insurance provider. Dealer confirmation The issue has not yet been reproduced due to the vehicle being totaled. Additional concerns The vehicle’s behavior was inconsistent with normal operation of safety-critical systems designed to maintain vehicle control. I am concerned about system performance, calibration, or possible electronic control failure. Manufacturer contact I reported this issue to Honda Customer Care by phone and requested escalation to engineering review.
