FORWARD COLLISION AVOIDANCE: WARNINGS

My 2025 Hyundai Tucson (VIN [XXX]) has been out of service since [XXX] (over 60 days) at CMA’s Colonial Hyundai in South Chesterfield, Virginia, waiting for a front camera related to safety Recall 302 (NHTSA Campaign 26V316 – Front View Camera FCA Update). The dealership previously replaced the front camera, but the issue was not resolved. They then ordered a new camera that remains on backorder with no firm ETA. I have been provided a rental during this time. Hyundai assigned Case #[XXX]. I have provided formal written notice under Virginia lemon law demanding a buyback because the vehicle has been out of service far beyond the 30-day threshold. The safety-related defect remains unrepaired. FORWARD COLLISION AVOIDANCE: WARNINGS

My 2025 Hyundai Tucson required the front brake rotors to be resurfaced at approximately 12,200 miles. The Hyundai dealership diagnosed the rotors as overheated and warped, despite the brake pads being in good condition. The repair occurred before Hyundai issued Recall 302 (NHTSA Recall 26V-316). Hyundai Recall 302 identifies a defect in the Forward Collision-Avoidance (FCA) system’s front-camera software that can cause the FCA system to engage earlier than expected and potentially result in sudden braking. Hyundai’s FCA system is capable of automatically applying the brakes. I am concerned that premature or unintended brake application associated with this recalled software condition may have contributed to excessive brake heat and premature overheating/warping of my front rotors. I am NOT claiming that this causal relationship has been established. Rather, I am reporting a potential safety defect and asking NHTSA to evaluate whether there may be a relationship. The dealership confirmed that the rotors were overheated/warped but characterized the condition as wear and tear. Hyundai Motor America subsequently denied my reimbursement request on the same basis. I have repeatedly asked Hyundai to evaluate whether Recall 302 could have contributed to the brake condition, but Hyundai has not acknowledged or technically addressed this potential relationship. The original rotors are no longer available for inspection because they were resurfaced during the repair. The dealership performed and documented the diagnosis and repair. I am submitting this complaint because I believe the combination of premature rotor overheating/warping at only 12,200 miles and a subsequently identified FCA defect involving unexpected braking warrants investigation.

FORWARD COLLISION AVOIDANCE

The contact owns a 2025 Hyundai Tucson. The contact stated that while driving at various speeds, the Forward Collision Avoidance Assist (FCA) warning light was intermittently illuminated. The dealer was notified of the failure and attributed the issue to inclement weather. The contact stated that while stopped, the vehicle was rear-ended because the brake lights failed to illuminate. The contact sustained lower back and shoulder injuries, was transported by ambulance to the hospital, and required ongoing injection treatments. A Police report was filed. The vehicle was towed to a dealer and repaired for rear-end and passenger-side damages; however, the cause of the brake lights' failure was not determined. Additionally, the contact stated that while attempting to accelerate from a complete stop at a traffic light, the vehicle stalled and was struck on the front driver's side by another vehicle. The crash caused the vehicle to spin, and the front end detached. The air bags failed to deploy, and a system failure message was displayed. The contact sought medical attention the following day. The contact sustained a dislodged left thumb bone which required surgery, a cast, and injections for left arm pain, as well as shoulder, neck, hip, and aggravated lower back pain. A Police report was filed. The second failure had occurred at [XXX]. The vehicle was towed to the auto body shop at the dealer and repaired; however, neither the dealer nor the insurance adjuster determined the cause of the vehicle stalling. FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

This is not the first situation NOR the first time it is being "fixed". I took my car in early March for an oil change and informed the representative about the same thing. The time that made me say something is when I was switching lanes and the car must have sensed me being close causing me to stop completely and the car behind me having to hard break and swerve. Rick Case Hyundai in Duluth, Georgia had my car for a MONTH, just for me to experience the same thing over 3 times since then. I received my car in April, and the recall for this wasn't put out until May. I am constantly scared for my life and the passengers I may have lives, as well as the other drivers lives, considering I got this car due to being in a rear end collision. When I took it in April, they CONSTANTLY, drove my car to diagnose it but kept telling me they didn't see the problem, and it wasn't coming up for them on their internal tracking system for their cars(webDCS). Once they finally talked to the CORPORATE field technician, they advised they replace the whole front bumper, including sensors, for the solution. But like stated before, since then I have experienced near crash stops AT LEAST 3 times since then, two of them being in the same month (July 16 and July 28). If this is the same solution, I don't see it being beneficial and I would like to be bought out at this point. Especially if I have to be without my car for a long period of time again.

The contact stated that the vehicle decelerated to 60 MPH and the brake pedal was abnormally firm. The contact stated that the animals in the rear of the vehicle were thrown into the front seat of the vehicle. The contact stated that the seat warmer independently turned on, the touch screen activated and deactivated, and the oil change warning light illuminated. The contact received notification of NHTSA Campaign Number: 26V316000 (Forward Collision Avoidance). The vehicle was taken to the dealer; however, the failure was not duplicated. I have had nothing but trouble with software issues since the very first recall (software related) on my vehicle, just a few months after I purchased it. I have had a very difficult time having Hyundai address these issues, and recently opened my third case with Hyundai. The issue: On 6/24/26 Peddar Hyundai conducted the software update for the recall regarding the front collision system. Later in the day on 6/24/26 and in the days following, whenever I turned on the car, Googlemaps would automatically pop up on the infotainment screen, the car’s navigation voice (not the googleMaps voice) would instruct me to turn right or left, etc. throughout the drive, despite no route being set. I have a video of this occurring. MOST CONCERNING: 6/25/26 I was pulling out of a parking lot, with no other cars or obstacles in my path, going slowly, and the automatic braking system ground the car to a stop. There was no warning on the dash. It took a few seconds before the car would allow me to accelerate. This is the very problem the software update was supposed to fix. 7/2/26 I was using GoogleMaps navigation at night and as I was driving, the infotainment system went blank and I had no navigation. After a minute or so the map came back on, but it was just spinning in circles and couldn’t right itself. Additional issues: the infotainment system randomly keeps popping up the question, asking if I want to open Google Maps. Sometimes when using the parking camera, an informational window keeps popping up stating “Press the button to display the camera view, or press and hold to activate the parking assist system”. I use this function every day, but for some reason, at times it keeps trying to teach me how to use it. (These types of glitches have happened before after software updates.) 6/26/26 I opened a case with Hyundai. Case number 42790134. I have yet to hear from them. I've called several times and they won't return my call or address this safety issue.

The automatic emergency braking system activated early and aggressively causing a rear end collision. It is available for inspection upon request. While driving in busy traffic on a highway I was lightly braking to avoid sudden stops or unexpected movements of the vehicle while traveling between 20-30 mph. The vehicle then activated the emergency braking system and came to a sudden and unexpected complete stop. This sudden stoppage caused myself and my passenger to be thrown forward harshly against the seatbelts. Additionally I was unable to steer the vehicle towards the side of the road as the wheel had locked and avoid an SUV that was rapidly approaching my rear. This SUV subsequently rear ended me. The actual collision was minor thankfully due to the low speed of the traffic, but could have been much more severe if traffic conditions had been different. Has the problem been reproduced or confirmed by a dealer or independent service center? Not yet. (I am in the process of this right now, but because my car was in a collision the dealer will not fix the recall or inspect the vehicle until the collision damage is fixed) Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The contact owns a 2025 Hyundai Tucson. While the contact’s son was driving approximately 55 MPH on a three-lane highway, the brakes locked up. While attempting to move the vehicle to the shoulder, the driver lost control of the vehicle and struck the guardrail and rolled over several times. The air bags deployed, and the glass shattered. There were no injuries reported. A Police report was filed. The contact associated the failure with NHTSA Campaign Number: 26V316000 (Forward Collision Avoidance). The vehicle was towed from the scene to a tow lot. The local dealer was not contacted, and the vehicle was not diagnosed or repaired. The contact stated that while driving at an undisclosed speed, another vehicle slowed down in front of the vehicle, and the Forward Collision Avoidance system activated, the brakes engaged, and caused another vehicle behind to crash into the rear of the contact’s vehicle. No warning lights were illuminated. A Police report was not filed. No injuries were sustained. Medical attention was not needed. The air bags did not deploy. The vehicle was taken to an independent mechanic, where it was diagnosed, and it was determined that the rear bumper, rear hatch, and several other unknown parts under the bumper needed to be replaced. FORWARD COLLISION AVOIDANCE: WARNINGS

I had the Hyundai recall 302 performed on 5/27/2026 and after the software update none of the front sensors worked. My wife had a 2 mph bump with the car in front of her on [XXX]and there was no warnings, also no damage no injuries that I'm aware of. So I started to investigate why there was no alarm sounded or any red warning light on the dashboard. Pulled the car into the garage and up to 6 inches from the garage wall no warning on the dash. Took the car back to the dealership on 6/5/2026 they changed some settings and it still failed to warn of an still failed. The gave me a service appointment on 6/9/2026 they calibrated the front windshield sensor and now the parking assist sensors worked. When I pulled into the garage the front camera showed yellow and red warnings as I approached the wall. That fixed the parking sensors issue. Still no forward collision avoidance warning shortly there after was referred to the service manager Rich Block he stated that the front bumper cover would need top be removed to calibrate the radar behind the bumper, but Hyundai's warranty would not cover it and, I should call Hyundai Customer care. I did and was contacted by Mohamed at National Consumer Affairs (714) 922 - 3276. and here we we sit. when they pulled the bumper they found the mounting bracket was bent. Mohamed at National Consumer Affairs (714) 922 - 3276 stated could I prove that all this happened after recall 302. I can only prove the parking sensor issue because after recalibration they worked. I was driving in a line of cars when there was a sudden slowing of all cars ahead of my 2025 Hyundai Tucson Hybrid. Car's Forward Collision Avoidance system applied brakes too early and too hard. Car behind me almost collided with my car's rear. My 2025 Hyundai Tucson Hybrid had less than 1,300 miles. I confirmed this early collision avoidance early braking incident to salesman, Stephan, on May 7, 2026 during service at Piazza Hyundai of West Chester. I explained it was a near collision by car behind me. No action taken by dealership, and no recall information was provided to me.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

On XXX, my vehicle unexpectedly activated its automatic braking system while driving, resulting in a rear-end collision. My infant daughter was in the vehicle at the time. While we were fortunate to avoid catastrophic injuries, the incident placed both of us in significant danger and could easily have resulted in a much more serious outcome. The other driver may also have sustained injuries as a result of the collision. The vehicle sustained approximately $20,000 in damage. Since the accident, I have learned of Hyundai’s recall involving this braking system. Given the circumstances, I am deeply concerned that the condition addressed by the recall contributed to the collision. What is particularly troubling is that this was not the first time I raised concerns regarding the vehicle’s braking behavior. On February 23, 2026, at approximately 11:00 a.m., I spoke with my dealership, North Bay Hyundai in [XXX] regarding unexpected braking behavior that I was experiencing with the vehicle. My concerns were not escalated, and I was not advised that a potential safety issue might exist. What component or system failed or malfunctioned, and is it available for inspection? The forward collision avoidance (FCA) system malfunctioned. The front camera system appears to have triggered unintended hard braking beyond normal driver input. The vehicle is available for inspection upon request. How was safety put at risk? While approaching a vehicle in front of me, I was gradually slowing down and applying the brakes normally. Without warning, the vehicle suddenly applied significantly stronger braking than intended. This abrupt and excessive braking caused the vehicle behind me to collide with my car, creating a dangerous situation and increasing the risk of injury. Has the problem been reproduced or confirmed? UNKNOWN. The issue has not been reproduced since the accident, and it has not yet been confirmed by a dealer. However, the incident is consistent with Hyundai Recall NHTSA Campaign Number 26V316000, which describes unexpected and premature activation of the forward collision avoidance system. Has the vehicle been inspected? The vehicle has been inspected by insurance representatives following the accident. It has not yet been inspected by Hyundai or a dealership specifically for this recall-related issue. Were there any warning signs before the failure? There were no warning lights, alerts, or messages prior to the sudden braking event. The braking intervention occurred without advance notice. Additional description of incident: At the time of the incident, I was approaching a vehicle ahead and applying the brakes gradually under normal driving conditions. The vehicle then suddenly applied much stronger braking than expected, consistent with automatic emergency braking intervention. This abrupt deceleration caused the vehicle behind me to rear-end my car. This behavior matches the defect described in Hyundai Recall 26V316000, where the forward collision avoidance system may activate unexpectedly and apply excessive braking.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

My vehicle has an ongoing safety‑critical defect involving the braking system, powertrain, and ADAS logic. I have not driven the vehicle since 04/27/2026; all diagnostic trouble codes were recorded while the vehicle was in the dealership’s possession. On 05/05/2026, the vehicle stored multiple DTCs, including C164286, P071300, P076800, U060FF1, U060F29, and U061386. On 05/13/2026, P071300, P076800, U060FF1, U060F29, and U061386 were recorded again, showing repeated powertrain, communication, and control‑system faults while the vehicle remained with the dealer. On 05/22/2026, two additional safety‑related DTCs (P107600 and C272146) were first recorded, and both were last reported again on 05/27/2026, still while the vehicle was in dealer custody. These DTCs now show as Historical because the vehicle has not been operated since the last fault was recorded. Their Historical status only means the system has not run a new check — it does not mean the defect is repaired. The vehicle has also exhibited brake‑logic abnormalities, including a persistent Brake‑ON signal at highway speeds, which violates Brake‑Throttle Override safety logic and affects ADAS functions. The powertrain shows signs of torque converter instability, mechanical drag, abnormal slip, and heat buildup documented across these dates. Two OTA updates were performed during periods of instability, but neither corrected the defect. The dealership has not provided documentation confirming the safety issues were resolved. Because the braking logic, powertrain behavior, and repeated DTC activity remain unexplained and unresolved, I do not believe the vehicle is safe to operate.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

The forward collision automatic braking happens randomly and for no reason causing immediate and complete stops in the middle of traffic. It whips the head forward as if a crash had occurred and causes drivers to the rear to have to suddenly brake to avoid collision. Ive had it occur multiple times since purchase with the most recent being this morning in the middle of a roundabout. I know I can disable the system but 1) that's also a safety risk and 2) it turns back on every time I start the car which is unacceptable. Ive asked the dealership about it and was told that it cannot be disabled. I already have the sensitivity turned down as much as possible and it still is happening. This issue makes an otherwise wonderful vehicle almost uunrivaled. When this happens there's no warning before the sudden braking. It also will randomly just slow speed down for no reason and with no way for the driver to stop it. Something needs to be done. Fix the system and make available a way to disable it without it auto turning back on. Im fed up.
