Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. Nissan North America, Inc. (Nissan) is recalling certain air bag inflator replacement service parts that may have been installed as part of a theft or collision repair. The parts include certain passenger-side air bag inflators designed to fit Infiniti 2002-2003 QX4, 2006-2010 M35 and M45; Nissan 2001-2003 Maxima, 2002-2006 Sentra, and both driver and passenger-side inflators designed to fit 2007-2012 Versa Sedan and Versa Hatchback vehicles. VEHICLE SPEED CONTROL:ACCELERATOR PEDAL

Nissan North America, Inc. (Nissan) is recalling certain model year 2006-2010 Infiniti M35 and M45 vehicles manufactured April 4, 2004, through October 5, 2010. Over time, the accelerator pedal sensor signal may deteriorate resulting in the output of an incorrect signal causing the engine to go into fail-safe (limp home) mode.

Risk: In this fail-safe mode, throttle valve deposits may cause the engine to stall when the vehicle is coming to a stop or at idle, increasing the risk of a crash.

Airbag

Issue: This recall is for certain vehicles that had an airbag inflator replaced as a part of an earlier recall campaign. This recall provides a final repair. Long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, may eventually degrade the propellant contained in the passenger-front airbag. This could cause the airbag to deploy with more force than normal. If the airbag inflator ruptures, fragments could be propelled toward vehicle occupants or cause damage to the airbag assembly, preventing its proper function. Safety Risk: If the vehicle is involved in a crash requiring an airbag deployment, the airbag inflator could rupture and create a risk of injury. Corrective Actions: Owners will be notified by mail and instructed to take their vehicle to a dealer to inspect the passenger-front airbag inflator. The passenger-front airbag module or inflator will be replaced as necessary. Note: The Canadian climate results in the propellant degrading slowly. This recall is being conducted as a precaution to address future risk. It is expected that all airbag inflators will be replaced before their function would be affected.

The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected.

Engine

On certain vehicles, the accelerator pedal throttle position sensor could fail. A faulty throttle position sensor will return the engine to idle. In certain cases, an abrupt return to idle could result in the engine stalling, causing a loss of motive power which, in conjunction with traffic and road conditions, and the driver's reactions, could increase the risk of a crash causing damage and/or personal injury. Correction: Dealers will replace the accelerator pedal throttle position sensor and reprogram the engine control module.

On 7/13/21 afternoon, I parked my car in a parking garage of Purdue University, located in West Lafayette, IN. Around 6 pm, as I was backing out of my parking spot on reverse gear and turning the steering wheel, the car seemed sluggish. So, I pressed the accelerator -- lightly, I believe . All of a sudden, the car just took off, moving backward very fast in a semicircle. It first hit the back of a parked car with its front right side and then a concrete wall with its back with tremendous force. I was alone in the car. I called my wife. She came from our home and called 911. The university police and medic arrived promptly, as did two of our friends. One friend drove my wife and me to IU Health Hospital emergency in Lafayette, IN, and the other friend waited at the accident spot for AAA to come. Apparently, AAA came a little later and towed the car to Nissan of Lafayette body shop. Based on the x-rays and CT scans done at the emergency, the attending physician's assistant told me -- apparently, after consulting with a neurosurgeon -- that I suffered two fractures in a neck bone ("transverse process of cervical vertebra"). She gave me a hard collar and asked me to wear it 24 hours a day, expect when taking a shower, for 6 to 8 weeks. She also asked me to see a neurosurgeon, whom I will see on 7/26/21. The body shop person I spoke with next day told me that the car would likely be totaled (the car is still there). Since then, I have been thinking why the car moved so fast and also searching the web to find out any reason for it. Could it be an instance of the "sudden acceleration" problem? I must say that I have been driving Infiniti cars for many years. I have been very pleased with them so far, as well as the service provided by Infiniti dealers, especially my current one, Dreyer & Reinbold Infiniti of Indianapolis. I have decided to inform NHTSA of this accident so that no other person driving an Infiniti car meets with an accident like this.

I am filing a complaint regarding a safety-related failure of the driver’s seat bracket in my vehicle. The bracket has broken, causing the driver’s seat to rock back and forth while the vehicle is in motion. I took the vehicle to an authorized dealer for inspection, and they confirmed that the seat bracket has failed. I was also informed that the replacement part is no longer in production and is unavailable. This condition poses a significant safety risk. A loose or unstable driver’s seat can impair vehicle control, increase driver distraction, and reduce the effectiveness of seatbelt and airbag systems in the event of a crash. As a result, this defect substantially increases the likelihood of an accident and the risk of serious injury. I am aware that there have been numerous similar complaints regarding this issue, yet no recall has been issued. Given the safety implications and apparent frequency of this defect, I believe this matter warrants further investigation and corrective action.

I was driving home recently and the back of my seat near the base gave out on me, the lower back/bottom raising motor was working but the seat was not going up. Upon inspection I found that the welds on the seat bracket had broken, going online and searching found me countless forums of seat brackets breaking during driving. Two main issues with this 1. I almost lost control due to the sudden drop 2. If I was to get into a collision the seat is not secure whatsoever. I have a 2007 Infiniti M35S that has been garaged kept since purchase. My dashboard has started to crack and exude a shiny, sticky, messy substance. I do not know whether this substance is toxic to the touch or my respiratory system. I have never used any type of cleaner or solvents on my dashboard, only a microfiber towel and water. I'm not sure how to answer how my safety or the safety of others is at risk since I don't know what the particular substance is. Secondly does the fragile state of the dashboard put me and my passengers at greater risk of injury with the discharge of an airbag. I have read several complaints where other Infiniti owners are experiencing the same issue with Infiniti vehicles circa 2006 - 2008. Infiniti should do the right thing and fix this problem!!

DURING NORMAL OPERATION, THE SEAT HAS FAILED IN IT'S ORIGINAL DESIGN INTENT. I BELIEVE THAT THE A FAILURE IN SOME COMPONENT UNDER THE SEAT CAUSED STRESS ON THE PIVOT POINT FOR UP-DOWN MOVEMENT IN THE SEAT AND OVER AN EXTENDED PERIOD OF TIME THE POINT FINALLY SHEARED. I BELIEVE THE EXTENSIVE STRESS WAS CAUSED BECAUSE OF THE AUTOMATIC SEAT MOVEMENT THAT OCCURS WHEN THE DRIVER ENTERS AND EXITS THE VEHICLE THE BRACKET WHICH ALLOWS THE SEAT TO MOVE UP AND DOWN WITH THE POWERED CONTROL HAS FAILED ON BOTH SIDES OF THE LEFT FRONT BUCKET SEAT. THE SEAT NOW MOVES FREELY FORWARD AND BACK AND POSES A SERIOUS SAFETY RISK TO MYSELF AND ANYONE ELSE WHO MIGHT BE DRIVING THE VEHICLE. THIS PUTS ME IN A TERRIBLE POSITION BECAUSE AS A CONCERNED CONSUMER I HAVE TO WORRY ABOUT THE FACT THAT AN ACCIDENT COULD BE FATAL DUE TO COMPONENT FAILURES. SHOULD I MOVE FORWARD WITH GETTING RID OF THE VEHICLE, EITHER BY SELLING IT TO ANOTHER PERSON OR TRADING IN THE VEHICLE, I PUT SOMEONE ELSE IN RISK AND/OR WILL TAKE A HUGE LOSS ON THE VALUE OF THE VEHICLE BECAUSE OF THE CONDITION WHICH WAS CAUSED BY POOR DESIGN AND/OR QUALITY TESTING.
