Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. In December 2019, the Office of Defects Investigation (ODI) opened this investigation based on 235 Vehicle Owner Questionnaires (VOQs) alleging doors failed to latch when closed, and/or inadvertently opened while driving, on model year (MY) 2012-2014 Ford Fiesta, and 2013-2014 Fusion and Lincoln MKZ. Ford Motor Company (Ford) is recalling certain 2013-2018 Focus, 2013-2019 Escape, and 2015-2016 MKC vehicles equipped with a 2.0L engine. The engine block heater may crack and develop a coolant leak, causing it to short circuit when the block heater is plugged in.

Risk: An electrical short circuit can increase the risk of a fire.

These vehicles were previously recalled and repaired under recalls 15V-246, 16V-643 or 17V-210, however the repair may not have been completed properly. A component within the door latches may break making the doors difficult to latch and/or leading the driver or a passenger to believe a door is securely closed when, in fact, it is not.

Risk: A door that is not securely latched could open while the vehicle is in motion, increasing the risk of injury to a vehicle occupant.

Ford Motor Company (Ford) is recalling certain 2014 Ford F-150 and E-350, 2014-2015 Ford Escape, and 2015 Lincoln MKC vehicles. Improperly manufactured bolts may have been used to install certain seats, seatbacks, seatbelt buckles, or seatbelt anchors on these vehicles.

Risk: The improperly manufactured bolts may fracture. If a seat, seatback, or seatbelt anchor attaching bolt fractures, the affected component may not be properly secured, increasing the risk of injury in the event of a crash.

A component within the door latches may break, preventing the doors from latching and/or leading the driver or a passenger to believe a door is securely closed when, in fact, it is not.

Ford Motor Company (Ford) is recalling certain model year 2015-2016 Lincoln MKC vehicles manufactured November 25, 2013, to January 25, 2016, and 2016 Ford Explorer vehicles manufactured October 20, 2014, to January 28, 2016. The affected vehicles are equipped with 2.3L GTDI engines and engine block heaters. These engine block heaters have elements that may overheat while plugged in.

Corrective Actions: To reduce the safety risk, Ford advises you not to use the engine block heater until the recall repairs have been completed. Ford will notify owners by mail and advise you to take your vehicle to a dealership to replace the block heater. Safety Risk: A short circuit could create a fire risk, even while the vehicle is parked and turned off. Corrective Actions: Ford will notify owners by mail and instruct you to bring your vehicle to a Ford dealership to install an in-line fuse for the battery monitor sensor. Ford advises to park your vehicle outdoors and away from other vehicles or structures until the recall repairs have been completed.

Seats And Restraints

On certain vehicles, improperly tempered bolts may have been used to install certain seats, seatbacks, seatbelt buckles, or seatbelt anchors due to a manufacturing issue. An improperly tempered bolt could fracture and compromise the structural integrity of the seat and/or seatbelt system's performance in a sudden stop or crash, which could increase the risk of injury to vehicle occupants. Correction: Dealers will inspect the bolts, and will remove and replace as necessary. Note: Only E-350 Extended Wagon models with 11-, 12- or 15-passenger seating configurations are involved in this recall.

Structure

On certain vehicles, the pawl spring tab in the side door latch could break. This condition typically will prevent the door from latching. In certain situations where the door is able to be closed, the door may be partially latched and could unlatch while driving. This could increase the risk of injury to a vehicle occupant seated next to the door, or result in damage to property. Correction: If a door will not latch, dealers will replace all four door latches with a more robust part. Note: A door that is partially latched can cause the courtesy light to remain on, set a door ajar message in the instrument cluster and sound a door ajar chime to alert the driver that there is a door latch problem.

Engine

On certain vehicles equipped with a 2.3 litre GTDI engine and an engine block heater, due to the engine’s orientation, the block heater could be susceptible to overheating. An overheated engine block heater may result in engine coolant leakage, or heat damage to the electrical connector of the block heater. Overheating of the engine block heater may increase the risk of an underhood fire causing injury and/or property damage. Correction: Dealers will replace the engine block heater element with one of a different design, and will inspect the block heater's electrical cord, and replace if required. In the interim, owners are advised to stop using their block heater until the campaign is completed. Note: This risk only exists when the block heater is plugged in and the vehicle is parked.

On certain vehicles, improper nickel plating of certain fuel pump internal components could contaminate the pump, potentially resulting in an inoperative fuel pump. This could result in a no-start condition, or an engine stall without warning and without the ability to restart the vehicle, which could increase the risk of a crash. Correction: Dealers will replace the Fuel Delivery Module (FDM).

The contact owns a 2015 Lincoln MKC. The contact stated that while the wife was driving into an intersection the vehicle crashed into a second vehicle. During the crash, the front of the vehicle was severely damaged but only the front driver air bag located under the dashboard console had partially deployed. No other airbags in the vehicle were deployed to provide protection to the driver. No injuries were reported. A police report was taken at the scene and the vehicle was towed away. During the crash, the vehicle was destroyed. The cause of the failures was not determined. The manufacturer and local dealer were contacted regarding the failure. The failure mileage was 75,000.

I OPENED MY GARAGE, SMOKE WAS POURING OUT AND THERE WERE FLAMES COMING OUT OF THE HOOD AT THE WINDSHIELD LINE. The dealership made two attempts ordering parts on backorder to address the recall at my service appointment today. After receiving parts last week, Ford told dealers they do not recommend installing these parts to address the recall—the dealer also said they do not know the procedure or replacement parts Ford will recommend to address the rear view camera recall on a later date. They also believe Ford will now replace the entire camera vs replace components. I have no problem with the approach to replace the camera if that’s the case. And I also realize this is not the dealership’s fault; however, there is a huge disconnect and communication gap from Ford quality that must be addressed. It’s been challenging to order parts for this recall and wait months, only to discover Ford no longer knows how to address a fix my vehicle needs. The contact owns a 2015 Lincoln MKC. The contact stated that while the vehicle was parked, the front driver's side door failed to unlock as intended, which required the contact to enter the vehicle through the front passenger's side door and to crawl over to the front driver's side seat. The vehicle was taken to the dealer, and the contact was informed that the repair was not covered under warranty or recall. The vehicle was not repaired. The contact related the failure to NHTSA Campaign Number: 23V378000 (Engine). The contact stated that while the dealer was previously performing the recall repair, the dealer informed the contact that the battery monitor sensor was damaged. Now, if we put gas in, the car struggles to start or won’t start. This happens all the time since April 2025. We looked at recalls and there were 2 with the fuel system that were apparently done. We feel that they may have been improperly repaired? The dealer service said this problem happens all the time with Lincolns. We’re afraid to put gas in the tank. We don’t know what will happen next? Car stopping at lights? Too much vapor coming out now? Repair is all over the place price wise wherever we check…but we don’t want to be stuck at a gas station or have someone light a cigarette near the car with gas vapor leaking out!

This is my 3rd Ford/Lincoln vehicles that I have needed to replace ALL lugnuts due to 'swollen' lugnuts! I assure you that I am not soaking them in water. Give me a break. Make them address this issue already. I put in a complaint to NHTA for my prior vehicle without resolution. 200-300 dollars to replace. Lincoln complaint desk acknowledges the common complaint and only say that it has been brought to upper managements attention. We all are aware that they will do nothing about it until their hand is pressed! Just a note..prior to these wholesale lugnut problems, I replaced 1 in my 45+ years of driving.
