Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. Mazda North American Operations (Mazda) is recalling certain 2003-2008 Mazda6, 2004-2005 MPV, 2004 RX-8, and 2006-2007 Mazdaspeed6 vehicles. These vehicles had their passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.

Risk: An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.

Mazda North America Operations (Mazda) is recalling certain 2003-2008 Mazda6, 2006-2007 Mazdaspeed6 and 2004 MPV vehicles nationwide. Mazda is also recalling 2005-2006 MPV vehicles in Alabama, Arizona, Arkansas, California, Delaware, the District of Columbia, Florida, Georgia, Hawaii, Illinois, Indiana, Kansas, Kentucky, Louisiana, Maryland, Mississippi, Missouri, Nebraska, Nevada, New Jersey, New Mexico, North Carolina, Ohio, Oklahoma, Pennsylvania, South Carolina, Tennessee, Texas, Virginia, West Virginia, Puerto Rico, American Samoa, Guam, the Northern Mariana Islands (Saipan), and the U.S. Virgin Islands. On July 9, 2019 this recall was expanded to include certain 2004-2011 RX-8 vehicles that were previously recalled under 16V-354 but have not yet been repaired. These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules, and used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the passenger frontal air bag, these passenger air bag inflators may explode due to propellant degradation occurring after long-term exposure to absolute humidity and temperature cycling.

MAZDA IS RECALLING 7,200 MY 2007-2008 MAZDA6 VEHICLES WITH CALIFORNIA EMISSION SPECIFICATIONS. THE METAL FUEL TANKS ON THE AFFECTED VEHICLES MAY HAVE HAD THE PVC PROTECTIVE COATING ON THE OUTSIDE OF THE FUEL TANK DAMAGED DURING THE ASSEMBLY PROCESS WHICH MAY RESULT IN REDUCED CORROSION RESISTANCE.

Risk: THIS COULD LEAD TO PERFORATION OF THE FUEL TANK, CAUSING FUEL LEAKAGE. FUEL LEAKAGE IN THE PRESENCE OF AN IGNITION SOURCE COULD RESULT IN A FIRE.

On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the driver frontal airbag. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. These issues could increase the risk of injury. Correction: Dealers will replace the driver frontal airbag inflator. Note: Canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected. DECEMBER 14, 2015 AROUND 6:40PM I WAS DRIVING HOME FROM WORK ON HIGHWAY 275E AT APPROXIMATELY 70 MPH APPROACHING THE 71N EXIT RAMP . CAR AND DRIVER'S SEAT HEATER HAD BEEN RUNNING ABOUT 8 - 10 MINUTES. I SMELLED A BURNING SMELL. I WAS WEARING JEANS. WHEN LOOKING AROUND TO FIGURE OUT WHERE SMELL WAS COMING FROM, MY LEFT LEG CAME IN CONTACT WITH SEAT, I FELT A "BITE" SENSATION ON THE BACK OF MY LEG - IMMEDIATELY I THOUGHT PERHAPS MY SEAT WAS WAS ON FIRE. I DIDN'T SEE ANY ACTUALLY FLAMES, SO I TURNED OFF SEAT HEATER, AND OPENED THE WINDOWS - HOPING TO COOL OFF THE INTERIOR OF THE CAR TO PREVENT ANY SMOLDERING IN THE SEAT TO ADVANCE TO A FIRE (SOUNDS A STUPID NOW, BUT AT THE TIME IT SEEMED TO MAKE SENSE). CONTINUE DRIVING WITH ONE HAND, WINDOWS DOWN, AND FANNING SEAT WITH THE OTHER. WITH THE WINDOWS DOWN, THE SMOKE/BURNING SMELL DISSIPATED. BY THE TIME I GOT HOME (ABOUT ANOTHER 10-15 MINUTE DRIVE FROM THE INITIAL OCCURRENCE), THE SEAT HAD COOLED. I OBSERVED A BURN SPOT ON DRIVER'S SEAT WHERE I FELT THE "BITE" ON THE BACK OF MY LEG. I ALSO THOUGHT MY JEANS WERE BURNT; HOWEVER, THAT TURNED OUT TO BE TRANSFER FROM THE BURNT MATERIAL FROM THE SEAT - I WAS ABLE TO SCRATCH MOST OF IT OFF WITH FINGERNAIL. ALSO, MY SKIN WAS NOT INJURED. I WILL BE CONTACTING MY LOCAL MAZDA DEALERSHIP TODAY.

FUEL SYSTEM, GASOLINE

The contact owns a 2008 Mazda Mazda6. The contact stated that while the vehicle was parked, there was fuel leaking from the fuel tank. The vehicle was taken to the dealer where it was diagnosed with a leaking fuel tank. The contact was informed that the fuel tank needed to be replaced. The dealer added a protective coating. The failure reoccurred. The contact stated that an abnormal fuel odor was on the interior of the vehicle, and fuel was leaking onto the ground. The dealer was contacted and made aware of the failure. The contact received notification of NHTSA Campaign Number: 08V412000 (Fuel system, Gasoline); however, the part to do the recall repair was unavailable. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the failure; however, the contact was informed that the part needed to be purchased to complete the repair. The failure mileage was 235,000. The front subframe totally rusted out and the wheel disconnected. I purchased this car in 2009, but it was a 2008 vehicle. Apparently there was a recall on this part for the 2009 through 2018 vehicles but it did not include the 2008, which had the exact same problem. My car was kept in the garage its entire life and the last 10 years, I was retired and did not even drive it in the snow. The car only has 79,000 miles. Even if my car was not included in the recall, you would think the manufacturer would have notified me of the flaw and the danger. I sold this car to my nephew in February of this year and he was deriving it when the wheel fell off and separated from the subframe. He could have been killed. Had I know the issues with this vehicle, I would never have sold it to him and would have just traded it in. I believe that we are owed some type of compensation from the dealer for this. Fortunately he was not driving on a highway when this happened. We have a quote of $1400 to replace this part and weld it back on.
