The contact owns a 2019 Tesla Model X. The contact stated that the passenger’s side rear falcon wing door closed unexpectedly. The door sensor failed to sense the contact’s [XXX] daughter’s leg. The contact’s [XXX] daughter’s leg was fractured. The dealer was contacted, and after having the vehicle inspected, no cause for the failure was found. The vehicle was not diagnosed or repaired. The manufacturer was informed of the failure. The contact was informed that an Incident Report would be available within 30 days, but received no additional information from the manufacturer. The falcon doors on the model X, when it is a hot day, and the sun is hitting the door, these doors will not open under any circumstance, resulting in a major safety issue. should there be an accident, a fire or other issue where someone needs to exit the vehicle, they are simply not able to do so under any circumstance! This is a known problem with these vehicles, if the sun hits these doors or it’s a hot day. There are hundreds, if not thousands of posts about these doors, not opening when the sun is hitting the door for a prolonged period of time or the door gets hot. There needs to be a recall for this issue. From my research, seems that there’s some sort of sensor adhesive that comes loose or something similar to that. And either case this is a major safety issue, as it does not allow the passenger to exit the vehicle under any circumstance. Even holding the override button on the screen does not open the door. My kids have been stuck in this car on a repeated basis due to this issue of the door, not opening due to this failed sensor. Tesla is aware of this issue, and has never issued a recall, even though they know it affects all of the model X models

The Automated Emergency Brakes did not activate. This car hit the next car on the street. I was driving when I noticed the brake lights on the car in front of me, so I applied the brakes. Though it did not work, as this car is equipped with the Automated Emergency Brakes feature, it was expected that the car would activate the feature to stop to avoid impact and an accident. Which did not happen, and this car has a tremendous loss; the car in front also suffered a loss. The car manufacturer has given data in Excel files showing the events; it has no data at the time of the impact, but it has data well before and well after. Due to the failure of this important feature, the car has suffered a loss (potentially a total loss), the insurance process has been affected, and there has been a mental impact on my family members and me. Hence, requesting help in this regard.

The falcon doors will not open under any circumstance if the car is sitting in the sun. This is a known issue by Tesla with thousands of online complaints by owners. It seems the glue that holds the sensor comes loose in the heat. This is a major safety issue by NHTSA’s own standards. Not being able to exit the vehicle is directly listed as a reason for a recall by NHTSA. You need to force Tesla to issue a recall for this known issue by Tesla

When I brought the vehicle to the manufacturer's official service center to have the defective safety latch inspected and repaired, they confirmed the behavior but refused to fix the underlying hardware. Instead, the service center officially documented on my repair invoice that this latch failure is "frequently present and deemed a characteristic of the vehicle." A malfunctioning, binding, or misaligned front trunk latch poses a severe safety risk. If the primary latch is failing to release properly, it indicates a defect in the retention mechanism that could lead to the hood unexpectedly opening, flying up, and obstructing the driver's vision at highway speeds (a known issue that has caused recalls in the past). The manufacturer's refusal to repair a failing primary safety latch, and instead documenting it as a normal "characteristic," leaves a critical safety component in a compromised and dangerous state. There were no warning lights prior to this failure; it is a physical, mechanical defect that the manufacturer has acknowledged in writing but refuses to remedy.

COMPONENT: Rear right falcon-wing door electronic control system. Door becomes stuck in a partially closed position and cannot be opened from inside or outside. Vehicle and component are available for inspection. SAFETY RISK: Falcon-wing doors have NO exterior mechanical release accessible to occupants when the electronic system fails. An occupant of the rear right seat can be trapped inside the vehicle. This creates a foreseeable risk of serious injury or death in any emergency requiring rapid egress: collision, fire (including thermal-runaway events documented across the lithium-ion EV fleet), submersion, or smoke inhalation. The risk is especially severe for child passengers using this seating position. CONFIRMED BY DEALER: Yes. Tesla Service Aliso Viejo and Tesla Collision Aliso Viejo (CA) inspected and attempted repair on at least 5 separate occasions between October 2025 and April 2026. Tesla represented in writing each time that the issue was resolved. The defect has recurred after every repair attempt. As of May 2026 the door remains unsafe. INSPECTED BY: Tesla service technicians only. No police, insurance, or independent inspection. WARNING SYMPTOMS / TIMING: The defect began in September 2025. No prior warning lamps or messages. The door operated normally for the preceding 12 months. Tesla initially attributed the failure to a minor cosmetic scratch sustained 12 months earlier — a theory inconsistent with the timeline, since the scratch existed during 12 months of normal door operation. Recurrence after 5+ failed repair attempts confirms the defect is electronic in nature, not cosmetic.

My 2020 Tesla Model X (VIN: [XXX] , approx. 121,000 miles) experienced a dangerous defect with the automatic front passenger door opening with excessive and uncontrolled force. I noticed this issue within the past three months. The vehicle's front doors are fully automated — the driver and passengers have no manual control over the force at which the door opens. Because of this, a person or object standing near the vehicle could be struck without warning and without any ability to stop or slow the door. The door was opening with such excessive force that it nearly struck a bystander on at least one occasion. This posed a clear and immediate safety hazard to anyone standing near the vehicle. When I brought the vehicle to Tesla Service, their own technician documented the issue in the repair notes as a "safety concern." The technician confirmed the cause was a defective door drive unit and a corroded connector in the wiring harness. Tesla replaced the right front door drive unit and the associated wiring harness. Tesla charged me over $1,000 to repair this defect, despite the fact that their own technician's notes explicitly identify this as a safety concern — not normal wear. I believe this is a manufacturing defect that should be covered under a safety recall or goodwill reimbursement, and I am reporting it here so that NHTSA can investigate whether other Tesla Model X vehicles share this defect. I am requesting that NHTSA investigate this issue and determine whether a safety recall is warranted for this component. My 2023 Tesla Model X Plaid's falcon wing doors have had a persistent rattling/squeaking defect since August 2023 that has not been resolved after 9+ repair visits to Tesla service centers. More critically, in November 2025 (Invoice #3000S0014846906), the left falcon door's ultrasonic sensor failed to detect an adjacent vehicle's side mirrors, causing the falcon door to strike and scratch the other vehicle's mirrors — a safety incident for which I was forced to pay out-of-pocket. Tesla replaced the ultrasonic sensor under warranty but did not compensate me for the third-party damage. Additionally, in January 2026, the right falcon door was showing a "press and hold to override" error approximately 20% of the time despite no obstacles being present, requiring replacement of the sensor carrier (Invoice #3000S0015725078). The door sensor failures represent a serious safety hazard as the falcon wing doors can strike pedestrians, cyclists, other vehicles, and property. These issues have not been permanently resolved despite repeated warranty repairs spanning nearly 3 years.

The contact was able to grab the chrome piece before the chrome piece flew off the vehicle. The chrome piece was reinstalled on several occasions; however, the failure recurred while rolling up the window several times. A Service Center was contacted through the Mobile App and provided an estimate for the part replacement. The vehicle was not repaired due to the cost. The contact related the failure to an unknown recall. The manufacturer was notified of the failure. The failure mileage was approximately 50,000.

On 03/13/2026, while driving my 2020 Tesla Model X at approximately 65 mph on a freeway, the front hood (frunk lid) suddenly flew open without warning and slammed into the windshield. The hood rotated upward violently and struck the windshield completely blocking the driver’s forward visibility. This created an immediate safety hazard while traveling at highway speed. Prior to the incident there were no warning messages or alerts displayed on the vehicle screen indicating that the hood was open or improperly latched. Tesla vehicles normally provide a dashboard and a sound alert if the hood is not properly latched before driving. No such warning occurred. The vehicle’s front camera recorded the incident. The video does not show any hood movement or flutter prior to the failure, suggesting that the hood was fully closed before the event. After the incident, Tesla service personnel inspected the vehicle. The technician stated that the hood may not have been properly latched prior to driving. However: The vehicle gave no dashboard warning No hood movement or vibration was visible prior to the event The hood opened suddenly and violently rather than gradually If the secondary latch alone had been holding the hood, some hood movement or vibration would normally be expected prior to full release. Safety Concern A hood opening at highway speed that: occurs without warning results in complete loss of forward visibility presents a severe safety risk to the driver and surrounding traffic. I am submitting information regarding repair access and repair estimate concerns involving a Tesla vehicle after a collision. During an insurance claim handled by Tesla Insurance Company, a repair estimate from a Tesla-preferred repair facility was used to declare the vehicle a total loss. However, a manufacturer-owned repair facility estimate would be significantly lower and could make the vehicle repairable. When I attempted to obtain an inspection and estimate from a Tesla manufacturer repair facility, the appointment was cancelled after communication between the insurance company and the repair facility. This prevented me from obtaining a competing repair estimate. Because Tesla operates the vehicle manufacturing, repair network, and insurance services within the same ecosystem, I am submitting this information for review regarding potential impacts on repair access, repairability, and consumer choice.

Approximately one to two months ago, the driver-side front door latch on my 2017 Tesla Model X malfunctioned and prevented me from exiting the vehicle through the driver door. I first attempted to open the door using the manual interior door handle (manual mechanical release), but the door did not unlatch. I then attempted to open the door using the electronic door release, but it also failed to open the door. When attempting the electronic release, the door actuator clicked multiple times but the door did not open. Even while pushing outward on the door while activating the release, the latch would not disengage. Because the driver door would not open using either the manual or electronic release mechanisms, I exited the vehicle through the passenger side door. Later, after applying significant pressure to the driver door and repeatedly attempting to open it, the door eventually opened. Tesla service has indicated that earlier front door latch assemblies require a retrofit and that both front doors must be retrofitted. I was given an estimated cost of approximately $1,500 or more for the latch retrofit plus an estimated diagnostic fee of approximately $200 or more. I am concerned this represents a safety defect involving failure of both the electronic and manual door release mechanisms, which prevented normal occupant egress through the driver door.

Sept 20, 2025. 2025 Model X door alarmed for being open when it was actually closed. Door would only open intermittently. Your in car and cannot escape through drivers door. It was locked shut and thought it was open. Parked car requested Tesla service. Mobile unit came out replaced electric opener. Door continued to lock driver in and out of car randomly for the month of October 2025. We parked car and scheduled appointments with Tesla multiple times. They were not concerned this was a safety issue. They reschedule our appointments multiple times. At the end of October after several service appointments and multiple parts replaced the door started working. Their final comments we "THEY WERENT ABLE TO VERIFY THERE WAS AN ISSUE."

Water entered the passenger cabin of my vehicle during normal rain exposure while the vehicle was parked overnight. The failure occurred at the roof glass/seal system, allowing rainwater to intrude into the interior behind the second-row seating area and soak the carpet and underlying padding. This resulted in persistent moisture and a strong musty odor inside the cabin, consistent with microbial growth and mold risk. The condition affects interior air quality and poses a potential health risk to vehicle occupants, particularly children or passengers seated in the rear. Prolonged moisture inside the cabin also raises safety concerns related to corrosion and possible impact to electrical components, sensors, and restraint systems routed through the interior floor area. The condition substantially impairs safe and normal vehicle use. The presence of moisture and microbial odor makes the vehicle unsuitable for passenger transport and creates uncertainty regarding the integrity of interior and electrical systems. The issue was inspected and confirmed by the manufacturer’s authorized service center. The service center documented that water intrusion occurred through failed or missing sealant around the roof/door glass assembly and confirmed saturation of interior carpet and padding. The manufacturer recommended replacement of interior padding for safety reasons but declined responsibility due to the vehicle being outside the basic warranty period. There were no warning lamps or messages prior to discovery. The issue was identified after rain exposure when wet carpet and odor were observed. The problem is reproducible under rain conditions. Vehicle: 2023 Tesla Model X Component: Power-operated doors / door latching (auto-close / “cinch” close) Incident type: Finger pinch injury during powered door closing Summary of incident: While using my 2023 Tesla Model X, a powered door closing sequence resulted in a finger pinch injury. The door appeared to close normally and then performed an additional “cinch”/pull-in motion to fully latch. During this final motion, my thumb was caught in a very narrow gap and was pinched. There was no adequate pinch detection/anti-pinch reversal before contact and compression occurred. The gap was extremely small, leaving little time to react. Injury: Thumb crush injury with immediate blood under the nail (subungual hematoma), swelling, and altered sensation/numbness. I obtained medical evaluation and an X-ray, which showed no fracture. A splint was prescribed to protect the thumb. Safety concern: This is a significant pinch hazard because the powered closing/cinching can occur quickly and unexpectedly, and the door does not reliably detect or reverse when a finger is in the closing path. This presents a risk of more severe injury (fracture, nail-bed injury, nerve damage), especially for children. When it happened / conditions: Normal use, low speed/parked. (Add date/time/location if you want.) The incident occurred during the final powered latch/close step (“close then push/pull for full close”). What I want investigated / remedy requested: Request NHTSA to investigate the Model X powered door closing/cinching behavior and pinch detection effectiveness, including whether there are known blind spots or missing/insufficient anti-pinch protections. Recommend Tesla provide a software update or hardware change to improve pinch detection/reversal and/or limit cinch force/speed, and provide clearer warnings/settings to prevent unexpected powered closure.

The vehicle presents a compounded safety defect creating a risk of passenger entrapment and high-voltage fire. 1. High Voltage Manufacturing Defect: On September 3, 2024, authorized Tesla Service technicians diagnosed a critical safety fault. The service record states technicians 'found A/C compressor HV harness was incorrectly routed, rubbing against the steering column' and 'found sand inside the connector'. Tesla repaired this defects under the 'Basic Vehicle Limited Warranty,' confirming it was a factory assembly error. This defect created an imminent risk of a high-voltage short to the steering column, which could result in driver electrocution or total vehicle power loss at speed. 2. Passenger Entrapment: The vehicle has a documented history of Rear Falcon Wing Door failure (7 repair attempts from April 2023 to May 2025). The doors frequently fail to latch or open. On July 8, 2025, the Left Rear Door failed to open due to a sensor fault. 3. Combined Hazard: If the incorrectly routed High Voltage harness causes a vehicle power failure, the rear doors' electronic latches will fail. Because the doors have a history of mechanical jamming, passengers in the rear seats would be trapped in a vehicle with a compromised High Voltage system.
