The contact owns a 2018 Tesla Model X. The contact stated that while attempting to park the vehicle, the vehicle became stuck in drive(D) and failed to shift to park(P). Due to the failure, the safety system locked the doors, and the contact was trapped inside the vehicle. The front end of the vehicle struck the garage door, causing damage to the garage door. The vehicle then rolled backwards and came to a complete stop after depressing the brake pedal. No air bags had deployed during the failure. The message “Self-Driving Feature Disabled” was displayed. A Police report was filed. No injuries were reported. The Fire Department arrived at the scene and had to open the frunk to cut the wires and disconnect the battery to turn off the vehicle, and the contact safely stepped out of the vehicle. The dealer was notified of the failure. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 72,929.

On 7/29/2026, 2:45 PM PST, I tapped the in-car myQ interface to send an "Open" command. Due to network latency/ connection drops in the Tesla infotainment system, there was a significant delay before the open command executed. After it finally opened, I proceeded to back out in Reverse. However, due to buffering/retry logic during the network reconnect, the system unexpectedly transmitted a follow-up "Close" command without any driver input. Because this automated ghost command executed while the vehicle was actively backing out, the overhead garage door unexpectedly began closing, striking the car and shattering the left Falcon Wing door glass. Safety Concern: The infotainment software suffers from a critical retry/buffering flaw where latent network API payloads execute out of sequence or trigger unintended secondary commands after initial connection drops. Transmitting an unprompted "Close" signal to an overhead garage door while a vehicle is actively moving in Reverse creates a severe collision hazard and safety risk for occupants and bystanders.

While driving the vehicle on the freeway, the driver noticed the car was handling unusually at highway speeds. After exiting the freeway at approximately 45 mph, the accelerator pedal failed to decelerate properly when the foot was lifted from the pedal. A warning message appeared on the display screen indicating the vehicle was shutting down while the vehicle was still in motion. The vehicle subsequently became completely inoperable and had to be towed to a Tesla service center. Tesla's own technician confirmed multiple Battery Management System faults and an internal fault within the high voltage battery pack. A full HV battery replacement was required. The vehicle had been purchased 14 days prior and had only been driven approximately 723 miles at the time of the incident. I own a 2016 Tesla Model X P100D equipped with the Large Drive Unit (LDU). The component that fails is the rotor coolant seal in the rear drive unit motor. Coolant leaks internally into the motor and inverter, causing bearing wear, corrosion, isolation faults, and eventual complete drive unit failure. The affected drive units are still installed and available for inspection upon request. This defect puts my safety and the safety of others at risk because it can cause sudden loss of propulsion (especially at highway speeds), power reduction, or total drivetrain shutdown, potentially leading to a crash. Coolant in high-voltage areas also raises electrical fault/fire risks. The problem is widely reproduced and confirmed: Tesla service centers are familiar with it and replace failed units with updated “-U” revisions that include a coolant bypass. Independent EV shops confirm the exact root cause (rotor seal failure) via encoder inspections showing coolant intrusion and through disassembly. This is a documented pattern failure in all 2012–2020 LDU vehicles. No police or insurance inspections have occurred yet, as no crash or total failure has happened. Tesla is aware of the recurring issue through service records on similar vehicles. Early symptoms before full failure typically include rear motor whining/grinding (especially under load), reduced power warnings, “Rear Drive Unit Fault” messages, and visible coolant on the speed sensor. These can appear gradually. My vehicles have not yet experienced total failure, but the defect is known to be inevitable. I am proactively monitoring encoder sensors. In April 13th, 2026 while driving from the house through community, I suddenly stopped at the cross roads due to unexpected conditions. During this event, a coffee mug placed in the cup holder tipped over and spilled liquid onto the center console area—near the phone charging pad and critical control panel used for gear selection and essential driving functions. In about 15 minutes after the spill, when I reached the highway at about 70 mph, the vehicle experienced a rapid and severe system-wide failure. The dashboard displayed multiple warnings and errors, including: * “Steering assist reduced – Steering may require increased effort” * “Center console buttons to select gears unavailable – Use touchscreen to select gears – Schedule service” * Repeated alerts indicating braking and system limitations * Conflicting and inaccurate readings, including the speedometer displaying “0 mph” while the vehicle was still moving at highway speed At the same time: * Braking response was severely compromised despite full pedal application * Hazard lights and turn signals were non-functional * Gear selection controls became unavailable or unreliable * Driver assistance and safety systems were disabled * The system issued instructions to “press and hold brake” while braking performance was inconsistent This resulted in a complete loss of situational awareness and control while I was in the car with my older parents and my child. l. I was unable to signal to surrounding vehicles while attempting to maneuver into a narrow emergency lane at highway speed. My family and I had fear for our lives. Critically, the vehicle instructed to exit immediately while we were still in a dangerous roadside position without functioning hazard lights. We were exposed to active highway traffic without any ability to alert others. was eventually able to maneuver the vehicle into the emergency lane under extremely dangerous conditions. The vehicle was towed, and the issue was reported immediately.

For the past year, the battery will not hold it charge. I am getting about 30% of predicted mile range in colder weather and about 50% of predicted range in warmer weather. This makes the car unusable except for short trips and even then requires frequent charging. In addition, intermittantly, it will not charge quickly at a Supercharger. When others cars present are charging 180 range mph, I am charging at 20-50 range mph. I have had Tesla evaluate the issue and they claim there is nothing wrong but refused to share their evaluation with me. The rep would not give me any specific reason for my problem other than they found nothing wrong. I started the process before the end of the battery warranty (8 years) which has now expired. I feel Tesla service is committing systemic fraud in their approach to warrantied battery failure.

High voltage battery management system generated fault codes BMS_u008 and BMS_f071 during a Supercharger charging session, prohibiting drive mode. Vehicle had been driven normally to the Supercharger with no drive-prohibiting alerts during transit. After connecting to the Supercharger, fault codes manifested. Despite fault condition, vehicle continued charging to 93% state of charge before requiring towing. This is the second such event — original HV battery also generated BMS fault alerts prohibiting drive mode during a charging session in January 2024 and was replaced with a remanufactured assembly. Third-party firmware alert data shows recurring hardware charging fault alerts (CHG_f018, CHG_f076, customer message: Unable to charge) occurring in January and February 2026, weeks prior to the March 2026 failure. Alert DI_u014 also fired repeatedly on March 18, 2026 indicating insufficient 12V supply contributing to undriveable condition — a separate system from the HV battery. Tesla Service Center has not investigated whether Supercharger charging events contributed to either failure, has not produced Supercharger session logs, and has not attempted software-level fault resolution before recommending full hardware replacement. Tesla's own firmware documentation describes BMS_u008 as potentially self-clearing and BMS_f071 as having a documented clear condition. Remanufactured battery subsequently drained from 88% to 0% over 11 days while unplugged at service center — 6-9% per day versus normal 1-3% per day vampire drain, consistent with active BMS fault causing abnormal parasitic draw.

On 03/13/2026, while driving my 2020 Tesla Model X at approximately 65 mph on a freeway, the front hood (frunk lid) suddenly flew open without warning and slammed into the windshield. The hood rotated upward violently and struck the windshield completely blocking the driver’s forward visibility. This created an immediate safety hazard while traveling at highway speed. Prior to the incident there were no warning messages or alerts displayed on the vehicle screen indicating that the hood was open or improperly latched. Tesla vehicles normally provide a dashboard and a sound alert if the hood is not properly latched before driving. No such warning occurred. The vehicle’s front camera recorded the incident. The video does not show any hood movement or flutter prior to the failure, suggesting that the hood was fully closed before the event. After the incident, Tesla service personnel inspected the vehicle. The technician stated that the hood may not have been properly latched prior to driving. However: The vehicle gave no dashboard warning No hood movement or vibration was visible prior to the event The hood opened suddenly and violently rather than gradually If the secondary latch alone had been holding the hood, some hood movement or vibration would normally be expected prior to full release. Safety Concern A hood opening at highway speed that: occurs without warning results in complete loss of forward visibility presents a severe safety risk to the driver and surrounding traffic. The electric power assist steering system on my 2022 Tesla Model X began displaying a "Steering Assist Reduced / Steering May Require Increased Effort" warning intermittently beginning in approximately May 2024, shortly after I purchased the vehicle. This warning appeared more times than I can count over the following months. In most instances the warning appeared but the steering continued to function normally. On approximately March 2, 2026, the warning appeared and for the first time the steering became extremely difficult to turn, requiring significantly increased physical effort to control the vehicle. The first physical failure occurred while parking at low speed in reverse. The steering failure also occurred subsequently while driving at highway speeds. In both instances I stopped the vehicle, exited, waited briefly, and upon re-entering the vehicle the warning cleared and steering returned to normal. This same sequence repeated the following day. The vehicle was brought to a Tesla authorized service center on March 13, 2026. Tesla's technician confirmed the steering assist reduction as reported and determined the steering gear required full replacement. The failed part, steering gear left hand drive part number 1070801-00-F, was replaced. Only full hardware replacement resolved the issue permanently. Prior power cycling provided only temporary relief. My safety was directly at risk. I am a full-time rideshare driver operating at highway speeds. The failed component has been replaced and is in Tesla's possession. Tesla is charging the full repair cost out of pocket despite this matching a documented pattern of identical failures reported to NHTSA by multiple Tesla owners.

Compounding this concern, on the morning following the incident at approximately 6:00 AM, I discovered one of the vehicle doors open without any user interaction. The vehicle had been secured the night before. This raises serious questions regarding electronic reliability and system integrity. Given the severity of these failures, I have lost confidence in the safety and dependability of this vehicle. My family’s safety cannot be subjected to further uncertainty. The 2018 Model X experienced a sudden and complete loss of power and became totally inaccessible. All doors and access points were inoperable and the vehicle could not be unlocked, disconnected, powered on, or placed into any service or tow mode. Emergency front-trunk release cables had to be used and external 12-volt power applied at the jump terminals to wake the vehicle. The inability to access the vehicle was unexpected and alarming. Children, pets, elderly, and others could have been inside of the vehicle. Thankfully, they were not. Tesla inspected the vehicle and stated the failure was related to the battery coolant heater. The failure resulted in a complete vehicle immobilization and access failure. I had previously experienced restart warnings on the dashboard, but I did not expect a total lockout and loss of access. The inability to access the vehicle was unexpected and alarming. It may be a known safety issue. It should be recalled. This condition presents a serious safety risk because occupants could be trapped inside the vehicle and the vehicle cannot be accessed or moved without external power. In extreme heat or cold, or in a medical or emergency situation involving a child or pet, this failure mode could create a life-threatening situation. Importantly, even after multiple requests, I have not received a copy of the diagnostic report, or anything other than informal emails or calls. Tesla has not provided information regarding this failure other than broad statements, and visuals of the location of the battery pack, in an attempt to deny warranty coverage. The Tesla reports a High Voltage System alert currently. The vehicle is subject to Tesla’s Battery and Drive Unit Limited Warranty. Tesla has stated that the failed component was the battery coolant heater, but has refused to cover the repair under warranty. Tesla confirmed the failure but has not provided the diagnostic report, fault codes, or vehicle logs supporting its determination.

The contact stated that the speed regulation was set to "Standard Mode" and the vehicle accelerated to 70 MPH in a 55 MPH Zone. The contact stated that was not the mode selected. In addition, when the contact set the speed regulator to "Crawl Mode," the vehicle drove 10-15 MPH below the speed limit. No warning light was illuminated. The contact was unable to control the speed of the vehicle. The vehicle was not taken to the dealer. The vehicle was not repaired. The manufacturer was not notified of the failure. My 2018 Tesla Model X experienced repeated over-the-air software update failures starting in late December 2025. Each update attempt would run for hours and then fail with a timeout error. This pattern continued for several weeks with the same error every time. After these repeated failed update attempts, the Autopilot ECU (computer) completely failed. Tesla service diagnosed the issue on January 6, 2026 as a failed Autopilot ECU 3.0 requiring full replacement at a cost of $1,808. The failure has disabled Full Self-Driving, adaptive cruise control, lane keeping assistance, automatic emergency braking, and all driver-assistance safety features. I am concerned this represents a systemic defect where Tesla's software updates are damaging or exposing failures in Autopilot hardware. Multiple other Tesla owners have reported this same pattern in 2024-2025: update timeout errors followed by complete Autopilot computer failure requiring expensive replacement. Tesla is charging customers to fix problems created by their own update process. The loss of critical safety features occurred solely due to Tesla's software update attempts.

Vehicle: 2018 Tesla Model X Recall Reference: Tesla eMMC Recall – NHTSA Campaign Number 21V-035 (Recall 21V035000) Defect Description: In March 2021, Tesla notified me by email of a recall involving failure of the eMMC memory chip in the Media Control Unit (MCU). This defect is known to cause loss of vehicle system functionality and safety-related features. Despite this notification, Tesla has refused to honor the recall and replace the eMMC when I brought my vehicle in for service. Current Vehicle Condition: My vehicle exhibits severe and worsening symptoms consistent with eMMC failure, including: System freezing and frequent unresponsiveness GPS extremely slow or nonfunctional Turn signal indicators intermittently fail Horn fails to operate approximately 95% of the time Vehicle reset/reboot does not work Touchscreen and vehicle controls are very slow or nonresponsive These issues occur regularly and without warning. Safety Risk: This defect creates a significant safety hazard. The failure of turn signals, horn, and system responsiveness increases the risk of accidents. The inability to reset or restore the system while driving further endangers occupants and other road users. Due to these failures, the vehicle is unsafe to operate. Manufacturer Response: Although Tesla issued a recall notice in March 2021, they have declined to perform the recall repair on my vehicle. No effective remedy has been provided, and the vehicle’s condition continues to deteriorate. Request for NHTSA Action: I am requesting that NHTSA investigate Tesla’s failure to honor Recall 21V-035 and require Tesla to provide the mandated repair. This unresolved defect affects safety-critical systems and presents an ongoing risk. When I went there last Dec 4 to get this repaired, they wanted me to upgrade and pay almost $2000. I am willing to pay half of the cost but not the entire amount.
