2nd incident. As I was driving on the highway, approaching an exit on my righthand side. I kept following the straight path in my lane, the lane assit of my vehicle assumed I was exiting my lane and jerked the steering wheel from my hands. My car dangerously veered into the lefthand lane.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

UNKNOWN I was driving on an expressway around 2:00 AM June 22nd using my cars adaptive cruise control. There was light rain and my car kept a safe distance from the car in front of me that was going slower than the speed I had set my cruise control to. Suddenly my car accelerated as if the car in front of me was no longer there. I had to quickly apply the brakes to avoid a collision. I have around 8,000 miles on my car and I had no problems the last time I went on a trip using the adaptive cruise control.

2025 Toyota Corolla Cross at 6,500 miles Silver Spring, MD Crash

The contact owns a 2025 Toyota Corolla Cross. The contact stated that while in a parking lot, a parking spot opened, and the contact planned to take the spot. While the vehicle was in reverse(R), the vehicle crashed into the front end of another vehicle. The contact stated that the back over prevention camera failed to alert the contact of an object or another vehicle nearby. The contact did not file a Police report. There were no injuries sustained. The vehicle was not yet diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 6,500.

On April 14, 2026 the unit went without brakes. The unit can not be change to parking mode or neutral mode. An in the Dash it show all kind of message, ( System Malfunction Brakes) (Collision-Contact Dealer) (AntilockBrake SystemMalfunction)(Secondary Collision Brake System Malfunction) Visit Your Dealer). Contact Dealer. I made a police station Notification # XXX. The next day the unit was pickup by a tower car an send to the Toyota Calesa Motors Dealer they asign Job order XXX. An after 6 hours waiting the mecanic Mr. [XXX] tell me that a brake cable was broking and that was the reason the unit [XXX] ithout brakes. The Unit has only 1498 milles. I purchase 3 week ago brand new. Them tell me they can repair it. But I toll Toyota Mecanic that I don't want that unit. I also went to DACO OFFICE in Puerto Rico and report the incident. They asign me a case # XXX against Calesa [XXX] , HNC CALESA TOYOTA, TOYOTA FINANCIAL SERVICES,INC TOYOTA DE PUERTO RICO. The unit is at the Dealer I Don't want it. Thanks! GOD nothing happens to my family, neigbhor. The unit don't crash. Is like new with also all the plastic on their seat. Only run XXX. The Unit Total cost is $ 52,730.16. FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

Vehicle displayed a “power brake loss” on the 2023 Corolla Cross Hybrid instrument cluster upon trying to start it. After not being able to turn the vehicle on, I exited the vehicle to ensure my key fob was in my pocket. All electronics worked. After roughly 4 minutes of being in the car all power was loss - battery drained. Towing company that my Toyota Dealership Provides had a number out of service so a different towing service came out and jumped my car - my warranty couldn’t be used since the towing company didn’t answer. The towing technician let me know that Toyota put in an extremely low voltage battery in my Hybrid. He expressed how the standard for these vehicles is AGM or high performance batteries to ensure stable voltage for hybrid management, and compatibility with regenerative systems - using the right part prevents electrical issues. My brakes are connected to my electrical system. By putting in weak batteries and below standard expectation is putting their consumers at many risks with electrical malfunctions. I was informed by the dealer after getting it replaced this is the Toyota Manufacturer issue and I had a technician even agree the weak batteries will only help in getting consumers back to a dealer quicker.

The contact stated that while operating the vehicle, the messages "System Malfunction - Contact Dealer" and "Secondary Collision Brake System Malfunction" were displayed. The contact stated that the messages were not displayed while starting the vehicle, but later were displayed while driving. The vehicle was taken to the local dealer, who was unable to determine the cause of the failure, and the codes were cleared. Shortly after retrieving the vehicle, while driving, the messages were displayed. The manufacturer was not yet contacted regarding the failure. The failure mileage was 2,800.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

The pre-collision system acted as if it sensed a car or object in the road. It had loud warnings and slammed my brakes very hard to stop the car. There were no objects or cars around. If a car was behind me, it would have been a guaranteed crash. Very dangerous especially if it happened on the highway. Dealership said it could have been from a loss of communication. A loss of communication should have the pre collision system disabled not force brake. This is a potentially life threatening issue with this car.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

This is a safety concern: 2025 Toyota Corolla cross with no damage history and happening since brand new. 1) Lane keeping: there are 2 types, one with cruise control and one without. For the one with cruise control, the system has a hard time tracking center and shoulder lines and OFTEN pulls in the wrong direction. This is ESPECIALLY true in certain circumstances and the vehicle has almost caused a collision several times, with both the owner and other Toyota-experienced drivers, necessitating turning off this feature. When the road is a rural one, without center or shoulder lines, the car OFTEN pulls (“assists” or “corrects”) in the wrong direction and twice has pulled the offending vehicle onto the soft curb. Dealer reviewed it and noted this is expected behavior, they recommended turning it off. This is less common on roads with white and yellow lines, but DOES happen if the vehicle is driven too close to the white line and the vehicle sometimes pulls towards the shoulder. 2) Auto-braking. Often when going around a curved turn from 30-55 MPH, the brakes sometimes deploy (usually without aural warning.) This is usually with a left curve, while driving in the right lane. Service manager drove the vehicle and thought this aberrant but tech AND Toyota rep noted this was expected behavior, Several times these safety features have caused near rear-end collisions and off-road incidents and COULD cause a head-on . Dealer recommendation is to simply turn it off.

2024 Toyota Corolla Cross Valley Stream, NY

FORWARD COLLISION AVOIDANCE: WARNINGS

On July 2, 2025, while driving at approximately 8:35 PM my 2024 Toyota Corolla Cross suddenly and unexpectedly came to a complete stop. The vehicle's Pre-Collision System (PCS) activated without any obstacle, pedestrian, or vehicle in front of me. I was decelerating at about 15–20 mph in clear weather and road conditions. The abrupt stop was extremely dangerous and could have caused a serious rear-end collision had another car been behind me. This created a significant safety risk to me and to others. I had already completed the Toyota recall related to the pre-collision/braking system weeks prior. Despite this, the malfunction still occurred. The issue has not yet been confirmed or reproduced by a dealer, but I have contacted Toyota and scheduled a full inspection. The vehicle is available for inspection upon request. There were no warning lamps, messages, or error alerts before the incident. The car simply braked suddenly without warning. Out of fear for my safety, I have since disabled the PCS system. I am now afraid to drive the car, especially at higher speeds, and have requested that Toyota escalate the matter for further resolution or potential buyback.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

January of 2025 the vehicle was put in drive and the vehicle shut down. "Power steering low egine stopped" message appeared. I Put the vehicle in park and started the car back up. I took it to the dealership. The battery was low they charged it and released it back. The vehicle was brought back shortly after that due to the vehicle being put in park and shutting down displaying the same "low power steering. Egine stopped" message. Along with "EPB stopped Incompletely". Brought it back in and the battery was low and they replaced it. A few months later the vehicle was brought in due to multiple braking events either when pulling forward with no acceleration (nothing was around the vehicle) or putting the vehicle into reverse with no acceleration (nothing behind the vehicle). During these events the car made a loud grinding sound and displayed BRAKE across the dash. The 3rd time at the dealership was due to the car shutting down when put into park and braking when reversing out of a parking spot. The code "epb stopped Incompletely" and the BRAKE message appeared.The dealership checked for codes but the car didn't have any saved. They can't recreate it. They had a feild tech come out but I'm unsure of what was found because they haven't released it. As of June 22, 2025 I was driving and the car braked on me while going 40 mph. The seat belt locked up and threw me and my passenger forward. A red BRAKE message appeared and the car sounded an alarm. The vehicle was brought back into the dealership on June 23, 2025 the dealership turned me away. Corporate started a case.

On [XXX], at the intersection of [XXX] and [XXX] in Los Angeles, [XXX] was operating the subject vehicle when it was involved in a hit-and-run that resulted in a secondary collision with multiple vehicles. Photos (attached) document the significant impact. Emergency responders were required to extricate [XXX]from the vehicle, during which they informed her that the airbags failed to deploy—a fact also documented in the official LAPD report. The failure of the airbag system represents a significant and dangerous malfunction of the vehicle’s safety features. This is particularly troubling in light of Toyota’s recent recall, which affects more than 400,000 vehicles. 2024 Toyota Corolla Cross Los Angeles, CA Crash

FORWARD COLLISION AVOIDANCE: WARNINGS

On January 10, 2025, I was driving my 2024 Toyota Corolla Cross and entered a parking lot to park. I started a three-point turn to park head-in and was slowly turning the steering wheel to the right. I was about to switch from the accelerator to the brake when the car suddenly accelerated rapidly. It felt like the front of the car lifted, and I lost control, colliding with a parked car to the left of the space. The distance between the two cars was about 5 feet, and the sudden acceleration lasted only 1-2 seconds. My car has a proximity warning and automatic emergency braking system, (Pre-Collision System) but neither activated, and the airbags did not deploy. My husband, in the passenger seat, heard a loud engine noise when the car surge forward unexpectedly. After the crash, I parked the car, checked for any objects near the pedals but found nothing. The other car had rear bumper damage, and mine had significant front-right damage, including a severely worn tire. I reported the incident to the police, exchanged information with the other driver, and called a tow truck. I later suspected a malfunction in the throttle or safety systems, such as the proximity warning or braking systems, which failed to activate. No prior warning lights or issues were present. The failure of these systems increased the risk to both myself and others in the parking lot. The cause of the malfunction has not been reproduced or confirmed by a dealer or service center. The vehicle’s onboard diagnostic (OBD) system could be checked for errors, as a potential cause of the sudden acceleration but was told that it is difficult to diagnose such issues and was told that it was slim to find anything even if I did a diagnostic check. Given the car’s advanced systems, I believe these components should be thoroughly inspected to prevent future incidents.
