LANE DEPARTURE: ASSIST

Hello, I am writing to formally request escalation and review of a potential vehicle safety concern involving my recent accident. I previously filed a complaint with the National Highway Traffic Safety Administration (NHTSA), contacted Honda, State Farm, and the investigating police department regarding this incident. I am following up because I am concerned this may represent a broader public safety issue, not limited to my individual case. Given the nature of the incident and vehicle behavior involved, I respectfully request: * Confirmation that my complaint has been received and assigned for review * Evaluation of whether this issue may indicate a potential safety defect or recurring pattern * Preservation and review of all relevant vehicle data, including Event Data Recorder (EDR), ECU/diagnostic logs, and any available telematics data * Any guidance on additional information or documentation needed to support a safety investigation I am concerned that if similar incidents are occurring in other vehicles, this may present a risk to public safety. I am requesting that this matter be reviewed accordingly for potential defect or safety trend analysis. Please confirm receipt of this request and advise on the next steps in the review process. Thank you for your time and attention to this matter. Sincerely, [XXX] Report# [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

After using the lane assist the steering wheel becomes sticky when I turn it off. Then if I turn it back on letting the lane assist correct my driving to turning it back off when turning around corners my steering wheel becomes really sticky. Then when driving the vehicle with lane asssist on the steering wheel will feel slightly different then I’ll turn it back off and it won’t stick around corners anymore (so then I can’t use my lane assist without my steering wheel getting sticky again) it really makes me worried. I don’t know if it’s connected but it makes me feel unsafe. When getting a hold of the dealership, they told me that I had to pay $150 to get a diagnostic because I didn’t have any active recalls. Which I think is a little crazy considering I keep reading about all these recalls about sticky steering wheels, but not specifically mine even though I’m experiencing the same thing they are. I’ve talked to somebody about three different times about recalls on this and they just keep saying that I don’t have a recall, but I’m telling them about what I’m experiencing is similar to people with active recalls, but they will still charge me a diagnostic fee that I can’t afford. And tells me I’m not in warranty. So my safety is at risk.. I have children in this car. I have pets in this car and I have family in this car…..

LANE DEPARTURE: ASSIST

During normal driving in rainy conditions, I experienced a sudden and unexpected loss of vehicle control involving steering behavior. At the time of the incident, the lane-keeping/driver-assist system was active, and I had both hands on the steering wheel while actively controlling the vehicle. Without any prior warning lights, dashboard messages, or alerts, the vehicle suddenly began to behave abnormally and unexpectedly pulled/dragged. I experienced a sudden loss of effective steering control and was unable to fully correct or regain control of the vehicle before a collision occurred. The incident happened abruptly without any gradual warning or system notification. Road conditions were wet due to rain, but there was no prior indication of mechanical or electronic malfunction before the event. Following the crash, the vehicle was declared a total loss by insurance. There are no open recalls listed for my VIN at this time. I am reporting this incident due to the sudden steering/control behavior and potential safety concern.

Waynetown, IN Crash 1 injured

LANE DEPARTURE: ASSIST

Accident happened while driving on a two-way county road without center lane markings. The driver was leaning to the right of the road. The lane keep assisting overcorrected to the left hitting a culvert. Upon the crash the seat unlocked from its position and slid towards the steering wheel. The seat belt did not lock in time to prevent the seat from considerable forward movement. The airbag located inside steering wheel seemed only partially inflated. No other airbags deployed. The combined malfunctions resulted in the driver's left foot being crushed (various fractures and joints dislodged), and right ankle fracture and dislocation. The driver was the only one involved in the cash. However, other drivers could have been affected by the sudden change of lanes - especially if driving the opposite direction. The issue has not been reproduced as far as known (unable to find any reports, images or videos online). There was no inspection following the accident. The car had previously shown unexplainable lane keeping assistance overcorrections before, but there were no system failure warning lights. The seat sometimes needed readjustment (the locking system would click, but the seat would not remain in place). Summary of issues: -Lane keep assisting - overcorrected leading to hitting a culvert on the opposite side of the road. -Driver's seat unlocked forward toward the steering wheel when car hit the culvert. -Seatbelt did not lock appropriately to prevent seat moving forward toward the steering wheel. I am unsure if the car is still available for inspection. It was totaled by the insurance company. FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

This vehicle suffers from phantom braking. With the adaptive cruise control set, at times, in the display of the instrument cluster, the display shows a semi, motorcycle or car that is not there in real life. When this happens the car applies the brakes and starts slowing down. Sometimes this is bad enough that the collision warning message comes on and the brake application is very aggressive. The type of weather or road surface does not seem to matter. It seems to be very random. This happens once a week. Not just on the date provided below.

LANE DEPARTURE: WARNING

A complaint is being filed regarding a **2025 Honda Civic** involving failure to diagnose and repair a safety-related suspension defect during the warranty period. While under warranty (~20,000 miles), a **clunking noise from the rear suspension** was repeatedly reported to Tamaroff Honda [XXX] ). Despite multiple service visits and complaints to both the dealership and American Honda (Case #XXX and #XXX), no proper diagnosis or repair was performed. After continued inaction, another Honda dealership identified a **broken rear shock absorber with an active fluid leak**, a safety-critical defect affecting vehicle stability and control. This condition existed and was reported during the warranty period but was dismissed. There is also a pattern of complaint mishandling, including lack of follow-up and unresolved case activity. Requests for service records and complaint documentation have not been fulfilled. This complaint requests investigation into the safety risk and failure to diagnose and repair a known defect during warranty. LANE DEPARTURE: ASSIST

I am filing a formal safety-related complaint regarding my 2025 Honda Civic Sport Touring Hybrid and the performance of its driver assistance systems. This vehicle uses a camera-only driver assist and lane keeping system. I previously owned a 2018 Honda Civic Touring that used both radar and camera. The earlier radar+camera system performed significantly better, maintained the lane more accurately, and provided smoother, more predictable behavior. The 2025 camera-only system frequently: – drifts within the lane – loses lane tracking unexpectedly – requires frequent driver intervention – feels less stable at highway speeds In my opinion, the newer vehicle’s system represents a downgrade in functional safety compared with prior-generation Honda systems. This change was not disclosed during the sales process, and many owners may assume newer models offer equal or better driver-assist performance. My concern is that removal of radar and reliance on camera-only sensing has resulted in degraded lane keeping performance that could increase crash risk due to unexpected disengagement or poor lane centering. I request this information be reviewed as part of consumer safety monitoring of driver assistance systems. FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

While driving on Rt.119 through Uniontown PA, I was going 60-65 mph on a four lane road passing a cat on the right. I was about 3/4 of the way passing the car when three emergency braking kicked in and applied full braking. The road was straight, there was no vehicle or other obstruction in the road. This happened at approximately 3:30 P.M on 11/14/2025 with overcast sky and dry conditions. This caused the full sized tractor trailer behind me to lock up his brakes to attempt to not hit me. The only thing I can think of that kept me from getting hit, was I was able to hit the gas pedal.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

While driving at highway speeds with the Adaptive Cruise Control system active, the system is causing the vehicle to come to a complete stop to avoid a collision that does not exsist. In all cases when this has occurred, there are no other vehicles on the road. This inlcudes in front, beside or coming toward me from the opposite direction. When this does occur, the screen does show a vehicle icon as if it is detecting a vehicle in front of me but there is none. Since purchasing this vehicle in May 2025, this issue has occurred 6 times. I have brought this to my dealers attention during my most recent visit and was advised Honda was working on an update to resolve this issue but I have not received any communication from Honda indicating a recall was coming or an update to resolve.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

While utilizing adaptive cruise control in clear weather, the system will harshly brake when there are no obstacles or vehicles ahead. I have almost been rear ended on numerous occasions due to unexpected braking while at highway speeds. I have not gone to the dealership yet, but intend to do so. During these braking events there are no other warning lights, and no other visual or audio alerts.

When traveling with cruise control enable the vehicle random slams on the brakes by itself even when their is no vehicle present in front. This has happened mutiple times on the highway and interstate. It has almost gotten me rearened from a vehicle deiving behind me. This is extremely dangerous. I made the dealership aware of the issue and they refused to address it.
