The contact owns a 2024 Honda HR-V. The contact stated that while the contact's daughter was driving at 42 mph with the cruise control engaged, the collision mitigation sensor independently activated. The contact's daughter noticed that the steering wheel had almost seized, and the vehicle's speed abruptly decreased. The vehicle stopped in the middle of the road. Another vehicle was making a turn and crashed into the contact's vehicle. In addition, the "Brake" message was illuminated. The vehicle was driven to the contact's residence. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The contact mentioned that the failure was a known failure with the vehicle. The contact's daughter was taken to the local medical center. The contact's daughter sustained muscle pain in the back, neck, both shoulders, both jaws, and severe headaches. The manufacturer was not made aware of the failure. The failure mileage was approximately 61,000.

The contact stated while driving approximately 60 MPH, the Collision Mitigation Braking System (CMBS) warning light illuminated, and the vehicle independently decelerated. Several unknown warning lights were illuminated. Additionally, the contact stated that the vehicle warned the driver of the vehicle proximity to another vehicle, or of driving out of the intended lane erroneously. The contact stated that while attempting to roll down the passenger’s side window, the window failed to respond as intended. The vehicle was taken to an unknown local dealer on several occasions and was diagnosed with an engine oil leak, debris in the window module, and the battery needed to be replaced. No additional information was available. The manufacturer was made aware of the failure. The failure mileage was approximately 10,000.

FORWARD COLLISION AVOIDANCE: WARNINGS
I was driving normally, and a car was coming up to a stop sign from my right side on a completely different road and the car jerked and the brakes automatically applied to the car and the dash blinked “BRAKE” for a brief moment. I’ve had the car for awhile and it has never done anything like that. Some days the blind spot doesn’t work either, when I know there is a car next to me the light on the mirrors and the beeping does not activate.

On several occasions the automatic brights have turned on when there were cars at crossroads, and when cars were within sight. The regular lights of the car are repeatedly interfering with visibility of other drivers, as the lights are much too bright and angled right in other driver's eyes. We avoid driving at night due to blinding other vehicles and preventing crashes. We are flashed constantly. The automatic braking has turned on when there were no objects, pedestrians, or cars in front of the vehicle - nearly causing us to get rear-ended by the vehicle behind us. The brakes because sticky after only 16 months of use. The steering is sticky as well.

While driving, the car did an automatic hard brake for no reason. Traffic was accelerating after being stopped at a red light. The car in front of me was not braking and was accelerating away from me when suddenly the car hard brakes with no warning beforehand. It braked hard enough to throw me forward and cause traffic behind me to have to hard brake in order to keep from rear-ending me. There was an on ramp coming in near me but again that traffic was not in my lane. I have not complained to the dealership yet so it has not been reviewed and I have no reports since an accident did not happen. There were no warning lamps or messages before the event; although the brake light came on when the hard brake occurred, but not before. This was the first time it has occurred since purchasing the car about a year and a half ago. It was terrifying. It’s an hour later and I’m still shook up.

Vehicle has been brought to dealership multiple times due to messages coming up on dashboard stating road departure mitigation system problem. See your dealer and Collision mitigation breaking system problem. See your dealer. Each time the car has been taken in the dealership has stated that the problem cannot be duplicated or replicated and no error codes found. Vehicle is operating as manufactured. Detailed have been requested. On one occasion the dealership stated the vehicle would be needed for multiple days but tried to return it the next day. After calls were ignored for days for answer the vehicle was returned covered in ice and windshield wipers not in the proper position. There is believed to be minor damage in the drivers side wiper. After the message came up again the dealership has stated they have exhausted all attempts to diagnose issue and will no longer work on the vehicle. Multiple attempts been made to gain American Honda's participation but they have not proactively participated. The car has also attempted to leave the lane/road on to occasions. This seems connected to the Honda Sensing probe NHTSA has expanded to include 2020-2022 Honda CR-V and Accord. This message was received yesterday and today upon start up and putting the car into gear.

The vehicle keeps displaying a dashboard notification indicating "Road departure mitigation system problem. See your dealer. The vehicle was most recently brought back from the dealer and after about 100 miles the car displayed this message again. The dealer continues to maintain there are no codes found and issue is not duplicated. The vehicle on two occasions has tried to depart the lane on the highway with well marked lanes. I have ask numerous times for more information about attempts to duplicate and what codes should be displayed. This message has shown up around 20 times at this point. This is very concerning and poses a serious safety concern. Photos are from two instances. This message only comes up for a few seconds. The computer is failing to record this and I am being told there is noting that can be done since it cannot be duplicated. I have the photos and have experiences attempts by the vehicle to leave the lane. This is a hazard that could lead to injury or path of an vehicle occupants or general public. Please assist before injury or death occurs. There maybe an effort by both Honda and the dealership to conceal this issue as I do not believe that is not being duplicated even if there are no codes. When I'm driving, the safety brake has engaged while I was already braking. Also the emergency brake system has engaged while no car was on front of me. The closest car was on a side street, braked, and waiting for me to pass so they could onto the street after me.

While driving on the highway my power steering went out making it extremely hard to control the vehicle. As soon as I left the highway several indicator lights came on saying power steering not working, STOP driving and call dealer. Also, accident avoidance not working, lane departure not working, etc. Called Honda Roadside Assistance and had the vehicle towed to a dealer. This was at 6:00PM on a Saturday (Nov 16). Vehicle was inspected on Monday Nov 18 and everything was working fine. The vehicle history showed a drop in battery voltage which Honda believes caused the warnings to go off. I'm concerned that I had NO power steering which could have resulted in an accident.

LANE DEPARTURE: WARNING
My car has been brought in for service multiple times due to airbag issues. After the initial repair resembling of the car panels was not done correctly forcing me to bring back the vehicle. After this on another occasional a strange rattling noise began to happen while driving the vehicle. This was scheduled for service but then the noise went away. After this another airbag issue came up saying the vehicle had to be taken to the dealer. The dealer was inititally saying the issue could not be duplicated and I could pick up the vehicle. At this point I looked up recalls and found the recall regarding steering and that noises could be attributed to this. I ask the recall be performed and was told it could not be. Another issue was however found with the airbags and repaired. I was told to get vehicle as it was fine. Since then the noise has occurred on 3 seperate occasions. Additionally while driving with lane control the vehicle has 2 twice left lane on highway forcing me to correct. I feel things are being dealt with in a very dishonest way by Honda of America and it is instructing dealers to avoid correctly diagnoising issues and falsling claiming it cannot be duplicated to avoid itself costly repairs.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
While on cruise control on the freeway, car suddenly decelerated 20+ MPH. There was nothing ahead of me on the road at the time, no cars, no debris or construction materials, nothing. I was nearly rear ended. Car has not been inspected yet. Many alerts go off on the dash for unexplained reasons - that various safety mitigation systems are offline due to debris on windshield for example, when the windshield is totally clean. But there were no alerts prior to this deceleration at all. Drivers side speaker shorts out intermittently. Car is available for inspection on request. I haven't used cruise control since this incident.

For the second time, while driving normally, with no obstructions in my lane ahead of me, my car’s collision mitigation system abruptly and harshly hit my brakes while warning sounds deployed. This was enough to cause me and my passenger to leap forward in our seats; thankfully we were wearing our seatbelts. I have not been to the dealership yet for this issue, but will tell them at my first oil change. I’m really angry because the jolt this faulty sustem gave us was enough to trigger a panic attack or even a heart attack.
