BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING

The entertainment system shuts down and repeatedly restarts during driving. When it shuts down, an error screen states "Network Communication Lost" and "MOST NET OFF". When it shuts down, the rearview camera function fails when the vehicle is in the reverse gear. Using today's date. This failure has occurred repeatedly. The NHTSA Recall Number: 23V-431 does not include this VIN; however, the failure symptoms appear to be identical

* Collision Mitigation Braking System (CMBS) – Southeastern Honda: Shortly after purchase, the CMBS repeatedly slammed on emergency brakes without cause, incorrectly flagging adjacent or oncoming vehicles as collision threats. I presented video and photo evidence to Southeastern Honda. The Service Manager dismissed the hazard, telling me I "just have to learn how to drive the car with these driver assist features." Fearing an accident, I was forced to permanently disable the system. * Starter Failure / Battery Issues – Space Coast Honda: The vehicle went through four batteries and repeatedly failed to restart when Auto Idle Stop engaged, stranding my wife and children in parking lots and active traffic. I brought the car in multiple times insisting the starter was the issue, but Space Coast Honda refused to replace it because no codes were present and my VIN wasn't attached to a recall. Once out of warranty, I replaced the starter out-of-pocket, which permanently resolved the issue. Teardown of the original starter revealed extensive heat damage, burned insulation, and charred components. * Current Engine Concern – Space Coast Honda: The engine now exhibits rod bearing failure symptoms aligned with Recall 23V-751, including knocking and metallic debris in the oil and filter. I brought the vehicle and photo evidence to Space Coast Honda. One staff member claimed metal in the oil was "normal," while another disagreed. I left the car for evaluation, but two days later learned no inspection was done; they claimed Honda would deny it based on VIN exclusion and quoted $13,500 for a used engine. When I insisted they present my case to Honda anyway, they abruptly raised the inspection fee to $800—despite having completed zero diagnostics over two days.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

While operating my 2019 Honda Pilot, the vehicle experienced a sudden and complete failure of its advanced driver safety systems. Without warning, a cascade of simultaneous error messages illuminated on the dashboard, completely disabling the Collision Mitigation Braking System (CMBS), Road Departure Mitigation, Adaptive Cruise Control, and Lane Keeping Assist networks. This defect renders critical safety features completely inoperable and causes severe driver distraction due to persistent dashboard warnings flashing on the loop. This failure occurred when turning on the car. The sudden loss of safety systems while traveling at highway speeds poses an unreasonable risk of a crash, as the vehicle can no longer utilize automatic emergency braking or lane assistance features. The vehicle currently has about 88000 miles. The dealership has indicated the issue stems from a failure of the front-facing windshield camera sensor, matching the systematic defect patterns alleged in active federal class-action litigation against Honda. I am requesting that the NHTSA formally investigate this widespread camera component failure to mandate a safety recall.

Additional Comments for Complaint #11750912:I am uploading photographic evidence of severe material delamination on my vehicle's front radar emblem shield (Part #71125-TG7-A51) [71125-TG7-A51].This manufacturing defect caused a critical safety failure: my vehicle recently executed a sudden, violent phantom braking event on the highway with absolutely no prior warning lights or error codes on the dashboard. The peeling layers and air bubbles on the emblem physically refracted the radar signals, creating a false target that locked the brakes and nearly caused a severe rear-end collision. While I was driving alone during this specific incident, this car is my primary family vehicle used daily to transport my wife and my three young children. I am now genuinely terrified to drive my family in this vehicle and must manually turn off the CMBS system every time I start the car to protect them. American Honda Corporate (Case #16495852) has formally refused to address this active braking hazard, deflecting the issue by focusing only on unrelated radio glitches.I request a federal safety investigation into Honda’s radar emblem material degradation, as it directly compromises collision-mitigation systems without triggering automated dashboard fault alerts, putting families and young children at immediate risk.

LKAS, CMBS AND BRAKE SYSTEM AMBER ICONS ARE ON. INFOTAINMENT DISPLAY SHOWS ACC AND CMBS HAS A PROBLEM. I TRIED CLEANING THE WINDSHIELD AND SENSORS IN THE FRONT BUT SAME WARNINGS STILL CONTINUED TO SHOW AT NEXT START UP AND DRIVE. LAST FEW DAYS I STARTED IT STILL POPS UP SO I TOOK IT TO INDEPENDENT SHOP YESTERDAY 7/15/26. THEY PULLED A U-CODE FOR MULTIPURPOSE CAMERA, TRIED TO CLEAN THE SENSORS BUT WARNINGS WOULD STILL SHOW SO IT'S A HARD CODE NOW. I HAD THEM DOCUMENT IT FOR ME. THESE ARE SAFETY FEATURES THAT CAME WITH THE CAR, SHOULD THEY BE FAILING THIS EARLY ON? I HAVE NOT DONE ANYTHING TO THERE, HAVEN'T BEEN IN ANY ACCIDENTS INVOLVING THAT AREA AND HAVEN'T HAD WINDSHIELD REPLACED EITHER. THEY WEREN'T ABLE TO FIX IT AS THEY DON'T HAVE THE CALIBRATION EQUIPMENT SO SAID WE'D NEED TO SEND TO HONDA DEALER.

FORWARD COLLISION AVOIDANCE: WARNINGS

VEHICLE SAFETY COMPLAINT - RECALL REPAIR FAILURE AND CMBS MALFUNCTION I am filing a formal vehicle safety complaint regarding two critical defects on my vehicle (VIN: [XXX]) and the refusal of American Honda Motor Co. to address them under Corporate Case #XXX. 1. CRITICAL SAFETY HAZARD (PHANTOM BRAKING): The vehicle's Collision Mitigation Braking System (CMBS) / Automatic Emergency Braking (AEB) is experiencing severe and dangerous malfunctions. While driving at highway speeds through construction zones with absolutely zero obstacles, vehicles, or pedestrians ahead, the vehicle violently and unpredictably slams on the brakes by itself. This unprovoked emergency braking creates an immediate and extreme risk of a catastrophic rear-end collision on the highway. 2. FEDERAL SAFETY RECALL REPAIR FAILURE (CAMPAIGN ZEM): The vehicle's instrument cluster, infotainment display screen, backup camera, and audio system repeatedly go completely black and reset while driving, disabling all driver visibility of vehicle speed, safety alerts, backup assist, and vital indicators. While NHTSA records indicate that the ZEM Recall Campaign was previously logged as "completed" on this VIN, the defect is fully active. This constitutes an official federal recall repair failure. The original safety hazard remains unresolved. MANUFACTURER RESPONSE & REFUSAL: American Honda Corporate was formally notified of both issues under Case #XXX. Corporate responded by completely ignoring the severe phantom braking defect and refused to authorize a safety diagnostic. Furthermore, they used the administrative status of the closed ZEM recall to deny any responsibility for the ongoing screen, camera, and audio failure. American Honda is actively forcing me to drive a vehicle with a failed safety recall repair and a defective autonomous braking system unless I pay out-of-pocket diagnostic fees to the dealer (Dave Mungenast St. Louis Honda). I request an immediate federal investiga INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

My 2019 Honda Pilot EX-L (perfectly maintained and with only 72K miles) just started displaying a "Braking System" failure warning light along with dash notifications that a number of critical safety systems are no longer functioning, including: forward collision warning, automatic emergency breaking, lane keeping assistance and departure warning, adaptive cruise control, and more. The car registers a failure code of "U3000-49" which I've learned is related to a bad front camera - a problem apparently plaguing so many Honda vehicles like mine that there is now a class action lawsuit (Jones, et al. v. American Honda Motor Co., Inc.) related to it. Given the lawsuit shows how pervasive this situation is, and the nature of the safety risks involved, I hope this issue will be elevated to a formal NHTSA Safety Recall along with reimbursement for repair expenses (mine will be about $1500).

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

I am filing this complaint because my 2019 Honda Pilot has developed a serious safety-related defect. When I started the vehicle, multiple warning lights remain on, including the LKAS, ACC, CMBS, and Brake System warnings. I haven't had it inspected by the dealer yet, as the closest dealer is 25 miles away down a curvy mountain road. I expect that it will be determined that the front-facing camera has failed. Because this camera supports several driver-assistance and warning systems, the failure has made those systems unreliable and prevents the vehicle from accurately alerting me to other potential problems. This creates a significant safety concern because, if another malfunction occurs, I may not be able to distinguish it from the existing warnings. The warning lights remain active unless the camera is replaced at an estimated cost of $2,600. I do not believe owners should be responsible for such an expensive repair when the failure affects critical safety and warning functions. This issue appears to be recurring among 2019 Honda Pilots, with similar complaints reported by other owners online and in social media forums. I respectfully request that Honda investigate this defect and provide an appropriate remedy, including repair assistance or coverage for the failed camera system.
