My 2022 Honda Pilot developed an intermittent failure of the multipurpose forward camera used by the Honda Sensing system. Multiple dashboard warnings appeared simultaneously, including Adaptive Cruise Control (ACC), Lane Keeping Assist (LKAS), Collision Mitigation Braking System, Road Departure Mitigation, Auto High Beam, Blind Spot Information, and a Brake System warning. The vehicle’s normal brakes continued to function, but the affected driver-assistance and collision-avoidance features became unavailable. A Honda dealership diagnosed DTC U3000-49, documented as an internal failure of the multipurpose camera, and recommended replacement and aiming/calibration of the camera. The quoted repair cost was $2,356.95 plus tax. The failure is intermittent. On one occasion, all warning lights disappeared after starting the vehicle and the Honda Sensing safety features operated normally. After the vehicle was shut off and restarted, the warnings returned and the safety features were again unavailable. Honda declined my request for goodwill assistance and advised that this is not considered a known camera problem. I am reporting this because failure of a single camera disables multiple collision-avoidance and driver-assistance safety systems. I am concerned that other 2022 Honda Pilot owners may experience the same U3000-49 multipurpose-camera failure.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
My 2022 Honda Pilot Black Edition (VIN [XXX], 62,900 miles) has multiple intermittent safety‑critical failures involving the transmission, acceleration, adaptive cruise control, and HVAC blower. These issues occur without warning and without diagnostic trouble codes. Transmission/Acceleration Failure: Most recent incident was XXX on Highway 71 during heavy traffic. The vehicle twice experienced rapid deceleration with no ability to reach a safety stop. RPM dropped below 1000, then spiked to 6,000–8,000 while the vehicle continued to lose speed. Acceleration returned only after several seconds. This also occurs during highway merging and uphill acceleration. Additionally, when disengaging cruise control and immediately pressing the accelerator, the vehicle loses speed, RPM drops abnormally low, then spikes before power returns. Adaptive Cruise Control/Honda Sensing Failure: On XXX the vehicle rapidly decelerated when a semi‑truck was beside me, despite no vehicle ahead. The week prior, while driving several states away, setting cruise at 70 mph caused sudden deceleration and cruise resetting itself to ~54 mph. These events resemble phantom braking and radar mis‑targeting. HVAC Blower Failure: During a long multi‑state trip the week prior, the AC blower (speed 3–4) intermittently stopped blowing for several seconds, then resumed at the same speed with no change to the display. These failures affect powertrain, forward collision avoidance, vehicle speed control, electrical systems, and cabin equipment. The dealership (Roush Honda, Westerville OH) cannot replicate the issues and reports no stored codes. These defects pose significant safety risks. Brake light wont cut off and drains battery. Car wouldn't start and once battery was charged all steering, cruise control, hill assist, lane assist and other lighs came on. 7 total alerts came on and car struggled to go up hills and speed up after braking at lights. Brake light stayed on entire drive and didnt go off even when the car was off.

I was driving home at 4am on I-95 with my children in the car. My car has had a history of "juttering" and making a vibrational noise, but on the way home there was an issue with acceleration when using the gas pedal, and slow to gain speed. There were times when the gas pedal didn't respond to my foot depression at all, and did not accelerate. When I was almost home, many lights came on my dash including: Blind Spot warning, emissions systems problem, transmission problem apply parking brake when parked, stop driving when safe transmission problem see manual. The car is not driveable at this time and sitting in my driveway. I have no idea where to tow it to or what to do or how much it will cost

Windshield cracked majorly after being hit by a small rock on the highway. Even after having it replaced and the camera recalibrated, 7 safety warnings of the ADAS come on. Reportedly, the frontcamera has failed, which is concerning in a car that has less than 50k and is just about 4yrs old. I believe this is a larger issue, the windshield truly should not have broken that easily and the camera failing so soon is problematic. It is also a very expensive fix.

While attempting to move forward in a parking lot, the vehicle remained in Reverse despite selecting Drive. This failure led to a low-speed collision with a truck’s tow hitch. This defect creates a high risk of crashes, injuries to pedestrians, or property damage because the vehicle does not move in the direction the driver has selected. 3.Has the problem been reproduced or confirmed by a dealer or independent service center? The vehicle was previously serviced at a Honda dealership for a radio/infotainment recall. Since that service, the transmission has failed to engage Drive multiple times at random. 4.Has the vehicle or component been inspected by the manufacturer…? The vehicle was inspected during the initial recall fix. And brought back because after the fix because the charging port stopped working/continued screen glitches. Following the recent collision, it is awaiting further inspection by a Honda dealership to determine the extent of the mechanical failure. 5.Were there any warning lamps, messages prior…? Symptoms began after the recall fix & included intermittent electronic failures of the charging ports/screen. The blind spot monitoring system has also failed to properly detect nearby vehicles at times. The front windshield camera is causing the brake system error light to come on and triggering the safety system to malfunction. This failed camera system disables safety features and causes lane assist, cruise control , lighting features, back-up camera, driving assist, and the collision system to be disabled, causing a major safety concern. This front camera system has failed and has been diagnosed by the Certified Honda dealer that my vehicle was purchased from. My vehicle is 4 years old and has 35k miles, and the system has failed. Each component was inspected and issues diagnosed by the dealer. The warning light first displayed on the dashboard and showed as a "brake system" error light.
