The vehicle repeatedly displays a "Stop Vehicle and Check Brake System" warning together with an ABS warning, accompanied by a loss of normal braking response. On XXX, while driving at approximately 60 mph on I-85 northbound near Exit XXX in [XXX], the warnings appeared and the brakes did not respond normally. I was maintaining a substantial following distance, and the braking system restored itself before I reached the point of needing to brake hard, so no crash or injury occurred. Had hard braking been required in that window, I do not believe the vehicle would have stopped as commanded. This is a recurring fault. The vehicle, purchased certified pre-owned and still under the Hyundai factory warranty, has been taken to the dealer (Ed Voyles Hyundai, [XXX]) three times for the same condition. On the first visit the Integrated Electronic Brake (IEB) unit was replaced; on the second, the Smart Junction Box was replaced; on the third, the dealer removed an aftermarket tracking device wired to the data link connector and referenced Hyundai TSB 24-GI-011H, representing the problem as corrected. The braking-loss event on XXX occurred after that repair. The vehicle has been out of service approximately 30 days total. Because the failure is an intermittent loss of braking response at highway speed, I consider it a serious safety hazard and no longer drive the vehicle. 1. Sudden Unintended Acceleration On two (2) separate occasions, after coming to a complete stop at a red light with foot on the brake pedal, the vehicle suddenly lurched forward on its own. 2. Brake Pedal Malfunction On more than ten (10) occasions, when attempting to stop at a light, the brake pedal briefly behaves like the accelerator for approximately 1–2 seconds. Driver has to release the brake and press it again to restore normal braking function. 3. Doors Unlocking/Opening on Their Own On more than one hundred (100) occasions, approximately 10 minutes after parking — with no key fob or phone nearby — a door opens on its own. Confirmed via mobile app notification and visual confirmation from a distance each time. 4. Apple CarPlay Connectivity Issues CarPlay frequently fails to connect. During phone calls specifically, the connection drops in and out intermittently. 5. Sticky Residue on Door Handles A sticky, adhesive-like residue is present on the interior passenger door grab handle and rear door handles. Residue does not clean off and transfers to hands on every use.

OnXXX, as we approached what I call the 6 o’clock position of a round-about in [XXX], we had braked almost to a full stop (0-5 mph) to wait on the car ahead of us to enter the round-about when we felt increasing movement. Despite braking, to wait on that car, our car with increasing speed started shoving that car into the circle. Fortunately, he was able to drive away from us, but that car had been damaged on the back and right side. With that obstacle out of the way, our car rapidly and exponentially increased speed - all while we were fully braking. Two witnesses said they saw our brake lights as we sped along. We hit the curb as we approached the 12 o’clock exit, and that turned us so that we crossed both lanes of the road and headed towards the far sidewalk and storefronts. When we hit that curb it pushed us diagonally to face the street again. Hard to estimate but we must have been doing 50 mph when we hit a very large brick planter and we finally were wedged to a stop when we slammed in between two vehicles parked parallel to the sidewalk. The motor ran for a few seconds after we stopped, and finally quit on its own. We KNOW the brake was applied the whole time; there were no visual or audible alarms from the car as we entered “UNINTENDED ACCELERATION”. Despite the incredibly strong impact (our car has been deemed totaled) the front air bags did not deploy. Only the driver’s side curtain deployed. The bumper of the parked transport van on our front left was damaged, and the small SUV on our front right was deemed totaled. No people were in those two parked cars; the first car was hit at the initial slower speed so those people were not injured. By some grace, no pedestrians were in our path or they would surely have been killed. The driver of our car (my husband) was miraculously not injured, but CT scans and x-rays showed I had several contusions and a fractured tibia that will require surgery due to brake, propulsion, no air bags. On [XXX], I received a warning on my 2023 IONIQ 5 that said “stop vehicle and check braking system” and the car went into “turtle mode”. I took it into the dealer for repairs and they said the issue was related to the 12V battery and returned the car to me as repaired. Then, on [XXX], I was driving the car and I got the same error message but this time the car’s brakes didn’t work. Thankfully, I was able to pull off the surface road I was on and pulled into a residential area where I used the parking brake to stop the car. I then had it towed to the dealer and they are currently in the process of repairing it. My son and I were in the car at the time of the [XXX] incident and were very fortunate that we did not hurt anyone or ourselves when the brakes didn’t work. As of today, the Hyundai dealer (Roseville, CA) says they are repairing a brake component that was defective but they missed this on the 2/28/26 inspection. I am reporting a safety-related defect involving unintended vehicle movement on my Hyundai Ioniq 5 (Model Year 2023, SEL AWD). Observed behavior: With the vehicle in Drive and the Auto Hold function active (indicated by the white “AUTO HOLD” indicator in the instrument cluster), I brought the vehicle to a complete stop by pressing the brake pedal. Auto Hold engaged (text now green), the vehicle remained stationary, and I removed my foot from the brake. After approximately three minutes of remaining stopped, a driver-assistance system error message appeared on the instrument cluster. At that point, Auto Hold disengaged (the “AUTO HOLD” indicator disappeared), and the vehicle began to creep forward on its own. I did not command acceleration, did not press the accelerator pedal, and did not intentionally release the brake. The movement was unexpected and created a safety risk. The first occurrence happened in a school drop-off line with the vehicle stopped near other vehicles and pedestrians. I have since reproduced the behavior and captured it on video. Related observations: Similar driver-assistance system errors have occurred while the vehicle was in Park and in Drive, with or without Auto Hold active. In those cases, the vehicle did not move because it was either mechanically locked or I had my foot on the brake. In all cases, Auto Hold deactivated. Safety concern: This behavior raises concerns about the reliability of Auto Hold and its interaction with braking and driver-assistance systems, particularly after the vehicle has been stationary for an extended period. Unintended forward movement presents a significant safety hazard. I am reporting this issue for investigation of potential defects in software, sensors, or control logic. I am writing to report a serious safety incident involving my 2022 Hyundai Ioniq 5 and to request a thorough investigation and guidance on next steps. On Jan. 3, 2026, at approximately 6:15 p.m. on a snowy day, we parked our car in the driveway of a weekend rental home near Lake Tahoe, CA. The driveway had a slight slope toward the house, and we backed the car down the driveway with the rear of the car facing the house. My husband turned off the car, then all five occupants exited the vehicle, closed the doors, and began unloading items from the trunk. The car began rolling backwards toward us and the house. It only stopped after pinning my husband into a snowbank created by a snowplow. The other three adults and our [XXX] niece narrowly avoided being struck. While pinned behind the vehicle, my husband had to dig out some snow to reach into his pants pocket and retrieve the key fob. I then moved the car forward a few feet to free him. Although he was pinned in the snow, my husband was (thankfully) not seriously injured. After freeing my husband, I put the car in Park and turned it off. In this model, the parking brake engages automatically; there is no separate parking brake button or lever. A few minutes later, without anyone in or near the vehicle, the car again rolled backwards down the driveway into the same snowbank. Tracks in the snow clearly indicate that the wheels rotated and the car rolled; it was not sliding on ice with the brakes applied. In other words, the parking brake and/or Park mechanism failed to hold the vehicle in place in sub freezing, snowy conditions. The temperature at this location and time was in the mid 20s °F. After returning home from our trip, we took the car to our Hyundai service center, which has performed all maintenance since we purchased it in 2022. When coming to a stop on a slight incline, the vehicle will come to a full and complete stop and occasionally begin to roll backwards after half a 2nd without changing brake pedal position. If the brake pedal is not push further down, the vehicle will continue to roll backwards. I have tested this on multiple IONIQ 5 models with the same result (2022 and 2023). This could cause a collision if there is another car behind the problem vehicle. No warning lamps. Both vehicles have done this since new. Nothing is wrong with either of the vehicles according to multiple dealers.

On [XXX], I was in New Jersey waiting for my mother to finish shopping. In an effort to preserve battery life, I turned off the engine while waiting. Upon her return, I placed my foot on the brake pedal and started the car. As soon as I shifted the gear into "Drive," the vehicle made an unusual sound and began to roll forward. At no point did I remove my foot from the brake pedal, and I immediately applied more pressure. However, the brakes did not respond. Panicked by the fact that a parked car was directly in front of me, I quickly shifted the gear into "Park" to engage the emergency brake. Fortunately, this action brought the vehicle to a stop, and no damage occurred. I turned the vehicle off and restarted it after which the brakes functioned properly. The experience was extremely alarming. This is not the first time I have experienced this issue. On [XXX], a similar incident occurred while I was stopped at a red light. At that time, I assumed I may not have applied sufficient pressure to the brake pedal. The car rolled forward and made contact with the vehicle in front of me, resulting in a minor accident and a cracked front headlight. Given that this has now happened twice, I am concerned about the possibility of a recurring brake system failure and am worried for my safety and the safety of others. - Brake Failure, it is available for inspection upon request - if I didn’t act quickly and pressed the emergency brake, it would have resulted in an accident. Damage to my car and damage to the car In front of me. And could have resulted in injury to myself. -I have not taken the vehicle to the dealership or service center yet as I have not driven it since the incident. The Forward Attention Warning system in the refreshed 2025 Ioniq 5 gives constant false positives for both my wife and myself when we are driving the car. With Hyundai's HDA and lane keep active, the system keeps chiming, claiming that the driver is not looking at the road when we are. In fact, the calibration is so poor that it is a safety issue. The constant random warnings are very distracting, and sometimes the system does not recapture your eyes before timing out and hard braking the car on the highway. My wife was parking the 2024 ioniq and it lurched forward at a high rate of speed and slammed into another car parked in our driveway and drove that vehicle through our home garage door. She feels certain that her foot was on the brake, but even if she accidentally slipped and appleid the brake the forward collision system should have prevented the accident additionally the impact occurred with such force it is unlikely due to human error as it pushed our very heavy 4Runner back 3 feet and thorough a closed garage door.

The contact owns a 2022 Hyundai Ioniq 5. The contact stated that after the vehicle was parked on a slight incline on the road, the gear shifter was shifted to park(P), and the vehicle was turned off. The contact stated that while exiting the vehicle, the vehicle started to roll backward. The contact was able to jump back into the vehicle and depress the brake pedal to stop the vehicle. The contact stated that the vehicle was still in park(P); however, when the brake pedal was released, the vehicle started rolling away again. The contact had not engaged the parking brake. The contact stated that the vehicle was turned off and restarted and was able to shift into park(P) and the vehicle remained stationary. The contact stated no warning light was illuminated. The contact had not taken the vehicle to a local dealer or independent mechanic. The vehicle was not diagnosed or repaired. The manufacturer was not informed of the failure. The contact researched online and related the failure to NHTSA Campaign Number: 22V324000 (Parking Brake); however, the VIN was not included. The failure mileage was approximately 25,000.

When driving in i-Pedal, which is a "one pedal driving option", sometimes after pausing the car rolls backs several inches. This appears to be a defect with the powetrain, regenerative braking engine, or otherwise. Hyundai's dealership has been advised of this issue; they have escalated this internally. A car in drive forward should never due to electrical or software reasons be told to move backwards -- which is what is happening here.

I received this car new on 9/2/2024. I was driving at the freeway speed of traffic on 9/5/2024 when there was a sudden alarm and warning lights flashing on the dashboard with a message that said “Stop vehicle and check brake system.” At this same time, the brakes stopped functioning normally, and I had to use extreme pressure with my foot pressing down on the brake pedal all the way to the floor to have the car slow down even marginally. All of this happened with traffic in front, behind, and on both sides of me going 65 mph! I was very luckily able to maneuver to the right lane and slowly decrease speed without getting rear-ended and without slamming into a car in front of me while going at high speed unable to stop. While waiting for a tow, I left the car on, as I slowly edged forward to a safer stopping location. The flashing red ring around the message to “stop vehicle and check brake system” remained, and then just after moving forward, another message with an amber circle around it said “Check ABS system.” This latter message stayed on for only a short time, then was replaced by the initial message. I had the car towed to the dealership, and their assessment was that there was a failure of the IEP module that communicates between aspects of the braking system. The final service report is attached, confirming the need for replacement of the IEP assembly. I was very lucky to come away from this event with nothing more than a very frightening experience. I hope that this report will initiate a recall to prevent a similar situation happening to someone else, and hopefully avoid tragedy involving serious injury or death.

Backing vehicle out of driveway and went to engage brakes and they did not engage. began rolling backwards down street with steep grade. Kept stepping on brakes with no response. Had to turn into a parked vehicle to keep from rolling down steep grade and potentially off a substantial cliff. Happened today and dealership said that it may be because the "battery for the brake" had not received a charge from the main battery. Very dangerous and never disclosed. If I had not taken evasive action I could have easily been killed. Car is being dropped off at collision center today but is available for inspection. There were no warnings or other symptoms.
