NHTSA investigations into the 2009 Lexus IS 250

An investigation is the step before a recall: the agency has seen enough complaints to look into a pattern itself. An open one means it thinks something may be wrong and nothing has been fixed yet.

Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. Recalls on the 2009 Lexus IS 250

A complaint is one owner's allegation. A recall is the point at which the manufacturer or a regulator agreed the defect is real and dangerous enough to fix for free — however old the car is.

VISIBILITY/WIPER

Toyota is recalling certain model year 2006-2012 Lexus IS250 and IS350 vehicles manufactured August 23, 2005, through October 5, 2011; 2010-2012 Lexus IS250C and IS350C vehicles manufactured April 10, 2009, through October 6, 2011; and 2008-2011 Lexus IS-F vehicles manufactured December 14, 2007, through July 10, 2011. The nuts that fasten the wiper arms may be insufficiently torqued such that if the wiper arms are restricted by an external load, such as a buildup of heavy snow, one or both of the wipers may become inoperative.

Risk: Inoperative wipers could reduce driver visibility and increase the risk of a vehicle crash.

Canada runs its own recall programme, and its campaigns do not always mirror the American ones.

Recall 2021723 55,683 units in Canada

Electrical

Issue: This recall is for certain block heater kits installed on certain vehicles or sold as accessories at Toyota or Lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt. Note: This recall only affects the following block heater/cord part numbers: C014003034 PK5A408J09 PU14000904 C014003151 PK5A408J13 PU14000905 C014000885 PK5A489J11 PU14000907 C014000888 PK5A489J22 PU14000909 C014000907 PK5A410H00 PU14000911 C014100904 PU14000900 PU14000913 C014100907 PU14000901 PU14003571 C014100911 PU14000902 PK5A489J14 PK5A408J05 PU14000903 Safety Risk: Engine block heater wiring that overheats or melts could create the risk of a fire. Corrective Actions: Toyota recommends that you should stop using the engine block heater immediately. Toyota will notify owners by mail and instruct you to take your vehicle to a dealer to disable the engine block heater. For some vehicles, the corrective actions for this recall are under development. For others, a new engine block heater cannot be installed, and the owner will receive a refund of the purchase price of the engine block heater.

Recall 2018014 123,165 units in Canada

Airbag

On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the passenger frontal airbag. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. These issues could increase the risk of injury. Correction: Depending on the vehicle model, dealers will either replace the passenger frontal airbag inflator, or replace the airbag assembly. Note: Canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected.

Recall 2014458 18,949 units in Canada

Fuel System

On certain vehicles, the fuel delivery pipes in the engine compartment may have been produced with particles from the anti-corrosion plating on the gasket seating surface where the fuel pressure sensor is installed. This could affect the sealing properties of the gasket seated between the pressure sensor and the pipe, potentially resulting in a fuel leak. A fuel leak in the presence of an ignition source could result in a fire causing injury and/or property damage. Correction: Dealers will replace the gasket and repair the gasket seating surface.

Recall 2013031 16,756 units in Canada

Visual System

On certain vehicles, the wiper arm securing nuts could have been insufficiently tightened during assembly. If the movement of the wipers is restricted by an external load, such as heavy snow on the windshield, the wiper arms could slip and one or both wipers could become inoperative. If this occurs, driver visibility could be reduced, which could increase the risk of a crash causing injury and/or property damage. Correction: Dealers will clean the mating surface of the wiper arm and motor shaft and install improved wiper arm nuts.

Recall 2011037 11,773 units in Canada

Fuel System

On certain GS 300, GS 300 AWD, GS 350, IS 250, IS 250 AWD and IS 350 vehicles, the fuel pressure sensor connected to the engine fuel delivery pipe may not have been sufficiently tightened during vehicle assembly. Should the pressure sensor loosen, fuel could leak past a gasket used in the connection between the sensor and pipe, and through the threaded portion of the sensor. Fuel leakage, in the presence of an ignition source, could result in a fire causing property damage and/or personal injury. Correction: Dealers will inspect for fuel leakage from the threaded portion of the fuel pressure sensor. If there are no signs of a fuel leak, the fuel pressure sensor will be tightened to specifications. If a fuel leak is confirmed, the gasket between the sensor and the fuel delivery pipe will be replaced, and the sensor will be tightened.

Recall 2009290 437,445 units in Canada

Other

On certain vehicles, the accelerator pedal may become stuck in the wide open position due to an unsecured or incompatible driver's floor mat. A stuck open accelerator pedal may result in very high vehicle speeds and make it difficult to stop the vehicle, which could cause a crash, serious injury or death. Correction: Dealers will reconfigure the shape of the accelerator pedal. Certain models will also have the shape of the floor underneath the accelerator pedal modified and/or a brake override system installed.

MELTING DASHBOARD RECALL. I HAVE A 2009 LEXUS IS 250 MELTING CHIPPED DASHBOARD AND DOORS. IT'S VERY HARD TO SEE WHILE DRIVING DUE TO THE FACT THAT THE DASHBOARD CAUSES A GLARE IN THE WINDOW. ALSO THE SMELL OF MELTING PLASTIC DASH IS SICKENING. A YEAR OR SO AGO MY CAR STARTED OVERHEATING. I STOPPED AND THE CAR SHUT OFF. THE COMPUTER SYSTEM FROZE AND SAID CHECKING SYSTEM AND SHOWED A DRAWING OF A CAR FRAME AND WHAT I GUESS IS THE HEAT SOURCE. I TOOK MY CAR TO THE MECHANIC AND HE REPLACED THE WATER PUMP. IT HAD CRACKED AND BOILED OVER AND EVER SINCE I HAVE HAD NOTHING BUT PROBLEMS. I'VE REPLACED THE ALTERNATOR, THE BATTERY, AND MY NAVIGATION SYSTEM IS BLANK AND COMPLETELY DOESN'T WORK. NOR DO THE BUTTONS AROUND IT. THE MOTHERBOARD IS FRIED. ALSO SOMETIMES WHEN I'M DRIVING AT NIGHT MY HEADLIGHTS TURN OFF IN THE PITCH BLACK DRIVING DOWN THE ROAD. MY DASHBOARD IS MELTING AND SINCE I'M A REALTOR WHO PUTS A LOT OF MILES ON HER CAR, I'M UNDERWATER. I'M EITHER GOING TO HAVE A HEAT STROKE DRIVING THAT SAUNA AROUND (THE HEAT IS STUCK ON) OR CRASH WHEN MY HEADLIGHTS SUDDENLY STOP WORKING. MY DAUGHTER IS LEARNING TO DRIVE AND I CANNOT LET HER DRIVE ONCE IT GETS DARK. IT'S OKAY FOR ME TO DIE BUT NOT HER. THERE DOESN'T NEED TO BE A SENSOR RIGHT UNDER THE WATER PUMP THAT CAN DESTROY YOUR HOLE CAR. THANK YOU FOR YOUR TIME.

MY DASHBOARD OF MY 2009 LEXUS IS 250 IS MELTING AND CAUSING A SIGNIFICANT GLARE ON MY WINDSHIELD WHEN I'M DRIVING THE VEHICLE. IT IS PROGRESSIVELY GETTING WORSE. CONCERN FOR VISIBILITY THROUGH THE WINDSHIELD AS THE DASHBOARD CONTINUES TO MELT. THE GLARE IS WORSE DEPENDING ON THE TIME OF DAY AND DIRECTION I'M HEADING. THERE WAS A RECALL FOR THIS ISSUE BUT ONLY WENT UP TO 2008 MODELS. RECALL SHOULD HAVE INCLUDE 2009 MODELS AS WELL.
