Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. Toyota is recalling certain model year 2006-2012 Lexus IS250 and IS350 vehicles manufactured August 23, 2005, through October 5, 2011; 2010-2012 Lexus IS250C and IS350C vehicles manufactured April 10, 2009, through October 6, 2011; and 2008-2011 Lexus IS-F vehicles manufactured December 14, 2007, through July 10, 2011. The nuts that fasten the wiper arms may be insufficiently torqued such that if the wiper arms are restricted by an external load, such as a buildup of heavy snow, one or both of the wipers may become inoperative.

Risk: Inoperative wipers could reduce driver visibility and increase the risk of a vehicle crash.

Issue: This recall is for certain block heater kits installed on certain vehicles or sold as accessories at Toyota or Lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt. Airbag

Issue: On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, may eventually degrade the propellant contained in the passenger-front airbag. This could cause the airbag to deploy with more force than normal. If the airbag inflator ruptures, fragments could be propelled toward vehicle occupants or cause damage to the airbag assembly, preventing its proper function. Safety Risk: If the vehicle is involved in a crash requiring an airbag deployment, the airbag inflator could rupture and create a risk of injury. Corrective Action: Owners will be notified by mail and instructed to take their vehicle to a dealer to have the passenger-front airbag inflator or airbag module replaced. Note: The Canadian climate results in the propellant degrading slowly. This recall is being conducted as a precaution to address future risk. It is expected that all airbag inflators will be replaced before their function would be affected.

On certain vehicles, the wiper arm securing nuts could have been insufficiently tightened during assembly. If the movement of the wipers is restricted by an external load, such as heavy snow on the windshield, the wiper arms could slip and one or both wipers could become inoperative. If this occurs, driver visibility could be reduced, which could increase the risk of a crash causing injury and/or property damage. Correction: Dealers will clean the mating surface of the wiper arm and motor shaft and install improved wiper arm nuts.

THIS IS A STUPID AND UNACCEPTABLE RESPONSE. THIS TAKATA AIRBAG RECALL HAS BEEN OUT FOR SEVERAL YEARS AND SOMEONE IN TOYOTA/LEXUS HAS KNOW ABOUT IT? SOMEONE HAS BEEN COVERING UP THE MESS AT LEXUS? THIS BORDERS ON FRAUD AND AS A MINIMUM SOMEONE HIGH IN THEIR ORGANIZATION HAS LIED TO KEEP THIS UNDER COVER FOR THIS LONG. NOT DRIVING THE VEHICLE IS NOT AN OPTION. I HAVE PURCHASE 3 LEXUS IN THE PAST BUT THIS WAS MY LAST.

THE FAILURE OCCURRED MORE THAN ONCE. THE VEHICLE WAS NOT DIAGNOSED OR REPAIRED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 17,000. THE CONSUMER STATED WHEN SHE STEPPED ON THE BRAKE TO START THE VEHICLE AND PUT THE GEAR IN REVERSE, THE VEHICLE LURCHED BACKWARDS AS SHE LIFTED HER FOOT OFF THE BRAKE. SHE WAS UNABLE TO STOP THE VEHICLE WITH THE BRAKE. SHE THREW THE VEHICLE INTO PARK AND TURNED IT OFF. THE VEHICLE WAS TOWED TO THE DEALER, WHERE THEY KEPT IT OVERNIGHT AND RAN PERFORMANCE CHECKS. THEY DID NOT FIND ANYTHING WRONG AND SAID IT COULD HAVE BEEN THE WEATHER MAT THAT WAS ON TOP OF THE LEXUS CARPET MAT. THE CONSUMER PAID FOR THE REPAIRS, BUT DID NOT STATE WHAT THEY WERE. THE CONSUMER STATED IT WAS THE SECOND TIME, SHE EXPERIENCED SUDDEN ACCELERATION. UPDATED 09/17/15
