The front and rear backup cameras on my 2022 Mitsubishi Outlander have completely failed, leaving me with a blank, black screen. This creates a severe safety hazard when parking and reversing. My vehicle's VIN explicitly shows an active "Remedy Status: Repair Available" on the official Mitsubishi portal for safety recalls SR-25-001, SR-23-001, and SR-22-003.The local dealerships claim the recall was already completed in April. However, if the safety issue was truly and permanently fixed, the "Remedy Status" column would match and say "Completed." Instead, the manufacturer's live system explicitly says "Repair Available," proving my vehicle is legally owed the updated software fix.Despite this clear safety status, Jeremy Franklin Mitsubishi (demanding $182) and Northtowne Mitsubishi (demanding $169) are refusing to perform these safety repairs unless I pay an out-of-pocket diagnostic fee. Mitsubishi Corporate Customer Relations has also refused to stop the dealerships from imposing these illegal financial barriers on an open federal safety recall

The contact owns a 2024 Mitsubishi Outlander. The contact stated that while driving approximately 30 MPH, the Forward Collision Avoidance system failed to activate as needed and the vehicle nearly crashed into the rear of another vehicle. The contact stated that during the failure, the vehicle in front struck a pedestrian, and the pedestrian went airborne and landed on the hood of the contact's vehicle. The contact stated that the Forward Collision Avoidance system failed to immediately activate, and the brake pedal was depressed to avoid crashing into the rear of the vehicle ahead. The pedestrian sustained unknown injuries and was transported to the hospital. The contact mentioned that the vehicle was taken to a dealer on several occasions for the seat belts failing to latch and secure the occupants. The dealer mentioned that there might be crumbs preventing the latch however, the contact mentioned that theory was not possible for the driver's seat belt. The failure was present with the seat belts in the second and third row seats. The cause of the failure was not yet determined. A Police report was filed at the scene. The contact rented a 2024 Mitsubishi Outlander from Drivo Car Rental. The contact stated that while driving at an undisclosed speed, there was an abnormal sound. The contact stated that there was a vibrating sound coming from the hood. The front windshield was fractured on the passenger's side, and the instrument cluster failed to function as intended. The contact stated that the safety features were inoperable, and the vehicle was almost involved in a crash. No warning lights were illuminated. The contact stated that the failure had occurred while traveling with the family. The contact stated that the vehicle was unsafe to drive. The vehicle was returned to Drivo Car Rental. The contact stated that while returning the vehicle to the agency, the contact was informed that the vehicle should not have been rented. In addition, the contact researched and became aware of an unrepaired recall associated with the VIN under NHTSA Campaign Number: 25V369000 (Back Over Prevention). The manufacturer was not made aware of the failure. The failure mileage was approximately 45,000.

While putting my car in reverse my backup camera froze and then went completely black. I have tried the hard and soft reset with no luck. You can hear the infotainment system trying to load over and over again with a black screen. While researching, I see there was a previous recall for this exact problem that required a software update which mine had right before I bought it used less than a year ago. If the update did not work or only worked temporarily, the unit had to be replaced. I’m not getting anywhere with my dealership for help. They offered to sell me a $5,000 unit or told me to buy an extended warranty for electronics, wait a month and file a claim. I find it hard to believe that this is not related to recall. I had a 13 year old Nissan pathfinder before his and my backup camera/screen was still going strong.

Recall repair did not solve the camera problem. Rear camera and parking assist camera's do not come on. When car is shifted into reverse, camera does not appear. This issue was already reported and I have completed the recall maintenance for this issue and the problem still persists. This is a massive safety liability. I took the car into the dealership and they confirmed the recall maintenance has already been completed. They then charged me $100 to do a diagnostic test, but they couldn't determine why the cameras were failing. I paid $100 and left with nothing fixed.

2 issues contributed to accident: 1st - Vehicle did not go into Park when button on gear shift was pushed. Driver did not notice that vehicle remained in Drive and exited the vehicle. 2nd - Vehicle did not automatically engage the emergency brake/park mode it is designed to do when Vehicle is in drive and driver door opens. As a result, Vehicle began to roll forward. Driver was knocked down and fell to ground and Vehicle ran over her legs, coming to a stop with the rear Driver side tire on her leg.

When driving with the adaptive cruise control on, the cruise will shut off without notice at random times. The forward collision warning light then comes on. After driving for some time the light will go out and the adaptive cruise will work again. This repeats many times on a trip randomly. This also leaves the vehicle without forward collision protection and automatic emergency braking. This is very dangerous at highway speed when you lose power driving with traffic. When reported to the dealer, they said it was a known issue without a solution and to not use the adaptive cruise and drive carefully without the forward collision protection active. This has been an issue since the car was new in November of 2024.

ENGINE MALFUNCTION SERVICE NOW APPEARED (ERROR) I took the vehicle to the dealership for evaluation. The mechanic checked, reset, and reprogrammed the module. I was instructed to return if the error occurred. It did after driving 5 miles. The mechanic rechecked for possibly having different errors. I was instructed that it was okay to drive with the errors. Unfortunately, after coming to a stop, I was unable to accelerate! I was on a major road and I was unable to move off to the side of the road. The vehicle in this state is life-threatening. (A tanker truck thankfully was able to stop!) I was able to limp to the dealer where I purchased the vehicle. NOTE: This has been a well-documented issue (Check OUTLANDER FORUM). My 2024 Outlander briefly had the errors, but they cleared themselves. I feel this needs expedited to severe, requiring a mandated software update before a life is lost! This error inhibits frontal collision sensors!

The contact stated that on several occasions while driving at 25 MPH, a "be careful due to icy roads" message appeared, but it was the middle of May and there was no ice or cold weather in sight. Automatic Emergency Breaking sometimes occurs while driving in rainy weather even when there are no obstructions in the front of the vehicle. This causes spinouts and exccessive wear and tear on break pads and rotors. Also, many shops and dealerships are having issues with putting 2022 year model outlanders in "maintenance mode" to be able to change rear ebs brakes. Due to this you cannot drive your vehicle and/or rish not having proper break functions. THIS IS URGENT!!!

Vehicle: 2025 Mitsubishi Outlander PHEV I am filing this safety complaint regarding a recurring electrical/software defect in my 2025 Mitsubishi Outlander PHEV affecting the backup camera display and infotainment screen. The backup camera screen repeatedly freezes and goes completely black while the vehicle is in reverse gear. This eliminates all rear visibility during backing maneuvers, posing a serious risk of striking pedestrians, cyclists, or objects — especially children in blind spots who cannot otherwise be seen. In addition, the infotainment screen has shut off entirely at least once while the vehicle was being driven, creating a dangerous distraction and loss of vehicle controls. I want to be clear that my dealership has acted in complete good faith and has done everything within their power to diagnose and resolve this issue. They have worked diligently on my behalf and are not the source of the problem. The failure to provide a fix lies squarely with Mitsubishi Motors North America, which has been unable or unwilling to supply an adequate repair, software update, or resolution despite being made fully aware of this ongoing safety defect. I have contacted Mitsubishi Motors North America directly on multiple occasions. They have failed to provide a satisfactory explanation, a committed timeline for a fix, or any path toward resolution. This is not a dealer-level issue — this is a manufacturer-level failure to address a known defect. This is not a convenience issue — the failure of a federally mandated backup camera system is a safety defect. Federal law (49 CFR Part 571, FMVSS 111) requires rear visibility systems to function properly at all times. I respectfully request that NHTSA open a formal investigation into this defect and determine whether a recall or technical service bulletin is warranted for the 2025 Mitsubishi Outlander PHEV.

David the Mitsubishi technician drove with me today and witnessed the issues with the adaptive cruise control system. 1. Slow to gain speed on resume in any situation. 2. Car slamming on brakes when sensing a car at a red light, when a car leaves the lane that I am in and there is a stopped car at a red light or not stopping at all. 3. When making a turn into a left or right lane, car will not sense the other car or it will slam the brakes unsafely
