Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. AIR BAGS:FRONTAL:PASSENGER SIDE:INFLATOR MODULE

General Motors LLC (GM) is recalling certain 2003-2004 Pontiac Vibe vehicles equipped with a front passenger air bag assembly replaced under a prior recall. The replacement air bag may not unfold as designed during inflation in high temperature conditions, possibly resulting in the air bag not inflating properly.

Risk: An air bag that does not inflate properly increases the risk of injury in the event of a crash.

18V883000

AIR BAGS:FRONTAL:PASSENGER SIDE:INFLATOR MODULE

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2002-2005 Lexus SC and Toyota Sequoia, 2003-2005 Toyota Corolla and Tundra, and 2003-2007 Pontiac Vibe vehicles. These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the passenger frontal air bag, these inflators may explode due to propellant degradation.

Risk: An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.

15V285000

AIR BAGS

Toyota Motor Engineering & Manufacturing (Toyota) notified the agency on May 13, 2015 that they are recalling certain model year 2004 Sequoia vehicles manufactured August 18, 2003, to August 19, 2004, and 2003-2004 Tundra vehicles manufactured March 18, 2003, to August 25, 2004. On June 16, 2015, Toyota expanded the recall to include an additional 1,270,468 vehicles, including model year 2002-2003 and 2005-2007 Sequoia, 2005-2006 Tundra, 2003-2007 Corolla, 2003-2007 Corolla Matrix, 2003-2007 Pontiac Vibe and 2002-2007 Lexus SC vehicles. This is to address a safety defect in the passenger side frontal air bag inflator which may produce excessive internal pressure causing the inflator to rupture upon deployment of the air bag. This recall addresses both the passenger side frontal air bags that were originally installed in the vehicles, as well as replacement air bags that may have been installed as replacement service parts. A replacement air bag may have been installed, as one example, if a vehicle had been in a crash necessitating the replacement of the passenger side frontal air bag.

Risk: In the event of a crash necessitating deployment of the passenger's frontal air bag, the inflator could rupture with metal fragments striking the vehicle occupants potentially resulting in serious injury or death.

15V286000

AIR BAGS

Toyota Motor Engineering & Manufacturing (Toyota) notified the agency on May 13, 2015, that they are recalling 2005-2007 Lexus SC, Toyota Corolla, Corolla Matrix, Sequoia, and Pontiac Vibe, and 2005-2006 Toyota Tundra vehicles that were originally sold, or ever registered, in Florida, along the Gulf Coast, Puerto Rico, Hawaii, the U.S. Virgin Islands, Guam, Saipan and American Samoa. On June 16, 2015 Toyota expanded the recall to cover an additional 769,055 vehicles, including certain model year 2002-2004 Lexus SC, Toyota Sequoia, 2003-2004 Toyota Corolla Matrix, Toyota Corolla, Pontiac Vibe, and Toyota Tundra vehicles, as well the vehicles covered by the May 13, 2015 notification in the portions of Georgia, Alabama, Louisiana, Mississippi and Texas that were not previously included. The inflators in the front passenger air bags are susceptible to rupture in the event of a crash necessitating the deployment of the front passenger air bag.

Risk: In the event of a crash necessitating deployment of the passenger's frontal air bag, the inflator could rupture with metal fragments striking the vehicle occupants potentially resulting in serious injury or death.

15V043000

AIR BAGS:FRONTAL

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2003-2004 Toyota Corolla vehicles manufactured December 28, 2001, to May 2, 2004, 2003-2004 Toyota Corolla Matrix vehicles manufactured January 6, 2002, to April 29, 2004, 2003-2004 Avalon vehicles manufactured June 5, 2002, to December 20, 2004, and 2003-2004 Pontiac Vibe vehicles manufactured January 18, 2002, to April 27, 2004. Due to electrical noise, a component in the air bag control module may fail, causing the front air bags, side air bags, and/or seatbelt pretensioners to deploy inadvertently while the vehicle is being operated.

Risk: Inadvertent deployment of the air bags or the seat belt pretensioners increases the risk of an injury or crash.

13V029000

AIR BAGS

Toyota is recalling certain model year 2003-2004 Corolla and Corolla Matrix vehicles and 2003-2004 Pontiac Vibe vehicles. The supplemental restraint system (SRS) circuits are susceptible to internal shorting. The electrical short may create an abnormal current flow and increased heat which can damage the circuits. This could result in an inadvertent deployment of the front airbags and/or seat belt pretensioners.

Risk: Inadvertent deployment of the airbags or the seat belt pretensioners may increase the risk of injury and the possibility of a vehicle crash.

08V162000

VISIBILITY:GLASS, SIDE/REAR

TOYOTA IS RECALLING 662,178 MY 2003-2004 COROLLA, COROLLA MATRIX AND PONTIAC VIBE VEHICLES EQUIPPED WITH POWER WINDOWS. THE DRIVER AND FRONT PASSENGER DOOR GLASS BOLTS MAY LOOSEN AND COME OFF, CAUSING THE DOOR GLASS TO SEPARATE FROM THE WINDOW REGULATOR. DUE TO THE COMPONENTS BECOMING LOOSE AND POSSIBLY RATTLING, AN ABNORMAL NOISE MAY BE HEARD FROM THE DRIVER AND /OR FRONT PASSENGER DOOR WHEN OPERATING THE POWER WINDOWS.

Risk: THE DOOR GLASS MAY SEPARATE FROM THE WINDOW REGULATOR, BIND AND SHATTER DURING OPERATION OF THE POWER WINDOWS, CAUSING DRIVER DISTRACTION AND/OR INJURY.

02V074002

POWER TRAIN:AXLE ASSEMBLY

ON CERTAIN TWO-WHEEL DRIVE (2WD) PASSENGER VEHICLES, IT IS POSSIBLE FOR THE BOLTS THAT JOIN THE REAR BRAKE AND HUB ASSEMBLY TO THE REAR AXLE CARRIER TO BE LOOSE DUE TO INSUFFICIENT TIGHTENING DURING THE ASSEMBLY PROCESS.

Risk: IF THE VEHICLE IS OPERATED IN THIS CONDITION, ONE OR MORE OF THE FOUR BOLTS COULD COME LOOSE, CAUSING AN ABNORMAL NOISE FROM THE REAR AXLE AREA. IN THE EVENT THAT ALL FOUR BOLTS LOOSEN COMPLETELY, SEPARATION OF THE REAR BRAKE AND HUB ASSEMBLY FROM THE REAR AXLE CARRIER COULD OCCUR, WHICH COULD LEAD TO A LOSS OF VEHICLE CONTROL.

Canada runs its own recall programme, and its campaigns do not always mirror the American ones.

Recall 2020021 88 units in Canada

Airbag

Issue: Certain cars that were repaired under a previous airbag recall may have had an incorrect passenger-front airbag installed. As a result, the airbag may not unfold properly when it deploys. This could cause the airbag to tear or not to inflate properly in a crash. Safety Risk: An airbag that does not work properly could create an increased risk of injury in a crash. Corrective Actions: General Motors will notify owners by mail and instruct you to take your vehicle to a dealer to inspect the passenger-front airbag assembly. It will be replaced if necessary.

Recall 2018707 29,827 units in Canada

Airbag

On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the passenger frontal airbag. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. These issues could increase the risk of injury. Correction: Dealers will replace the passenger frontal airbag module.<br><br> Note: Canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected. Vehicles may have had the original front passenger airbag module replaced as a "like-for-like" replacement under recall 2015-272 (GM Recall #14491) as a temporary remedy while evaluating a permanent remedy. These vehicles will receive the permanent remedy passenger airbag assembly under this recall.

Recall 2016106 60,672 units in Canada

Airbag

On certain vehicles, the passenger (frontal) airbag inflator could produce excessive internal pressure during airbag deployment. Increased pressure may cause the inflator to rupture, which could allow fragments to be propelled toward vehicle occupants, increasing the risk of injury. This could also damage the airbag module, which could prevent proper deployment. Failure of the passenger airbag to fully deploy during a crash (where deployment is warranted) could increase the risk of personal injury to the seat occupant. Note: This recall supersedes recalls 2013-116, 2014-301 and 2015-272. Correction: Dealers are to replace the front passenger airbag assembly with one equipped with a newly specified inflator.

Recall 2014568 20,605 units in Canada

Airbag

On certain vehicles, a defect in the supplemental restraint system could result in an inadvertent deployment of the front airbag(s) and/or seatbelt pretensioner(s). Unintended seatbelt pretensioner and/or airbag deployment, in a non-warranted (non-impact) situation, could startle the driver, which could result in a vehicle crash causing property damage and/or personal injury. In some instances, inadvertent deployment could cause minor injuries to vehicle occupants. Correction: Dealers will replace the airbag control module and may install an in-line jumper wiring harness to correct the issue. Note: This recall supersedes recall 2013033. Vehicles that were repaired under the previous campaign and did not receive a new airbag control module will also need to be repaired under this campaign.

SINCE THE DAY OF THE REPAIR THE LID ON TOP LEFT SIDE OF THE DASHBOARD HAD POPPEDUP AND DID NOT CLOSE FULLY. BUT RECENTLY THE NOISE HAS BECOME STEADY. I HAD MY MECHANIC WHO SPECIALIZES IN TOYOTA CARS CHECK THE CAR IN APRIL 2021. MY MECHANIC HAPPENS TO BE FAMILIAR WITH THE TAKATA AIRBAG ISSUE. HE SAYS THAT THE AIRBAG REPAIR DONE ON THE CAR IN 2020 IS THE ROOT CAUSE OF THE NOISE. THAT IS TO SAY MY CAR WAS MESSED UP AS A RESULT OF THIS SO CALLED RECALL REPAIR. TAKATA RECALL TWO TIMES TRIED TO GET THIS RECALL FIXED FOR AIR BAGS, AS MY PRECIOUS DAUGHTER DRIVES THIS CAR. BEEN SCREWED AROUND FROM THE DEALERSHIP. WASTED SO MUCH TIME ON THE PHONE AND TAKING THE VEHICLE IN TO THE DEALER. BOTH TIMES THEY WERE UNABLE TO FIX THE RECALL SAYING THEY DIDN'T HAVE THE RIGHT PART. I BELIEVE THIS RECALL IS A FAKE AND GM IS GOING THROUGH THE MOTIONS ONLY TO APPEASE THE NHTSA, THE REALITY IS VEHICLES ARE NOT BEING FIXED AS PROMISED! SO DISAPPOINTED.

TAKATA RECALL I TOOK MY VEHICLE IN FOR A TAKATA RECALL. AFTER THE WORK WAS DONE, MY SRS LIGHT CAME ON. WHEN I WENT TO THE DEALER, THEY SAID IT HAD NOTHING TO DO WITH THE WORK THEY DID. THEY DIDN'T LOOK AT THE CAR, THEY DIDN'T DIAGNOSE THE PROBLEM, THEY JUST SAID IT WASN'T THEIR FAULT, BUT I COULD PAY TO HAVE THEM HOOK UP THEIR COMPUTER TO DIAGNOSE THE CODE. I DIAGNOSED THE CODE MYSELF AND IT IS FOR AN "AIRBAG STAGE 2- OPEN CIRCUIT." I FIND IT HARD TO BELIEVE THAT THE DEALER (AN AUTHORIZED GM DEALER - BILL PIERRE CHEVROLET IN SEATTLE) WORKED ON MY AIRBAG SYSTEM AND THEIR WORK HAS NOTHING TO DO WITH THIS AIRBAG SYSTEM FAULT THAT POPPED UP AFTER THE WORK WAS PERFORMED. THEY DIDN'T DIAGNOSE THE CODE, THEY DIDN'T LOOK AT THE CAR, THEY JUST DISMISSED MY COMPLAINT, AND TOLD ME TO PAY TO HAVE IT REPAIRED OR EVEN LOOKED AT.

I RECENTLY FOUND OUT FROM OTHER PONTIAC VIBE AND TOYOTA MATRIX OWNERS THAT THE ODOMETER STOPS RECORDING AT 299,999 MILES...THIS WAS A FACTORY GOOF AND QUITE A FEW PEOPLE HAVE TRIED TO GET WITH THEIR MANUFACTURER ABOUT IT AND SAID THAT IT WAS THE OWNERS RESPONSIBILITY TO HAVE THE PROBLEM FIXED/ODOMETER REFLASHED AT OWNERS EXPENSE...WE KNOW THAT THE OTHER VEHICLES THAT ARE BEING DRIVEN TODAY, THEIR ODOMETER CAN MAX OUT AT 999,999 MILES...IN MOST TO ALL STATES, IT IS ILLEGAL TO ALTER THE ODOMETER IN ANY WAY AND SHOULD HAVE CORRECT MILEAGE STATED EVERY TIME WE RENEW OUR REGISTRATION, BUT HOW CAN WE WHEN THEIR IS STILL ALOT OF THESE VEHICLES ON THE ROAD, BUT CANNOT TRULY ADDRESS THEIR CORRECT MILEAGE BECAUSE THE MANUFACTURER PROGRAMMED THE ODOMETER INCORRECTLY....PLEASE OPEN A CASE AND CONSIDER A RECALL TO HAVE THE MANUFACTURER FIX THEIR PROBLEM....THANK YOU

TAKATA RECALL; I RECEIVED A RECALL NOTIFICATION DIRECTING ME TO GO TO KOOL CHEVROLET FOR REPAIRS. PARTS WERE ORDERED AND A SERVICE PERSON CALLED AND AN APPOINTMENT WAS MADE. THE SERVICE MANAGER JASON IMMEDIATELY CALLED ME TO SAY THEY WOULD NOT HONOR THE APPOINTMENT AND WORK ON MY CAR BECAUSE OF AN INCIDENT AT KOOL TOYOTA NEXT DOOR RELATING TO INCOMPETENT WORK PERFORMED ON MY 2012 TOYOTA RAV4 TWO YEARS AGO. ARE TOYOTA AND GM NOW ONE AND THE SAME? WHY SOLICIT MY BUSINESS AND THEN REFUSE TO DO IT AFTER ALL ARRANGEMENTS HAVE BEEN MADE? CAN A GM DEALER "BLACKBALL" A CONSUMER TRYING TO COMPLETE A LIFE THREATENING RECALL REPAIR? I WILL CONTEST IN ADVANCE ANY EXCUSE THAT KOOL CHEVROLET COMES UP WITH THAT THEY SAY JUSTIFIES THEIR INACTION TO PERFORM THIS LIFE THREATENING RECALL REPAIR.

I PURCHASED THIS 2003 PONTIAC VIBE FOR $4000 ON 7/27/2013 FROM 49TH STREET AUTO SALES & REPAIR. THIS CAR HAD INCORRECT ENGINE INSTALLED. VIN: [XXX] WAS INSTALLED WHEN I PURCHASED. GM DEALER VERIFIED THE ENGINE VIN # IS FROM A 2007 PONTIAC VIBE 4 CYL 1.8 L AUTOMATIC TRANSMISSION ENGINE. NOT A 16 VALVE 6 SPEED VEHICLE. THEREFORE THERMOSTAT SENSOR CAN'T BE ATTACHED, TEMPERATURE SENSOR FOR RADIATOR CAN'T BE ATTACHED, ENGINE LIGHT HAS BEEN DISABLED AND TRANSMISSION ERRORS CAN'T BE CORRECTED. WARNING ASK OWNER WHY HE THIS VEHICLE WAS ADVERTISED TO HAVE A 4 CYLINDER TOYOTA CELICA GT-S MOTOR INSTALLED. PER MECHANIC ([XXX]) INSPECTION MY SON WAS ASSURED BY HIM IT ONLY HAD 900 MILES ON IT. IT DID NOT AFTER $1241.20 IN REPAIRS JUST TO MAKE IT SAFE TO DRIVE AFTER THE AXLE SHAFT FELL OUT OF THE TRANSMISSION AT 70 MPH. I'M FROM MICHIGAN AND DROVE THIS DEATH TRAP OF A CAR HOME WITH INTENSE SHAKING ON FRONT RIGHT BECAUSE AXLE WASN'T EVEN ATTACHED. MY MECHANIC HAD TO WELD THE AXLE. I HAVE DOCUMENTATION FROM GM STATING THIS ENGINE SHOULD NOT BE INSTALLED INTO THIS VEHICLE. VIDEO FROM MECHANIC WHO IDENTIFIED ENGINE VIN DETAILING ALL PROBLEMS. I RECEIVED A REFUND FROM THIS DEALER THROUGH MEDIATION WITH CONSUMER PROTECTION AGENCY OF PINELLAS COUNTY. THIS DEALER IS TRYING TO SELL THIS DANGEROUS VEHICLE AGAIN! .49THSTREETAUTOSALES.COM/VIEW/1427/TAN-2003-PONTIAC-VIBE-IN-CLEARWATER-FL INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6).
