Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. Issue: On certain cars, the passenger-front airbag that was installed during prior recall repair must be replaced. The airbag may not unfold properly when it deploys. This could cause the airbag to tear or not inflate properly in a crash. Safety Risk: An airbag that does not work properly could create an increased risk of injury in a crash. Corrective Actions: General Motors will notify owners by mail and instruct them to take their vehicle to a dealer to replace the passenger-front airbag assembly.

Airbag

On certain vehicles, long-term exposure to high absolute humidity and temperature, combined with high temperature cycling, could eventually degrade the propellant contained in the passenger frontal airbag. This could cause the airbag to deploy with more force than normal. As a result, if the vehicle were involved in a collision that warrants airbag deployment, fragments could be propelled toward vehicle occupants, and the airbag assembly could be damaged, preventing proper function. These issues could increase the risk of injury. Correction: Dealers will replace the passenger frontal airbag module.<br><br> Note: Canadian climate results in the propellant degrading slowly. The recall is being conducted as a precaution to address future risk, and is expected to replace airbag inflators before their function would be affected. Vehicles may have had the original front passenger airbag module replaced as a "like-for-like" replacement under recall 2015-272 (GM Recall #14491) as a temporary remedy while evaluating a permanent remedy. These vehicles will receive the permanent remedy passenger airbag assembly under this recall.

On certain vehicles equipped with a 1.8L MFI engine and a two-wheel drive transmission, the Engine Control Module (ECM) may have been improperly manufactured. There is a possibility that a crack may develop at certain solder points or on varistors on the circuit board. In most cases, if a crack occurs at certain points or on certain varistors, the engine warning lamp could be illuminated, harsh shifting could result, or the engine may not start. In some instances, if cracking occurs on particular solder points or varistors, the engine could stop while the vehicle is being driven, increasing the risk of a crash causing personal injury. Correction: Dealers will inspect and, if necessary, replace the ECM.

FORWARD COLLISION AVOIDANCE

The contact stated their fiancé owned a 2007 Pontiac Vibe. The contact stated that while their fiancé was driving, MPH unknown, the vehicle was drifting, then suddenly veered towards the opposite lane going south, causing the vehicle to crash into two vehicles in that lane. The vehicle spun from the impact and ended up in the opposite lane, stopped. The front driver-side air bag deployed. The vehicle was towed and expected to be totaled. The other two vehicles involved in the crash were towed and had damage as well. The driver of the vehicle was injured at the location, following the incident, but received medical attention on the spot. At the hospital, the driver's injuries were fatal. The contact spoke with a certified mechanic who informed them that the Forward Collision Avoidance feature seemed to fail due to other safety features in the vehicle not activating and preventing the crash. The mechanic stated the safety feature seemed to take full control of the vehicle, not allowing the driver to safely operate the vehicle. The dealer was not contacted. The manufacturer was not made aware of the failure. The failure mileage was 140,000.

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

MY DAUGHTER WAS DRIVING AT A SPEED OF 45 TO 50 MPH ON A ROAD WITH A POSTED SPEED LIMIT OF 55 WHEN ANOTHER CAR HIT A PATCH OF ICE WHILE COMING OUT OF A DRIVEWAY AND SLID INTO THE ROAD IN FRONT OF HER. MY DAUGHTER COULD NOT STOP IN TIME AND RAN INTO THE LEFT FRONT THIRD PART OF THE OTHER CAR. THE OTHER CAR WAS CONSIDERED TOTALED BY THEIR INSURANCE COMPANY. THE FRONT OF OUR CAR HAD SIGNIFICANT DAMAGE SO BECAUSE OF IT'S AGE AND BECAUSE WE DIDN'T HAVE COLLISION COVERAGE I DECIDED IT WASN'T WORTH TRYING TO FIX SO IT WILL BE AUCTIONED AT A SALVAGE YARD. THE REASON FOR THE COMPLAINT IS THAT THE DRIVER SIDE AIR BAG DID NOT DEPLOY CAUSING MY DAUGHTER'S FACE TO HIT THE DASH CAUSING SIGNIFICANT FACIAL INJURIES. ALMOST EVERYONE INVOLVED SEEMED TO THINK THAT IT WAS ODD THAT THE AIR BAG DID NOT DEPLOY SO WE THOUGHT WE SHOULD REPORT IT.

There were two incidents. 1. I was pulling into a parking spot at a Walmart driving very slowly, to almost reach the parking sign when the vehicle revved and lurched forward. I had to put my full pressure on the brake pedal, and eventually shifted to neutral and the revving subsided. I turned the car off and looked at the floor mat, which was in its usual position, and did not seem to be interfering with the pedal. Since this had not happened before, I rationalize that perhaps my foot had been on both the brake and accelerator. But I did not notice that my foot had slipped at all, and it was definitely on the brake when the revving occurred. 2. Today, my wife called me to tell me she had experienced the same revving while she was parking.

The contact owns a 2007 Pontiac Vibe. The contact stated that the air bag had been previously replaced however, the air bag warning light illuminated after the replacement of the air bag. The vehicle was taken to an independent mechanic who diagnosed the vehicle and determined that the air bag inflators failed and needed to be replaced. The mechanic informed the contact of NHTSA Campaign Numbers: 19V627000 (Air Bags), 18V883000 (Air Bags), 15V285000 (Air Bags) and 15V286000 (Air Bags). The dealer had been contacted and it was confirmed that the VIN was not included in any of the recalls. The vehicle was not repaired. The manufacturer was made aware of the failure and stated there were recalls for the air bags then referred the contact back to the dealer. The contact was also referred to NHTSA Hotline to file a complaint. The approximate failure mileage was 139,000.

Manufacturer is unable to remedy NHSTA Recall # 15V 285, also known as GM Manufacturer Recall # N 140491. It was brought to the GM Dealership, T&T Coast 2205 Highway 35, Sea Girt NJ 08750 THREE times. The first time they could not properly install an airbag, claiming the airbag they were trying to install was defective. The 2nd time another new airbag was installed but the SRS/Airbag dashboard light went on and would not go off. The THIRD time the dealership ran a OBD scan (and charged me) only to say, yep Code B-1911 showed on the scanner --- this is an SRS AIRBAG failure. They refused to fix it. UPDATE 06/29/2023- Consumer mailed in copy of invoice showing the issue and no repair, case number with GM.

THE COVER FOR THE AIRBAG ON THE DASH IS PUSHED UP AT LEAST 1/2 INCH ABOVE THE DASH ON THE WINDSHIELD SIDE OF THE AIRBAG COVER. I WAS NOTIFIED OF THIS AFTER THE TECHNICIAN INSTALLED THE REPLACEMENT PARTS AND WAS ALSO TOLD THAT THESE WERE THE PARTS THAT WERE SENT TO THEM TO REPLACE THE PROBLEM AIRBAG PARTS.
